Giant Cell Tumor of the Proximal phalanx of the Little Finger about One Case in Our Milieu

Giant cell tumor is a benign primary bone tumor. The phalangeal localization is rare. It is distinguished from other locations by the local aggressiveness of the tumor and a high rate of recurrence. We report one case of a 25-year-old right-handed nurse with no reported pathological history who complained about an ulcero-budding mass of the right little finger evolving one year ago. She would have initially consulted a bonesetter who would have taken care of her by using decoctions. The hand X-ray showed an osteolytic tumor of the first phalanx of the right little finger with a “honeycomb” appearance invading the soft tissues. We carried out the amputation of the radius. The postoperative course was simple with healing of the surgical wound and disappearance of the axillary adenopathy. The surgical specimen after anatomopathology exam concluded to be a grade 2 giant cell tumor of Senerkin.

Osteoid Osteoma of Proximal phalanx of the Index Finger of the Right Hand

Osteoid osteoma is a benign bone forming tumor of the growing skeleton that is most often seen in young men. It represents by pain and radiologic appearance of anidus surrounded by osteosclerosisthat occurs mostly in long bones of the lower extremity. Occurrence of this tumor in the hand is an uncommon condition. We present a 23-year-old man with an osteoid osteoma in the proximal phalanx of the right index finger. The radiological examination showed a lytic lesion with irregular border with a central sclerotic nidus and also sclerotic changes around the lytic lesion and bone expansion in the proximal phalanx of the right index finger. He was suffering from a history of pain and swelling of his finger since 8 months. The pain disappeared after surgical excision of nidus. The patient was pain-free and there was no evidence of recurrence at 15 months follow up.

BACKGROUND Dedifferentiated chondrosarcoma(DDCS)accounts for 10%of all chondrosarcomas and has the poorest outcome,with a 5-year survival rate of 7%-25%.DDCS commonly occurs in the femur and pelvis,whereas DDCS of the finger is extremely ***,the histological findings of preexisting solitary enchondroma samples are important and valuable for diagnosing malignant *** SUMMARY We report our experience with DDCS in the proximal phalanx of the left middle finger of an 87-year-old *** had undergone surgery for enchondroma,with curettage and artificial bone grafting,11 years ago,in the same *** years after the primary surgery,the left middle finger gradually started to enlarge,and the growth speed increased in the past *** radiographs showed an expansive osteolytic lesion with calcifications and residual grafting *** to the suspicion of malignancy,we performed ray *** findings revealed an abrupt transition between the low-grade chondrosarcoma and dedifferentiated sarcoma *** dedifferentiated components showed the features of a high-grade undifferentiated pleomorphic *** patient was diagnosed with DDCS arising from a preexisting *** had no local recurrence or distant metastasis and died of pneumonia 6 years and 10 months after the second *** The histological findings of a precursor lesion showed a typical enchondroma,suggesting that DDCS can arise from enchondroma.

Anatomical and Histological Study of the Foot of Iraqi Endogenous Buffaloes （Bubalus bubalis）

The anatomical and histological study were conducted on 72 specimens of buffaloes foot, regardless of the age and species The foot structures were described which included first phalanx, second phalanx, third phalanx, digital cushion, joint, ligaments and tendons, as well as the foot＇s sole, blood and nerve supply. The main source of blood supply is the median artery, the radial artery, dorsal metacarpal artery. On the other hand, the metacarpophalangeal joint, proximal interphalangeal joint and joint＇s capsules were shown by radiographic pictures. The median nerve is the main source of the nerve supply of foot, as well as ulnar and radial nerve. However, the digital tendons on the dorsal surface and palmer surface were represented by common, proper flexor digital tendons and deep superficial digital extensor tendons respectively, the histological picture of coronary region revealed that it consistd of horn tubules and intertubular horn. While the lamellae region showed that it consisted of primary epidermal lamellae, containing the epidermis and dermis regions, also, noticed that the epidermal lamellae was interfered with faced dermis lamellar. The histological structure of the hoof revealed that it consisted of keratinocytes which connected with each other by desmosal adhesion. Digital cushion contained of elastic and collagen fibers with presence of adipose cells masses greater than that at foot sole.

Traumatic Floating Bone. About 11 Cases and a Literature Review

Introduction: Floating bone injury or bipolar dislocation is a concurrent dislocation at both ends of a long bone or a flat bone. It is an exceptional injury. We report 11 consecutives cases of floating bones. Objectives: The aim of the study is to analyze the frequency, the mechanism of injury and clinical forms of floating bones, and to present the results of their management. Material and Methods: A ten years ongoing retrospective study was held in four trauma departments. Eleven cases of floating bones were identified. The floating bones characteristics such as the injured bone, the patient age and sex, the mechanism of injury, the dislocations directions, the associated complications, the treatment and the outcome were studied. Results: Nine male and two female patients, with a mean age of 37 years [19 - 65 years range] presented a concurrent bipolar dislocation of one bone or a group of bones. They sustained a road traffic accident (n = 5), a workplace accident (n = 3), a fall from height (n = 2), and a sport accident (n = 1). The clavicle (n = 3) and the first metatarsal (n = 3) were the most frequently involved. The others floating bones were the radius-ulna complex (n = 1), the radius-lunatum complex (n = 1), the first metacarpal (n = 1), the first phalanx (n = 1) and the femur (n = 1). The floating bones ends displacement occurred in the sagittal plane (the forearm, the femur, the first phalanx and the first metatarsal) and in the horizontal plane (the clavicle, the first metatarsal and the first metacarpal). We defined direction displacements as bidirectional asymmetric (n = 10) or unidirectional symmetric (n = 1). Associated complications were fractures (wrist, hip, tarso-metatarsal joints) and wounds (fingers, metatarso-phalangeal joint). Dislocations were treated conservatively (n = 5) and surgically (n = 17) with excellent results (n = 13/16 joints). Three patients were lost of view.
