Efficacy and safety of Shaoyang Xibi decoction in patients with knee osteoarthritis:a multi-center,single-blind,randomized controlled trial

PURPOSE: To observe the efficacy and safety of Shaoyang Xibi decoction(SYXBD) in patients with knee osteoarthritis(KOA), and to verify that the theory of "Shaoyang dominating bone" in Traditional Chinese Medicine(TCM) can be applied to KOA ***: Participants were randomly allocated to two groups: SYXBD(treatment group, n = 66)and Meloxicam(control group, n = 66). Western Ontario and McMaster Universities osteoarthritis Index(WOMAC) and 36-Item Short Form Health Survey(SF-36) were used to assess efficacy before the treatment and 8 weeks after the ***: Baseline data before the treatment between the two groups were similar. The WOMAC scores significantly decreased and the SF-36 scores significantly increased after 8-week treatment in both groups compared with before the treatment(P < 0.05). SYXBD significantly decreased pain scores(P < 0.001), physical function scores(P <0.001) and the total scores(P < 0.001) in WOMAC compared to Meloxicam. SYXBD significantly improved physical function(P = 0.021), bodily pain(P = 0.002) and general health(P = 0.014), with no significant difference in role emotional(P = 0.053),role physical(P = 0.517), vitality(P = 0.241), social function(P = 0.712) and mental health(P = 0.800)in SF-36 compared to Meloxicam. No adverse events were reported in the treatment group while13 adverse events happened in the control group during the ***: SYXBD, prepared based on the theory of "Shaoyang dominating bone", has a better curative efficay and safety in patients with KOA compared with Meloxicam. The TCM theory of "Shaoyang dominating bone" may be useful in KOA treatment.

Scraping therapy for knee osteoarthritis: a systematic review and Meta-analysis

OBJECTIVE:To systematically evaluate the efficacy of scraping therapy on knee osteoarthritis(KOA).METHODS:A computerized search was conducted in several databases,including PubMed,Web of Science,Cochrane Library,China National Knowledge Infrastructure Database,China Science and Technology Journal Database,Wanfang,and Chinese BioMedical Literature Database,from the establishment date of these databases to April 9,*** controlled trials(RCTs)on scraping therapy for KOA were collected in accordance with the“Participants-Intervention-Control-Outcome-Study design”*** methodological evaluation of the included studies was carried out using RoB 2.0,and Meta-analysis was conducted using *** Grades of Recommendations Assessment,Development and Evaluation(GRADE System)was applied to grade the quality of ***:A total of 12 RCTs with 997 subjects were included in the *** indicated that compared with the control group,the scraping therapy treatment group exhibited increased Lysholm scores[mean difference(MD)=4.96,95%confidence intervals(CI)(3.18,6.74),Posteoarthritis Index scores[MD=-6.77,95%CI(-8.99,-4.56),P<0.00001)].The quality of evidence obtained in this Meta-analysis was low according to the GRADE ***:The available evidence suggests that scraping therapy may have certain clinical efficacy in the treatment of ***,further high-quality RCTs are needed to confirm the findings.

Effect of acupuncture therapies combined with usual medical care on knee osteoarthritis

OBJECTIVE: To observe the effect of acupuncture or electroacupuncture(EA) combined with usual medical care for treating knee osteoarthritis(KOA).METHODS: A total of 90 patients with KOA were randomly allocated to 3 groups: usual care group(UC group, n = 30) was treated by pharmacological treatment of non-steroidal anti-inflammatory drugs(NSAIDs) and drugs for activating blood circulation(Ds-ABC), acupuncture(AP) combined with usual care group(UC group)(AP + UC group, n =30) and EA combined with UC group(EA + UC group, n = 30). The primary outcome measurements included pain visual analogue scale/score(VAS), Western Ontario and McMaster Universities osteoarthritis Index(WOMAC Index) and its subscales. Secondary outcome measurement was Assessment of Quality of Life instrument version of the 36-item Short Form Health Survey(AQoL-SF36).RESULTS: By the end of the 1 st week, AP + UC group and EA + UC group exhibited statistically significant improvements in primary outcome measures, except for WOMAC stiffness, compared with the UC group(P < 0.05). Moreover, the energy/fatigue domain of AQoL-SF36 in the AP + UC group showed better results than UC group(P < 0.05). By the end of the 2 nd week, all the primary outcome measures revealed that either the AP + UC or EA +UC group demonstrating remarkable advantages compared with the UC group(P < 0.05). The social functioning and general health domains of AQoLSF36 in the two acupuncture-intervention groups were improved significantly than UC group(P <0.05). We also found the energy/fatigue and emotional wellbeing domains of AQoL-SF36 in the EA +UC group demonstrated better results than UC group(P < 0.05).CONCLUSION: AP or EA combined with usual care is more effective than usual care alone for the treatment of KOA, the intervention of electric current in the process of acupuncture may improve more domains of AQoL-SF36 in KOA patients.

