Satisfaction with care is an important indicator for family caregivers of patients with terminal cancer and is linked to the quality of life. Despite this, few studies have examined the aspects of satisfaction with care of family caregivers of inpatients with cancer in general wards. This qualitative study aimed to elucidate the elements of satisfaction with the care that inpatients with terminal cancer in general wards and their family caregivers receive from medical staff, as perceived by the family caregivers. Semi-structured interviews were conducted with 10 family caregivers of inpatients with terminal cancer. Participants were asked about the care received until then from medical staff, the features of satisfactory care, and the aspects of care they felt were unsatisfactory or could become satisfactory with improvement. The data were analyzed with the content analysis method and the six categories were extracted. For family caregivers of inpatients with terminal cancer in general wards, along with the care identified as important in palliative care, the methods of alleviating symptoms and explaining the patient’s condition were also important. The results highlight the importance of determining a patient-oriented approach and explanations together with each patient and family caregiver, based on an understanding of the long treatment process.

This work is based on an on-going research on medical informed consent in Papua New Guinea(PNG)with 19 doctors from hospitals,health care centers and private medical establishments in the National Capital District and Central Province who voluntarily participated in the study.The authors conducted an examination of doctors’understanding of informed consent for medical procedures through a qualitative interview study and we describe our findings.We saw a need to involved patients in decision-making about their care,ethical imperative and concerns about litigation and their complaints highlighted the issue of informed consent.In order for the patients to make an informed decision about medical procedure,the doctors involved in the proposed treatment should conduct the informed consent discussion.The discussion should include the treatment,the risks and benefits of treatment,and alternative therapies with associated risks and benefits.We studied doctors’perspectives of gaining informed consent for routine medical procedures.We used qualitative study methods using structured interviews selected by purposive sampling.The data collected were entered into a data base and were analyzed thematically.The discussion is based on review of legal decisions,commentaries and our personal experience in studying medical malpractice cases.We have utilized case reports and several informative writings that have appeared in the world literature,as well as selections from vast amounts of material available in USA,UK,Australia,India and PNG.The current informed consent processes do not appear to be ideal for many doctors in PNG.In particular,there are inhibiting factors that affect patients from making medical informed decisions,doctors find time not enough to run discussions on informed consent,Department of Health does not have a standard informed consent form,patients complain about no consent form,and they have not signed consent forms.These are but some issues that affect patients when trying to make informed decisions.We say that

A qualitative study on Experiences of Patients with Esophageal Speech Training after Total Laryngectomy

Background: Patients who had total laryngectomy not only have a pressure from the possible diagnosis of laryngeal cancer, but also suffer from unspeakable misery. To relieve the suffering, some patients choose esophageal speech training. However, most of the current researches focus solely on the results of the training with the negligence of the patients’ mental health and their life experience during the training. A qualitative research is highly sensitive to patients’ subjective feelings, and thus able to capture their subtle emotional changes. Objective: To explore deeply patients’ real experiences of esophageal speech training after total laryngectomy and reveal the public their training experience of the unique and special group. Methods: In May 2018, researchers conducted a two-month field observation on and in-depth interviews with 15 patients who were on esophageal speech training after total laryngectomy in the esophageal speech training room of the Eye and ENT Hospital of Fudan University. The research applied phenomenological research method and Colaizzi Data Analysis. Results: The data analysis indicated four main themes: changes of mindset;showing positive attitudes and behaviors in training;shift in social relations;and arduous training process. Conclusion: The study has the finding that patients on esophageal speech training had negative emotions in the early stage of the training and dissatisfaction with its teaching. The repercussion of the surgery brought them changes of social relations, social bias and prejudice ensued. It is, therefore, urgent and crucial to give the speech trainees psychological counseling in the early stage to improve their qualities of life, to mobilize social support to incentivize their training, to optimize esophageal speech training teaching, and to regulate the management of the esophageal speech training environment.

BACKGROUND Parents of children with type 1 diabetes mellitus(T1DM)are under heavy caregiving stress,and parental caregivers'experience can affect the health outcomes of children with T1DM.AIM To describe the true inner feelings of parents caring for children with T1DM.METHODS Descriptive research methods were used to classify and summarize parents'experience when adapting to the role of caregivers for children with T1DM.The data was sorted and analyzed using content analysis.Themes of parents'experience caring for children with T1DM were refined,and their feelings were deeply investigated.RESULTS A total of 4 themes and 12 subthemes were identified:(1)Desire for information(disease-related information,home care information,and channels of information acquisition);(2)Skill guidance needs(insulin injection techniques,skills required for symptom management,and skills for parent-child communication);(3)Seeking emotional support(family support,peer support from other parents of children with T1DM,and professional support);and(4)Lack of social support(needs for financial support and needs for social security).Exploring the true experience of parents caring for children with T1DM is of great significance for helping them adapt to their role as caregivers.Nurses should provide professional guidance in terms of information,skills,emotion,and social support to parental caregivers.

