Attention was recently drawn to the production of D-glucose by salivary glands and its concentration in the saliva of both normal and diabetic subjects. Likewise, the identity and expression of selected transporters involved in the handling of the hexose by salivary glands were recently investigated in both normal and diabetic rats. In consideration of this information, the present report aims at providing an integrated review on the physiopathology of parotid cell energetics.

Objective: To report an extremely rare case of neuroendocrine tumour with simultaneous involvement of both the facial nerve and the deep lobe of parotid gland. Method: case report and English language literature review concerning neuroendocrine neoplasm involving the facial nerve with emphasis on clinical presentation. Results: We report a unique case of adenoma with neuroendocrine differentiation which involved both the mastoid segment of the right facial nerve and also the deep lobe of the parotid gland on the ipsilateral side. Both tumours were not contiguous and were anatomically separate from each other. A CT scan of the whole body revealed no other neuroendocrine tumours. Conclusion: To the best of our knowledge, this is the first report in the English language literature of a neuroendocrine tumour to involve both the vertical mastoid segment of the facial nerve with simultaneous involvement of the deep lobe of parotid gland as a separate lesion.

Background: Mucoepidermoid carcinoma ex-pleomorphic adenoma (MECxPA) is an extremely rare salivary gland malignancy. With only nine prior reported cases, this entity represents a challenging histopathological diagnosis. Methods: We present a case of a 71-year-old male with an enlarging left neck mass over several months. CT showed both a parapharyngeal space mass and a separate level II neck mass. Results: The patient underwent resection of the left parapharyngeal mass and ipsilateral selective level II-IV lymphadenectomy. The final pathologic diagnosis was metastatic high-grade mucoepidermoid carcinoma ex-pleomorphic adenoma. Conclusions: We describe a novel presentation of high-grade mucoepidermoid carcinoma ex-pleomorphic adenoma as a metastatic parapharyngeal mass.

Primary Malignant parotid Melanoma: Searching for a Rare Entity

Objectives: The aim of this paper is to establish a correct approach to the diagnosis of a primary malignant melanoma of the parotid gland. Methods: We present a case of a patient with a painless mass situated in the left parotid region after an initial cytological and radiological study suggesting a Warthin tumor whose treatment involved a total parotidectomy with a facial nerve reconstruction. Results: The histopathological result showed a malignant melanoma that after various studies and evaluations showed no other primary origin different from the parotid gland itself. Conclusions: Salivary gland melanomas are relatively rare entities that when being of a primary origin and localized in the parotid gland become even more uncommon. Upon a melanoma found in the parotid gland, the first suspicion should be a metastatic origin, being the most common sites the oral cavity and the skin from the head and neck region.

Purpose: To describe the extremely rare occurrence of acinic cell carcinoma of the parotid in a young child. Materials & Methods: A 14-year-old boy presented with a left sided facial mass. It was initially thought to be a bug bite or perhaps a wrestling injury. However, it progressed rapidly overall several weeks. CT revealed a well-circumscribed, cystic, 3.9 × 2.8 × 3.2 cmmass centered in the deep lobe of the left parotid gland, also involving the superficial lobe and extending medially through the stylomandibular tunnel with a component extending to the stylomastoid foramen. There was no solid enhancing portion. There were several enlarged level II lymph nodes suspicious for nodal metastases. Results: The mass was biopsied and pathology was consistent with acinic cell carcinoma. Complete left parotidectomy and left selective neck dissection was performed, confirming the diagnosis. Conclusions: Salivary gland neoplasms are rare in children. Less than 5% of salivary gland tumors occur in children. Salivary gland neoplasms account for less than 8% of pediatric head and neck tumors. Approximately 1/3 of salivary gland tumors arising in childhood are malignant;most being mucoepidermoid. Acinic cell carcinoma of the parotid gland in children is extremely rare, especially in those under age 16, with only a few available case reports. This case was also unusual in that the lesion was predominantly cystic.

Definitive diagnosis of parotid gland masses is required optimal management planning and for prognosis. There is controversy over whether fine needle aspiration cytology(FNAC) or ultrasound guided core biopsy(USCB) should be the standard for obtaining a biopsy. The aim of this review is to assess the current evidence available to assess the benefits of each technique and also to assess the use of intra-operative frozen section(IOFS). Literature searches were performed using pubmed and google scholar. The literature has been reviewed and the evidence is presented. FNAC is an accepted and widely used technique. It has been shown to have variable diagnostic capabilities depending on centres and experience of staff. USCB has a highly consistent diagnostic accuracy and can help with tumour grading and staging. However, the technique is more invasive and there is a question regarding potential for seeding. Furthermore, USCB is less likely to be offered as part of a one-stop clinic. IOFS has no role as a first line diagnostic technique but may be reserved as an adjunct or for lesions not amenable to percutaneous biopsy. On balance, USCB seems to be the method of choice. The current evidence suggests it has superior diagnostic potential and is safe. With time, USCB is likely to supplant FNAC as the biopsy technique of choice, replicating that which has occurred already in other areas of medicine such a breast practice.

Epithelial-Myoepithelial Carcinoma of parotid Gland—A Surprising Clinical Entity

Objective: Objective of this study is to highlight the importance of considering clinical and radiological findings in parotid tumors before operating tumors with high suspicion of malignancy though reported as benign on histopathology. Material and Methods: 20 years old female patient underwent radical parotidectomy for epithelial-myoepithelial carcinoma of the left parotid gland which was diagnosed on FNAC. Contrast CT and contrast MRI also showed a tumor involving left parotid gland with destruction of ramus of mandible and loss of tissue planes medially & laterally. Facial nerve was infiltrated by tumor, thus sacrificed. Post operatively, patient had left facial palsy and was discharged after 7 days without any other sequelae. Postoperative histopathology showed pleomorphic adenoma with positive resection margins medially, superiorly & posteriorly. No further treatment was given in view of the benign nature of the tumor. Results: Patient was kept on monthly follow up and was disease free at one year follow up. Conclusion: Aggressive surgical treatment should be planned for malignant parotid tumors to give a better tumor free survival. Misdiagnosis can occur on FNAC as it can hit only one part of the tumor thus missing a malignant tumor or can interpret pleomorphic adenoma as epithelial-myoepithelial carcinoma.