Effect of Gubi prescription on caveolin-1 expression and phosphoinositide 3 kinase/protein kinase B and Fas signal pathways in rats with knee osteoarthritis

OBJECTIVE: To investigate the effects of Gubi prescription on the expression of caveolin-1, and the phosphoinositide 3 kinase/protein kinase B(PI3 K/Akt) and Fas signal pathways in rats with knee osteoarthritis(KOA).METHODS: Forty KOA model rats were established using a modification of Hulth's method. Rats were divided into five groups by the random number method: model, positive drug(Vicolli group), and high-, medium-, and low-dose Gubi prescription groups(n = 8/group). In the sham surgery group(n = 8), only anterior and posterior cruciate ligaments of rats were exposed during surgery. A normal group(n = 8) consisted of rats with no treatment. Rats were intragastrically administered corresponding drugs once every day for eight consecutive weeks. Then, rat synovial membranes were extracted and histomorphological changes were recorded. m RNA expression was measured by *** superoxide dismutase(SOD), malondialdehyde(MDA), nitric oxide(NO), and interleukin 1β(IL-1β) levels were measured. Western blotting determined the effects of Gubi prescription on protein expressions of caveolin-1, Bax, Bcl-2, Fas, and caspase-3 in chondrocytes from KOA rats. The knee cartilage of rats was excised and cultured under aseptic conditions. After coincubation of chondrocytes with Gubi prescription-containing serum, IL-1β, and si RNA, Western blotting was used to determine the protein expressions of caveolin-1, Bax, Bcl-2, Fas, and ***: The morphological score of the articular synovium in the model group was significantly higher than in the normal group(P < 0.01). The morphological score in the high-and mediumdose Gubi prescription groups was lower than in the model group(P < 0.05). OBJECTIVE: To verify the action of non-pharmacological interventions, whole body vibration exercise (WBVE) and auriculotherapy (AT) on the management of knee osteoarthritis (KOA) analyzing the handgrip strength (HS). METHODS: One hundred twelve participants with KOA were allocated in (a) WBVE group with peak-to-peak displacement of 2.5 to 7.5 mm, frequency from 5 up to 14 Hz, acceleration peak from 0.12 up to 2.95 g (2 d/weekly for 5 weeks),(b) AT group, points of both ears (Kidney, knee-correspondent point and Shenmen) were stimulated with seeds,(c) WBVE + AT group and (d) respective control groups. HS was assessed in all the participants, in acute and cumulative responses. RESULTS: The intervention with WBVE alone and combined with AT improved (P < 0.05), in a cumulative response, the HS. CONCLUSION: WBVE alone or combined with AT might promote biological effects that interfere with the HS in individuals with KOA.

Effect of acupotomy versus electroacupuncture on ethology and morphology in a rabbit model of knee osteoarthritis

OBJECTIVE: To evaluate the treatment effect of acupotomy(Apo) in a rabbit model of knee osteoarthritis(KOA), compare the results of Apo versus electroacupuncture(E-Apu) on ethology, morphology, and structure of the articular cartilage surface in a rabbit model of KOA, and analyze the differences in the treatment effects of Apo versus ***: Twenty-eight male New Zealand white rabbits were randomized into four groups: normal control, blank model, Apo, and E-Apu(n = 7 in each group). Except for the normal control group, the left hindlegs of all rabbits were fixed in an extended position for 5 weeks to establish the KOA *** passive range of motion(PROM) and Lequesne index were measured before and after the establishment of the KOA model to assess the ethology in all groups. Warming moxibustion attenuates inflammation and cartilage degradation in experimental rabbit knee osteoarthritis

Protocol-optimizing study of combining Tuina and horse-riding squat exercise for knee osteoarthritis
