Effect of pediatric tuina on children's recurrent acute respiratory tract infections: a retrospective cohort study in Southern China

OBJECTIVE: To determine the effectiveness of pediatric tuina(PT) in preventing recurrent acute respiratory tract infections(ARTIs) in children. METHODS: This is a retrospective cohort study based on the electronic medical records of children with recurrent ARTIs in 2016. Children were divided into a PT group or a non-PT group, according to whether they had received PT or not in 2016. The primary outcome was the number of ARTI episodes in 2017 and 2018. The secondary outcomes were the number of ARTIs leading to outpatient department visits and outpatient antibiotic prescriptions due to ARTIs in the same time period. Negative binomial regressions were used to detect the association between PT and the outcomes. RESULTS: A total of 2303 children were included in the analysis, including 94 in the PT group and 2209 in the non-PT group. Children who received PT six or more times in 2016 had fewer episodes of ARTIs in 2017 [incidence rate ratio(IRR): 0.59, 95% confidence interval(CI)(0.42-0.84)] and 2018 [IRR: 0.58, 95% CI(0.36-0.94)] and fewer outpatient department visits due to ARTIs in 2017 [IRR: 0.56, 95% CI(0.38-0.83)] than children who had not received PT in 2016. There was no significant difference in the number of outpatient antibiotic prescriptions between the two groups. CONCLUSIONS: Receiving PT six or more times within one year is associated with a decrease in recurrent ARTIs in children in the following two years. Randomized controlled trials are needed for effect evaluation prior to establishing PT as a method for preventing recurrent ARTIs among children.

Effect of pediatric tuina on hypothalamic metabolites in young rabbits using liquid chromatography-mass spectrometry

Objective: To investigate the changes in the hypothalamic metabolites of lipopolysaccharide(LPS) febrile young rabbits after the treatment with pediatric tuina.Methods: A total of 30 young rabbits were randomly assigned into three groups: the normal group, the model group, and the tuina group. Both the model group and the tuina group were injected intravenously with LPS. “Six antipyretic manipulations”(pushing Tianmen, pushing Kangong, kneading Taiyang,kneading Erhougaogu, clearing Tianheshui, and pushing Jizhu) were administered 1 h after the LPS injection in the tuina group. The rectal temperatures of the young rabbits were monitored during the experiment to explore the antipyretic effect. Three hours after the injection, the content of interleukin-1β(IL-1β), tumor necrosis factor(TNF)-a, and prostaglandin E;(PGE;) in the serum was detected. In addition, liquid chromatography-mass spectrometry(LC-MS) was used for the hypothalamus metabolomics.Results: Compared with the model group, the rectal temperature of the tuina group was decreased at 2 h and 3 h after the LPS injection(P =.04, P =.03, respectively), and the content of IL-1β, TNF-a, and PGE2was decreased(P =.03, P =.003, and P =.008, respectively). The metabolomics results showed that there were 23 potential biomarkers after the tuina intervention, enriching 27 pathways. Lipid metabolites,especially glycerophospholipids, were a majority of the altered metabolites. The primary metabolic pathways affected by tuina included the arachidonic acid metabolism, the GABAergic synapse, D-glutamine and D-glutamate metabolism, and the glutamatergic synapse.Conclusion: pediatric tuina reduced the temperature of the febrile rabbits and downregulated the expression of IL-1β, TNF-a, and PGE2, and the antipyretic mechanism may be related to changes in metabolites and metabolic pathways in the hypothalamus.

