Heavy metals in maternal and cord blood in Beijing and their efficiency of Placental transfer

This study aimed to determine the effect of exposure to heavy metals in pregnant women in Beijing, China. We also evaluated the association of these heavy metals with birth weight and length of newborns. We measured the levels of 10 heavy metals,including lead(Pb), titanium(Ti), manganese(Mn), nickel(Ni), cadmium(Cd), chromium(Cr), antimony(Sb), stannum(Sn), vanadium(V), and arsenic(As), in 156 maternal and cord blood pairs. An inductively coupled plasma mass spectrometry method was used for measurement. Pb, As, Ti, Mn, and Sb showed high detection rates(> 50%) in both maternal and cord blood. Fourteen(9%) mothers had blood Pb levels greater than the United States Center for Disease Control allowable threshold limit for children(50 μg/L).In prenatal exposure to these heavy metals, there was no significant association between any heavy metal and birth weight/length. Moreover, we estimated the Placental transfer efficiency of each heavy metal, and the median Placental transfer efficiency ranged from 49.6%(Ni) to 194%(Mn)(except for Cd and Sn). The level and detection rate of Cd in maternal blood were much higher than that in cord blood, which suggested that Cd had difficulty in passing the Placental barrier. Prospective research should focus on the source and risk of heavy metals in non-occupationally exposed pregnant women in Beijing.

Placental pathologies and fetal outcome in pregnant women with COVID-19: A retrospective study

Objective:To describe histopathologic findings in the placentas in women with coronavirus disease 2019(COVID-19)during pregnancy.Methods:In a cross-sectional study,38 pregnant women with COVID-19 and undergoing delivery between March 2020 and January 2022,were included.The patients had positive polymerase chain reaction(PCR)test for SARS-CoV-2 infection and the placentas after delivery were sent for histopathologic evaluation based on the Amsterdam Placental Workshop Group Consensus Statement and assessed by two pathologists.Results:Our results showed that maternal vascular malperfusion was the most common and was present in 17 cases(44.7%).These features included accelerated villous maturation(36.8%)distal villous hypoplasia(5.3%),Placental infarction(5.3%)and intervillous fibrin deposition(10.5%).Other pathologic findings included focal calcification(10.5%),intravillous congestion and hemorrhage(10.5%),sub-chorionic hemorrhage(5.3%),acute villitis,chronic histiocytic intervillositis and delayed villous maturation each in one case(2.6%).Twelve out of 38 cases showed no significant pathologic changes.Fetal outcomes included neonatal intensive care unit admission rate of 13.2%,dyspnea 31.6%,newborn’s anosmia 7.9%,intrauterine fetal demise 2.6%,asphyxia 2.6%and neonate COVID infection 5.3%.Conclusions:Microvasculopathy,as a sign of maternal vascular malperfusion,is a common finding in placentas from SARS-CoV-2 positive pregnant women in the present study.Further studies with larger sample sizes and comparative studies between COVID-19 positive and negative,as well as information from patient follow-up are suggested.

BACKGROUND Clinically significant portal hypertension(CSPH) and severe portal hypertension(SPH) increase the risk for decompensation and life-threatening complications in liver cirrhosis. Pathologic angiogenesis might contribute to the formation of these conditions. Placental growth factor(PlGF) and Nogo-A protein are biomarkers of pathological angiogenesis, but data on their role in liver cirrhosis and portal hypertension is scarce.AIM To determine plasma levels of PlGF and Nogo-A in patients with liver cirrhosis,CSPH, SPH and potential to predict portal hypertension.METHODS A cohort of 122 patients with hepatitis C virus and/or alcohol-induced liver cirrhosis with characterized hepatic venous pressure gradient(HVPG) were included in the study. Demographic data, medical history, Child-Turcotte-Pugh and Model of End Stage liver disease score, clinical chemistry, liver stiffnessvalues were recorded on the day of the procedure prior HVPG measurement. The degree of portal hypertension was determined by the invasive HVPG measurement. Nogo-A and PlGF plasma levels were evaluated using enzyme linked immunosorbent assay. The control group consisted of 30 healthy age-and sex-matched individuals.RESULTS Peripheral PlGF levels were higher and Nogo-A levels were lower in patients with liver cirrhosis(23.20 vs 9.85;P < 0.0001 and 2.19 vs 3.12;P = 0.004 respectively). There was a positive linear correlation between peripheral levels of PlGF and HVPG(r = 0.338, P = 0.001) and negative linear correlation between the peripheral Nogo-A levels and HVPG(r =-0.267, P = 0.007). PlGF levels were higher in CSPH and SPH(P = 0.006;P < 0.0001) whereas Nogo-A levels were lower(P = 0.01;P < 0.033). Area under the curve for the diagnosis of CSPH for PlGF was 0.68(P = 0.003) and for Nogo-A-0.67(P = 0.01);for SPH 0.714(P <0.0001) and 0.65(P = 0.014) respectively. PlGF levels were higher and Nogo-A levels were lower in patients with esophageal varices(P < 0.05). PlGF cut-off value of 25 pg/mL distinguished patients with CSPH at 55.7% sensitivit

