Relation of supplementary feeding to resumptions of menstruation and ovulation in lactating postpartum women

Background Resumption of menstrual cycles is one of the indicators for restoration of reproductive capability in postpartum women. However, menstruation does not necessarily mean that ovulation has taken place. The aim of this study was to investigate the relation of supplementary feeding to return of menstruation and ovulation after delivery. Methods A questionnaire was used to obtain data from 101 breasffeeding mothers. The following elements were analyzed： age, education level, breasffeeding practice, time of return of menstruation, contraceptive practice, and starting time of supplementary feeding during the lactation at intervals of 6 weeks to 18 months after delivery. The ovulation was continuously monitored by ultrasonography and basal body temperature （BBT） measurement. Results By ultrasonography, 53 of the 101 women （52.5%） had the first ovulation （follicle 〉1.8 cm in diameter） within 154 days after delivery on average, among whom 11 （10.9%, 11/101） had restoration of ovulation within 4 months and 42 （41.6%, 42/101） had it after 4 months. In women with follicles 〉1.8 cm in diameter （n=53）, the menstruation resumed （138±84） days after delivery, and the supplementary feeding was started at （4.0±1.1） months, which were significantly earlier than those in the women with follicular diameter 〈1.7 cm （n=48; （293±88） days, （5.1±1.3） months; t=9.003, P〈0.01 and t=4.566, P〈0.01）. In the women with follicles 〉1.8 cm in diameter, 30 had return of menstruation before the end of ultrasonographic monitoring, while only 8 in the women with follicular diameter 〈1.7 cm had menstrual resumption at the same time （χ^2=16.91, P〈0.01）. The starting time of supplementary feeding was positively correlated with the time of the restoration of menstruation （n=100, r=0.4764, P〈0.01） and first ovulation after delivery （n=53, r=0.5554, P〈0.01）. A GnRH agonist (5-50 μg) is capable of inducing ovulation in PMSG-primed hypophy-sectomized immature rats, as is the case in hCG-induced ovulation, but 2-4 h earlier than hCG. GnRH-induced ovulation is effectively blocked by the concomitant administration of the GnRH-antagonist which failed to interfere with hCG-induced ovulation, indicating that GnRH and its agonists do not share a receptor with LH/hCG. Like hCG, GnRH is also capable of inducing tissue type (tPA), but not urokinase type (uPA) PA. The plasminogen activator activity in ovarian homogenates and the granulosa and theca-interstitial cells increase in a time-dependent manner, reaching maximum levels just prior to ovulation. Similar to hCG, GnRH also increases tPA activity in cumulus-oocyte complexes in a time-dependent fashion.

Malignant transformation of hepatocellular adenoma in a young female patient after ovulation induction fertility treatment:A case report

BACKGROUND Hepatocellular adenoma(HCA) is a rare benign liver tumor usually affecting young women with a history of prolonged use of hormonal contraception.Although the majority is asymptomatic, a low proportion may have significant complications such as bleeding or malignancy. Despite responding to the hormonal stimulus, the desire for pregnancy in patients with small HCA is not contraindicated. However, through this work we demonstrate that intensive hormonal therapies such as those used in the treatment of infertility can trigger serious complications CASE SUMMARY A 33-year-old female with a 10-year history of oral contraceptive use was diagnosed with a hepatic tumor as an incidental finding in an abdominal ultrasound. The patient showed no symptoms and physical examination was unremarkable. Laboratory functional tests were within normal limits and tests for serum tumor markers were negative. An abdominal magnetic resonance imaging(MRI) was performed, showing a 30 mm × 29 mm focal lesion in segment VI of the liver compatible with HCA or Focal Nodular Hyperplasia with atypical behavior. After a total of six years of follow-up, the patient underwent ovulation induction treatment for infertility. On a following MRI, a suspected malignancy was warned and hence, surgery was decided. The surgical specimen revealed malignant transformation of HCA towards trabecular hepatocarcinoma with dedifferentiated areas. There was non-evidence of tumor recurrence after three years of clinical and imaging follow-up.,CONCLUSION HCAs can be malignant regardless its size and low-risk appearance on MRI when an ovultation induction therapy is indicated.

