Use of machine learning models for the prognostication of liver transplantation: A systematic review

BACKGROUND Liver transplantation(LT)is a life-saving intervention for patients with end-stage liver disease.However,the equitable allocation of scarce donor organs remains a formidable challenge.Prognostic tools are pivotal in identifying the most suitable transplant candidates.Traditionally,scoring systems like the model for end-stage liver disease have been instrumental in this process.Nevertheless,the landscape of prognostication is undergoing a transformation with the integration of machine learning(ML)and artificial intelligence models.AIM To assess the utility of ML models in prognostication for LT,comparing their performance and reliability to established traditional scoring systems.METHODS Following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines,we conducted a thorough and standardized literature search using the PubMed/MEDLINE database.Our search imposed no restrictions on publication year,age,or gender.Exclusion criteria encompassed non-English studies,review articles,case reports,conference papers,studies with missing data,or those exhibiting evident methodological flaws.RESULTS Our search yielded a total of 64 articles,with 23 meeting the inclusion criteria.Among the selected studies,60.8%originated from the United States and China combined.Only one pediatric study met the criteria.Notably,91%of the studies were published within the past five years.ML models consistently demonstrated satisfactory to excellent area under the receiver operating characteristic curve values(ranging from 0.6 to 1)across all studies,surpassing the performance of traditional scoring systems.Random forest exhibited superior predictive capabilities for 90-d mortality following LT,sepsis,and acute kidney injury(AKI).In contrast,gradient boosting excelled in predicting the risk of graft-versus-host disease,pneumonia,and AKI.CONCLUSION This study underscores the potential of ML models in guiding decisions related to allograft allocation and LT,marking a significant evolution in the field of prog

Acute non-variceal upper gastrointestinal bleeding(ANVUGIB)is a common medical emergency in clinical practice.While the incidence has significantly reduced,the mortality rates have not undergone a similar reduction in the last few decades,thus presenting a significant challenge.This editorial outlines the key causes and risk factors of ANVUGIB and explores the current standards and recent updates in risk assessment scoring systems for predicting mortality and endoscopic treatments for achieving hemostasis.Since ANUVGIB predominantly affects the elderly population,the impact of comorbidities may be responsible for the poor outcomes.A thorough drug history is important due to the increasing use of antiplatelet agents and anticoagulants in the elderly.Early risk stratification plays a crucial role in deciding the line of management and predicting mortality.Emerging scoring systems such as the ABC(age,blood tests,co-morbidities)score show promise in predicting mortality and guiding clinical decisions.While conventional endoscopic therapies remain cornerstone approaches,novel techniques like hemostatic powders and over-the-scope clips offer promising alternatives,particularly in cases refractory to traditional modalities.By integrating validated scoring systems and leveraging novel therapeutic modalities,clinicians can enhance patient care and mitigate the substantial morbidity and mortality associated with ANVUGIB.

Based on the El Nino event data sequence from 1854 to 1993, the nature of sequences was de-termined by using statistical normal and independent tests, etc. With the Markov random process and first order auto-regression predictive model, we set up the prognostication mode and give the time limit of the occurrence of next El Nino event, which probably occurs around 2002.The occurring probability for 2001 is 44 %, and it is 61 % for 2002.

The measurement and modification of hypoxia in colorectal cancer:overlooked but not forgotten

Tumour hypoxia is the inevitable consequence of a tumour’s rapid growth and disorganized,inefficient vasculature.The compensatorymechanisms employed by tumours,and indeed the absence of oxygen itself,hinder the ability of all treatmentmodalities.The clinical consequence is poorer overall survival,disease-free survival,and locoregional control.Recognizing this,clinicians have been attenuating the effect of hypoxia,primarily with hypoxicmodification or with hypoxia-activated prodrugs,and notable success has been demonstrated.However,in the case of colorectal cancer(CRC),there is a general paucity of knowledge and evidence surrounding themeasurement andmodification of hypoxia,and this is possibly due to the comparative inaccessibility of such tumours.We specifically review the role of hypoxia in CRC and focus on the current evidence for the existence of hypoxia in CRC,themajority of which originates from indirect positron emission topography imaging with hypoxia selective radiotracers;the evidence correlating CRC hypoxia with poorer oncological outcome,which is largely based on themeasurement of hypoxia inducible factor in correlation with clinical outcome;the evidence of hypoxicmodification in CRC,of which no direct evidence exists,but is reflected in a number of indirectmarkers;the prognostic andmonitoring implications of accurate CRC hypoxia quantification and its potential in the field of precision oncology;and the present and future imaging tools and technologies being developed for themeasurement of CRC hypoxia,including the use of blood-oxygenlevel-dependentmagnetic resonance imaging and diffuse reflectance spectroscopy.

