Social insights on the implementation of One Health in zoonosis prevention and control: a scoping review

Background:The One Health (OH) concept has been promoted widely around the globe. OH framework is expected to be applied as an integrated approach to support addressing zoonotic diseases as a significant global health issue and to improve the efficiency and effectiveness of zoonosis prevention and control. This review is intended to overview the social impact of the implementation of OH on zoonosis prevention and control.Methods:A scoping review of studies in the past 10 years was performed to overview the integration feature of OH in zoonosis prevention and control and the social impacts of OH. PubMed and Web of Science were searched for studies published in English between January 2011 and June 2021. The included studies were selected based on predefined criteria.Results:Thirty-two studies were included in this review, and most of them adopted qualitative and semi-qualitative methods. More than 50% of the studies focused on zoonosis prevention and control. Most studies were conducted in low- and middle-income countries in Africa and Asia. Applying OH approach in diseases control integrates policymakers, stakeholders, and academics from various backgrounds. The impact of OH on economic is estimated that it may alleviate the burden of diseases and poverty in the long term, even though more financial support might be needed at the initial stage of OH implementation. OH implementation considers social and ecological factors related to zoonosis transmission and provides comprehensive strategies to assess and address related risks in different communities according to regions and customs.Conclusions:Based on reviewed literature, although there seems to be a lack of guidelines for assessing and visualizing the outcomes of OH implementation, which may limit the large-scale adoption of it, evidence on the contributions of implementing OH concepts on zoonosis prevention and control indicates long-term benefits to society, including a better integration of politics, stakeholders and academics to improve their cooperation,

Background One Health approach is crucial to tackling complex global public health threats at the interface of humans, animals, and the environment. As outlined in the One Health Joint Plan of Action, the international One Health community includes stakeholders from different sectors. Supported by the Bill & Melinda Gates Foundation, an academic community for One Health action has been proposed with the aim of promoting the understanding and real-world implementation of One Health approach and contribution towards the Sustainable Development Goals for a healthy planet.Main text The proposed academic community would contribute to generating high-quality scientific evidence, distilling local experiences as well as fostering an interconnected One Health culture and mindset, among various stakeholders on different levels and in all sectors. The major scope of the community covers One Health governance, zoonotic diseases, food security, antimicrobial resistance, and climate change along with the research agenda to be developed. The academic community will be supported by two committees, including a strategic consultancy committee and a scientific steering committee, composed of influential scientists selected from the One Health information database. A workplan containing activities under six objectives is proposed to provide research support, strengthen local capacity, and enhance global participation.Conclusions The proposed academic community for One Health action is a crucial step towards enhancing communication, coordination, collaboration, and capacity building for the implementation of One Health. By bringing eminent global experts together, the academic community possesses the potential to generate scientific evidence and provide advice to local governments and international organizations, enabling the pursuit of common goals, collaborative policies, and solutions to misaligned interests.

One Health: navigating plague in Madagascar amidst COVID-19

Background Africa sees the surge of plague cases in recent decades,with hotspots in the Democratic Republic of Congo,Madagascar,and Peru.A rodent-borne scourge,the bacterial infection known as plague is transmitted to humans via the sneaky bites of fleas,caused by Yersinia pestis.Bubonic plague has a case fatality rate of 20.8%with treatment,but in places such as Madagascar the mortality rate can increase to 40–70%without treatment.Main text Tragedy strikes in the Ambohidratrimo district as three lives are claimed by the plague outbreak and three more fight for survival in the hospitals,including one man in critical condition,from the Ambohimiadana,Antsaharasty,and Ampanotokana communes,bringing the total plague victims in the area to a grim to five.Presently,the biggest concern is the potential plague spread among humans during the ongoing COVID-19 pandemic.Effective disease control can be achieved through training and empowering local leaders and healthcare providers in rural areas,implementing strategies to reduce human–rodent interactions,promoting water,sanitation and hygiene practices(WASH)practices,and carrying out robust vector,reservoir and pest control,diversified animal surveillance along with human surveillance should be done to more extensively to fill the lacunae of knowledge regarding the animal to human transmission.The lack of diagnostic laboratories equipped represents a major hurdle in the early detection of plague in rural areas.To effectively combat plague,these tests must be made more widely available.Additionally,raising awareness among the general population through various means such as campaigns,posters and social media about the signs,symptoms,prevention,and infection control during funerals would greatly decrease the number of cases.Furthermore,healthcare professionals should be trained on the latest methods of identifying cases,controlling infections and protecting themselves from the disease.Conclusions Despite being endemic to Madagascar,the outbreak’s pace is unparalleled,and it ma

Roadmap to tackle antibiotic resistance in the environment under the One Health framework

