Cholestasis, ascites and pancytopenia in an immunocompetent adult with severe cytomegalovirus hepatitis

Human cytomegalovirus(CMV) is a herpesvirus,which establishes lifelong latency after primary infection and leads to severe disease in immunocompromised patients. However,CMV infection in immunocompetent patients is usually asymptomatic and severe organ damage is rarely reported. We report a case of severe CMV hepatitis in an immunocompetent patient presenting with cholestasis,portal hypertensionrelated ascites and pancytopenia. The patient was asymptomatic with normal liver function and negative CMV DNA after two weeks of antiviral therapy. This case is an example of a common infection with an uncommon presentation,and suggests that testing for CMV should be carried out,even in patients with normal immune status,presenting with severe liver damage or cholestasis.

Is necrosectomy obsolete for infected necrotizing pancreatitis? Is a paradigm shift needed?

In 1886,Senn stated that removing necrotic pancreatic and peripancreatic tissue would benefit patients with severe acute *** then,necrosectomy has been a mainstay of surgical procedures for infected necrotizing pancreatitis(NP).No published report has successfully questioned the role of ***,however,increasing evidence shows good outcomes when treating walled-off necrotizing pancreatitis without a *** literature concerning NP published primarily after 2000 was reviewed;it demonstrates the feasibility of a paradigm *** majority(75%)of minimally invasive necrosectomies show higher completion rates:between 80%and 100%.Transluminal endoscopic necrosectomy has shown remarkable results when combined with percutaneous drainage or a metallic *** morbidities range from 40%to 92%.Single-digit mortality rates have been achieved with transluminal endoscopic necrosectomy,but not with video-assisted retroperitoneal necrosectomy *** procedures without necrosectomy have evolved from percutaneous drainage to transluminal endoscopic drainage with or without percutaneous endoscopic gastrostomy access for laparoscopic *** series have reached higher success rates of 79%-93%,and even 100%,using transcystic multiple drainage *** is becoming evident that transluminal endoscopic drainage treatment of walledoff NP without a necrosectomy is *** further refinement of the drainage procedures,a paradigm shift from necrosectomy to drainage is inevitable.

Plk1在胰腺癌中的表达及临床意义

目的研究胰腺癌组织标本及细胞系中Plk1的表达及临床意义。方法应用免疫组化EnVision两步法检测33例胰腺癌及其癌旁组织、3例胰腺浆液性囊腺瘤、3例黏液性囊腺瘤、5例慢性胰腺炎的石蜡标本中Plk1的表达。用半定量RT—PCR检测5例新鲜胰腺癌组织及其癌旁组织中Plk1 mRNA的表达。用Westernblot检测两株胰腺癌细胞panc1和CFPAC1中Plk1表达。结果胰腺癌组织中Plk1的阳性表达率为72、7％（24／33），其表达与胰腺癌患者的年龄、性别、临床分期、组织学分级、淋巴结转移、肿瘤的部位无显著相关（P〉0.05）。癌旁组织、慢性胰腺炎和良性胰腺肿瘤组织中Plk1的表达均为阴性。5例新鲜胰腺癌组织中都有Plk1mRNA的高表达，两株胰腺癌细胞中也有Plk1蛋白的表达。结论 Plk1在胰腺癌中的表达具有一定肿瘤特异性，有可能成为胰腺癌诊治的理想靶标。

D-泛酸(维生素B5)在饲料、食品、化工、医药等行业中被广泛的应用,市场需求量较大。目前生产D-泛酸多使用化学拆分法,不仅繁琐耗时而且对环境产生很大的影响。本文以泛酸合成酶(Pantothenatesynthetase,PS)作为关键酶进行的生物酶法生产路线具有环境友好,工艺简洁的特性,并且能够大大降低生产成本。主要研究内容及结果如下:(1)泛酸合成酶的筛选本研究对11个来源的泛酸合成酶基因(panc)进行克隆,使其在Escherichia coliBL21(DE3)中表达,以泛酸合成酶的酶活性为指标对构建成功的菌株进行筛选,使用高效液相色谱(HPLC)检测PS酶的活性,结果发现短短芽孢杆菌来源的泛酸合成酶活性最高,为1562.3U/mL。(2)泛酸合成酶性质分析对重组菌株PSBb的泛酸合成酶进行酶学性质研究,首先研究了其最适温度和最适pH,然后又对其动力学参数及其细胞内ATP含量进行了测定。从实验结果中发现PSBb的最适温度是30℃,最适pH是7.0,kcat为115.4s,细胞中ATP含量为 1.58×10μmol/mg。(3)重组大肠杆菌在5 L发酵罐中边发酵边转化的条件优化以D-泛解酸和β-丙氨酸为底物,在5L发酵罐中对重组菌株生产D-泛酸的培养基种类,诱导剂的种类,诱导剂的最适浓度,诱导的最适温度和培养基中残糖量进行优化。结果发现反应进行46h后使用葡萄糖发酵培养基的D-泛酸产量为65.1 g/L比使用TB培养基的产量高6.3g/L,使用0.1g/L的IPTG作为诱导剂,同时控制诱导温度为30℃,控制培养基中残糖量在0.1-0.5g/L时D-泛酸产量可以达到86.4g/L,转化率达 87.8%。(4)补料分批发酵为了降低β-丙氨酸对细胞的毒害作用,进一步提高D-泛酸的产量,我们尝试将500mL包含60g/L的D-泛解酸和40 g/L的β-丙氨酸混合溶液作为底物分为三批,五批,八批进行补料分批实验。结果发现底物分五批投入时效果最好,转化产生的D-泛酸可达92.2g/L,转化率达到93.7%。我们进一步对发酵罐的转速与溶氧进行优化后发现,转速与溶氧关联,控制溶氧在15%(v/v)时D-泛酸产量可达95.7g/L,转化率达到97.2%,时空产率为50.4g/L/d。目前对于PS酶的活性和动力学方面的资料很少,本研究为泛酸合成酶的活性和动力学研究及酶法生产D-泛酸奠定了良好的基础。

Bursectomy at radical gastrectomy

Radical gastrectomy with extended lymph node dissec tion and prophylactic resection of the omentum, peri toneum over the posterior lesser sac, pancreas and/o spleen was advocated at the beginning of the 1960 s in Japan. In time, prophylactic routine resections of the pancreas and/or spleen were abandoned because of the high incidence of postoperative complications. However omentectomy and bursectomy continued to be standard parts of traditional radical gastrectomy. The bursaomentalis was thought to be a natural barrier against invasion of cancer cells into the posterior part of the stomach. The theoretical rationale for bursectomy was to reduce the risk of peritoneal recurrences by eliminating the peritoneum over the lesser sac, which might include free cancer cells or micrometastases. Over time, the indication for bursectomy was gradually reduced to only patients with posterior gastric wall tumors penetrating the serosa. Despite its theoretical advantages, its benefit for recurrence or survival has not been proven yet. The possible reasons for this inconsistency are discussed in this review. In conclusion, the value of bursectomy in the treatment of gastric cancer is still under debate and large-scale randomized studies are necessary. Until clear evidence of patient benefit is obtained, its routine use cannot be recommended.
