Objective:To perform a network meta-analysis on four commonly used acupuncture therapies(electroacupuncture,fire acupuncture,warm acupuncture,and filiform needling) and rehabilitation therapy in the treatment of Post-stroke spastic hemiplegia so as to compare the differences in clinical therapeutic effect between acupuncture therapy and rehabilitation therapy as well as among different therapeutic methods of acupuncture.Methods:A computer-based retrieval was conducted in Chinese and English databases,i.e.CNKI,WanFang,VIP,SinoMed,PubMed,Embase,Web of Science and Cochrane library.The search period limit was from the database establishment to April 17,2020.Data analysis was performed through Revman 5.3,Gemtc 0.14.3 and Stata 14.2.Results:A total of 27 trials were finally eligible,including 1880 patients,943 patients of which were in observation group and 937 patients in control group.In terms of the improvement of effective rate,electroacupuncture,fire needling,warm acupuncture and filiform needling methods were all better than rehabilitation therapy,and among which,warm acupuncture was the highest in probability to be the optimal measure.Regarding the improvement in Fugl-Meyer Assessment(FMA) motor function score,fire needling,warm acupuncture and filiform needling methods were all better than rehabilitation therapy,and among which,fire needling method was the highest in probability to be the optimal measure.In view of the improvement of BI score,fire needling method was better than electroacupuncture and filiform needling method,while,warm acupuncture was better than electroacupuncture,thus,fire needling method may be the optimal measure.For the modified Ashworth muscular tension assessment,there was no significant differences in pairwise comparison among different interventions and warm acupuncture was probably the optimal measure.Conclusion:The overall therapeutic effect of 4 acupuncture therapies is better than rehabilitation therapy on Post-stroke spastic hemiplegia,respectively,among which,the therapeutic eff

Objective:To explore the clinical therapeutic effect and the impacts of acupuncture on cerebral blood flow and serum S100β and alkaline phosphatase(ALP) in the patients with Post-stroke mild cognitive impairment in convalescence stage.Methods:A total of 66 cases were randomized into a control group(33 cases) and an observation group(33 cases).The conventional treatment was applied in the control group.On the base of the treatment of the control group,acupuncture was added at Shéntíng(神庭GV24),Bǎihuì(百会GV20),Dàzhuī(大椎GV14),Mìngmén(命门GV4),Shènshū(肾俞BL23),Shénmén(神门HT7) and Xuánzhōng(悬钟GB39) in the observation group.Before and after treatment,separately,Montreal cognitive assessment(MoCA),mini-mental state examination(MMSE) and modified Barthel index(MBI) were assessed in the patients,peak systolic velocity(PSV) and resistance index(RI) and the levels of serum S100β and ALP were measured.At the end of treatment,the clinical therapeutic effect was evaluated in patients of the both groups.Results:After treatment,the scores of MoCA,MMSE and MBI in the observation group were all higher than those in the control group.PSV in the observation group was higher than the control group,while RI,S100β and ALP were lower than those in the control group,indicating the statistical differences(all P Post-stroke mild cognitive impairment in convalescence stage.This therapy improves the cognitive function,the quality of life and cerebral blood flow in the patients and also down-regulates the levels of serum S100β and ALP.Hence,this acupuncture therapy is worthy of clinical promotion.

Efficacy and safety of Di-Tan Decoction for treating Post-stroke neurological disorders:a systematic review and Meta-analysis of randomized clinical trials

The management of Post-stroke complications plays an important role in the quality of life.Di-Tan Decoction(DTD;涤痰汤)is a widely used traditional Chinese medicine.This study incorporated systematic review and meta-analysis to evaluate the efficacy of DTD in Post-stroke neurological disorders.Randomized clinical trials(RCTs)were searched from English,Chinese and Korean electronic medical databases,by including the keywords"Di-Tan Tang","Di-Tan Decoction","Scour Phlegm Decoction","stroke",and"RCT.Each RCT included control(placebo,conventional therapy,or Western medicine)and experimental(DTD treatment)groups.For patients inflicted with stroke for 1-6 weeks,the outcomes of Post-stroke neurological disorders were measured by scales for Post-stroke symptoms and were classified as"completely healed","markedly effective","effective"and"ineffective".Totally,11 RCTs(n=490 controls and n=502 DTD subjects)were selected from 210 articles identified in the initial search.A metaanalysis of evaluation criteria in Post-stroke symptoms revealed that the overall odds ratio(ORs)for alleviating Post-stroke neurological disorders were 0.30-fold lower(95%CI=0.21-0.43)in the DTD group than the control(Western medicine)group(Pstroke diagnostic scale applied(including NFA,HDS,and NIHSS),the overall Post-stroke symptoms determined were less severe in the DTD group(n=219)than the control group(n=217).No adverse effects of DTD were observed in the 11 RCTs reviewed.All 11 studies used an appropriate method for randomization of subjects to evaluate the risk of bias(ROB),and 7 studies included allocation concealment as well as blinding of patients and practitioners.High-risk ROB was included in6 RCTs.No significant publication bias was derived from the funnel plot.Our results indicate that the administration of DTD alone,and DTD in combination with Western medicine,exert greater efficacy for Post-stroke complication therapy,than Western medicine administered alone.More rigorous and regulated studies a