A good sleep is essential for the health and development for all persons and in special for kids,that are more vulnerable to the effects of not having a good sleep.The sleep problems in children are very common,and many parents do not recognize it as a sleep problem.The most common are resistance to going to bed and difficulties falling asleep,followed by bruxism,sleep talking and snoring,as well as difficulty getting out of bed in the morning and early morning fatigue.The incidence of this problems in daily life for children and parents is considerable and require a professional guidelines and treatment.Objective:This study was oriented to the implementation of an intervention plan with pediatric tuina in the framework of kindergarten,to observe the effects of pediatric tuina on children with sleep disorders.The intervention method was focused on teaching and guiding parents to do a pediatric tuina protocol to solve their children's sleep problems.Method:From different Kindergarten of the Nanjing City,62 children were selected according to the evaluation of sleep disorder with CSHQ questionnaire.The enrolled children were randomly divided in two groups,tuina group and control group;both groups participate in a lecture about routine and life style to relieve sleep problems;in addition,in the tuina group the parents performed a 4-week course protocol.The control group only received the lecture.The questionnaire was done before and after the intervention.Results:The results showed that the total scores of the questionnaire in tuina group decreased significantly after intervention(p=0.000),while in the control group the differences where no significative(P=0.292).The analysis confirmed that 84%of the cases in the tuina group had a relevant improvement in the signs of sleep problems,confirming the effectiveness of the intervention with tuina.Conclusion:The pediatric tuina performed in a homecare form under professional guide is effective in solving sleep disorders in children;this kind of intervention is significant

A female patient,13 months old,suffering from limited extension of the right thumb for 7 days,diagnosed as stenosing tenosynovitis of the thumb in children.tuina(Chinese massage) therapy was adopted,in which,stimulation was applied with fingers instead of needles,including pressing and kneading,twisting and kneading,plucking,dorsal extending,rotating as well as pushing and rubbing manipulations.The whole procedure of manipulation lasted around 15 min,once every two days,3 times weekly.Meanwhile,the parents were advised to bath the child patient's right hand into warm water every day and fix the affected thumb with splint at night.After 12 treatments with tuina therapy,the flexion and extension function of the affected thumb were returned to normal and the thumb motor function was recovered.In 2 months of follow-up,the thumb motor function was normal and the stenosing tenosynovitis was not recurred.In this case,tuina therapy is adopted in treatment of stenosing tenosynovitis of the thumb in children.As an alternative method of surgery,tuina therapy has a satisfactory clinical effect.

The article introduced the application of tuina at"Simingxue"(four points named after"ming")and pressing therapy at auricular points in treatment of 17 cases of anisometropic amblyopia in children.The methods:pressing and kneading with the thumb at Jingming(睛明BL1),Shangming(上明Extra),Yiming(翳明EX-HN14)and Guangming(光明GB37).Pressing with the finger digital at Yintang(印堂EX-HN3),Cuanzhu(攒竹BL2),Yuyao(鱼腰EX-HN4),Taiyang(太阳EX-HN5),Yingbai(阳白GB14),Sizhukong(丝竹空TE23),Tongziliao(瞳子髎GB1),Qiuhou(球后EX-HN7)and Chengqi(承泣ST1).Scraping the upper and lower orbits,nipping the eyebrows,pushing the orbits,pushing baoping and warming the eyes.Pressing Quchi(曲池LI11),Hegu(合谷LI4),GB37,Zusanli(足三里ST36),Xuehai(血海SP10)and Sanyinjiao(三阴交SP6).Rolling on the bilateral distributions of bladder meridian on the back.Pressing and kneading at Touwei(头维ST8),Qucha(曲差BL4),Baihui(百会GV20),Sishencong(四神聪EX-HN1),Fengchi(风池GB20)and Dazhui(大椎GV14).Grasping five meridians,scattering bilateral temporal regions,grasping and kneading the neck and nape region and holding Jianjing(肩井GB21).Auricular pressing therapy was exerted at Yan(眼eye,LO5),Gan(肝liver,CO12),Shen(肾kidney,CO10),Pi(脾spleen,CO13),Wei(胃stomach,CO4),Pizhixia(皮质下subcortex,AT4),Pingjianqian(屏间前anterior intertragic notch,TG21)and Pingjianhou屏间后(posterior intertragic notch,AT11).The treatment was given 3 times a week,6 treatments made one course and 4 courses of treatment were required.Of 17 patients,2 cases were cured,13 cases cured basically,1 case improved and 1 case failed.The total effective rate was 88.2%.
