原发性卵巢鳞癌(primary ovarian squamous cell carcinoma, pscC)很罕见,在所有类型的卵巢恶性肿瘤中发病率不足1%[1]。pscC绝大部分为成熟囊性畸胎瘤恶性转化,少数起源于卵巢Brenner瘤或子宫内膜异位症恶变,而无卵巢基础疾病,由卵巢表面上皮组织鳞状化生的单纯pscC是极为罕见的类型[2]。本文报道本院1例罕见的单纯pscC并进行文献复习。

Rare and undiagnosed diseases substantially decrease patient quality of life and have increasingly become a heavy burden on healthcare systems.Because of the challenges in disease-causing gene identification and mechanism elucidation,patients are often confronted with difficulty obtaining a precise diagnosis and treatment.Due to advances in sequencing and multiomics analysis approaches combined with patient-derived ipsc models and gene-editing platforms,substantial progress has been made in the diagnosis and treatment of rare and undiagnosed diseases.The aforementioned techniques also provide an operational basis for future precision medicine studies.In this review,we summarize recent progress in identifying disease-causing genes based on GWAS/WES/WGS-guided multiomics analysis approaches.In addition,we discuss recent advances in the elucidation of pathogenic mechanisms and treatment of diseases with state-of-the-art ipsc and organoid models,which are improved by cell maturation level and gene editing technology.The comprehensive strategies described above will generate a new paradigm of disease classification that will significantly promote the precision and efficiency of diagnosis and treatment for rare and undiagnosed diseases.

Primary sclerosing cholangitis: Updates in diagnosis and therapy

Primary sclerosing cholangitis (psc) is a chronic cholestatic syndrome of unknown origin mostly found in males, and characterized by diffuse inflammation and fibrosis of both intra- and extra-hepatic bile ducts. So far, psc is considered as an autoimmune hepatobiliary disease. In most cases the progression of psc towards liver cirrhosis and liver failure is slow but irreversible, and liver transplantation is currently the only definitive treatment. In recent years,psc has been an area of active research worldwide with great interest in etiology, pathogenesis, diagnosis, and therapeutic options such as hydrophilic ursodeoxycholic acid and immunosuppressive agent tacrolimus. Recent updates on clinical and therapeutic aspects of psc are discussed in the present review.

Synergistic effect of fluorination on both donor and acceptor materials for high performance non-fullerene polymer solar cells with 13.5% efficiency

A high performance polymer solar cells(pscs) based on polymer donor PM6 containing fluorinated thienyl benzodithiophene unit and n-type organic semiconductor acceptor IT-4 F containing fluorinated end-groups were developed. In addition to complementary absorption spectra(300–830 nm) with IT-4 F, the PM6 also has a deep HOMO(the highest occupied molecular) level(-5.50 e V), which will lower the open-circuit voltage(V_(oc)) sacrifice and reduce the E_(loss) of the IT-4 F-based pscs. Moreover, the strong crystallinity of PM6 is beneficial to form favorable blend morphology and hence to suppress recombination. As a result, in comparison with the pscs based on a non-fluorinated D/A pair of PBDB-T:ITIC with a medium PCE of 11.2%, the PM6:IT-4 Fbased pscs yielded an impressive PCE of 13.5% due to the synergistic effect of fluorination on both donor and acceptor, which is among the highest values recorded in the literatures for pscs to date. Furthermore, a PCE of 12.2% was remained with the active layer thickness of up to 285 nm and a high PCE of 11.4% was also obtained with a large device area of 1 cm^2. In addition, the devices also showed good storage, thermal and illumination stabilities with respect to the efficiency. These results indicate that fluorination is an effective strategy to improve the photovoltaic performance of materials, as well as the both fluorinated donor and acceptor pair-PM6:IT-4 F is an ideal candidate for the large scale roll-to-roll production of efficient pscs in the future.

The microstructure and texture development in a Mg-0.41Dy(wt%)alloy after plane strain compression(psc)at 300℃under a strain rate of 10^(-2)and 10^(-4)s^(-1),up a final true strain of—1.2 were investigated using electron backscatter diffraction(EBSD)and X-ray diffraction.At high strain rate of 10^(-2)s^(-1),the microstructure exhibited massive twins mainly{101^(-)2}extension,{1011}contraction and{101^(-)3}-{101^(-)2}double twin due to the random texture of the as-cast alloy.Meanwhile,at a low strain rate of 10^(-4)s^(-1),the microstructure was characterized by dynamic recrystallization at the{101^(-)2}extension,(101^(-)1)contraction twins and grain boundaries.Twin dynamic recrystallization(TDRX),rotational dynamic recrystallization(RDRX)and discontinuous dynamic recrystallization(DDRX)were the main mechanisms responsible for the formation of recrystallized grains.The texture was characterized by the formation of three fibers:basal,//CD and//CD where CD is the compression direction.Moreover,the texture was less sensitive to the deformation conditions since the recrystallized grains showed the same orientation than twins.The changes of the mechanical properties of the alloy were ascribed to the resulting microstructure due to the twinning and dynamic recrystallization.

倾向性评分的敏感性分析及SAS软件实现

目的介绍三种倾向性评分的敏感性分析方法的原理及SAS实现,包括Rosenbaum、propensity score calibration、VanderWeele&Arah敏感性分析。方法利用真实世界研究——纤维肌痛的模拟数据集,分别采取Rosenbaum、propensity score calibration和VanderWeele&Arah三种方法进行敏感性分析,并通过Rosenbaum界限、平均干预效应等指标衡量隐藏偏倚,进一步评价主要分析结果的稳健性。结果Rosenbaum、propensity score calibration、VanderWeele&Arah方法均可实现对倾向性评分的敏感性分析,Rosenbaum方法应用最为普遍,propensity score calibration和VanderWeele&Arah方法适用范围更广,对于隐藏偏倚的解释更加具体可靠。结论随着倾向性评分方法的广泛应用,其敏感性分析的重要性也逐渐突显,本研究中介绍的三种方式建议综合使用,全方面评估隐藏偏倚,以提高研究结果的可信度。
