pillcam Colon 2 capsule in patients unable or unwilling to undergo colonoscopy

AIM: To assess the feasibility, accuracy and acceptability of pillcam Colon 2 in detection of significant lesions in colorectal cancer risk patients, unable or unwilling to perform colonoscopy. METHODS: This is a prospective, single center study using the second generation of pillcam Colon capsule. In all patients the readers were instructed to review the entire colon capsule endoscopy(CCE) examination using Rapid 7 software and additionally to note significant extra-colonic findings. Colonic significant findings were described according to European Society of Gastrointestinal Endoscopy guidelines. CCE procedure completion rate, level of bowel preparation and rate of adverse events were assessed. RESULTS: A total of 70 patients at risk of colorectal cancer were enrolled in the study. In three patients the procedure failed because the capsule was not functioning when entered the colon. pillcam Colon 2 showed positive findings in 23(34%, 95%CI: 21.6%-44.1%) of the remaining 67 patients. Six patients were diagnosedwith tumors: 4 with colon cancers, 1 with gastric cancer and 1 with a small bowel cancer. The capsule findings were confirmed after surgery in all these patients. The capsule excretion rate in twelve hours was 77% with 54 patients having a complete examination. The rectum was not explored during CCE procedure, in 16 patients(23%, 95%CI: 13.7%-34.1%). Every patient accepted CCE as an alternative exploration tool and 65/70(93%) agreed to have another future control by CCE. No complications were reported during or after CCE examination. CONCLUSION: pillcam Colon 2 capsule was effective in detecting significant lesions and might be considered an adequate alternative diagnostic tool in patients unable or unwilling to undergo colonoscopy.

目的通过pillcam SB胶囊内镜取像及IPP专业图像分析软件检测,研究分析小肠的衰老进程,探寻小肠衰老的评估指标。方法将在北京荣祥再生医学研究所进行pillcam SB胶囊内镜检测的115例健康人群作为研究对象,截取其近端空肠典型图像后,用IPP专业图像分析软件测量绒毛长度、绒毛宽度、绒毛密度及黏膜皱褶厚度,计算绒毛长宽比及黏膜指数,并对比分析各项指标与年龄的关系。结果随着年龄的增长,小肠绒毛长度逐渐变短,绒毛宽度逐渐变宽,绒毛密度逐渐变小,黏膜皱襞厚度逐渐变薄,绒毛长宽比值逐渐变小,黏膜指数逐渐变小,其中30~39岁年龄段绒毛长度变化最为明显;30~39岁及70~79岁年龄段绒毛密度变化最为明显;70~79岁年龄段黏膜皱襞厚度变化最为明显;各年龄段黏膜指数变化均较明显。结论随着人体年龄的增长,小肠黏膜的形态结构会随之出现不同程度的退行性改变,进而影响小肠的消化吸收功能。通过对人体小肠衰老状态的评估,可早期了解小肠的衰老程度,以便给予有效的防治措施,为胃肠功能障碍性疾病的预防和治疗提供理论依据。

First pillcam Colon 2 capsule images of Whipple's disease:Case report and review of the literature

Whipple's disease is a rare chronic systemic infection determined by the Gram-positive bacillus Tropheryma whipplei.The infection usually mainly involves the small bowel,but sometimes other organs are affected as well.Since the current standard clinical and biological tests are nonspecific,diagnosis is very difficult and relies on histopathology.Here we present the case of a 52-yearold man with chronic diarrhea and weight loss whose symptoms had been evolving for 2 years and whose diagnosis came unexpectedly after capsule examination.Diagnosis was confirmed by the histopathologic examination of endoscopic biopsy samples,and treatment with co-trimoxazole resulted in remission of symptoms.We present the first images of Whipple's disease obtained with the pillcam Colon 2 video capsule system.

目的通过与全消化道造影检查的对比,探讨pillcam SB胶囊内镜在慢性腹泻疾病中的诊断价值。方法对64例慢性腹泻的患者进行pillcam SB胶囊内镜及全消化道造影检查,并对结果进行对比分析。结果 64例因病因未明确而治疗效果不佳的患者进行胶囊内镜检查后,再接受全消化道造影检查,在胶囊内镜检查中,发现1例Crohn病;3例小肠憩室炎;22例小肠糜烂;7例小肠肿瘤;31例未见病变者。全消化道造影检查中,发现2例小肠糜烂;1例Crohn病;1例小肠憩室炎;7例小肠肿瘤;49例小肠功能紊乱;4例未见病变者。结论胶囊内镜在慢性腹泻疾病诊断中对于小肠黏膜的器质性病变,尤其是黏膜平坦病变的诊断有明显优势。而全消化道造影在肠道形态、肠动力学方面的诊断有一定的价值。