Antibiotic resistance has been recognized as a major challenge worldwide for humans.“One Health”has been recognized as a key concept for containment of antibiotic resistance.Under the framework,the role of the environment in the development of antibiotic resistance genes(ARGs)has become increasingly obvious.Despite numerous efforts,response to antibiotic re-sistance is considered to be inadequate,which is probably due to the lack of a clear roadmap.Here,we propose a“One Health”roadmap to combat antibiotic resistance in the environment through(1)understanding environmental resistome.The environmental gene pool has long been recognized as the single largest reservoir of both known and novel ARGs.(2)Standardizing ARG quantification.Systematic joint efforts based on standardized quantification are urgently needed to un-derstand the true tempospatial profiles of the environmental resistome.(3)Identifying mechanisms of resistome development.Horizontal gene transfer and co-selection have been recognized as the two main mechanisms contributing to the environmental resistome.(4)Establishing a risk-assessment framework.The first critical step for large-scale cost-effective targeted ARG management in the environment is the risk assessment to identify the priority ARGs for control.(5)Formulating regulatory standards.By correlating the environmental ARG profile with public health,we may identify the indicator ARGs that can be integrated into current environmental quality standards.(6)Developing control strategies.Systematic analysis of available control technologies is required to identify the most feasible ones to curtail the spread of ARGs in the environment.The proposed roadmap under the“One Health”framework provides a guide to tackle antibiotic resistance in the environment.

Due to the increase in human population,habitat destruction,and the close interaction between humans,animals and the environment,these dynamic changes are a threat to the health of human,animal and environment.Therefore,we required intervention consisting of medical,veterinary,environment and other relevant disciplines and sectors in combination.If biological and environmental health is supposed to be optimum,cooperation and coordination is necessary between the multi‐sectoral stakeholders responsible for the efforts.It not limits to a region or country on a worldwide scope.One Health encourages a collaborative,coordinated,and multidisciplinary approach to ensure the health and wellbeing of humans,animals,and the environment across different spatial levels,it aims to optimal health for lives and the environment,and recognizing the interconnection between people,animals,plants,and their shared environment.This review discussed the essential to integrate One Health in the areas of infectious diseases,food safety,antimicrobial resistance,ecological environment,and chronic diseases etc.,and discuss the practice of One Health in each area with some examples,hoping that One Health will serve as a framework to solve the challenges and issues facing China.

Coronavirus disease 2019(COVID-19)is as an emerging infectious disease(EID)that has caused the worst public health catastrophe of the 21st century thus far.In terms of impact,the COVID-19 pandemic is second only to the Spanish Flu pandemic of 1918 in modern world history.As of 7 September 2021,there have been 220 million confirmed cases of COVID-19 and more than 4.5 million deaths.EIDs pose serious public health and socio-economic risks,and 70%of EIDs originate from wildlife.Preventing development of EIDs such as COVID-19 is a pressing concern.Here,taking the COVID-19 pandemic as an example,we illustrate the disastrous effects of EIDs and assess their emergence and evolution from a One Health perspective.We propose a One Health strategy,centered on‘moving the gates forward’,for EID prevention and control at the human-animal-environment interface.This strategy may be instructive and provide early warnings of EIDs in the future.

There are increasing concerns over the threat of nanoplastics to environmental and human health.However,multidisciplinary barriers persist between the communities assessing the risks to environmental and human health.As a result,the hazards and risks of nanoplastics remain uncertain.Here,we identify key knowledge gaps by evaluating the exposure of nanoplastics in the environment,assessing their bio-nano interactions,and examining their potential risks to humans and the environment.We suggest considering nanoplastics a complex and dynamic mixture of polymers,additives,and contaminants,with interconnected risks to environmental and human health.We call for comprehensive integration of One Health approach to produce robust multidisciplinary evidence to nanoplastics threats at the planetary level.Although there are many challenges,this holistic approach incorporates the relevance of environmental exposure and multi-sectoral responses,which provide the opportunity to identify the risk mitigation strategies of nanoplastics to build resilient health systems.

