OBJECTIVE: To investigate the effect of Huangqi decoction( 黄芪汤) on renal interstitial fibrosis and its association with the transforming growth factor-β1(TGF-β1)/mitogen-activated protein kinase(MAPK) signaling pathway. METHODS: 120 C57/BL mice were randomly divided into six groups: sham group, Enalapril(20 mg/kg) group, 5/6 Nephrectomy model group, and 5/6 Nephrectomy model plus Huangqicoction(0.12, 0.36 and 1.08 g/kg respectively) groups. Detecting 24hours urinary protein, blood pressure, serum creatinine, urea nitrogen content changes. Periodic Acid-Schiff stain(PAS) and Masson’s trichrome staining was used to observe the renal tissue pathological changes. Protein expression of TGF-β1, Phosphorylated P38 mitogen activated protein kinases(P-P38), Phosphorylated c-jun N-terminal kinase(P-JNK), Phosphorylated extracellular regulated proteinhnase(PERK), Fibroblast-specific protein-1(FSP-1), Alpha smooth muscle actin(α-SMA), Type Ⅲ collagen(Collagen Ⅲ), Connective tissue growth factor(CTGF),Bcl-2 Assaciated X protein(Bax) and B cell lymphoma 2(Bcl-2) were measured with western blot and immunohistochemical. RESULTS: Both Huangqi decoction and Enalapril improved the kidney function, 24 h urinary protein and the fibrosis in 5/6 Nephrectomy mice, Huangqi decoction downregulated the expressions of TGF-β1, FSP-1, α-SMA, Collagen Ⅲ and CTGF in a dose-dependent manner, and it has a significant difference(P < 0.01) compared with model group.Huangqi decoction downregulated the expressions of P-P38, P-JNK, P-ERK and Bcl-2 in a dose-dependent manner, while upregulated the expression of Bax. CONCLUSIONS: The protective effect of Huangqi decoction for renal interstitial fibrosis in 5/6 nephrectomized mice via the inhibition of EpithelialMesenchymal Transitions and downregulating the TGF-β1/MAPK signaling pathway.

Background:Although the impact of tumor complexity on peri-operative outcomes has been well established using several nephrometry scoring systems,the impact of adherent perirenal fat remains poorly defined.This study aimed to develop a novel nephrometry scoring system for predicting the peri-operative outcomes of laparoscopic partial Nephrectomy(LPN)by integrating and optimizing the RENAL score(RNS)and Mayo adhesive probability(MAP)score.Methods:We retrospectively evaluated 159 patients treated with retroperitoneal LPN.The patients’demographic parameters,RNSs,and MAP scores were evaluated as potential predictors of perioperative outcomes,including operation time,estimated blood loss(EBL),and margin,ischemia,and complication(MIC)achievement rate.The independent predictors were used to develop a novel nephrometry scoring system.The predictive value and inter-observer agreement for the novel nephrometry scoring system were evaluated.Results:Tumor radius(R score),nearness to the renal sinus or collecting system(N score),and posterior perinephric fat thickness were independent predictors of peri-operative outcomes and were used to develop the RNP score.The univariate analysis revealed that the RNP score was significantly associated with operation time,EBL,and MIC achievement rate(P<0.050).The RNP score was an independent predictor of operation time(P<0.001),EBL(P=0.018),and MIC achievement rate(P=0.023)in the multivariate analysis.The RNP score was not inferior to RNS in the area under the curve for predicting peri-operative outcomes and performed better in inter-observer agreement(76.7%vs.57.8%)and kappa value(0.804 vs.0.726).Conclusion:The RNP score,combining the advantages of the RNS and MAP score,demonstrated a good predictive value for the peri-operative outcomes of retroperitoneal LPN and better inter-observer agreement.

