Maternal supply of cysteamine alleviates oxidative stress and enhances angiogenesis in porcine Placenta

Background:Oxidative stress in Placenta is associated with the occurrence of adverse pregnancy outcomes in sow,but there are few satisfactory treatment strategies for these conditions.This study investigated the potential of cysteamine(CS)as an antioxidant protectant for regulating the reproductive performance,redox status,and Placental angiogenesis of sows.Methods:The Placental oxidative stress status and vascular density of piglets with different birth weights:Placenta,serum,and colostrum samples of sows or piglets were collected,and the characteristics of sows and piglets were recorded.Furthermore,the in vivo results were validated using porcine vascular endothelial cells(PVECs).Results:Compared with the NBW Placentae,the LBW Placentae showed increased oxidative damage and were vulnerable to angiogenesis impairment.Particularly,H2O2-induced oxidative stress prompted intracellular reactive oxygen species generation and inhibited the tube formation and migration of PVECs as well as the expression of vascular endothelial growth factor-A(VEGF-A)in vitro.However,dietary CS supplementation can alleviate oxidative stress and improve the reproductive performance of sows.Specifically,compared with the control group,dietary 100 mg/kg CS could(1)decrease the stillbirth and invalid rates,and increase both the piglet birth weight in the low yield sows and the Placental efficiency;(2)increase glutathione and reduce malondialdehyde in both the serum and the colostrum of sows;(3)increase the levels of total antioxidant capacity and glutathione in LBW Placentae;(4)increase the vascular density,the mRNA level of VEGF-A,and the immune-staining intensity of platelet endothelial cell adhesion molecule-1 in the LBW Placentae.Furthermore,the in vitro experiment

Steroid hormones,including progestagens,estrogens,androgens,corticosteroids,and their precursor cholesterol,perform essential functions in the successful establishment and maintenance of pregnancy and normal fetal development.As the core endocrine organ at the prenatal stage,the human Placenta is involved in the biosynthesis,metabolism,and delivery of steroid hormones.Steroidogenic pathways are tightly regulated by Placenta-intrinsic cytochrome P450 and hydroxysteroid dehydrogenase.However,the relationship between Placental steroidogenic enzyme expression and adverse pregnancy outcomes is controversial.In this review,we summarize the possible upstream regulatory mechanisms of Placental steroidogenic enzymes in physiologic and pathophysiologic states.We also describe the human Placental barrier model and examine the potential of single-cell sequencing for evaluating the primary functions and cellular origin of steroidogenic enzymes.Finally,we examine the existing evidence for the association between Placental steroidogenic enzyme dysregulation and adverse pregnancy outcomes.

Comparison of cell salvage with one and two suction devices during cesarean section in patients with Placenta previa and/or accrete: a randomized controlled trial

Background:Cell salvage has recently been recommended for obstetric use in cases with a high risk of massive hemorrhage during cesarean section(CS).However,limited data are available to support the use of one suction device to collect lost blood.This study aimed to investigate the volume of red blood cells(RBCs)salvaged and the components of amniotic fluid(AF)in blood salvaged by one suction device or two devices during CS in patients with Placenta previa and/or accrete.Methods:Thirty patients with Placenta previa and/or accrete undergoing elective CS in the Women's Hospital of Zhejiang University School of Medicine were recruited for the present study from November 1,2017 to December 1,2018.The patients were randomly assigned to one of the two groups according to an Excel-generated random number sheet:Group 1(w=15),in which only one suction device was used to aspirate all blood and AF,and Group 2(w=15),in which a second suction device was mainly used to aspirate AF before the delivery of the Placenta.Three samples of blood per patient(pre-wash,post-wash,and post-filtration)were collected to measure AF components.The salvaged RBC volumes were recorded.Continuous data of pre-wash,post-wash,and postfiltration samples were analyzed by using one-way analysis of variance with Tukey5s test for multiple comparisons,or Kruskal-Wallis test with Dunn test for multiple comparisons.Comparisons of continuous data between Group 1 and Group 2 were conducted using Student's t test or Mann-Whitney U test.Results:The salvaged RBC volume was significantly higher in Group 1 than that in Group 2(401.6±77.2 mL vs.330.1土53.3 mL,?=4.175,P<0.001).In both groups,squamous cells,lamellar bodies,and fat were significantly reduced by washing(all P<0.001)and squamous cells were further reduced by filtering(P<0.001).Squamous cells were found in six post-filtration samples(three from each group).Lamellar bodies and fat were completely removed by filtering.Insulin-like growth factor binding protein 1,alphafetoprotein,albumin,lactate dehydrogenase