Despite the importance of Placental function in embryonic development,it remains poorly understood and challenging to characterize,primarily due to the lack of non-invasive imaging tools capable of monitoring Placental and foetal oxygenation and perfusion parameters during pregnancy.We developed an optoacoustic tomography approach for real-time imaging through entire ~4 cm cross-sections of pregnant mice.Functional changes in both maternal and embryo regions were studied at different gestation days when subjected to an oxygen breathing challenge and perfusion with indocyanine green.Structural phenotyping of the cross-sectional scans highlighted different internal organs,whereas multi-wavelength acquisitions enabled non-invasive label-free spectroscopic assessment of blood-oxygenation parameters in foeto-Placental regions,rendering a strong correlation with the amount of oxygen administered.Likewise,the Placental function in protecting the embryo from extrinsically administered agents was substantiated.The proposed methodology may potentially further serve as a probing mechanism to appraise embryo development during pregnancy in the clinical setting.

Preeclampsia(PE)remains a leading cause of maternal and perinatal morbidity and mortality in obstetrics worldwide.No effective treatments to reduce its incidence and severity in clinical practice are currently available.A variety of hypotheses have been generated aiming to explain the origins of PE,notably being the genetic predispositions and Placental dysfunction.As regard to Placental dysfunction,much progress has been made in basic research and several potential therapeutic targets have been identified.This review will discuss in detail the potential therapeutic targets in PE models including uteroPlacental blood flow,oxidative stress,vasoactive factors and inflammation/immune response,and introduce the evolving technologies for Placental research nowadays.

Influence of human albumin combined with low-dose heparin on disease condition, serology and Placental blood flow of severe preeclampsia patients

Objective:To study the influence of human albumin combined with low-dose heparin on disease condition, serology and Placental blood flow of severe preeclampsia patients. Methods:A total of 78 patients with severe preeclampsia in our hospital between May 2011 and February 2017 were chosen as study subjects, treatment methods were reviewed and all patients were divided into control group (n=37) who underwent low-dose heparin plus conventional treatment and observation group (n=41) who underwent human albumin combined with low-dose heparin plus conventional treatment, both lasting for 7 d. Before and after treatment, mean arterial pressure (MAP) and 24 h urinary protein were measured, serum markers of disease were measured by enzyme-linked immunosorbent assay (ELISA), and umbilical artery hemodynamics were detected by color Doppler ultrasound.Results:Before treatment, there was no significant difference between the two groups in MAP, 24 h urine protein, serum disease-related indexes or umbilical artery hemodynamic indexes. 7 d after treatment, MAP and 24 h urine protein levels in observation group were lower than those in control group;serum disease-related indexes TGF-β1 and VEGF contents in observation group were higher than those in control group whereas VCAM-1 and sFlt-1 contents were lower than those in control group;umbilical artery hemodynamic indexes PI, RI and S/D levels in observation group were lower than those in control group.Conclusion: Human albumin combined with low-dose heparin can optimize the disease condition, promote the angiogenesis of placenta and increase the blood supply of placenta in patients with severe preeclampsia.

Clinical study in diagnosis of Placental implantation abnormality with transabdominal Doppler ultrasound

Objective: To evaluate the ultrasonographic charateristics and the diagnostic value in Placental implantation abnormality (PIA) with transabdominal Doppler ultrasound, and improve the rate of diagnosis accuracy in the prenatal and postnatal of PIA with transabdominal Doppler ultrasound, for early diagnosis and treatment, to avoid hysterectomy caused by fatal bleeding. Methods: There were 61 cases complicated with PIA by operation and pathologically confirmed in our hospital. 33 cases were prenatal PIA, and 28 cases were postnatal PIA. The sonographic characteristics of PIA were summarized, the sensitivity and false negative rate of transabdominal Doppler ultrasound were analyzed with the results of surgical and pathological diagnosis. Results: In the images of prenatal PIA, the incidence of abundant blood flow signals in muscular layer of the placenta was the highest, which could reached 85.7%, followed by placenta 'vortex' and placenta previa;Placental remnants were major sonographic performance in postnatal PIA, the incidence rate was 100%, followed by abundant blood flow signals in muscular layer of the placenta and disappeared boundary between myometrium and placenta. 14 cases could be diagnosed as prenatal PIA accurately by transabdominal Doppler ultrasound, the sensitivity was 42.4%, and the false negative rate was 57.6%. 21 cases could be diagnosed as postnatal PIA accurately by transabdominal Doppler ultrasound, the sensitivity was 75.0%, and the false negative rate was 25.0%. Conclusions: The specificity was low in prenatal PIA by transabdominal Doppler ultrasound, but the false negative rate was high. If necessary, the diagnostic accuracy could be improved combined with MRI. The specificity was much higher in postnatal PIA, and the false negative rate was low. The ultrasonography of PIA has some certain characteristics, we should strengthen the consciousness of the diagnosis in PIA, especially in the prenatal PIA, and improve the auxiliary diagnosis.

