physicians’ Instructions to Patients Regarding Medications in a Saudi University Hospital

Objectives: To estimate the proportion of patients who received instructions regarding their medications’ functions, methods of administration, dosages, adverse effects, drug-drug interactions, as well as to identify the sources of knowledge concerning medications’ instructions. Methods: A cross-sectional study was carried out in King Khalid University Hospital (KKUH) in the out-patient pharmacy in 2013. The data collection method includes personal interview with patients who are randomly selected from adults above 18 years of age. The interview was conducted among patients and any person who attends the consultation. Results: The sample was 274 patients. Patients who received instructions for drugs’ functions 208 (75.9%), method of administration 229 (83.6%), doses of drugs 220 (80.3%), drugs’ adverse effects 47 (17.1%), and drug-drug interactions 41 (15%). Sources of medications’ instructions were physicians (73.6%), pharmacists (42.3%), patient information leaflets (PILs) (40.5%) and family or friends (12.8%). Conclusion: The provided instructions about prescribed medications to patients in KKUH were incomplete that may lead to therapeutic failure.

Assessment of physicians’ Knowledge, Attitude and Practices of Radiation Safety at Suez Canal University Hospital, Egypt

Doctors who request imaging must be well trained in deciding whether diagnostic imaging is indicated and have an accurate knowledge of the associated risks. Although radiological doses are low and the chance of late effect is minimal, it should be kept as low as reasonably achievable. This cannot be achieved without a proper knowledge and adherence to safe practices. This cross-sectional study investigates the level of physicians’ knowledge about radiation safety and their attitude towards radiation protection. A self-administered questionnaire, for radiation safety was sent to a purposive sample of 120 physicians at Suez Canal University Hospital. Eighty questionnaires were filled by participants (response rate;66.7%). The sample included 22 radiologists, 15 oncologists, 25 surgeons and 18 orthopedists. Most participants did not receive any radiation safety-related training (88.8%). Radiologists and oncologists were exposed to ionizing radiation more frequently;however, their knowledge was as low as that of other physicians. The overall knowledge score ranged from 40% - 60% (mean;56.5 ± 15.2), with a low score among surgeons and orthopedics. The most deficient knowledge was in the dose of background radiation and the radiation dose received by patients in each type of radiation procedure. Adherence to safe radiation practices was violated by most of participants, especially surgeons and orthopedics, but they attributed it to the poor applicability of the protective measures during performing the procedures. This study concluded that physicians at the Suez Canal University Hospital had deficient knowledge, unsafe practices and negative attitude towards radiation safety policies & precautions.

What factors affect physicians’ decisions to use an e-health care system?

Prior studies have not explored physician’s attitudes toward, and behavior and willingness to accept an e-health care system. However, physicians can induce demand for their services. The development of the high-tech asthma care mobile service (ACMS) in Taiwan provided a means of exploring key factors in a physician’s choice of using an ACMS. The study was based on the technology acceptance model (TAM) and integrated “subjective norm,” “innovativeness,” and “managerial support” to understand and predict physicians’ attitudes and behavioral intentions toward adopting high-tech healthcare systems such as the ACMS. Of 700 questionnaires distributed to physicians with experience using ACMS, 504 completed returns were received (a 72% response rate). The data were analyzed using the structural equation modeling (SEM) method. The results of the study showed that the model selected to explain and predict utilization of the ACMS had high explanatory power and was a good-fit model. The most critical factor that affected behavioral intentions related to ACMS was user attitude, followed by perceived usefulness, managerial support, subjective norm, perceived ease of use, and innovativeness.

Residents Achieve a High Patient Satisfaction in Pre-Anesthetic Patient Assessment. An Observational Study

A physician’s empathetic attitude is regarded as an important influencing factor towards patient satisfaction. However, physicians’ self-assessed empathy declines during medical school and through residency, which may limit patient satisfaction in the doctor-patient contact. In conse-quence, we hypothesized that residents might fail in generating high levels of patient satisfaction. We conducted an observational study in the setting of a pre-anesthetic assessment center, Univer-sity Hospital Leipzig, Germany. Patient satisfaction was measured using the overall ZUF-8 score (8 - 32 points;increasing with patient satisfaction). Data were collected by questionnaires (ZUF-8 items, additional satisfaction items, and items of organizational and logistical processes). Key items for patient satisfaction as primary endpoints were defined. Statistical evaluation was performed in dependency of the anesthesiologists’ level of training (residents vs. consultants) by using the Mann-Whitney-U test or the chi square test, as required. Statistical significance was accepted for p < 0.05. In total, 2565 patients were assessed for eligibility. 986 patients met the criteria of inclusion. 978 questionnaires were submitted with 517 questionnaires being excluded because of missing values. Finally, 461 questionnaires could be analysed. Both residents and consultants achieved a high level of patient satisfaction. The ZUF-8-scores were comparable between the groups (p = 0.91), whilst the quality of consultation was rated slightly better by patients being assessed by residents (p = 0.047). The results in additional satisfaction items were comparable between the groups. We found a trend in residents assessing to be rated more friendly. Residents are able to generate a high level of patient satisfaction which is comparable to that of consultants with scores ranging between “good” and “very good”.
