pain threshold, anxiety and other factors affect intensity of postoperative pain in gastric cancer patients: A prospective cohort study

Objective: This prospective cohort study explored factors related to postoperative pain in gastric cancer patients.Methods: A total of 236 patients who underwent gastrectomy were enrolled. All patients enrolled in the study completed the Hospital Anxiety and Depression Scale(HADS) questionnaire and Life Orientation Test-Revised(LOT-R) questionnaire on the day before surgery. Heat pain threshold(HPT), cold pain threshold(CPT) and pressure pain threshold(PPT) were measured for all patients one day prior to surgery and demographic details were collected. All patients were connected to a patient-controlled intravenous analgesia(PCIA) pump at the end of the surgery. The occurrence of postoperative pain was used as a dependent variable, and multivariate logistic regression analyses were conducted to screen for factors affecting postoperative pain.Results: In total, 83 patients(35.2%) had postoperative pain. Body mass index(BMI) ≥28 kg/m^(2) [odds ratio(OR): 2.67;95% confidence interval(95% CI): 1.07-6.67], total gastrectomy(OR: 2.64;95% CI: 1.42-4.91),preoperative anxiety score ≥8(OR: 2.37;95% CI: 1.12-5.02), heat pain threshold ≤4.9 s(OR: 2.14;95% CI:1.06-4.32), pressure pain threshold ≤4 g(OR: 2.05;95% CI: 1.05-4.03), and female gender(OR: 1.99;95% CI:1.04-3.83) were risk factors for postoperative pain.Conclusions: Obesity, wide range of gastrectomy, high preoperative anxiety, low HPT and PPT, and female gender are associated with increased risk for postoperative pain.

Effects of Electroacupuncture on the pain threshold and the NMDA R1 mRNA in DRG on Neuropathic pain Rats

To observe the effect of multiple electroacupuncture (EA) on the pain threshold and the regulation of N-methyl-D-aspartate (NMDA) receptor in dorsal root ganglia (DRG) of neuropathic pain rats. Rats were prepared with a unilateral chronic constriction injury (CCI) to the sciatic nerve. EA was done in acupoints 'Huan Tiao' and 'Yang Ling Quan' for 30 min every day and the thermal thresholds were detected after EA at 3, 5, 7, 10, 14 days after operation. On day 14 after nerve injury, the in situ hybridization method was used to investigate the change of NMDA R1 mRNA in L4-L5 DRG. The thermal threshold reduced significantly from day 3 after operation in CCI rats. After multiple EA treatment, the ipsilateral thermal hyperalgesia relieved gradually and the thermal threshold had no difference with control side after day 5(P>0. 05). From Day 7 after operation, the thermal threshold at each time point were significantly different compared with CCI group respective-ly(P>0. 05). Moreover the EA had accumulative effect. On Day 14 after operation , the NMDAR1 mRNA positive neurons and the mean optic density in ipsilateral L4-5 DRG were less than that of control side(Ppain. The multiple EA treatment can attenuate the thermal hyperlgesia of neuropathic pain rats and regulate the NMDA receptor.

OBJECTIVE:To evaluate the sternocleidomastoid and the descending portion of the trapezius muscles in terms of electromyographic activity and pressure pain threshold in women with chronic cervical pain after acupuncture treatment.METHODS:Twenty-five women,aged 18-50 years[(31±3)years],with chronic neck and temporomandibular disorder participated in the study.The electromyographic activity of the sternocleidomastoid muscle and the descending portion of the trapezius muscle were recorded at rest,during shoulder elevation,(left and right)rotation of the neck and crucifix position.Maximum voluntary contraction was used to normalize the electromyographic activity data.The points used for acupuncture were Jiache(ST 6),Xiaguan(ST 7),Quanliao(SI 18),Tinggong(SI 19),Yifeng(TE-17),Fengchi(GB 20),Renzhong(GV 26),Yangbai(GB-14),and points of the lower and upper limbs:Tai chong(LV 3),Zusanli(ST 36),Sanyinjiao(SP 6),Kunlun(BL 60),He Gu(LI4)and Yanglingquan(GB 34).The treatment course was 10 sessions,30 min long per session,and twice a week.The final electromyographic activity and pressure pain threshold data collection were performed 15 d after treatment completion.RESULTS:The normalized electromyographic activity and pressure pain threshold data were tabulated and submitted to statistical analysis using t-test(Ppain threshold for the trapezius and sternocleidomastoid muscles,bilaterally,after the acupuncture treatment.CONCLUSION:The acupuncture treatment improved the neck muscles in terms of electromyographic activity and increased the tolerance to chronic pain in women.