Objective:To evaluate the differences in the clinical effect and the safety in the treatment of Post-stroke anxiety disorder(PSAD)between electroacupuncture(EA)and paroxetine on the base of the heart-brain theory.Methods:A total of 70 patients of PSAD were randomized into an EA group and a medication group,35 cases in each one.In the EA group,four acupoints were selected and divided into two pairs.In the Pair A,there were Baihui(百会GV20)and Fengfu(风府GV16).In the Pair B,there were Xinshu(心俞BL15)and Shenmen(神门HT7).After acupuncture,the electroacupuncture apparatus was applied.The duration of needle retention was 20 min.EA was given three times a week,totally for 4 weeks.In the medication group,paroxetine tablets were administered orally,10 mg daily in the first 4 days and 20 mg afterwards,totally for 4 weeks.Before and after treatment,as well as in the follow-up period,Hamilton anxiety scale(HAMA),Barthel index(BI)for daily life ability,and clinical global impression(CGI)were observed and the clinical therapeutic effect was evaluated separately.Results:A total of 3 cases were dropped-out in the medication group.Finally,35 cases in the EA group and 32 cases in the medication group were included in the analysis.Compared with the scores before treatment,the scores of HAMA and CGI-SI(severity of illness)were lower and the scores of BI were higher after treatment and during the follow-up in either the EA group or the medication group(all P0.05).Compared with the medication group,BI score in the EA group was higher in the follow-up,indicating the statistical significance(P<0.05).After treatment,CGI-SI score was different statistically between the EA group and the medication group(P<0.05).There were 5 cases with adverse reactions in the EA group and 9 cases in the medication group.Conclusions:The therapeutic effect of electroacupuncture on th

Mechanism of the acupoints of the governor vessel in treatment of Post-stroke depression on the base of the specificity of meridian points

Focusing on the effect specificity of meridian points along the running course of meridian,by taking the treatment of Post-stroke depression(PSD)with the acupoints of the governor vessel as an example,the correlation between meridian points and disease was explored.The specific regulatory effect of governor vessel,Baihui(百会GV20)and Shenting(神庭GV24)in physiology and pathology in PSD and the material basis of the specificity effect were analyzed.Finally,the specific relationship between the acupoints of the governor vessel and PSD was explained.

Research progress on the prediction of Post-stroke epilepsy

Epilepsy is a common neurological disease that not only causes difficulties in the work and life activities of patients,but also brings complex social problems.Cerebrovascular disease is currently the main cause of epilepsy in the elderly.With the increased survival rate of patients after stroke,the incidence of epilepsy after stroke has also increased.Effective prediction of epilepsy after stroke is extremely crucial for the prognosis of patients,the initiation of antiepileptic therapy and the reduction of epileptic seizures.In this review,we summarize and compare the current models for the prediction of epilepsy after stroke,including the SeLECT prediction model,Post-stroke Epilepsy Risk Scale(PoSERS),CAVE score,electroencephalogram(EEG)prediction model,and Scandinavian stroke Scale(SSS)score,in order to provide reference for clinical practice and future research.Prediction models can be selected based on the clinical classification of cerebrovascular events.The SeLECT score prognostic model is a better choice for ischemic stroke,especially for the exclusive prediction of mild post stroke epilepsy.The CAVE score model is suitable for intra-cerebral hemorrhage patients.It is simple and offers high correlation between the risk factors and epilepsy.The PoSERS score simultaneously predicts ischemic and hemorrhagic stroke,and is superior to other methods in specificity as well as positive and negative prediction rate.The SSS score,which only measures stroke severity,is not strictly considered as a mature predictor,but it can be used as a first step screening tool.A growing number of large studies are under the way to identify risk factors of poststroke epilepsy(PSE)and to improve the inclusion of predictive indicators.New and advanced findings by EEG recordings may further improve the prediction of PSE.

