pulmonary arteriovenous malformations （AVM） lung is defined by an abnormal communication between pulmonary artery（ies） and vein（s） responsible for a right-left shunt. Congenital forms are most common and usually associated with Rendu-Osler disease （ROD）. Inversely, 15-45% of patients with ROD present lung AVM Nowadays, embolization is preferred to surgical resection in the majority of cases. Except for certain cases where surgery is indicated. This paper shows the role of surgery in AVMs.

＜正＞Objective To cure congenital multiple pulmonary arteriovenous malformations （AVM） with transcatheter embolotherapy with coils and geofolm. While traditionally such kind of AVM were treated only by surgical operation, but with unideal results. Methods Two patients underwent intravascular embolotherapy （aged 18 and 21, both male）. Multiple pulmonary AVM in both lungs were shown by digital subtraction angiography. Superselective catheterization was taken first, then followed embolization with coils in different size and gelfoma particles. Results Three and two sites of pulmonary AVM of two patients were occluded successfully by transcatheter intravascular embolotherapy. Patients’ symptoms had greatly improved after operation. Shortness of breath and dark purple of lip had gone. Blood oxygen saturation had got to the normal level. Three years follow-up showed no recurrent on both two patients. Conclusion Compared with surgical operation, interventional embolotherapy is an ideal way to cure pulmonary AVM, especially for multiple ones. It ’s safe, less invasive, effective and short-recovered. So it is more acceptable for patient.
