Objective:To investigate the quality of life(qol)of living donor liver transplantation(LDLT)donors.Methods:The Euro qol 5-dimensional questionnaire(EQ-5D)was used to measure the qol of donors.Results:One donor repor ted“a little problem”of mobility(MO),and 2 donors(9.1%)repor ted“a little problem”of usual activities(UA).Moreover,there were 8 donors(36.4%)and 7 donors(31.8%)declaring“a little problem”of pain/discomfor t(PD)and anxiety/depression(AD),respectively.And both dimensions have a donor repor ting“moderate problem.”The mean visual analog scale(VAS)was 83.1±12.4.Conclusions:Donors can gain a stable and preferable qol after donation in both the shor t and long terms.ED-5D application in the field of liver transplant could be an effective choice in qol studies.

In 2003, a 61-year old highly respected director of the Surgery Department of Dongfang Hospital Affiliated to Beijing University of Chinese Medicine was diagnosed as advanced lung cancer, moderately differentiated adenocarcinoma, coverinqmore than two thirds of the thoracic aorta, multiple mediastinal nodal metastases, pleural effusion ＂Only 3 months could be expected＂, a cruel fact came out after deliberation of many seasoned medical specialists. In the whole career of this surgery professor, he used the lancet as his weapon to save lives. But this time after weighing the pros and cons again and again, he required a ＂tender＂ plan with no lancet.

In the 1960s, a number of integrative Chinese and Western medicine （ICWM） surgeons （such as Prof. WU Xian-zhong） first started the application of traditional Chinese medicine （TCM） in acute abdominal surgery, and thus they became the pioneers of perioperative therapy with ICWM. Since then, they haveaccumulated abundant experience in this field. At the 1st National Conference of Perioperative Therapy with ICWM in November 1994, the academician WU Xian-zhong, et al believed that perioperative therapy with ICWM hold promise for the future.

Understanding Patients’ Symptoms Management Using Objective and Subjective Indicators among Patients with Systemic Lupus Erythematosus

Background: Descriptive statistics have been used to document the high prevalence of sleep disorders, fatigue, and pain in patients with systemic lupus erythematosus (SLE). Objectives: To describe day-to-day variations in sleep indicators, physical activity level, pain, and fatigue measured using objective and subjective indicators in lupus patients with sleep disorders. Methods: We selected three patients with sleep disorders, measured by the Pittsburgh Sleep Quality Index score (≥9), out of 20 participants followed up for 4 weeks. We compared the self-rated quality of life (qol) measured using the Pittsburgh Sleep Quality Index (PSQI) with that of objectively measured sleep indicators monitored using the Nemuri scan. The detailed data collection methods have been previously published. Results: The sleep status generated by the Nemuri scan revealed that Case A with a flare and Case B without a flare required frequent daytime rest. The PSQI sleep duration and sleep latency were generally in agreement with the monitored data, although the PSQI failed to capture the complexity of sleep disorders, particularly the fluctuations in the quality of sleep indicators. Patient C approximately 4 hours on weekdays and 10 hours on Saturday nights. All three cases had high fatigue levels, and their moderate-to-vigorous physical activity duration was less than one-half of duration that recommended by the World Health Association. Conclusion: Visual display for objective monitoring of sleep quality is an excellent tool for understanding patients who require frequent resting and irregular sleep indicators. Objective monitoring of sleep quality, along with self-rated pain and fatigue, promotes an understanding of how patients with SLE cope with severe symptoms.

CM has got excellent reputation because of the effects. Nonetheless, few clinical articles of CM to treat cancer are published on major international journals up to now.

