Nocturnal hypertension and riser Pattern are associated with heart failure rehospitalization in patients with heart failure with preserved ejection fraction

BACKGROUND Nocturnal hypertension is reported as a risk factor for cardiovascular disease.This study aimed to explore the potential association between nocturnal hypertension and heart failure(HF)rehospitalization in patients with HF with preserved ejection fraction(HFpEF).METHODS A total of 538 patients with HFpEF from May 2018 to December 2021 were consequently recruited in this study and followed up until they were readmitted for HF or the end of this study.Cox regression analysis was used to reveal the potential association between nighttime blood pressure(BP)levels,nocturnal hypertension and nocturnal BP Patterns and HF rehospitalization.Kaplan-Meier curve was used to assess the cumulative event-free survival rate between groups.RESULTS There were 537 patients with HFpEF were included in the final analysis.The mean age of the study population was 77.14±8.68 years,and 41.2% of patients were men.After a median follow-up duration of 10.93(4.19–21.13)months,176 patients(32.7%)with HFpEF were readmitted for HF.Cox regression analysis had revealed that nighttime systolic BP level[hazards ratio(HR)=1.018,95%CI:1.008–1.028,P=0.001],nighttime diastolic BP level(HR=1.024,95%CI:1.007–1.042,P=0.007),nocturnal hypertension(HR=1.688,95%CI:1.229–2.317,P=0.001)were associated with HF rehospitalization.Kaplan-Meier analysis had demonstrated that patients with nocturnal hypertension had significantly lower event-free survival rate(log-rank PPattern had a higher risk of HF rehospitalization(HR=1.828,95%CI:1.055–3.166,P=0.031)and lower eventfree survival rate(log-rank P=0.003)than those with a dipper Pattern.These findings were also confirmed in patients with HFpEF and hyperuricemia.CONCLUSIONS Nighttime BP levels,nocturnal hypertension and a riser Pattern are independently associated with HF rehospitalization in patients with HFpEF,and prominently in patients with HFpEF and hyperuricemia.Well controlled nighttime BP levels should be emphasized and considered in patients with HFpEF.

A novel method for small and unbalanced sample Pattern recognition of gas insulated switchgear partial discharge using an auxiliary classifier generative adversarial network

Convolutional neural networks(CNNs)have been widely used for gas-insulated switchgear(GIS)partial discharge(PD)Pattern recognition due to their powerful feature extraction ability.However,there is commonly a scarcity of fault samples due to low insulation failure rate of GIS equipment,which degrades the diagnostic performance of these CNN networks when directly applied to small and unbalanced datasets.Therefore,we propose a novel auxiliary classifier generative adversarial network for GIS PD Pattern recognition for small and unbalanced samples.First,we propose using synchrosqueezed wavelet transform to extract time-frequency characteristics of PD pulses and obtain a time-frequency image with high energy aggregation and timefrequency distribution rate.Then,we propose an improved generative adversarial network with an auxiliary classier and self-attention mechanism,which can generate highquality PD samples for situations with few classes.Experiments show that our proposed method can reach 95.75%recognition accuracy for small datasets,which is the highest among several comparable methods.Furthermore,the proposed method has excellent and stable recognition performance for various unbalanced datasets.

Locomotion in mollusc Aplysia is implemented by a pedal rolling wave,a type of axial locomotion.Well-studied examples of axial locomotion(pedal waves in Drosophila larvae and body waves in leech,lamprey,and fish)are generated in a segmented nervous system via activation of multiple coupled central Pattern generators(CPGs).Pedal waves in molluscs,however,are generated by a single pedal ganglion,and it is unknown whether there are single or multiple CPGs that generate rhythmic activity and phase shifts between different body parts.During locomotion in intact Aplysia,bursting activity in the parapedal commissural nerve(PPCN)was found to occur during tail contraction.A cluster of 20 to 30 P1 root neurons(P1Ns)on the ventral surface of the pedal ganglion,active during the pedal wave,were identified.Computational cluster analysis revealed that there are 2 phases to the motor program:phase I(centered around 168°)and phase II(centered around 357°).PPCN activity occurs during phase II.The majority of P1Ns are motoneurons.Coactive P1Ns tend to be electrically coupled.Two classes of pedal interneurons(PIs)were characterized.Class 1(PI1 and PI2)is active during phase I.Their axons make a loop within the pedal ganglion and contribute to locomotor Pattern generation.They are electrically coupled to P1Ns that fire during phase I.Class 2(PI3)is active during phase II and innervates the contralateral pedal ganglion.PI3 may contribute to bilateral coordination.Overall,our findings support the idea that Aplysia pedal waves are generated by a single CPG.