BACKGROUND Endothelial colony-forming cells(ECFCs)have been implicated in the process of vascularization,which includes vasculogenesis and angiogenesis.Vasculogenesis is a de novo formation of blood vessels,and is an essential physiological process that occurs during embryonic development and tissue regeneration.Angiogenesis is the growth of new capillaries from pre-existing blood vessels,which is observed both prenatally and postnatally.The Placenta is an organ composed of a variety of fetal-derived cells,including ECFCs,and therefore has significant potential as a source of fetal ECFCs for tissue engineering.AIM To investigate the possibility of isolating clonal ECFCs from human early gestation chorionic villi(CV-ECFCs)of the Placenta,and assess their potential for tissue engineering.METHODS The early gestation chorionic villus tissue was dissociated by enzyme digestion.Cells expressing CD31 were selected using magnetic-activated cell sorting,and plated in endothelial-specific growth medium.After 2-3 wks in culture,colonies displaying cobblestone-like morphology were manually picked using cloning cylinders.We characterized CV-ECFCs by flow cytometry,immunophenotyping,tube formation assay,and Dil-Ac-LDL uptake assay.Viral transduction of CVECFCs was performed using a Luciferase/tdTomato-containing lentiviral vector,and transduction efficiency was tested by fluorescent microscopy and flow cytometry.Compatibility of CV-ECFCs with a delivery vehicle was determined using an FDA approved,small intestinal submucosa extracellular matrix scaffold.RESULTS After four passages in 6-8 wks of culture,we obtained a total number of 1.8×107 CV-ECFCs using 100 mg of early gestational chorionic villus tissue.Immunophenotypic analyses by flow cytometry demonstrated that CV-ECFCs highly expressed the endothelial markers CD31,CD144,CD146,CD105,CD309,only partially expressed CD34,and did not express CD45 and CD90.CV-ECFCs were capable of acetylated low-density lipoprotein uptake and tube formation,similar to cord blood-derived ECFCs(

To editor:The disorders associated with Placenta accreta spectrum(PAS)are potentially severe obstetric complications that should be managed by expert centers with substantial re-sources to provide both preoperative and intraoperative as-sessments and then prearrange a multidisciplinary team with an appropriate delivery plan.Placenta accreta spectrum was first described in 1937 as“attempts to remove the pla-centa led to major postpartum hemorrhage that required emergency or secondary hysterectomy to control bleeding.”1 In 1966,Luke et al-classified this condition into Placenta creta(or vera,adherenta),increta,and perceta based on the depth of Placenta villi adhesion or invasion,as determined by pathological features.Although infrequent,there have been significant advances in the global evolution of the strat-egies used to manage PAS,especially over the last decade.