In China, CM has been applied to treat almost every cancer patient, especially during the observation period after surgery, routine radiotherapy and chemotherapy. Several decades of CM treatment results have shown that CM treatment could be helpful in improving patients＇ physical rehabilitation, relieving symptoms and strengthening immunity. CM also could aid in the decline of rates of recurrence and metastasis, prolong the survival time, and especially, have significant effects on relieving symptoms such as relieving fatigue, improving syndromes caused by conventional therapies （surgery, radiotherapy, chemotherapy and biological treatment）, and indescribable discomforts caused by the unknown reason. It has been found in several studies that conventional medical treatment combined with CM could significantly improve qol, and reduce treatment-related and disease-related symptoms. 目的:探讨宫颈癌术后患者支持性照顾需求现状;了解患者支持性照顾需求与生活质量(Quality of life,qol)的变化与两者的关联;指导临床医护人员提供以患者需求为导向,更具针对性地为宫颈癌患者提供护理服务,提高患者生活质量。方法:本研究采用量性和质性研究相结合的方法,共包括以下两部分内容:第一部分为量性研究,选取济南市某三甲医院妇科病房符合纳排标准的宫颈癌术后患者为研究对象,使用支持性照顾需求量表(SCNS-SF34)、宫颈癌特异性评价量表(EORTC QLQ-CX24)和心理痛苦温度计(DT)对115例患者于术后7日(T1)进行调查,了解其支持性照顾需求、生活质量现状和主要影响因素。在此基础上,分别于术后3个月(T2)、术后6个月(T3)继续跟踪调查,动态分析患者支持性照顾需求和qol随时间变化的趋势及两者的关系。第二部分为质性研究,在T1阶段的问卷调查基础上通过目的抽样选取支持性照顾需求条目均分≥4分,且心理痛苦得分(DT)≥4分的患者为研究对象,采用质性研究中的现象学方法将每例研究对象视为个案并对其追踪,根据时机理论对同一患者于术后5-7日(调整期)、术后3个月(适应期)、术后6个月(稳定期)共进行3次一对一半结构化访谈。通过定性的深入访谈,应用支持性照顾需求多维模型归类结果,纵向探究宫颈癌术后患者疾病不同阶段支持性照顾需求变化。结果:1.宫颈癌术后支持性照顾需求得分为(110.08±24.339),条目均分为(3.114±0.680),属于中度需求,其中健康信息需求得分最高(3.676±0.949)分,性需求得分最低(2.403±1.434)分。2.患者的年龄、临床分期、文化程度、手术类型、对疾病认识、家庭人均月收入是支持性护理需求的影响因素(Pqol在术后3个月内(T2)下降至最低,然后逐步上升,且在术后六个月达到最高水平。5.T1、T2、T3三个阶段支持性照顾需求与qol的关系为负相关(P＜0.05),即患者需求水平越高其生活质量越差。6.质性研究阶段结果:患者术后3个阶段的支持性照顾需求体验,分别是:(1)调整期:身体不适及日常生活中断;心理调适需求;医务人员及家人支持需求;治疗方案及康复要点需求。(2)适应期:放化疗反应及手术远期影响;心理调节与建设需求;延续护理服务及同伴支持需求;症状及复查相关信息需求;“性”心理压力及性体验异常。(3)稳定期:回归社会需求;疾病复发担忧;获得认可;身体调理及不适症状处理;性信心不足及性知识缺乏。结论:1.宫颈癌术后患者均存在不同程度的需求未满足情况,健康系统与信息需求最高,且受多种因素影响,根据变化轨迹术后三个月内呈上升趋势,之后逐渐下降。2.不同阶段患者支持性照顾需求特点不同,术后三个月是生活质量最差的时机,也是干预的重点阶段。因此,医护人员应科学评估患者不同时期未满足的需求,制订与实施阶段性、个体化的措施,使其最大化地重返家庭与社会。3.患者支持性照顾需求与生活质量呈负相关关系,即患者需求水平越高其生活质量越差。根据患者在疾病不同阶段所表现出的需求,给予针对性的干预措施,提高患者生活质量

AIM: Quality of life (qol) is a concept that incorporates many aspects of life beyond 'health' The chronic liver disease questionnaire (CLDQ) was developed to evaluate the impact of chronic liver diseases (CLD) on qol. The objectives of this study were to translate and validate a liver specific questionnaire, the CLDQ.METHODS: The CLDQ was formally translated from the original version to Thai language with permission. The translation process included forward translation, back translation, cross-cultural adaptation and a pretest. Reliability and validity of the translated version was examined in CLD patients. Enrolled subjects included CLD and normal subjects with age- and sex-matched. Collected data were demography,physical findings and biochemical tests. All subjects were asked to complete the translated versions of CLDQ and SF36, which was previously validated. Cronbach's alpha and test-retest were performed for reliability analysis. One-way Anova or non-parametric method was used to determine discriminant validity. Spearman's rank correlation was used to assess convergent validity. P-value 0.45. Test-retest reliability done at 1 to 4 wk apart was 0.88 for the average CLDQ score and from 0.68 to 0.90 for domain scores. The CLDQ was found to have discriminant validity. The highest scores of CLDQ domains were in the normal group, scores were lower in the compensated group and lowest in the decompensated group.
