palpitations are one of the most common reasons for medical consultation. Theytend to worry patients and can affect their quality of life. They are often asymptom associated with cardiac rhythm disorders, although there are otheretiologies. For diagnosis, it is essential to be able to reliably correlate the symptomswith an electrocardiographic record allowing the identification or rulingout of a possible rhythm disorder. However, reaching a diagnosis is not alwayssimple, given that they tend to be transitory symptoms and the patient isfrequently asymptomatic at the time of assessment. In recent years, electrocardiographicmonitoring systems have incorporated many technical improvements thatsolve several of the 24-h Holter monitor limitations. The objective of this review isto provide an update on the different monitoring methods currently available,remarking their indications and limitations, to help healthcare professionals toappropriately select and use them in the work-up of patients with palpitations.

Introduction:palpitation is a common complaint among patients presenting to emergency departments or cardiologists.palpitation caused by organic heart diseases,such as coronary atherosclerotic heart disease and hypertensive heart disease,may be a sign of a life-threatening condition.In such cases,treatment with western medicines or surgeries is warranted.However,for patients with palpitation caused by noncardiac factors,western medicine treatment is often ineffective,and traditional Chinese medicine has comparatively superior advantage.With the change in people’s lifestyle and habits in the past decades,the time spent sitting and that with a forward head posture is increasing,which has accelerated the process of spinal aging and increased the incidence rate of spinal diseases.Case presentation:A 54-year-old female presented with paroxysmal palpitations,chest depression,and neck pain that had lasted for more than 2 months.Western medicine diagnosis was palpitations due to spinal disorders,while the traditional Chinese medicine diagnosis was palpitation(heart and spleen deficiency).The patient was treated with traditional Chinese massage,30 min sessions,5 times a week,3 weeks totally.Subsequently,the patient’s neck pain and numbness disappeared and the palpitations,chest tightness,and dizziness ameliorated.The patient resumed regular appetite,experienced better sleep,and had further improvement of spirit,enabling her to lead a normal life.Conclusions:Traditional Chinese massage therapy can relieve muscle spasm,regulate cervical facet joint disorder and dislocation,release adhesion,relieve synovial incarceration,restore the dynamic and static balance of the cervical spine,improve blood circulation of the cervical spine,increase the nutritional supply of the cervical spine and surrounding soft tissue,and promote metabolism and tissue repair.All these advantages make it a good complementary and alternative therapy for palpitation caused by spine disorders.

To the Editor： A 47-year-old male of chest discomfort for 2 weeks patient was admitted because He had no obvious incentive precordial discomtfort 2 weeks ago, accompanied by palpitation. He immediately went to the town hospital. Electrocardiogram （ECG） showed myocardial infarction. Coronary angiography showed coronary artery disease accompanied by a ventricular aneurysm. The patient was transferred to Beijing Anzhen Hospital. ECG showed sinus rhythm, heart axis deviation ＋ 111 °, anterior septal, anterior lateral, anterior myocardial infarction, and complete right bundle branch block. Chest X-ray showed no obvious abnormalities in heart and lung. Ecbocardiography showed abnormal motion of segmental ventricular wall; formation of a ventricular aneurysm in apex area of the heart, the diameter of the ventricular aneurysm was 2 cm, the diastolic function of left ventricular was reduced. Coronary angiography showed left anterior descending artery filling slowly and its intima was not smooth; the stenosis rate was 90%. The stenosis rate of the circumflex artery was 90% [Figure la and b]. He was diagnosed with acute anterior myocardial infarction, left ventricular aneurysm, and hypertension.

Traditional Chinese Medicine nursing protocols for paroxysmal atrial fibrillation

Atrial fibrillation(AF), the most common cardiac arrhythmia worldwide, is associated with a higher risk of mortality and morbidity, in which paroxysmal atrial fibrillation(PAF) approximately accounts for 25%-60% of AF. Anti-arrhythmic drugs(AAD) have been used to treat different indications of AF for a long time, but its use is limited due to potential proarrhythmic cardiovascular and non-cardiovascular toxicity as well as the modest impact on the maintenance of sinus rhythm. Recently, Traditional Chinese Medicine(TCM) exhibits an excellent effect on PAF. This article primarily investigated the key points of common syndromes, TCM nursing methods and healthy guidance of PAF, aiming at maintaining and developing the strengths of TCM, improving its efficacy and standardizing its behavior.