Histo-morphometric vicissitudes in diet-controlled diabetic Placenta:A quantitative stereological model

Background:An affordable approach accepted worldwide to successfully treat mild maternal hyperglycemia in gestational diabetes(GDM),is diet-controlled therapy.As no elaborate research was available,this study was crafted to investigate morphometric stereological details and to determine the mean Placental oxygen diffusion capacity for patients who were kept on diet therapy for mild gestational hyperglycemia.Methods:A clinical trial(NCT04907708)was conducted from January 2018 to February 2019.A total of 70 women completed the study,out of which 35 served as healthy controls as Group A and 35 were diagnosed as mild gestational diabetics according to WHO criteria(Group B).These patients were kept on a restricted calorie diet with light exercise during gestation and were followed regularly during gestation.Soon after delivery,conserved Placentas underwent complete gross,microscopic,and stereological investigations with the point and intersection-counting methods.Results:Significant differences were observed for Placental width and syncytial knots(pPlacental volume,mean Placental components volumes(villi,inter-villous space,fetal capillary,and fetal connective tissue),mean villi and mean fetal capillary diameter,mean villi and capillary surface density and mean morphometric diffusing capacity of Placenta showed non-significant results between the groups.Conclusion:Minimal changes were observed in gross,microscopy,and morphometric stereological details in the Placentae of GDM patients managed with nutritional therapy during gestation compared with the healthy controls.

Maternal Murine Cytomegalovirus Infection during Pregnancy Up-regulates the Gene Expression of Toll-like Receptor 2 and 4 in Placenta

Increasing evidence has revealed that maternal cytomegalovirus （CMV） infection may be associated with neurodevelopmental disorders in offspring. Potential relevance between the Placental inflammation and CMV-related autism has been reported by clinical observation. Meanwhile, abnormal expression of Toll-like receptor 2 （TLR2） and TLR4 in Placenta of patients with chorioamnionitis was observed in multiple studies. IL-6 and IL- 10 are two important maternal inflammatory mediators involved in neurodevelopmental disorders. To investigate whether murine CMV （MCMV） infection causes alterations in Placental IL-6/10 and TLR2/4 levels, we analyzed the dynamic changes in gene expression of TLR2/4 and IL-6/10 in Placentas following acute MCMV infection. Mouse model of acute MCMV infection during pregnancy was created, and pre-pregnant MCMV infected, lipopolysaccharide （LPS）-treated and uninfected mice were used as controls. At E13.5, E 14.5 and E 18.5, Placentas and fetal brains were harvested and mRNA expression levels of Placental TLR2/4 and IL-6/10 were analyzed. The results showed that after acute MCMV infection, the expression levels of Placental TLR2/4 and IL-6 were elevated at E13.5, accompanied by obvious Placental inflammation and reduction of Placenta and fetal brain weights. However, LPS 50 ktg/kg could decrease the IL-6 expression at E13.5 and E14.5. This suggests that acute MCMV infection during pregnancy could up-regulate the gene expression of TLR2/4 in Placental trophoblasts and activate them to produce more pro- inflammatory cytokine IL-6. High dose of LPS stimulation （50 gg/kg） during pregnancy can lead to down-regulation of IL-6 levels in the late stage. Imbalance of IL-6 expression in Placenta might be associated with the neurodevelopmental disorders in progeny.

Water buffalo(Bubalus bubalis)and domestic cattle(Bos taurus)are closely related species.However,embryo transfer interspecies has been attempted without any success.The failure in hybrid embryo-implantation is associated with the glycocode in the maternal-fetal interface.Glycosylation patterns have been studied in different species of ruminants;however,in B.bubalis,only the binucleated cells(BNC)have been analyzed.This glycocode is essential for a successful embryo-implantation and can be defined by Lectin-Histochemistry(LHC).The aim of this study is to compare the glycosylation pattern of Placenta and uterus in water buffaloes and cattle by LHC.Tissue samples of Placenta and uterus from pregnant Mediterranean female water buffaloes(Buf1)and Angus cows(Bov1)were analyzed.All animals were euthanized at 98 days of gestation.LHC was carried out using twelve lectins(Con A,LCA,PSA,sWGA,PHA-e,SBA,UEA-1,WGA,RCA-1,PNA,DBA,BSA-1).The intensity of lectin binding was semiquantitatively scored using a scale of 0(negative)to 3(strongly positive).Difference between species was found in trophoblast layer by PSA,SWGA,PNA and BSA-1,in BNC,and in the mononuclear cells by LCA,PSA,PHA-e,DBA,BSA-1,PNA.In utero,differences in the apical cellular membrane and the secretion of glands were identified by DBA and RCA-1,and in the cytoplasm of those glandular epithelial cells by PHA-e,BSA-1,WGA,and SBA.In both species,BNC presented a strong positive reaction with DBA and SBA,a moderate response by LCA,PHA-e,BSA-1 and PNA lectin,and a low reaction by PSA,UEA-1,SWGA,WGA,Con A and RCA-1.The results found in this study suggest that although both species are closely related,glycosylation patterns in the Placenta and uterus are different,thus providing a possible reason for embryo transfer not being possible between these species.

