The impact of emergency department length of stay on the Outcomes of trauma patients requiring hospitalization: a retrospective observational study

BACKGROUND: We aimed to explore the impact of the emergency department length of stay(EDLOS) on the outcome of trauma patients.METHODS: A retrospective study was conducted on all trauma patients requiring hospitalization between 2015 and 2019. Patients were categorized into 4 groups based on the EDLOS(24 h). Data were analyzed using Chi-square test(categorical variables), Student’s t-test(continuous variables), correlation coefficient, analysis of variance and multivariate logistic regression analysis for identifying predictors of short EDLOS and hospital mortality.RESULTS: The study involved 7,026 patients with a mean age of 32.1±15.6 years. Onefifth of patients had a short EDLOS(24 h were older(P=0.001) and had more comorbidities(P=0.001) and fewer deaths(P=0.001). Multivariate regression analysis showed that the predictors of short EDLOS were female gender, GCS, SI, hemoglobin level, ISS, and blood transfusion. The predictors of mortality were TTA-1(odds ratio [OR]=4.081, 95%CI: 2.364–7.045), head injury(OR=3.920, 95%CI: 2.413–6.368), blood transfusion(OR=2.773, 95%CI: 1.668–4.609), SI(OR=2.132, 95%CI: 1.364–3.332), ISS(OR=1.077, 95%CI: 1.057–1.096), and age(OR=1.040, 95%CI: 1.026–1.054). CONCLUSIONS: Patients with shorter EDLOS had different baseline characteristics and hospital Outcomes compared with patients with longer EDLOS. Patients with prolonged EDLOS had better Outcomes;however, the burden of prolonged boarding in the ED needs further elaboration.

Association between influenza vaccination and SARS-CoV-2 infection and its Outcomes: systematic review and meta-analysis

Background:World Health Organization recommends that influenza vaccines should benefit as much of the population as possible,especially where resources are limited.Corona virus disease 2019(COVID-19)has become one of the greatest threats to health systems worldwide.The present study aimed to extend the evidence of the association between influenza vaccination and COVID-19 to promote the former.Methods:In this systematic review,four electronic databases,including the Cochrane Library,PubMed,Embase,and Web of Science,were searched for related studies published up to May 2022.All odds ratios(ORs)with 95%confidence intervals(CIs)were pooled by meta-analysis.Results:A total of 36 studies,encompassing 55,996,841 subjects,were included in this study.The meta-analysis for severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection provided an OR of 0.80(95%CI:0.73–0.87).The statistically significant estimates for clinical Outcomes were 0.83(95%CI:0.72–0.96)for intensive care unit admission,0.69(95%CI:0.57–0.84)for ventilator support,and 0.69(95%CI:0.52–0.93)for fatal infection,while no effect seen in hospitalization with an OR of 0.87(95%CI:0.68–1.10).Conclusion:Influenza vaccination helps limit SARS-CoV-2 infection and severe Outcomes,but further studies are needed.Registration:PROSPERO,CRD42022333747.

Evaluation of follow-up and long-term Outcomes of gunshot and stab wounds in a French civilian population

Purpose The data concerning long-term follow-up and Outcomes of penetrating trauma are poorly detailed in the literature.The main objective of our study was to analyze the hospital and extra-hospital follow-up of penetrating trauma victims and to evaluate the late complications and long-term consequences of these traumas.Methods This work was a retrospective longitudinal monocentric observational study conducted at Laveran Military Hospital,from January 2007 to January 2017.All patients hospitalized for gunshot wound or stab wound management during this period were identified via a retrospective systematic query in the hospital information system using the ICD-10 codes.Epidemiological data,traumatism characteristics,hospital management,follow-up and traumatism consequences(i.e.,persistent disability)were analyzed.To improve evaluation of traumatism long-term consequences,extra-hospital follow-up data from general physicians(GP)were collected by phone call.During this interview,9 closed questions were asked to the GP.The survey evaluated:the date of the last consultation related to injury with the GP,the specific follow-up carried out by the GP,traumatism consequences,and recurrence of traumatism.Descriptive,univariate and multivariate with regression analysis were used for statistical analysis.Results A total number of 165 patients were included.Median(Q1,Q3)of hospital follow-up was 28(4,66)days.One hundred one patients(61.2%)went to their one-month consultation at hospital.GP follow-up was achieved for 76 patients(55.2%).Median(Q1,Q3)of GP follow-up was 47(21,75)months.Twenty-four patients(14.5%)have been totally lost to follow up.The overall follow-up identified 54 patients(32.7%)with long-term consequences,20 being psychiatric disorders and 30 organic injuries.Organic consequences were mainly peripheral nerve damages(n=20;12.1%).Most of the psychiatric consequences were diagnosed during GP follow-up(n=14;70%).Seventeen cases(10.3%)of recurrence were found and late mortality occurred in 4 patients(2.4%).High i

