Pre-diagnostic levels of adiponectin and soluble vascular cell adhesion molecule-1 are associated with colorectal cancer risk

AIM： To examine the relationships between pre-diag- nostic biomarkers and colorectal cancer risk and assess their relevance in predictive ***： A nested case-control study was designed to include all first primary incident colorectal cancer cases diagnosed between inclusion in the SUpplemen- tation en VItamines et Min^raux AntioXydants cohort in 1994 and the end of follow-up in 2007. Cases （n = 50） were matched with two randomly selected con- trols （n = 100）. Conditional logistic regression models were used to investigate the associations between pre- diagnostic levels of hs-CRP, adiponectin, leptin, soluble vascular cell adhesion molecule-1 （sVCAM-1）, soluble intercellular adhesion molecule-I, E-selectin, monocyte chemoattractant protein-1 and colorectal cancer risk. Area under the receiver operating curves （AUC） and relative integrated discrimination improvement （RIDI） statistics were used to assess the discriminatory poten- tial of the models. RESULTS： Plasma adiponectin level was associated with decreased colorectal cancer risk （P for linear trend -- 0.03）. Quartiles of sVCAM-1 were associated with increased colorectal cancer risk （P for linear trend = 0.02）. No association was observed with any of the other biomarkers. Compared to standard models with known risk factors, those including both adiponectin and sVCAM-1 had substantially improved performance for colorectal cancer risk prediction （P for AUC improve- ment = 0.01, RIDI = 26.5%）. CONCLUSION： These results suggest that pre-diag- nostic plasma adiponectin and sVCAM-1 levels are as- sociated with decreased and increased colorectal cancer risk, respectively. These relationships must be confirmed in large validation studies.

Prognostic value of human papillomavirus 16/18 genotyping in low-grade cervical lesions preceded by mildly abnormal cytology

Histological low-grade squamous intraepithelial lesion/cervical intraepithelial neoplasia grade 1 （LSIL/CIN1） preceded by normal or mildly abnormal cytology is recommended for conservative follow-up, with no separated management. In this study, we assessed the triage value of human papillomavirus （HPV） 16/18 genotyping in 273 patients with LSIL/CINI. HPV16/18 genotyping was performed at baseline and follow-up was at 6-monthly intervals for up to 2 years. At each follow-up, women positive for cytology or high-risk HPV （hrHPV） were referred for colposcopy. Enrollment cytology, HPV16/18 genotyping, and questionnaire-obtained factors were linked to the 2-year cumulative progression rate. Univariate and multivariate analyses were performed taking into account time-to-event with Cox proportional hazard regression. The results showed that 190 cases （69.6%） regressed, 37 （13.6%） persisted, and 46 （16.8%） progressed. HPV16/18 positivity （hazard ratio （HR）, 2.708; 95% confidence interval （CI）, 1.432-5.121; P=-0.002） is significantly associated with higher 2-year cumulative progression rate. Sub-analysis by enrollment cytology and age restricted the positive association among patients preceded by mildly abnormal cytology and aged 30 years or older. Immediate treatment is a rational recommendation for the high-risk subgroup, when good compliance is not assured.

High-dose thalidomide increases the risk of peripheral neuropathy in the treatment of ankylosing spondylitis

Thalidomide is an effective drug for the treatment of ankylosing spondylitis but might induce peripheral neuropathy. This major adverse reaction has attracted much concern. The current study aimed to observe the incidence of thalidomide-induced peripheral neuropathy among an- kylosing spondylitis patients for 1 year after treatment. In this study, 207 ankylosing spondylitis cases received thalidomide treatment, while 116 ankylosing spondylitis cases received other treat- ments. Results showed that the incidence of thalidomide-induced peripheral neuropathy in the thalidomide group was higher than that in the non-thalidomide group. There was no significant difference in the incidence of neuropathy between the 〈 6 months medication and 〉 6 months medication groups. There were no differences in the mean age, gender, or daily dose between the two groups. The incidence of peripheral neuropathy among patients receiving 25, 50, 75, or 100 mg thalidomide per day was 4.6%, 8.5%, 17.1%, 21.7%, respectively. The incidence was significantly different between the groups receiving 25 mg and 100 mg thalidomide. In conclu- sion, thalidomide can induce peripheral neuropathy within 1 year after treatment of ankylosing spondylitis; however, age and gender have no obvious impact on the incidence of peripheral neuropathy. The incidence of peripheral neuropathy is associated with increasing daily doses of thalidomide.