Emotions of Gynecologic Cancer Patients Dealing with Permanent Colostomy:A qualitative Interview study

Purpose: This study was conducted to describe emotional state of colostomized patients with gynecologic cancer. Patients and Methods: The study employed a qualitative design from May to October of 2010, in a private university hospitals outpatient gnecologic oncology clinic in Ankara, Turkey. Content analysis with an inductive approach had been used to analyze 32 interviews that served as the data base for this study. Interview form that focused on what gynecologic cancer women felt to living with a colostomy. Results: Findings also reveal that gynecologic cancer patients with colostomy experience many uneasy feelings that affect their emotional state. The content analysis resulted in the development of two thematic units. The first we described as “To experience the consequences of stoma as a gynecologic cancer patients”. The second was “suffering experienced-being challenged the consequences of the stoma”, and the third was “promote self management-normalization of the new condition”. For the psychological support, family and friends’ support has been mentioned as the most important part that can release their anxiety. Conclusions: We concluded that a having stoma was difficult for the women. The emotional state of the women when they get colostomy was typically characterized by fear, and worry about their current process. Body image, self-esteem, social activity, sexuality were the aspects that most affected the patients. Nurses have very important role within holistic approach and empathetic interaction, for determining major problem areas.

Objective: To explore the negative emotions of mothers during chemotherapy of their children with malignant brain tumors. Methods: The phenomenological method for qualitative studies was adopted;convenience sampling was used to conduct semi-structured interviews to the mothers of 9 children with malignant brain tumors who received chemotherapy from June 2019 to December 2019;the Colaizzi 7-step analytical method was implemented to collate and analyze the original data. Results: The results were summarized into six subjects: stressed, exhausted because of trying to control everything in home care, prone to excessive care, financially burdened, desperate for knowledge of patient care, and desperate for psychosocial support. Conclusions: Understanding the real experiences and inner feelings of mothers during chemotherapy of their children with malignant brain tumors is helpful for medical staff to learn about the psychological distress and needs of mothers of children with malignant brain tumors, so as to give them psychological support and even social support, and help the mothers and the families of children with malignant brain tumors get through a special period.

Purpose: To describe how patients living with psoriatic arthritis experience long-term hydrotherapy group treatment. Studies for this group of patients are lacking. Method: qualitative in terviews were conducted with ten informants after hydrotherapy treatment. The treatment included exercises for increased mobility, coordination, endurance, aerobic fitness, stretching and relaxation. The interviews were analysed with content analysis. Results: A theme “hydrotherapy—a multidimensional experience” and two categories emerged: situational factors and effects of hydrotherapy. The category “situ- ational factors” comprised the subcategories: warm water, training factors and a competent instructor, and the category “effects of hydro- therapy” comprised the subcategories: psy- chological effects, improved physical capacity, social effects, changed pain experience and changes in work and participation in daily life. Conclusion: Positive effects of regular hydro- therapy in a group setting were shown in physical ability, energy, sleep, cognitive function, work and participation in daily life. The instruc- tors had an important coaching role, which needs to be promoted.

Feelings and Spiritual Practices of Patients with Mastectomy: A qualitative study from Turkey

Purpose: This study was conducted to describe feelings and spiritual practices of patients with mastectomy. Method: The study was employed a qualitative design at a private university hospital oncology unit in Ankara, Turkey. Interviews were performed with 20 mastectomized women. Results: The findings indicated that commonly used in several spiritual practices;prayer was used by 96%, of the patients, and 94% requested that others pray for their health, the practice to positive reframing (76%), the practice to planning for family-friends activities (74%), visiting mos-que-shrine (73%), and the practice to reading Quran (71%). The categorization of the data led to identification of three thematic units. The first we described as “Experiencing the consequences of illness as a disorder”. The second was “Restoring a sense of order to life”, and the third was “Living with the new condition”. Conclusion: We concluded that a having mastectomy was difficult for the subjects. In this respect, nurses’ role is vital to help women with breast cancer in such a difficult and modifying process as breast removal.

Aim:The study aimed to explore factors related to the initiation and utilization of focused antenatal care(FANC)in the Southern District of Mzimba,Malawi.Methods:This study used an exploratory qualitative design.Total of 22 in-depth interviews with pregnant women and community midwife assistants were conducted from December 2015 to January 2016 in Mzimba.Thematic analysis approach was adopted to identify the facilitator and harriers of the FANC initiation and utilization.Results:Facilitator of FANC initiation and utilization included seeking pregnancy confirmation,medical treatment for an existing health problem and the support by community health extension workers.Barriers included the additional cost to free FANC service,lack of essential equipment,unfriendly adolescent reproductive health service,and HIV stigma.Conclusion:Early initiation of FANC relies on both woman’s awareness and community support.Promoting the use of FANC should focus on creating an enabling environment,e.g.,increasing investment of essential medical equipment,reducing additional costs of FANC services,eliminating the discrimination against adolescent pregnancy and people living with HIV,and strengthening health personnel’s training.

A qualitative study of Non-English Majors' Learner Autonomy in Out-of-class English Learning Contexts

The aim of the study was to explore non-Engish majors' actual practice of out-of class English learning activities and the relationship between out-of-class English learning and language proficiency. Data were gathered through qualitative methods by semi-structured interviews and observation with 6 non-English majors from Tianjin Foreign Studies University. Students' CET-4 scores were collected as well. Out-of-class observation was conducted with two participants who represent respectively high and low scores in CET. The results indicated that high scorers generally showed higher level of learner autonomy in out-of-class learning context. than low scorers. Most of the participants hoped to obtain more guidance and recommendations for learning materials and learning strategies so as to improve their language skills.