Objective:As exclusively endoscopic endonasal resection of benign orbital tumors has become more widespread,high-quality Outcomes data are lacking regarding the decision of when and how to reconstruct the medial orbital wall following resection.The goal of this study was to systematically review pertinent literature to assess clinical Outcomes relative to orbital reconstruction practices.Methods:Data Sources:PubMed,EMBASE,Web of Science.A systematic review of studies reporting exclusively endoscopic endonasal resections of benign orbital tumors was conducted.Articles not reporting orbital reconstruction details were excluded.Patient and tumor characteristics,operative details,and Outcomes were recorded.Variables were compared usingχ^(2),Fisher’s exact,and independentt tests.Results:Of 60 patients included from 24 studies,34(56.7%)underwent orbital reconstruction following resection.The most common types of reconstruction were pedicled flaps(n=15,44.1%)and free mucosal grafts(n=11,32.4%).Rigid reconstruction was uncommon(n=3,8.8%).Performance of orbital reconstruction was associated with preoperative vision compromise(POutcomes when considering all patients.In patients with intraconal tumors,however,there was a higher rate of short-term postoperative diplopia when reconstruction was foregone(P=0.041).This potential benefit of reconstruction did not persist:At an average of two years postoperatively,all patients for whom reconstruction was foregone either had improved or unchanged diplopia.Conclusion:Most Outcomes assessed did not appear affected by orbital reconstruction status.This general equivalence may suggest that orbital reconstruction is not a necessity in these ca

Potential impact of epidural labor analgesia on the Outcomes of neonates and children

This review summarizes recent evidences regarding the potential influences of epidural labor analgesia(ELA)on the Outcomes of neonates and children.Terms and relevant words including“ELA,”“ELA and neonatal Outcomes,”“ELA and children’s development,”and“ELA and children’s neurocognitive development”were used to search articles published in PubMed database up to October 2019.Original articles and reviews regarding potential influences of ELA on neonates and children were identified.Relevant references of the selected articles were also screened.The anesthetics used during ELA can be absorbed,enter the fetus,and produce neonatal depression;however,these effects are less severe than those during systematic opioid analgesia.The impact of anesthetic exposure during ELA on children’s neurodevelopment has not been fully studied,but would be mild if any.ELA increases the risk of intrapartum maternal fever;the latter may be harmful to neonatal Outcomes.The use of ELA may increase birth injury by increasing instrumental delivery,although long-term adverse events are rare.On the other hand,ELA may reduce maternal depression and,thus,produce favorable effects on neurocognitive development in childhood;but evidences are still lacking in this aspect.ELA may produce both favorable and unfavorable effects on neonates and children.These effects should be discussed with parturient women before making decisions.The potential harmful effects should be carefully managed.The overall impacts of ELA on neonatal and children’s Outcomes need to be studied further.

Beta-blockers and 1-year clinical Outcomes in hospitalized heart failure patients with atrial fibrillation

OBJECTIVE To assess the association between beta-blockers and 1-year clinical Outcomes in heart failure(HF)patients with atrial fibrillation(AF),and further explore this association that differs by left ventricular ejection fraction(LVEF)level.METHODS We enrolled hospitalized HF patients with AF from China Patient-centered Evaluative Assessment of Cardiac Events Prospective Heart Failure Study.COX proportional hazard regression models were employed to calculate hazard ratio of betablockers.The primary outcome was all-cause death.RESULTS Among 1762 HF patients with AF(756 women[41.4%]),1041(56%)received beta-blockers at discharge and 1272(72.2%)had an LVEF>40%.During one year follow up,all-cause death occurred in 305(17.3%),cardiovascular death occurred in203 patients(11.5%),and rehospitalizations for HF occurred in 622 patients(35.2%).After adjusting for demographic characteristics,social economic status,smoking status,medical history,anthropometric characteristics,and medications used at discharge,the use of beta-blockers at discharge was not associated with all-cause death[hazard ratio(HR):0.86;95%Confidence Interval(CI):0.65-1.12;P=0.256],cardiovascular death(HR:0.76,95%CI:0.52-1.11;P=0.160),or the composite outcome of all-cause death and HF rehospitalization(HR:0.97,95%CI:0.82-1.14;P=0.687)in the entire cohort.There were no significant interactions between use of beta-blockers at discharge and LVEF with respect to all-cause death,cardiovascular death,or composite outcome.In the adjusted models,the use of beta-blockers at discharge was not associated with all-cause death,cardiovascular death,or composite outcome across the different levels of LVEF:reduced(Outcomes,regardless of LVEF.