In this study we have demonstrated that both granulosa and theca-interstitial cells of hy-pophysectomized rat ovaries are capable of synthesizing tPA and PAI-1.Injection of a GnRH agonist canmarkedly induce these gene expressions in the ovary in a cell-specific and time-coordinated manner,so that asurge of tPA mRNA and its activity in both granulosa and theca-interstitial cells was obtained just prior toovulation.Theca-interstitial cells make PAI-1 become the most active in the ovary.Both the amount PAI-1mRNA and its activity in the cells reach the maximum level 6 h before the tPA peak.By contrast,granulosacells produce only a little amount of PAI-1 (most increase tPA activity),and both PAI-1 mRNA and activityin the cells reach the maximum after ovulation.The coordinated regulation of tPA and PAI-1 in the ovarymay fine-tune the peak of tPA activity which may be important for the regulation of the ovulatory process.The changes of tPA and PAI-1 in the ovarian cells of hypophysectomized rats during GnRHa-induced ovula-tion are similar to that in intact rats during hCG-induced ovulation,suggesting that the ovulatory process canbe modulated by different regulatory signals mediated by influencing the coordinated expression of both tPAand PAI-1.

Objective: To analyze the application value of combined detection of sperm quality, sex hormone and ovulation in the diagnosis of infertility. Methods: The study was conducted from June 2021 to June 2022. Sixty infertile couples who received IVF cycle treatment in our hospital during this period were selected as the observation group, and 60 couples with good sperm quality and follicle number ≥ 5 who conceived naturally after gynecological disease treatment were selected as the control group during the same period. The sperm quality, sex hormones and ovulation of the two groups were observed, and the change of positive rate was detected by the combined detection method. Results: Compared with the control group, the observation group had less semen (2.82 ± 0.12) ml, lower concentration (69.17 ± 1.28) × 106/ml, normal sperm morphology rate (2.92% ± 0.11%), lower survival rate (70.25% ± 1.16%), higher deformed sperm index (1.39 ± 0.11), and significant differences between groups (P ovulation can effectively clarify the problems existing in the couple, significantly improve the detection rate of the cause of the patient, and is more conducive to guiding clinical symptomatic treatment, which is worthy of promotion and reference.

Studies on the Relationship between the Follicular Blood Micro—Circulation and ovulation

Twelve purebred Har Bai rabbits with a body weight of 3-4 kg were seleeted and divided in-to four groups at random.they were treated with FSH and hCG atfter the second estrus,The four groups of rabbits were killed at the beginning of estrus ,the beginning of ovulation,15h and 39 h after the beginning of ovulation respectively ,The follicles of 0.5-2 mm in diameter were dissected from the ovaries were fixed ,sectioned ,mounted and stained for examination under light and elec-tron microscopes,The results showed that blood micro-circulation of follicles at the onset of esturs was normal and cells of the theca interna and granular cells were active in metabolism in most of the follicles ,examined at the beginning of ovulation,the blood micro-circulation became disordered,the cells became not active in matbolism and follicular stigmaes appeared at the upper wall of the follicles .At 15 h after the beginning of ovulation ,the unovulated follicles whic were developing into follicular corpus luteam had normal blood micro-circulation,At 39 h after the beginning of ovulation most of the follicles began to degenerate,In was coneluded that there was a close relationship between blood micro-circulation of follicele and the ovulation.