Impact of Libido at 2 Weeks after Stroke on Risk of Stroke Recurrence at 1-Year in a Chinese Stroke Cohort study

Background： There were few studies on the relation between changes in libido and incidence of stroke recurrence. The aim of this study was to investigate the relationship between libido decrease at 2 weeks after stroke and recurrent stroke at 1-year. Methods： It is a multi-centered, prospective cohort study. The 14th item of the Hamilton Depression Rating Scale-17 was used to evaluate changes of libido in poststroke patients at 2 weeks. Stroke recurrence was defined as an aggravation of former neurological functional deficit, new local or overall symptoms, or stroke diagnosed at re-admission. Results： Among 2341 enrolled patients, 1757 patients had completed follow-up data, 533 （30.34%） patients had decreased libido at 2 weeks, and 166 （9.45%） patients had recurrent stroke at l-year. Multivariate logistic regression analysis showed that, compared with patients with normal libido, the odds ratio （OR） of recurrent stroke in patients with decreased libido was reduced by 4 l% （OR = 0.59, 95% confidence interval [CI]： 0.40-0.87）. The correlation was more prominent among male patients （OR = 0.52, 95% CI： 0.31 0.85） and patients of≥60 years of age （OR = 0.57, 95% CI： 0.35-0.93）. Conclusions： One out of three stroke patients in China's Mainland has decreased libido at 2 weeks after stroke. Decreased libido is a protective factor for stroke recurrence at 1-year, which is more prominent among older male patients.

Relationship between Trp64Arg mutation in the β3-adrenergic receptor gene and metabolic syndrome： a seven-year follow-up study

Background It has been shown that the β3-adrenergic receptor （β3-AR） gene Trp64Arg mutation was closely related to obesity and insulin resistance, and may be related to the prevalence of metabolic syndrome （MS）. The aim of this study was to investigate the relationship between the 33-AR gene mutation and the prevalence of MS. Methods A seven-year follow-up study was initiated in 2000, with 496 samples of simplex obese subjects （body mass index ≥25 kg/m2） and 248 normal-weight subjects. According to the β3-AR genotypes, the subjects were classified as Trp64 homozygote group and Arg64 carrier group and after 7 years the prevalence of MS was determined. Results According to the baseline profile, there were no significant differences in the adiposity, blood pressure, lipid profile, fasting plasma glucose and fasting insulin between Trp64 homozygote group and Arg64 carrier group either in obesity or normal-weight subjects. The results of follow-up study indicated that in obese men the prevalence rate of MS was much higher in Arg64 carrier group than that in Trp64 homozygote group （54.76% vs. 40.85%, P 〈0.05）, but there was no statistical difference in women of the above groups. The prevalence rate of MS in obese men of both Trp64 homozygote group and Arg64 carrier obese group were obviously higher than that in women of the above groups （40.85% vs. 18.27% and 54.76% vs 21.28%, all P 〈0.005）. Differences were not statistically significant in the prevalence of MS for normal weight Trp64 homozygote group and normal weight Arg64 carrier group, either between men, between women, or between men and women. Comparison of populations indicated that no matter with the β3-AR gene mutation or not, the prevalence of MS in obese subjects was significantly higher than normal weight subjects （X2=28.240 and x2=15.586, all P 〈0.005）. Logistic analysis showed that the mutation of β3-AR gene was associated with the prevalence of MS in men.

Prediction of Early Rebleeding and Mortality after Acute Esophageal Variceal Hemorrhage among Yemeni Patients in Major Hospitals—Sana’a

Background and aims: Acute variceal hemorrhage (AVH) is the most serious encountered complication of liver cirrhosis and carries high mortality rate. Several risk factors that predict early rebleeding and mortality have been studied and there is no similar study in our country, so the aim of this study was to identify the risk factors of early rebleeding and mortality after bleeding episode in cirrhotic patients in Yemen. Patients and Method: It was a prospective study of cirrhotic patients with AVH who were admitted to the main public hospitals in Sana’a between April 2014 and March 2015. Demographic information, medical histories, physical examination findings, and laboratory test results were collected. Endoscopic and pharmacologic treatment was performed. The patients were followed up since admission and up to 6 weeks for the occurrence of rebleeding and mortality after the acute attack. Univariate and multivariate analyses were performed to identify independent risk factors for rebleeding and mortality. Survival analysis was estimated using the Kaplan-Meier method. Result: A total of 102 patients were analyzed. 26 patients (25.5%) rebleeded within 6 weeks period. The predictive factors significantly associated with rebleeding within 6 weeks period in univariate analysis were clot on varix at index endoscope (P - 47.29, P = 0.001) and high serum bilirubin (- 1.19, P = 0.01). Sixteen patients died (15.7%) within 6 weeks period. Predictors of mortality with significant difference in univariate analysis were hypovolemic shock (P = 0.001), high WBCs count (P - 1.41, P - 1.40, P 9/l.

