Determining Requirements for an Optimal Ad-Hoc Multicast protocol in Mobile Health Care Training Systems

Nowadays, Health Care Training-based System (HCTS) is a vital component in the education and training of health care in 3D Virtual Environment (VE). The practice of HCTS continues to grow at rapid pace throughout all of the healthcare disciplines, however research in this field is still in its early stage. Increasingly, decision makers and developers look forward to offer more sophisticated, much larger, and more complex HCTS to serve the desired outcome and improve the quality and safety of patient care. Due to the rapidly increasing usage of personal mobile devices and the need of executing HCTS applications in environments that have no previous network infrastructure available, Mobile Health Care Training-based System (MHCTS) is an expected future trend. In such systems, medical staff will share and collaborate in a 3D virtual environment through their mobile devices in an ad-hoc network (MANET) in order to accomplish specific missions’ typically surgical emergency room. Users are organized into various groups (Radiologists, Maternity departments, and General surgery etc...), and need to be managed by a multicast scheme to save network bandwidth and offer immersive sense. MHCTS is sensitive to networking issues, since interactive 3D graphics requires additional load due to the use of mobile devices. Therefore, we need to emphasize on the importance and the improvement of multicast techniques for the effectiveness of MHCTS and the management of collaborative group interaction. Research so far has devoted little attention to the network communication protocols design of such systems which is crucial to preserve the sense of immersion for participating users. In this paper, we investigate the effect of multicast routing protocol in advancing the field of Health care Training-based System to the benefit of patient’s safety, and health care professional. Moxibustion for Constipation in Patients with Stroke:protocol of a Systematic Review and Meta-analysis

Introduction:Constipation after stroke negatively affects the patients’quality of life.Moxibustion therapy may relieve the symptoms of constipation by moving intestinal Qi in accordance with traditional Chinese medicine theory.However,the effects of moxibustion therapy on constipation after stroke have not yet been systematically assessed.This review aims to assess the efficacy and safety of moxibustion therapy for constipation after stroke.Methods and analysis:We will search the following databases from inception to the present:Cochrane Central Register of Controlled Trials of The Cochrane Library,MEDLINE,EMBASE,Chinese Biomedical Database,China National Knowledge Infrastructure,VIP Database,and Wanfang Database.Randomized controlled clinical trials that assess the outcome of moxibustion for constipation after stroke will be included.Outcome measures will include mean weekly complete spontaneous bowel movements,the patient assessment of constipation,quality of life questionnaire,and others.Adverse events will assessed and reported for safety evaluation.Study selection and data extraction will be performed by two independent reviewers.Risk of bias assessment and data synthesis will be implemented using Review Manager software(version 5.3).Ethics and dissemination:This systematic review does not need ethical approval because no individual information will be involved.The protocol and full version of this systematic review will be published in a peer-reviewed journal.Updates will be conducted if there is enough new evidence that may cause any of the review conclusions to change.

Background: A Randomized Controlled Trial (RCT) has been elaborated where goal directed fluid and hemodynamic therapy (GDFHT) will be realized with trans-thoracic echocardiographic aortic blood flow peak velocity variation (ΔVpeak) and distance minute (DM) to guide fluid therapy and hemodynamics in high risk pediatric surgical patients. This RCT will clarify the impact of GDFHT with ΔVpeak and DM on postoperative outcome in terms of morbidity, length of stay in the intensive care unit (LOSICU), length of mechanical ventilation (LMV) and length of hospital stay (LOS) in children. To determine values of ΔVpeak, DM and VTI predictive of these postoperative outcomes, an observational pilot study will be realized. This pilot study is described here. The primary objective of this study is to determine values of ΔVpeak, DM and ITV predictive of postoperative outcome in children in terms of morbidity. The secondary objectives are to determine values of ΔVpeak, DM and ITV predictive of LOSICU, LMV, LOS, intraoperative, postoperative fluid administration and vasoactive-inotropic therapy. Methods: 500 - 1000 children aged less than 18 years will be included prospectively. Statistic analysis will be realized with XLSTAT 2019.4.2 software or plus. Results and Conclusions: This trial protocol will determine values of ΔVpeak, DM and ITV with echocardiography predictive of postoperative outcome in children.

Tree Based Energy and Congestion Aware Routing protocol for Wireless Sensor Networks

Wireless Sensor Networks (WSNs) have inherent and unique characteristics rather than traditional networks. They have many different constraints, such as computational power, storage capacity, energy supply and etc;of course the most important issue is their energy constraint. Energy aware routing protocol is very important in WSN, but routing protocol which only considers energy has not efficient performance. Therefore considering other parameters beside energy efficiency is crucial for protocols efficiency. Depending on sensor network application, different parameters can be considered for its protocols. Congestion management can affect routing protocol performance. Congestion occurrence in network nodes leads to increasing packet loss and energy consumption. Another parameter which affects routing protocol efficiency is providing fairness in nodes energy consumption. When fairness is not considered in routing process, network will be partitioned very soon and then the network performance will be decreased. In this paper a Tree based Energy and Congestion Aware Routing protocol (TECARP) is proposed. The proposed protocol is an energy efficient routing protocol which tries to manage congestion and to provide fairness in network. Simulation results shown in this paper imply that the TECARP has achieved its goals.