抗生素耐药性(antimicrobial resistance,AMR)是一个复杂的全球性问题,如今也被视为一个重要的One Health问题。One Health相互关联的领域促成了抗生素耐药微生物在地方和全球范围内的出现、演变和传播,这是全球健康的一个重要风险因素。鉴于抗生素耐药性的持续存在和传播,以及在人、动物和环境方面的相互依赖性,采取One Health的策略解决AMR问题是有意义的。肺炎克雷伯菌是一种重要的人兽共患机会致病菌,同时也是公认的耐药基因在不同生态位之间的贩运者。其中产超广谱β-内酰胺酶的肺炎克雷伯菌已被世界卫生组织列入至关重要的最优先级关注的病原微生物。CTX-M型β-内酰胺酶则是目前最流行的超广谱β-内酰胺酶(Extended-spectrum β-lactamases,ESBLs),可通过质粒、插入序列等可移动元件在不同来源肺炎克雷伯菌间传播,对公共卫生健康造成一定的威胁。本研究旨在基于One Health理念,探究bla阳性肺炎克雷伯菌在人、动物和环境界面的流行病学分布,并通过全基因测序技术分析不同来源的bla阳性肺炎克雷伯菌的特点及其传播相关性。本研究从扬州市共采集2470份不同来源的样品,分离到的332份头孢噻肟耐药肺炎克雷伯菌,其中人源103株,动物来源112株,环境来源117株。采用琼脂稀释法或微量肉汤稀释法测定分离株对16种抗生素的最小抑菌浓度(minimum inhibitory concentration,MIC),结果发现所有分离株对四环素、头孢唑林和复方新诺明的耐药率较高,分别为93.67%、91.27%和85.54%,对环丙沙星、氯霉素、萘啶酸等耐药率也均在50.00%以上。对美罗培南、替加环素和多粘菌素的耐药率相对较低,分别为25.60%、18.98%和3.31%。不同来源的分离株对部分抗生素的耐药率表现出明显的差异,尤其是人源菌株对美罗培南、替加环素和阿米卡星的耐药率显著高于其他来源,动物源菌株则是对四环素和庆大霉素的耐药率显著高于其他来源。同时,头孢噻肟耐药肺炎克雷伯菌多重耐药现象严重,所有分离株均为多重耐药菌,18.07%的菌株对11种抗生素耐药。对332株分离株进行全基因组测序,结果发现,所有分离株均携带bla基因,主要分为7个不同的变体。bla-24仅存在于环境源分离株,其余6中变体在人、动物和环境菌株中均被检测到。332株分离株除bla基因外,还携带多种抗生素耐药基因,例如氨基糖苷类(aac(3)-Ⅱd、aac(6’)-Ib-cr)、喹诺酮类(qnrS1)、四环素类(tet(A))、磺胺类(sul1、sul2)等。值得注意的是,在人、动物和环境源分离株中均检测到替加环素耐药基因tmexC1-tmexD1-toprJ1以及人和环境分离株中检测到碳青霉烯类耐药基因blaKPC-2、blaNDM-1和blaNDM-5。人、动物和环境共享耐药基因。bla阳性肺炎克雷伯菌ST类型丰富,不同界面均存在同一 ST型分离株。其中ST11、ST15、ST45和ST290均存在于人、动物和环境分离株。核心基因组SNP系统发育树表明存在同一 ST型不同来源分离株间共享非常少量的SNPs差异,且具有相似的耐药基因和毒力基因。这说明不同来源的分离株存在克隆关系。同时所有分离株均携带多种质粒类型,其中IncF质粒(311/332,93.67%)最为常见,是携带bla的主要群体。为进一步探究bla的传播,对部分菌株三代测序,结果显示bla-14基因主要位于大小不等的IncFIB-IncFII质粒,存在染色体定位。BLAST比较发现人与环境或动物分离株存在高度相似的质粒,例如携带blacCTX-M-14的环境源菌株质粒p22HJ094与人源菌株质粒pSBH247覆盖率为96%,核酸一致性为99.99%。不论是质粒携带还是染色体定位,均共享核心保守的遗传结构ISEcp1-bla-14-IS903B。bla基因同样主要位于IncFIB-IncFII质粒,存在染色体定位。遗传环境分析显示,均存在2959bp的核心遗传结构ISEcp1-bla-Δorf477。这两种核心遗传结构均位于更大的多重耐药区,该区域还包括aac(6’)-Ib-cr,strA/B,blaOXA-1,sul1和tet(A)等多个抗生素抗性基因以及穿插多个不同的完整或截短的插入序列和转座子(如IS26、IS5075、IS6100、IS3000和ΔTnAs1等)。这些耐药基因与插入序列形成常见的耐药盒式阵列,多个抗性模块的获得证实了插入序列在耐药基因传播和质粒进化中的重要作用,促进了耐药基因的共转移。综上所述,扬州市bla阳性肺炎克雷伯菌在人、动物和环境中广泛分布,多重耐药现象严重。人、动物和环境菌株存在的共享耐药基因、有限的克隆关系、相似的多重耐药质粒和核心遗传结构表明bla阳性肺炎克雷伯菌在人、动物和环境间存在关联和传播风险。

随着新冠肺炎(COVID-19)的暴发,野生动物、生物多样性和人类健康的关系再次引起广泛讨论。近20年来,国际社会对于生物多样性与健康的研究日益增多,并将它作为生物多样性保护与研究的重要方向之一。One Health作为一个新的理念框架,通过交叉学科的研究和行动来推动包括人、所有其他动物及环境的健康。这个理念被不同国家、国际组织及协定所接纳及推广,包括《生物多样性公约》等。本文通过总结近些年生物多样性对健康的影响方式、One Health的定义与发展历史、进入生物多样性议程的过程,提出中国应用One Health改进相关野生动物管理以降低公共卫生危机的可能性的建议,以及onehealth框架内增强生物多样性保护所需的研究方向。onehealth在中国的应用与发展应重视生物多样性研究和保护在其中的作用,利用在景观生态学、群落内物种关系动态变化、气候变化影响、土地利用变化模式与趋势的研究,与人类健康相结合,提高One Health在应对公共健康和环境健康风险方面的准确性与及时性。同时,需要加强我国在野生动物管理方面的投入和力度,增强生物多样性保护与公共健康的联系,将预警与干预措施前移,减少疾病暴发带来的社会经济成本。