Effects of Electroacupuncture on pain threshold and Prostaglandin E2 in Spinal Cord in Postoperative pain Rat

Objective:Electroacupuncture(EA)is good at post-surgical pain.But point selection method in treating incision-induced pain remains a major clinical challenge.We reasoned that different acupoints may work though adjusting prostaglandin E2 in spinal cord.We wish to explore the analgesic mechanism of electroacupuncture on plantar incision pain rats and provide more therapeutic ideas for acupuncture analgesia.Methods:A total of 50 male rats were randomly divided into a sham operation group,a model group,an EA 1,EA 2 and a drug group(n=10,each).A rat model of left plantar incision pain was established.The rats in EA1 group was needled at ipsilateral Yanglingquan(GB34)and Taixi(KI3).The rats in EA2 group was needled at ipsilateral Quchi(LI11)and Hegu(LI4).EA stimulation(2/100 Hz,1-2-3 mA)was administered 30 minutes immediately after operation.The rats in drug group were fed with Fenbid by gavage 20 minutes before incision(30 mg/kg,p.o.).The hot plate pain detector was used to measure the thermal pain threshold(TPT)before and 24 hours after operation Prostaglandin E2 content of spinal cord was detected by enzyme-linked immunosorbent assay(ELISA)at 1 and 24 hours after operation.Results:Compared with sham operation group,the TPT in model group decreased 41%.Compared with the model group,the TPT increased 56%in EA1,29%in EA2,190%in drug group(P>0.05).At 1 h after operation,compared with the sham operation group,PGE_2 in model group increased 15%.Compared with the model group,PGE_2 in drug group decreased 5%.At 24 hours after operation,compared with sham operation group,PGE_2 in model group increased 9%.Compared with model group,it decreased 4%in EA 1 group,8%in drug group and increased 3%in EA2 group.Conclusion:Both the drug and the electroacupuncture can adjust the 24-hour pain threshold and PGE_2 in spinal cord.The curative effects of the drug are better than that of electroacupuncture.The proximal point is better than that of the distal point.Electroacupuncture can treat postoperative pain by regulating PGE_2 in spinal

Objective:To explore the circadian rhythm of pain threshold and circadian changes of analgesic effect of panax notoginseng total glycosides in mice.Methods:Use the hot plate method and the radiation heat method to establish mouse pain model,measure the pain threshold in 08:00,12:00,16:00,20:00,24:00,04:00,analyzes its circadian rhythm.Given panax notoginseng total glycosides(100mg/kg) In 16:00,and04:00;after 3 days,measure the diurnal differences for effect of pain threshold in different administration time.Results:The pain threshold of mice pain model showed a peak in 16:00,while the valley is located in the night.The analgesic effect of panax notoginseng total glycosides in 04:00 administration more than 16:00 administration.Conclusion:The pain threshold of mice have a circadian rhythm,and analgesic effects of panax notoginseng total glycosides is dependent on circadian time.