Aim:To determine the tolerance and acceptance of hepatic venous pressure gradient(HVPG)measurements in patients with liver ***:This prospective international multicenter study included 271 patients with cirrhosis who were scheduled to undergo HVPG measurement between October 2019 and June *** related to the tolerance and acceptance of HVPG measurements were collected using descriptive ***:HVPG measurements were technically successful in all 271 patients,with 141(52.0%)undergoing HVPG measurement *** complication rate was 0.4%.Postoperative pain was significantly lower than preoperative expected pain(p<0.001)and intraoperative pain(p<0.001),and intraoperative pain was also significantly lower than preoperative expected pain(p=0.036).No,mild,moderate,severe,and intolerable discomfort scores were reported by 36.9%,44.6%,11.1%,6.3%,and 0.4%of these patients,respectively,during HVPG measurement and by 54.6%32.5%,11.4%,1.5%,and 0%,respectively,after HVPG *** these patients,39.5%had little understanding and 10%had no understanding of the value of HVPG measurement,with 35.1%and 4.1%regarding HVPG measurements as being of little or no help,*** patients reported that they would definitely(15.5%),probably(46.9%),or possibly(29.9%)choose to undergo additional HVPG measurements again,and 62.7%regarded the cost of the procedure as ***:HVPG measurement was safe and well‐tolerated in patients with cirrhosis,but patient education and communication are warranted to improve the acceptance of this procedure.

Prediabetes is a subclinical stage between normal glycemia and diabetes, featured with impaired glucose tolerance, and/or impaired fasting glucose. Despite the absence of diabetes, individuals with prediabetes may experience a higher risk of type 2 diabetes, cardiovascular disease, cancer, and premature mortality. Although low-carbohydrate diet and low-fat diet are beneficial in short-term weight loss and improvement in metabolic risk factors, the associations of these dietary patterns, particularly with different food sources and quality of macronutrients, with mortality remain unclear among people with prediabetes. The present study aimed to prospectively investigate the associations of overall, unhealthy, and healthy lower-carbohydrate-diet and lower-fat-diet scores with all-cause mortality among individuals with prediabetes. Methods This study included 9,793 adults with prediabetes. The dietary intake was measured by 24-hour recalls, and the National Cancer Institute method were used to estimate the usual intake of each macronutrient to correct for measurement bias. The overall, unhealthy, and healthy lower-carbohydrate-diet and lower-fat-diet scores were determined based on the percentage of energy from total and subtypes of carbohydrate, fat, and protein. The generalized linear model was used to examine the associations of lower-carbohydrate-diet and lower-fat-diet scores with cardiometabolic biomarkers at baseline. The Cox proportional hazards regression models were applied to calculate hazard ratios(HRs) and 95% confidence intervals(CIs), and the restricted cubic spline regression with 3 knots(25 th, 50 th, and 75 th) was used to explore the dose-response relationships between diet scores and mortality. Objective:To determine the efficacy of absorbable gelatin sponge in reducing blood loss as well as shortening the length of hospital stay in patients undergoing multilevel posterior lumbar spinal surgery. Background:Absorbable gelatin sponge is reported to decrease postoperative drain output and the length of hospital stay after multilevel posterior cervical spine ***,there is a dearth of literature on prospective study of the efficacy of absorbable gelatin sponge in reducing postoperative blood loss as well as shortening the length of hospital stay in patients undergoing multilevel posterior lumbar spinal surgery. Methods:A total of 82 consecutive patients undergone multilevel posterior lumbar degenerative procedures between June 2011 and June 2012 were prospectively randomized into one of two groups according to whether absorbable gelatin sponge for postoperative blood management was used or *** distribution, total drain output,blood transfusion rate,the length of stay,the number of readmissions,and postoperative complications were ***:Total drain output averaged 173 mL in the study group and 392 mL in the control group(p=0.000).Perioperative allogenic blood transfusion rate were lower in the Gelfoam group(34.1%vs.58.5%,P =0.046);Also length of stay in the patients with the use of absorbable gelatin sponge(2.21 days) was significantly shorter(p=0.000) than the patients in the control group(3.02 days).No patient developed adverse reactions attributable to the absorbable gelatin sponge. Conclusions:Application of absorbable gelatin sponge at the end of multilevel posterior lumbar fusion can significantly decrease postoperative drain output and length of hospital stay.