An Automated Negotiation protocol Based On Game Theory

With the goal of meeting one-to-many negotiation mechanism in the process of inter-basin water transfer, according to features of all interest subjects in analysis of inter-basin water transfer, a one-to-many synchronous bidding negotiation model based on bidding rules of game theory is structured, which analyzes and negotiates the functions, code of conduct and information expression of each subject. In order to optimize overall interests and individual interests, and a series of parameters in the negotiation, the paper propose various rules corresponding to the negotiation model so that the efficiency of each interest subject and fair distribution of negotiation proceeds can be safeguarded in the inter-basin water transfer system.

Effects of a Free Water protocol on Inpatients in a Neuro-Rehabilitation Setting

Background: Dysphagia is common among stroke and acquired brain injury (ABI) patients and may result in aspiration. To reduce the risk of thin liquid aspiration, patients are often restricted to thickened fluids. There is considerable clinical interest in the risks and benefits of offering oral water intake, in the form of water protocols, to patients with thin-liquid dysphagia. Objectives: A controlled pilot study of a free water protocol was undertaken at Riverview Health Centre, in Winnipeg, Manitoba to assess benefits, feasibility and possible complications of free water protocol. Methods: The study examined 16 individuals with stroke or ABI who were randomly assigned to either a control group on thickened fluids (six subjects) or a treatment group (ten subjects) that followed a free water protocol. The average length of time in the study was 4.3 weeks with a range of 1 - 17 weeks among all participants. Results: Throughout the study, there was no incidence of pneumonia in any of the sixteen participants. No statistically significant difference was noted in the weekly total liquid intake between the treatment group and the control group. Perceived swallowing related care results suggest statistically significant improvements from initial to final measures for both the treatment group (p = 0.004) and the control group (p = 0.02). However, a comparison of the change in both measures, between the treatment and control groups, shows no statistically significant differences. Conclusion: This pilot study suggests the need for larger scale work in order to more accurately identify the effects of free water protocols.

Home Automation Device protocol (HADP): A protocol Standard for Unified Device Interactions

Daily activities have become more efficient and convenient with home automation. There are many different home automation devices in the market for consumers to enjoy their home for being smarter, resourceful, and remotely accessible. However, current home automation protocols lack extensibility and compatibility. In this paper, we propose a protocol standard for home automation system called Home Automation Device protocol (HADP). This protocol standard aims for the interoperability of home automation devices across different platforms. Based on the IFTTT (IF-This-Then-That) model, we define a set of device communication protocols where devices’ triggers and actions are combined to generate and manage interactions through a central node. The proposed protocol standard offers low power consumption and low bandwidth requirements using the minimum data packets to trigger an action on a home automation device. The protocol supports various communication mediums such as Wi-Fi, Bluetooth 4.2, ZigBee IP, 6LoWPAN, IEEE 802.15.4 standards, and Ethernet orany network layer supporting IPv6 protocol.

Improved Routing protocol for Health Care Communications

Wireless Body Area Network (WBAN) technology has significantly increased the potential of remote healthcare monitoring systems. The devices used for WBAN have limited energy resources. For most devices it is impossible to recharge or change the batteries. Since the data mostly consists of medical information, high reliability and low delay is required. The main objective of this simulation study is to evaluate the performance of routing protocol on static IEEE 802.15.4 to determine the most suitable routing protocol for Wireless Body Area Networks. Here, investigations on the various routing protocol suggest cluster topology and AODV as the probable candidate. About 16% improvement in the energy consumption was observed when modifications were made considering the energy and mobility, thus achieving high residual battery capacity and eliminating the need for recharging the batteries.

A New Field protocol for Determination of Forest Structure, Biodiversity and Heath Status by Means of GPS Tools: A Case Study from Gaza Forest

This article describes the application of GPS, image processing analysis and statistical tools for determination of forest structure and biodiversity. The idea is based on determination of forest area using GPS tools, then taking representative plots “samples” about 5% - 10% of the forest and counting the tree species in each plot (sample). This process enables the estimation of species population in each plot. The sum of all species population enables the calculation of relative density of each species. Multiplying species population with plot number of the forest enables the calculations of total species population in the forest. Calculating the relative density enables the determination of dominant, co-dominant, suppressed and flour structure of the forest. To determine the age of the forest, we apply our method on determining the diameter of trees and grouping them to four groups, this enables the estimation of dominant age within single species. From these data, we can estimate the age of the forest. This field protocol is easy and applicable and provides good results in estimating structure, age and biodiversity in the forest.