Ultrasonic evaluation of uterine spiral artery characteristics in patients with gestational hypertension and their correlation with the Placental hypoxia injury

Objective: To investigate the ultrasonic evaluation of uterine spiral artery characteristics in patients with gestational hypertension and their correlation with the Placental hypoxia injury. Methods: A total of 128 puerperae with gestational hypertension who gave birth in this hospital between December 2014 and February 2017 were collected as gestational hypertension group, and 100 healthy puerperae who gave birth in this hospital during the same period were collected as normal control group. The differences in uterine spiral artery parameter levels as well as the contents of angiogenesis indexes, oxidative stress indexes and inflammatory mediators in the Placental grinding fluid were compared between the two groups 1 week before delivery. Pearson test was used to evaluate the correlation between uterine spiral artery characteristics and Placental hypoxia injury in patients with gestational hypertension. Results: Uterine spiral artery RI, PI and S/D levels in gestational hypertension group were higher than those in normal control group;angiogenesis indexes HIF-1α, VEGF and sFlt-1 contents in Placental grinding fluid of gestational hypertension group were higher than those of normal control group;anti-oxidation indexes CTA and SOD contents were lower than those of control group whereas LHP content was higher than that of normal control group;inflammatory mediators IL-6, TNF-α, NF-κB and HSP70 contents were higher than those of normal control group. Pearson test showed that the uterine spiral artery RI, PI and S/D levels in puerperae with gestational hypertension were positively correlated with the degree of angiogenesis, oxidative stress and inflammatory response in the placenta. Conclusion: There are abnormal changes of uterine spiral artery flow parameters in puerperae with gestational hypertension, and the abnormality can objectively reflect the degree of Placental hypoxia injury.

Effect of hypoxia on expression of Placental trophoblast cells SATB1 and β-catenin and its correlation with the pathogenesis of preeclampsia

Objective: To study the effect of hypoxia on the expression of Placental trophoblast cells SATB1 and β-catenin and its correlation with the pathogenesis of preeclampsia. Methods: Trophoblastic cell lines HRT8/SVneo were cultured, SATB1 and β-catenin expression and cell biological behavior were determined after hypoxia reoxygenation treatment; cell biological behavior and the expression of related genes were determined after the transfection of SATB1 and β-catenin siR NA; preeclampsia placenta and normal placenta tissues were collected and the expression of SATB1 and β-catenin were determined. Results: OD value, cell migration rate, m RNA contents of SATB1 and β-catenin of H/R group were significantly lower than those of Nor group, cell apoptosis rate was higher than that of Nor group and the number of invasive cells was less than that of Nor group; OD value and bcl-2 mRNA content of SATB1-siRNA group were lower than those of NC group; cell apoptosis rate as well as Bax, Caspase-3, caspase-6 and caspase-9 mRNA contents were higher than those of NC group; cell migration rate as well as CTSB, CTSD, MMP2 and MMP9 mRNA contents of β-catenin-siRNA group were lower than those of NC group; the number of invasive cells was less than that of NC group; the expression levels of SATB1 and β-catenin in preeclampsia placenta tissue were significantly lower than those in normal placenta tissue. Conclusions: Hypoxia can inhibit the expression of SATB1 and β-catenin in the pathogenesis of preeclampsia, which can affect the proliferation, apoptosis, migration and invasion of cells.

Relationship of serum folic acid and vitamin B12 contents with maternal endothelial injury and Placental ischemia hypoxia in patients with preeclampsia

Objective:To investigate the relationship of serum folic acid and vitamin B12 contents with maternal endothelial injury and Placental ischemia hypoxia in patients with preeclampsia. Methods:A total of 76 puerperae with preeclampsia who gave birth in this hospital between March 2016 and October 2017 were selected as the preeclampsia group, and 100 healthy puerperae who gave birth in this hospital during the same period were selected as the normal control group. The differences in serum contents of folic acid, vitamin B12, endothelial injury indexes and oxidative stress indexes were compared between the two groups 24 h before delivery. Pearson test was used to assess the intrinsic relationship of serum folic acid and vitamin B12 contents with the disease severity in puerperae with preeclampsia.Results:Serum folic acid and vitamin B12 contents of preeclampsia group were lower than those of control group;endothelial injury markers sEng, sFlt-1, ET-1 and TM contents were higher than those of control group whereas ADM content was lower than that of control group;oxidative stress indexes AOPPs and LHP contents were higher than those of normal control group whereas T-AOC content was lower than that of control group. The Pearson test showed that serum folic acid and vitamin B12 contents in puerperae with preeclampsia were directly correlated with vascular endothelial injury and Placental ischemia hypoxia.Conclusion: There is folic acid and vitamin B12 deficiency in patients with preeclampsia, and it can aggravate the vascular endothelial injury and Placental ischemia hypoxia.