Clinical Study on Tongyan Spray(通咽喷雾剂) for Post-stroke Dysphagia Patients:A Randomized Controlled Trial

Objective： To observe the effectiveness and safety of Tongyan Spray （通咽喷雾剂） composed of Chinese medicine for Post-stroke dysphagia patients. Method： One hundred and twenty-two Post-stroke dysphagia patients were randomly assigned to the treatment group （61 cases） and the control group （61 cases）. Basic treatment was given to both groups, with Tongyan Spray additionally used in oropharynx for the treatment group, and the placebo used for the control group. After 28-day treatment, the clinical effect and safety were evaluated according to the standard swallowing assessment （SSA） scale. Results： One patient dropped out in each group, and 120 patients reached the final analysis of the study. The total effective rate for the treatment group was 71.7% （43/60）, higher than 46.7% （28/60） in the control group （P〈0.05）, and the improvement on SSA scores of the two groups were significantly different after treatment （P〈0.05）. For grade 1 dysphagia patients （completely depending on nasogastric tube）, the effective rate of the treatment group was 40.9% （9/22）, and 12.5% （2/16） of the control group, without significant difference （P〉0.05）, while the improvement of SSA score was significantly different between the two groups after treatment （P〈0.05）. For grade 2-3 dysphagia patients （oral and nasogastric tube feeding）, the total effective rate of the treatment group was 89.5% （34/38）, higher than 59.1% （26/44） in the control group （P〈0.05）, and also the improvement on SSA scores was significantly different between the two groups after treatment （P〈0.05）. Conclusion： Tongyan Spray was an effective and safe method for Post-stroke dysphagia patients.

Role of Transcranial Direct Current Stimulation in the Management of Post-stroke Depression

Study Objectives: stroke is a major neurological disease significantly threatening the human health and life with a high morbidity, disability, and mortality. Post-stroke depression (PSD) is one of the common complications of stroke. PSD has been a major factor hindering the recovery of neurological functions and daily activities in stroke patients and is closely related to the social avoidance and increased mortality of stroke patients. Although antidepressant drug treatment has improved during the last decades, symptoms in about 20% of the patients are not in remission two years after initiation of pharmacological intervention. Nowadays, non-invasive brain stimulation;techniques;transcranial direct current stimulation (tCDS) has gained an important interest in the treatment of many neuropsychiatric disorders which might be of some help in treatment of PSD. The goal of the current study is to evaluate the beneficial short term role of tDCS on Post-stroke depression. Study Design: This was a randomized double blind placebo-controlled clinical trial included forty hemiparetic patients with Post-stroke depression from outpatient clinic of Faculty of Physical Therapy, Cairo University, in the period between June 2018 to June 2019. All patients diagnosed as having a PSD according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Depression was evaluated by Hamilton Depression Rating Scale (HAM-D) pre and post treatment and quality of life was evaluated by the stroke Specific Quality Of Life scale (SS-QOL). Patients were assigned randomly into two groups: the study group A and the control group B. Both groups received antidepressant medication in addition to physical therapy program. Group A received tCDS for 30 minutes, three times per week for one month, while the group B received sham transcranial direct current stimulation. Post-stroke Epilepsy within a Teaching Hospital in Cotonou, Benin

Seizures occurring after stroke are risk factors for the onset of ensuing epilepsy. However, the incidence of vascular epilepsy is low. The purpose of this study was to study vascular epilepsy features in a teaching university. It was a retrospective, descriptive and analytical study, conducted from 1st January 2015 to 31st December 2016 in Cotonou Teaching Hospital (CNHU-HKM), Benin. It involved 35 patients brought together through comprehensive sampling. Vascular epilepsy hospital attendance rate was 2%. There were simple partial seizures in 74.28% of cases while cerebral ischemia was recorded in 44.44% of patients. Neurological consultation after stroke occurrence was a protective factor against the occurrence of vascular epilepsy with significant p at 0.001.

Post-stroke neuropsychiatric(NP) disorders are common and have complex etiologies.Multiple factors such as lesion location, personality characteristics, socioeconomic status, pre-stroke psychiatric history, and family support are the determining factors in most cases. Although depression and anxiety are more common, emotional incontinence and catastrophic reactions are also frequently observed. Post-stroke depression(PSD)has been associated with an increased risk of stroke recurrence. Despite its serious consequences and high prevalence, PSD remains undetected and untreated. Treatment options of NP disorders depend on severity, presentation at the time of diagnosis, and patient cooperation, and include antidepressants, mood stabilizers, exercise therapy, psychotherapy, and even revascularization. The efficient management of NP disorders improves outcomes, quality of life, and survival rates in Post-stroke patients. However,very few studies have identified definitive treatment for these patients; therefore, further research is required.