BACKGROUND Renal duplication is a common deformity of the urinary system,with an incidence of approximately 1/125 in children.Symptomatic patients with hydronephrosis,vesicoureteral reflux,or incontinence may require surgical interventions.Laparoscopy and retroperitoneoscopy are the two main accesses for partial Nephrectomy.CASE SUMMARY A 9-year-old child was admitted to the hospital for hydronephrosis of the left kidney.Ultrasonography showed that the left kidney was larger,approximately 12.6 cm×6.3 cm×5.5 cm in size,with visible separation of the pelvis and an obviously separated lower portion.The upper segment of the left ureter was dilated(approximately 2.6 cm in width),and no significant dilation was observed in the middle and upper segments.The right kidney and ureter were normal.Primary diagnosis was left renal duplication malformation and hydronephrosis.Retroperitoneal laparoscopic Nephrectomy and ureterectomy were performed.Intraoperative exploration revealed a dilated pelvis and thin renal parenchyma at the lower pole of the left kidney.The upper left kidney was smaller than normal,and the pelvis and ureter were larger than normal.The renal artery was blocked for 40 min.A hemolock was used to clamp down the kidney ureter,and a drainage tube was retained in the retroperitoneal cavity.The operation was uneventful,and the estimated amount of blood loss was 100 mL.Total abdominal drainage amount was 116 mL.The drainage tube was removed on postoperative day(POD)3 and the patient was discharged on POD6.The pathological diagnosis confirmed the atrophy of the renal parenchyma,the dilation of the renal pelvis,hydronephrosis,and ureteral cystic dilation.CONCLUSION The retroperitoneoscopic approach for partial Nephrectomy is feasible and effective in selective pediatric patients with a duplex kidney.

Objective:Once chronic inflammatory renal disease(IRD)develops,it creates a severe peri-fibrotic process,which makes it a relative contraindication for minimally invasive surgery(MIS).Our objective is to show that laparoscopic Nephrectomy(LN)is a surgical option in IRD with fewer complications and better outcomes.Methods:Retrospective review of patients who underwent a modified-surgical laparoscopic transperitoneal Nephrectomy was performed.Data search included all operated patients between May 2013 and May 2018 that had a pathology result with any renal inflammatory condition(xanthogranulomatous pyelonephritis,chronic nephritis,and renal tuberculosis).We describe intra-operative variables such as operative time,blood loss,conversion rate,postoperative complications and length of hospital stay.Results:There were 51 patients who underwent laparoscopic Nephrectomy with a confirmatory pathology report for IRD.We identified four(8%)major complications;three of them required transfusion and one conversion to open surgery.The mean operative time was 233108 min.Mean estimated blood loss was 206242 mL excluding the conversion cases and 281423 mL including them.The mean length of hospital stay was 3.02.0 days.Conclusion:Laparoscopic Nephrectomy for IRD can safely be done.It is a reproducible technique with low risks and complication rates.Our experience supports that releasing the kidney first and leaving the hilum for the end is a safe approach when vascular structures are embedded into a single block of inflammatory and scar tissue.

The impact of intra-operative cell salvage during open Nephrectomy

Objective:To assess the impact of intra-operative cell salvage on outcomes in open Nephrectomy.Methods:A retrospective cohort study was performed of all patients undergoing open Nephrectomy for suspected malignancy from 1 October 2013 to 1 October 2017.Patients were grouped and compared based on whether they received intra-operative cell salvage(ICS).Primary outcomes were allogeneic transfusion rates(ATRs),and if histology confirmed cancer,disease recurrence.Secondary outcomes were complications and transfusion-related cost.Results:Forty patients underwent open Nephrectomy for suspected malignancy during the enrolment period.Sixteen patients received ICS while 24 did not(standard group).Compared with the standard group,ICS patients had similar median age(63.5 vs.61.0 years;p=0.83)but fewer females(19%vs.58%;p=0.013).The groups were similar in pre-operative and discharge haemoglobin,Charlson Comorbidity Index,length of hospital stay and proportion with thoracoabdominal surgical approach.The ICS group had a smaller proportion undergoing partial Nephrectomy(19%vs.54%;p=0.025)and shorter median follow-up(278 vs.827 days;p=0.0005).Histology was malignant for 14 ICS and 15 standard patients.The ICS group had more frequentT2 disease(79%vs.27%;p=0.005).There were no positive margins.Both groups had similar ATRs(6%vs.4%;p=0.96),complication rates(19%vs.29%;p=0.46)and recurrence rates(18%vs.7%;p=0.40).Transfusion costs were higher amongst ICS patients(AUD$878.18 vs.$49.65 per patient).Conclusion:ICS appears safe,with low rates of recurrence and complication.Both groups had low ATRs,and therefore cost benefit for ICS was not seen.

