Objective: To investigate the diagnosis and treatment for recurrent dysphagia of esophageal carcinoma after radical radiotherapy. Methods: The experince of 41 cases with recurrent dysphagia of esophageal carcinoma after radical radiotherapy were retrospectively summerized. Results: All of the cases were surgically treated form July, 1989 to May, 1997 with resectability rate in 97%, operative mortality in 4.8%, postoperative morbidity in 29%. Pathological examination showed squemous cell carcinoma in 82.8%, and serious radiation reaction in 13.2% after operation. The 1, 3, 5year survival rates after operation were 72.7%, 36.3%, 18.1% respectively, and those after radiation in 100%, 60%, and 35.5% respectively. Conclusion: Most of recurrent dysphagia for these cases were due to uncontrolled/recurrent cancer, and minority of them were due to constriction after radiotherapy. It is difficult to distinguish cancer with benigh constriction before operation. Some cases have lymphatic metastasis without local cancer recurrence. For those patients with recurrent disphagia should be treated with operation as early as possible.

Clinicopathologic Analysis of 2,889 Nanchang-Area Patients with Colorectal Polyps

OBJECTIVE To study the clinicopathologic characteristics, changes in the nature and incidence of colorectal polyps in the Nanchang area, Jiangxi, Province. METHODS We retrospectively investigated the patients with colorectal polyps who were diagnosed by colonoscopy and pathology.in our hospital from 1990 to 2004. The analysis involved the incidence, average patient age, polyp location and pathological types. We recorded the changes of the polyp clinicopathologic features by comparing the clinicopathologic types of colorectal polyps over fi ve-year periods. RESULTS Of the 21,853 patients who received a colonoscopy, 2,889(13.2%) were diagnosed with colorectal polyps. Their average age was 46.6±16.5 years, with a male to female ratio of 1.8:1. The males were older than females (47.1±17.5 vs. 45.5±14.5, P<0.05). Location of the polyps: 41% in the rectum, 27.7% in the sigmoid colon, 35.8% in the left side verus 23.1% in the right side (P<0.05). Patients with polyps located in the transverse and ascending colon were older than those with polyps in the rectum and sigmoid colon (P<0.05). Adenomatous polyps comprised the most common type (67%) and the rectum was the most common site for each type, especially juvenile and retention polyps. Juvenile polyps were found in the youngest patients(12±4.8, P<0.05) and the adenomatous in the oldest (52±14, P<0.05). The ratio of patients with polyposis comprised 1.2%, and patients with polyps accompanied with colorectal cancer comprised 6.1%. Examination of the changes in the incidence, the average patient age, and adenomatous type showed that they had all increased, but the frequency of infl ammatory and retention polyps decreased. CONCLUSION Colorectal polyps are a common problem. The frequency is greater in males compared to females and the rectum and sigmoid colon had the highest incidence. pathology.is an inherently subjective discipline, and therefore is often referred to as both an art and a science. Over the years, laboratory physicians have implemented special tissue stains and molecular techniques to limit subjectivity in the discipline. Beginning in the late 19th century, histochemical stains were developed to assess diagnostic biochemical reactions in tissue. Histochemistry has recently seen a resurgence in popularity because of the higher costs of other newer methods. Today, this technique is used by almost every pathology.laboratory across the world. Diagnostic and Therapeutic Cold Knife Conization for Cervical Intraepithelial Neoplasia

OBJECTIVE of using cold （CIN）. METHODS To evaluate the diagnostic and therapeutic efficacy knife conization for cervical intraepithelial neoplasia We retrospectively analyzed 186 cases with CIN diagnosed and treated in our hospital; compared the histologic diagnoses from cervical conization and from colposcopic multiple punch biopsies, and then evaluated their postoperative histologic findings and clinical outcomes. RESULTS Of the 186 cases, there was a correlation in histologic findings between cervical conization and colposcopic multiple punch biopsies in 138 cases （74.2%）, and there was no correlation in the other 48 cases （25.8%）. Incomplete excision was performed in 8 cases （4.3%）, but the failure rate was only 1.1%; the cure rate was 98.9%. Five cases with early invasive cancer were found. Eleven patients underwent subsequent hysterectomy. The main complications associated with conization were hemorrhage and cervical stenosis. Bleeding occurred in 8 （4.3%） of the patients, and cervical stenosis occurred in 3 （1.6%）. CONCLUSION Cervical intraepithelial neoplasia was diagnosed more accurately using conization than by colposcopic multiple punch biopsies. Conization can also play an important role in the treatment for CIN. If properly performed, the procedure has a low risk of complications. It can provide an accurate histologic representation of the disease process, and be curative in most cases.