Application of abdominal aortic balloon occlusion followed by uterine artery embolization for the treatment of pernicious Placenta previa complicated with Placenta accreta during cesarean section

Objective:This study aimed to investigate the clinical effects of abdominal aortic balloon occlusion followed by uterine artery embolization for the treatment of pernicious Placenta previa complicated with Placenta accreta during cesarean section.Methods:We performed a retrospective analysis of the clinical data for 623 patients who experienced pernicious Placenta previa complicated with Placenta accreta and received treatment in our hospital from January 2013 to January 2019.All patients underwent abdominal aortic balloon occlusion before their cesarean section.Seventyeight patients received bilateral uterine artery embolization,and among them,Placenta accreta was found at the opening of the cervix in 13 patients.Due to suturing difficulty after the removal of the Placenta,gauze packing was used to temporarily compress the hemorrhage.As soon as the uterus was sutured,emergent bilateral uterine artery embolization was performed.Active bleeding was noted in the remaining 65 patients when the lower part of the uterus was pressed after the Placenta was removed and the uterus was sutured,therefor,bilateral uterine artery embolization was performed urgently.Results:Of the 623 patients,545 patients underwent only abdominal aortic balloon occlusion and 78 patients underwent additional emergent bilateral uterine artery embolization due to hemorrhaging during or after their cesarean section.No hysterectomies were performed.In the 78 patients,the amount of bleeding was 800-3,200 ml with an average of 1,650 ml during the operation;the volume of blood transfused was 360-1,750 ml(average:960 ml).The fetal fluoroscopy time was 3–8 s(average:5 s).The dose of radiation exposure was(4.2±2.9) m Gy.Fetal appearance,pulse,grimace,activity,and respiration(Apgar) score were normal.No serious complications were observed during or after the operation in the follow-up visits.Conclusion:For patients with pernicious Placenta previa complicated with Placenta accreta who experience active bleeding after cesarean section and abdominal aortic

Uterine Artery Embolization for Management of Primary Postpartum Hemorrhage Associated with Placenta Accreta

Objective To evaluate the efficacy and safety of uterine artery embolization(UAE) in the management of primary postpartum hemorrhage associated with Placenta accreta. Methods We retrospectively reviewed the medical records of patients with Placenta accreta between January 2010 and August 2014. Totally 18 women(mean age 30.8±4.2 years) of primary massive postpartum hemorrhage with diagnosis of Placenta accrete received treatment of UAE after delivery. Images of DSA and medical records were reviewed. Technical success was defined as control of bleeding after embolization. The complications, control of hemorrhage and recurrent bleeding of the Placenta left inside the uterus were retrospectively collected for assessment. Results All patients underwent transcatheter embolization of bilateral uterine arteries. The technical success rate of embolization was 100%. Bleeding was controlled in 17 of 18 patients(94%) during follow-up period(median 18 months, 3-31months) without further bleeding recurred. One patient with Placenta percreta undertook an emergent hysterectomy along with surgical bladder repair after UAE because of persistent uterine bleeding. Eight patients had postembolization syndrome and no other complications occurred. Conclusion Uterine artery embolization is an effective and safe treatment for the management of primary postpartum massive hemorrhage associated with Placenta accreta.