Since the first reports almost ten years ago, wireless capsule endoscopy has gained new f ields of application. Colon capsule endoscopy represents a new diagnostic technology for colonic exploration. Clinical trials have shown that colon capsule endoscopy is feasible, accurate and safe in patients suffering from colonic diseases and might be a valid alternative to conventional colonoscopy in selected cases such as patients refusing conventional colonoscopy or with contraindications to colonoscopy or when colonoscopy is incomplete. Despite the enthusiasm surrounding this new technique, few clinical and randomized controlled trials are to be found in the current literature, leading to heterogeneous or controversial results. Upcoming studies are needed to prove the substantial utility of colon capsule endoscopy for colon cancer screening, especially in a low prevalence of disease population, and for other indications such as inflammatory bowel disease. Possible perspectives are critically analysed and reported in this paper.

Capsule endoscopy compared with conventional colonoscopy for detection of colorectal neoplasms

Colon capsule endoscopy(CCE) may be a means to overcome the low adherence to colorectal cancer screening. The device is an ingestible capsule with a video camera at both ends that can take photographs as it progresses through the gastrointestinal tract.pillcam colon(PCC1) may be used for structural evaluation of the large bowel following an adequate cleaning procedure.PCC1 measures 11 mm×31 mm and has dual cameras that enable the device to acquire video images from both ends with a wide coverage area,automatic light control and a frame rate of four frames per second.The system includes a sensor array and data recorder connected to the patient during the procedure.The recorded data are downloaded to the Given Imaging Rapid workstation for review of the colon video.The second generation of pillcam Colon(PCC2) is similar to PCC1 and incorporates new developments.The angle of view has been increased to 172 degrees.It has an adaptive frame rate,alternating from 35 frames per second while in motion to 4 images when virtually stationary.The new RAPIDsoftware now includes a simple graphic interface tool for polyp size estimation.The procedure of bowel cleansing until capsule ingestion is similar to that used for traditional colonoscopy.However it is more rigorous as the bowel cleanliness for capsule colonoscopy has to be excellent or at least good to result in an adequate sensi-tivity of the method.Briefly,it consists of 3.5-4 L of split dose polyethylene glycol.Oral NaP boosters are administered after 1-2 h if the capsule has entered the small bowel.Sodium phosphate(NaP) seems to be a necessary adjunct to the regimen because the total transit time is doubled without NaP.The cleansing level was considered to be good to excellent in 72%-88%in studies with PCC1.The sensitivity for significant polyps(>6 mm or more than 3 polyps>3 mm) ranged from 63%-88% with specificities between 64%-94%.PCC2 showed an improved sensitivity of 89%and a specificity of 76%. Use of enhancement algorithm to suppress reflections in 3-D reconstructed capsule endoscopy images

In capsule endoscopy(CE), there is research to develop hardware that enables ‘‘real'' three-dimensional(3-D) video. However, it should not be forgotten that ‘‘true'' 3-D requires dual video images. Inclusion of two cameras within the shell of a capsule endoscope though might be unwieldy at present. Therefore, in an attempt to approximate a 3-D reconstruction of the digestive tract surface, a software that recovers information-using gradual variation of shading-from monocular two-dimensional CE images has been proposed. Light reflections on the surface of the digestive tract are still a significant problem. Therefore, a phantom model and simulator has been constructed in an attempt to check the validity of a highlight suppression algorithm. Our results confirm that 3-D representation software performs better with simultaneous application of a highlight reduction algorithm. Furthermore, 3-D representation follows a good approximation of the real distance to the lumen surface.

AIM: To check the usefulness of blue mode (BM) review in lewis score (LS) calculation, by comparing it with respective LS results obtained by white light (WL) small-bowel capsule endoscopy (SBCE) review and mucosal inflammation as reflected by faecal calprotectin (FC) levels, considered as ’gold standard’ for this study. METHODS: Computational analysis of our SBCE database to identify patients who underwent SBCE with pillcam? and had FC measured within a 30-day period from their test. Only patients with prior colonoscopy were included, to exclude any colon pathology-associated FC rise. Each small bowel tertile was reviewed (viewing speed 8 fps) with WL and BM, in a back-to-back mode, by a single experienced reviewer. LS were calculated after each WL and BM reviews. Pearson rank correlation (rho, r) statistic was applied.RESULTS: Twenty-seven (n = 27, 20F/7M) patients were included. Thirteen (n = 13) had SBCE with pillcam?SB1, and the remainder (n = 14) with pillcam? SB2. The median level of FC in this cohort was 125 μg/g. LS (calculated in WL SBCE review) correlation with FC levels was r = 0.490 (P = 0.01), while for BM review and LS correlation with FC was r = 0.472 (P = 0.013).