To the Editor:Laparoscopic radical Nephrectomy(LRN)is the standard of care for patients with T2 tumors and localized masses not treatable by partial Nephrectomy according to the European Association of Urology Guidelines on renal cell carcinoma.[1]Transperitoneal and retroperitoneal are two main approaches in LRN.With the advantages of easier hilar control,shorter operation time,and less bowel irritation,retroperitoneal approach is preferable in our center.The peritoneal tear is a common intraoperative complication during retroperitoneoscopic procedures,leading to the collapse of the retroperitoneal space.

To the Editor:Erector spinae plane block (ESPB) has been reported to provide effective analgesia for various indications, including thoracic and breast surgery.[1] To the best of our knowledge, there is no case in which the ESPB provide postoperative analgesia in open Nephrectomy except for one pediatric patient.[2] We report a case in which intermittent ESPB provides effective pain control as a part of multimodal analgesia after open Nephrectomy.

We herein reported a 27-year-old woman with a right renal mass for two years.She underwent laparoscopic partial Nephrectomy.Immunohistochemical examination of the specimen confirmed the diagnosis of solitary fibrous tumor by revealing its positive staining for cluster of differentiation(CD)34,epithelial membrane antigen(EMA),B-cell lymphoma-2(Bcl-2)and CD99 in the tumor cells.No adjuvant treatment was carried out.The patient was in good health without local recurrence or metastasis during 2 years of follow-up.Laparoscopic partial Nephrectomy for renal solitary fibrous tumor is an alternative treatment to radical Nephrectomy.It can provide a good outcome.However,further follow-up and more cases of renal solitary fibrous tumor treated with laparoscopic partial Nephrectomy are necessary to compare the oncological outcome with radical Nephrectomy.

Preliminary experience with transperitoneal laparoendoscopic single-site radical Nephrectomy using a home-made single-port device in China

Background Laparoendoscopic single-site （LESS） surgery represents a technique to further reduce morbidity and scarring associated with surgery.We present our preliminary experience with transperitoneal LESS radical Nephrectomy （RN） using a home-made single-port device in China.Methods From July 2010 to November 2011,eleven patients with renal tumor not greater than T2 underwent LESS-RN by an experienced laparoscopic surgeon.A home-made single-port device was used through a 5-cm umbilical incision.A combination of standard and articulating laparoscopic instruments was used.The sequence of steps of LESS-RN was similar to transperitoneal laparoscopic RN.Patient characteristics,perioperative variables and postoperative outcomes were recorded and analyzed.Results Except for two transperitoneal laparoscopic conversions and one hand-assisted laparoscopic conversion,the other procedures were completed successfully without conversion to open surgery.The mean operative time was 224.5 （155-297） minutes,estimated blood loss was 270.9 （50-900） ml,and hospital stay was 10.4 （5-15） days.The mean visual analog pain scale （VAPS） on the first postoperative day was 4.0/10.Final pathological analysis revealed renal cell carcinoma in all cases with a stage distribution of three T1a,five T1b,and three T2a tumors.With the mean followup period of 21.4 （12-28） months,all patients were alive without evidence of tumor recurrence or metastasis,and were satisfied with the appearance of the scars.Conclusion Transperitoneal LESS-RN using a home-made single-port device is technically feasible and safe in a selected group of patients （low body mass index and stage tumor） and has excellent cosmetic results.Although preliminary oncologic outcome is not compromised,the long-term evaluation of these patients is awaited.