Proactive reconciliation as a tool for integrating mining and milling operations

Historically, reconciliation has been viewed as a quality test of model estimates as well as a powerful tool for detecting and correcting problems in all stages of mine operations from resource estimation to metal production. If used correctly, reconciliation helps to better predict the life of mine(LOM), improves the adherence of production plans to the respective budget, and allows for effective control of the mining and milling processes. However, the accuracy of reconciliation results requires that all input data must be generated in accordance with the principles of sampling correctness. Furthermore, complete reconciliation systems can become extremely complex and must be carefully analyzed in order to provide realistic and helpful conclusions. Based on these concepts, this paper presents a successful proactive reconciliation system applied to a copper and gold mine in Brazil for monitoring and integrating mining and milling operations.

Burkitt Lymphoma Presenting as Ileocolic Intussusception in an Adult

Adult intussusception is rare, highly associated with a malignant lead point, and often requires emergent surgical management. We report the case of a 44-year-old male who presented with generalized abdominal pain and was found to have early ileocolic intussusception secondary to a large ileocecal mass. Biopsies of the mass and an enlarged cardiophrenic lymph node, as well as pleural fluid cytology were all consistent with Burkitt lymphoma (BL). Curiously, the patient’s abdominal exam was reassuring, and the intussusception and malignant bowel obstruction resolved over 36 hours with conservative management alone. With a Burkitt lymphoma international prognostic index (BL-IPI) score of 2, the patient proceeded to treatment with combination chemoimmunotherapy and attained a complete response after four cycles. There was no bowel perforation or recurrent intussusception throughout treatment. Thus, this report marks the first reported case of adult BL-associated intussusception to resolve with non-invasive management and establishes a precedent for conservative management in select patients.

Nomogram using F-18 fluorodeoxyglucose positron emission tomography/computed tomography for preoperative prediction of lymph node metastasis in gastric cancer

BACKGROUND Lymph node(LN)metastasis is an important prognostic factor in patients with gastric cancer(GC).However,the evaluation of LN metastasis status in the preoperative setting is not accurate.Therefore,precise preoperative prediction of LN metastasis status is crucial for optimal treatment in patients with GC.AIM To develop a preoperative nomogram for LN metastasis using F-18 fluorodeoxyglucose(F-18 FDG)positron emission tomography/computed tomography(PET/CT)and preoperative laboratory test findings in GC.METHODS In this study,the data of 566 GC patients who underwent preoperative F-18 FDG PET/CT and subsequent surgical resection were analyzed.The LN metastasis prediction model was developed in the training cohort and validated in the internal validation cohort.Routine preoperative laboratory tests,including albumin and carbohydrate antigen(CA)19-9 were performed in all patients.Univariate and multivariable logistic regression was performed to validate the preoperative predictive indicators for LN metastasis.RESULTS Of the 566 patients,232(41%)had confirmed histopathologic LN metastasis.Univariate logistic regression revealed that the tumor location,blood hemoglobin,serum albumin levels,neutrophil to lymphocyte ratio,platelet to lymphocyte ratio,CA 19-9,maximum standardized uptake value(SUVmax)of the primary tumor(T_SUVmax),and SUVmax of LN(N_SUVmax)were significantly associated with LN metastasis.In multivariate analysis,T_SUVmax(OR=1.08;95%CI:1.02–1.15;P=0.011)and N_SUVmax(OR=1.49;95%CI:1.19–1.97;P=0.002)were found to be significant predictive factors for LNmetastasis.The LN metastasis prediction model using T_SUVmax,N_SUVmax,serum albumin,and CA 19-9 yielded an area under the curve(AUC)of 0.733(95%CI:0.683–0.784,P=0.025)in the training cohort and AUC of 0.756(95%CI:0.678–0.833,P<0.001)in the test cohort.CONCLUSION T_SUVmax and N_SUVmax measured by preoperative F-18 FDG PET/CT are independent predictive factors for LN metastasis in GC.

Multiple myeloma(MM)is an aggressive plasma cell malignancy with high degrees of variability in outcome,some patients experience long remissions,whilst others survive less than two years from diagnosis.Therapy refractoriness and relapse remain challenges in MM management,and there is a need for improved prognostication and targeted therapies to improve overall survival(OS).The past decade has seen a surge in gene expression profiling(GEP)studies which have elucidated the molecular landscape of MM and led to the identification of novel gene signatures that predict OS and outperform current clinical predictors.In this review,we discuss the limitations of current prognostic tools and the emerging role of GEP in diagnostics and in the development of personalised medicine approaches to combat drug resistance.

Clinical decision-making is what all cardiovascular physicians do.The purpose of decision-making is to make the diagnosis,decide on appropriate therapy and estimate the prognosis of the individual patient.Regarding the prognosis,population prognostic data is used but this may or may not apply to the individual patient.LV function,no matter how it is determined seems to correlate best with prognosis.But we need to remember that prognostication of the individual is,at best,an educated guess.

Purpose/Background:Precise estimation of prognostic outcomes of patients with Lip and oral cavity squamous cell carcinoma（LOSCC） is crucial for disease surveillance. However, patients with the same TNM stage sharing heterogeneous survival outcomes is commonly observed. Serum markers have been paid increasing attention during the past few years, due to their simplicity and easy acquirement. Here, we conducted a study to establish nomograms for refining predictions of LOSCC patients’ disease-free survival（DFS） and overall survival（OS） with TNM parameters.