Incidence and Pattern of traumatic spine injury in a single level I trauma center of southern Iran

Purpose:Spine injury is one of the leading causes of death and mortality worldwide.The objective of this study was to determine the incidence,Pattern and outcome of trauma patients with spine injury referred to the largest trauma center in southern Iran during the last 3 years.Methods:This is a cross-sectional study conducted between March 2018 and June 2021 in the largest trauma center in the southern Iran.The data collection form included the age,sex,injury location(cervical,thoracic,and lumbar),cause of injury(traffic accidents,falls,and assaults),length of hospital stay,injured segment of spine injury,severity of injury,and outcome.Statistical analyzes were performed using SPSS software version 24.Results:Totally 776 cases of spine injury were identified.The spine injury rate was 17.0%,and the mortality rate was 15.5%.Cervical spine injury(20.4%)more often occulted in motorcycle accident,and thoracic spine injury(20.1%)occulted in falls.The highest and lowest rates of spine injurys were related to lumbar spine injury(30.2%)and cervical spine injury(21.5%),respectively.There was a statistically significant relationship between the mechanism of injury and the location of spine injury(pPattern of spine injury may assist to prevent the missing diagnosis of spine injury in multiple trauma patients.

Effects of coke charging Pattern on burden movement in COREX melter gasifier based on physical simulation

A three-dimensional semicircle cold model of COREX melter gasifier(MG)was established,and the effects of coke charging Pattern on burden movement were investigated by adopting this model.The burden flow Pattern of packed bed was measured with different coke charging amounts,coke charging positions,and coke sizes.The results show that the solid flow Pattern presents a change from straight line type to reverse U type and finally to W type in MG with traditional coke charging,while the burden flow Pattern experiences the change from straight line type to W type with platform and finally W type under center coke charging,and the solid flow Pattern presents a change from straight line type to W type with platform and finally W type under intermediate coke charging.The difference demonstrates that coke charging Pattern affects the uniform descending in MG to some degree.The burden in the coke charging zone keeps a straight line descending.No matter what kind of coke charging Pattern is adopted,the burden residence time is shortened with the increase in coke charging amount,and the vertex of deadman decreases obviously,which is conducive to the renewal of the deadman.The appropriate coke charging amount and coke size should be well controlled at 8.7%and 5 mm,respectively.The results are useful not only in developing further understanding of solid flow in MG but also in verifying different mathematical models,particularly the coupled discrete element method with computational fluid dynamics simulation which has been increasingly used in the literature.

To the Editor:Ankle sprains are some of the most common emergency injuries from trauma.Due to the particularity of its anatomy,most sprains of the ankle are due to injuries of the lateral collateral ligaments,of which 80%are tears or ruptures of the anterior talofibular ligament(ATFL).[1]If the treatment is not appropriate,the injury will affect the patient’s work and life in the long term,and the patient can even develop chronic ankle instability(CAI),increasing the treatment duration and resulting inhigher economic burdens on the patients and themedical insurance system.Regarding the clinical epidemiology of ATFL injuries,at present,most research is focused on the exploration of the incidence,pathogenic causes,and treatment methods,but not enough attention has been given to individualized factors and the associated injuries.In addition,the guidance effect of the findings of these studies on clinical practice is limited.To the best of our knowledge,there have been no clinical epidemiological studies on ATFL injuries in ward patients in China.Therefore,this study retrospectively analyzed the clinical data of all patients with ATFL injuries in the Sports Medicine Center of the Southwest Hospital ofArmy Military Medical University from July 2007 to December 2020.The goal was to understand the characteristics and Pattern distributions of ATFL injuries to provide a basis for their prevention,early diagnosis,and treatment.This study was approved by the Ethics Committee of The First Affiliated Hospital of Army Medical University(KY 2021026).

1.Introduction Microstructures are at the centre of materials science and engineering and play an essential role in determining the in-service mechanical properties of metallic alloys[1–4].Extensive theoretical and experimental investigations have been undertaken to describe and determine solidification microstructures as a function of solidification conditions.The seaweed Pattern is one of the most important classes of microstructural Patterns[5–7].The characteristic of this Pattern is that its extending tips—unlike the behaviour observed in needle-like dendrites—split repeatedly into two or more parts.