Background Spermatozoa interact with oviduct secretions before fertilization in vivo but the molecular players of this dialog and underlying dynamics remain largely unknown.Our objectives were to identify an exhaustive list of sperm-interacting proteins(SIPs)in the bovine oviduct fluid and to evaluate the impact of the oviduct anatomical region(isthmus vs.ampulla)and time relative to ovulation(pre-ovulatory vs.post-ovulatory)on SIPs number and abundance.Methods Pools of oviduct fluid(OF)from the pre-ovulatory ampulla,pre-ovulatory isthmus,post-ovulatory ampulla,and post-ovulatory isthmus in the side of ovulation were collected from the slaughterhouse.Frozen-thawed bull sperm were incubated with OF or phosphate-buffered saline(control)for 60 min at 38.5℃.After protein extraction and digestion,sperm and OF samples were analyzed by nanoLC-MS/MS and label-free protein quantification.Results A quantitative comparison between proteins identified in sperm and OF samples(2333 and 2471 proteins,respectively)allowed for the identification of 245 SIPs.The highest number(187)were found in the pre-ovulatory isthmus,i.e.,time and place of the sperm reservoir.In total,41 SIPs(17%)were differentially abundant between stages in a given region or between regions at a given stage and 76 SIPs(31%)were identified in only one region×stage condition.Functional analysis of SIPs predicted roles in cell response to stress,regulation of cell motility,fertilization,and early embryo development.Conclusion This study provides a comprehensive list of SIPs in the bovine oviduct and evidences dynamic spatiotemporal changes in sperm-oviduct interactions around ovulation time.Moreover,these data provide protein candidates to improve sperm conservation and in vitro fertilization media.

This paper presents a real-time microcomputer-based logger for measuring basal body temperature (BBT). BBT is the normal resting body temperature of a healthy person immediately upon waking in the morning. The temperature for women normally rises after ovulation due to hormonal changes. The temperature is logged real-time into the computer over a period and the BBT chart produced is used to predict ovulation. There is high correlation between the approach developed by this study and other standard measuring equipments—correlation of 0.9945with standard thermistor, and correlation of 0.9977 with standard thermocouple. This development enables privacy of use, allowing women to predict their ovulation status at a personal level.

Background:Polycystic ovary syndrome(PCOS)is an endocrine disorder that occurs in women of child-bearing age.Moreover,PCOS patients have decreased pregnancy rates and clomiphene citrate resistance.The traditional Chinese medicine formula Kun-Tai-1S(KT1S),consisting of the seahorse species hippocampus,has been reported to elicit therapeutic effects in patients with PCOS.However,given the limited resources and global demand for wild hippocampus,whether KT1S with or without hippocampus can elicit similar therapeutic effects has not been confirmed.Methods:KT1S and Kun-Tai-1A(KT1A,KT1S without dry hippocampus)were used to treat a letrozole-induced rat model of PCOS with an established disease.The serum levels of testosterone,luteinizing hormone,anti-Müllerian hormone,and estradiol were determined,the luteinizing hormone/follicle-stimulating hormone ratio was determined,and the ovarian pathology was evaluated.Results:Similar to the therapeutic effects of cyproterone acetate,both the KT1S and KT1A treatments reduced the body weight and ovarian and uterine indices in the rats with PCOS.The serum levels of testosterone,anti-Müllerian hormone,and luteinizing hormone and the luteinizing hormone/follicle-stimulating hormone ratio were significantly lower in the KT1S and KT1A treatment groups compared to the model group(P<0.01 and P<0.05,respectively).Moreover,the histopathological assessment results suggested that both the KT1S and KT1A treatments significantly ameliorated the PCOS pathology in the rats with an established disease,with a reduced number of cystic and atretic follicles and an increased number of corpora lutea being observed in the ovaries.Notably,there was no obvious difference in the disease outcomes between the KT1S and KT1A-treated groups.Network pharmacology analysis revealed that 4’,7-dihydroxyflavanone,sinpemine A,quercetin,8-isopentenyl-kaempferol,and luteolin in KT1A may promote estrogen signaling;furthermore,the nitric oxide regulation pathway is also closely involved.Conclusion:KT1A and KT1S treatmen