Implanting iodine-125 seeds into rat dorsal root ganglion for neuropathic pain: neuronal microdamage without impacting hind limb motion

The use of iodine-125 （L251） in cancer treatment has been shown to relieve patients＇ pain. Consid- ering dorsal root ganglia are critical for neural transmission between the peripheral and central nervous systems, we assumed that 125I could be implanted into rat dorsal root ganglia to provide relief for neuropathic pain. 125I seeds with different radioactivity （0, 14.8, 29.6 MBq） were im- planted separately through L4-5 and L5-6 intervertebral foramen into the vicinity of the L5 dorsal root ganglion, von Frey hair results demonstrated the mechanical pain threshold was elevated after implanting 125I seeds from the high radioactivity group. Transmission electron microscopy revealed that nuclear membrane shrinkage, nucleolar margination, widespread mitochondrial swelling, partial vacuolization, lysosome increase, and partial endoplasmic reticulum dilation were visible at 1,440 hours in the low radioactivity group and at 336 hours in the high radio- activity group. Abundant nuclear membrane shrinkage, partial fuzzy nuclear membrane and endoplasmic reticulum necrosis were observed at 1,440 hours in the high radioactivity group. No significant difference in combined behavioral scores was detected between preoperation and postoperation in the low and high radioactivity groups. These results suggested that the mechan- ical pain threshold was elevated after implanting 125I seeds without influencing motor functions of the hind limb, although cell injury was present.

Analgesic Effect of Diprospan in Rats with Trigeminal Neuralgia

This study examined the analgesic effect of diprospan in rats with trigeminal neuralgia.Rat model of trigeminal neuralgic pain was established by loosely ligating the left infraorbital branch of the trigeminal nerve.After allodynia developed,the rats were randomly divided into 2 groups（n=20 in each）：diprospan group,in which the rats received diprospan（7 mg/mL,0.1 mL） injected to the left infraorbital foramen area;control group,in which saline（0.1 mL） was administered as the same manner as the diprospan group.The pain threshold（PT） in the left infraorbital area was measured before and 2,6,and 8 weeks after the administration.The expression of neuropeptides [substance P,preprotachykinin A（PPTA）,calcitonin gene-related peptide（CGRP）] in the trigeminal nerve was detected at the same time points as the PT measurement by immunohistochemistry or in situ hybridization method.The results showed that in the diprospan group,the PT was 10.65±1.26,10.77±1.19 and 14.13±1.34 g 2,6,and 8 weeks after the administration respectively,significantly higher than that before the administration（PT value：0.36±0.11）（P0.05 for each）.In the saline group,the PT was 0.37±0.13,0.66±0.09,4.45±1.29 and 13.72±1.72 g before and 2,6,and 8 weeks after the administration respectively with differences being significant between before and 6,8 weeks after the administration（P0.01）.No significant difference existed in the PT between the diprospan group and the saline group at pre-administration（P0.05）.The PT in the diprospan group was significantly greater than that in the saline group 2 and 6 weeks post-administration（P0.05）.In the diprospan group,the expression levels of neuropeptides were significantly reduced as compared with those in the saline group 2 and 6 weeks post-administration（P0.05）.It was concluded that diprospan has an obvious analgesic effect on the trigeminal neuropathic pain partly by reducing the expression of neuropeptides in the trigeminal ganglia.

The Stimulative Effect of SupracutaneousElectroacupuncture on Human Skin and Its Analgesic Effect on Rats

The stimulative effect of supracutaneous electroacupuncture using a blunt-tip electrode on120 points of human skin was observed in 12 healthy volunteers. A comparison of stimulation by the moreexpensive supracutaneous instrument specifically designed for this treatment and that of the rnore conventional transcutaneous electroacupuncture instrument, modified by use of a blunt-tip, belt-type electrodeinstead of the traditional electro-needle. showed similar therapeutic effects. An animal experiment, usingthe rat tail flick analgesia method to compare supracutaneous electroacupuncture. showed that the painthreshold was raised by both types of electroacupuncture. with both having an optimal stimulation time of10￣20 minutes. The percentage of change in maximum pain threshold was 206. 6 % for supracutaneouselectroacupuncture at 7V and 175. 4 % for transcutaneous electroacupuncture at 2V.