Severe aplastic anemia II(SAA-II)progresses from non-severe aplastic anemia(NSAA).The unavailability of efficacious treatment has prompted the need for haploidentical bone marrow transplantation(haplo-BMT)in patients lacking a human leukocyte antigen(HLA)-matched donor.This study aimed to investigate the efficacy of haplo-BMT for patients with SAA-II.Twenty-two patients were included and followed up,and FLU/BU/CY/ATG was used as conditioning regimen.Among these patients,21 were successfully engrafted,19 of whom survived after haplo-BMT.Four patients experienced grade II–IV aGvHD,including two with grade III–IV aGvHD.Six patients experienced chronic GvHD,among whom four were mild and two were moderate.Twelve patients experienced infections during BMT.One was diagnosed with post-transplant lymphoproliferative disorder and one with probable EBV disease,and both recovered after rituximab infusion.Haplo-BMT achieved 3-year overall survival and disease-free survival rate of 86.4%±0.73%after a median follow-up of 42 months,indicating its effectiveness as a salvage therapy.These promising Outcomes may support haplo-BMT as an alternative treatment strategy for patients with SAA-II lacking HLA-matched donors.

Infliximab therapy and Outcomes in patients with polyarticular juvenile idiopathic arthritis:a single-center study in China

Background Juvenile idiopathic arthritis(JIA)is a chronic inflammatory disease that includes seven heterogeneous subgroups with different prognoses.In particular,polyarticular JIA(pJIA)has a longer period of active disease and a poorer prognosis.Tumor necrosis factor(TNF)-alpha inhibitors are effective in patients with pJIA,but the therapeutic regimen remains controversial.Here,we performed a single-center study to determine the potential correlation between TNF-alpha inhibitor(infliximab)therapy and Outcomes in these patients.Methods Clinical data of 40 pJIA patients were collected at our center from January 1,2010 to January 1,2018,and patients were grouped according to the timing of infliximab therapy.The erythrocyte sedimentation rate(ESR),the number of joints with active disease,and the 27-point juvenile arthritis disease activity score(JADAS-27)were analyzed.Results The ESR,the active joint count,and the JADAS-27 decreased significantly in all groups after 3 months(P=0.041/0.415/0.008,0.022/0.030/Outcomes of patients with pJIA significantly,and should be introduced early during the clinical course.

Characteristics and Outcomes of patients with autoimmune pancreatitis after pancreatectomy:a retrospective study

Objective:Autoimmune pancreatitis(AIP)is a rare and special type of pancreatitis,which is often difficult to distinguish from pancreatic cancer.We aimed to discuss the clinical characteristics,operation characteristics,and therapeutic effects of AIP pa-tients who received surgical treatment based on retrospective analysis.Methods:The medical records of patients who underwent surgery at Peking Union Medical College Hospital from January 2012 to July 2021 and whose postoperative pathology was AIP were collected.Clinical symptoms,laboratory examinations,imaging features,surgical information,and prognostic data were analyzed.Results:Nineteen patients with AIP(median age 60 years old,14 males)underwent surgery,whose main indication for surgery was suspected of a malignant lesion(n=18,94.7%).Ten patients underwent pancreaticoduodenectomy,8 underwent distal pancreatectomy,and 1 underwent total pancreatectomy,the pathological result of whom were all AIP.The main postoperative complication was pancreatic fistula(n=11,57.9%),and one patient(5.3%)underwent a second operation due to gastrointestinal anastomotic bleeding.One patient(5.6%)had a recurrence of AIP,3 patients(16.7%)were treated with glucocorticoid after sur-gery,and one patient(5.6%)was treated with immunosuppressors.Conclusions:Differential diagnosis of AIP from pancreatic cancer is complex.Operation difficulty and complication incidence for patients with AIP were similar to pancreatic cancer patients.Although glucocorticoid is the first-line treatment for AIP,surgery may still be necessary for patients who are strongly suspected of a malignant lesion.

Ehlers-Danlos syndrome:prevalence and Outcomes in gender affirming surgery-a single institution experience

Aim:Patients with Ehlers-Danlos Syndrome(EDS)are considered to have an increased risk for wound healing complications.Surgeons may therefore be hesitant to offer elective surgeries,including gender-affirming surgeries(GAS),to EDS patients.At our center,we frequently encountered patients presenting for GAS evaluation with the co-existing diagnosis of EDS.This study aims to establish the prevalence of EDS diagnosis in our GAS patients and compare their post-operative complications to patients without EDS diagnosis.Methods:This is a single-institution retrospective case-control study on all patients who underwent GAS from 2016-2020.Data include EDS diagnosis,demographics,operation,and complications(including minor wound healing issues).Results:Of 1363 patients presenting for GAS,36(2.6%)had EDS diagnoses and were matched with 108 control patients.Major complications requiring surgical intervention in the OR occurred in 6 patients(4.2%),(2.8% EDS vs.5.4%controls;P=0.63),while 8.3% of EDS and 14% of controls required minor interventions(P=0.38).The rate of wound healing issues of any severity was 28% in EDS vs.47% in control groups(P=0.04).Conclusion:The prevalence of EDS diagnosis in our patient population is 132 times the highest reported prevalence in the general population.Wound healing issues and the need for additional post-operative interventions in the group with EDS diagnosis were not significantly different from the control group.Our findings suggest that patients with a diagnosis of EDS undergoing GAS have comparable Outcomes to patients without EDS.Concerns for post-operative complications should not be a barrier to offering GAS to patients presenting with an EDS diagnosis.
