Background: Neck pain caused by cervical spondylosis(CS) is a chronic pain condition, with an increasingly high incidence in the general population. Electroacupuncture is a common analgesic modality that has been widely applied in Neck pain treatment. However, current electroacupuncture instruments used in the clinic have low intelligence levels and obscure parameter standards. We here designed this study for assessing the effect and safety of a new, intelligent electroacupuncture instrument, the CX-DZ-Ⅱ, in treating Neck pain. Methods: The present study is a prospective, two-center, randomized, controlled, open-label, non-inferiority trial for CX-DZ-Ⅱ on treating Neck pain caused by CS. Totally 160 eligible patients will be included in this trial and randomly assigned to an experimental group and a control group in a 1:1 ratio. A semi-standard acupoint selection strategy will be employed. In the experimental group, selected acupoints will be stimulated by CX-DZ-Ⅱ. Electroacupuncture treatment will be accomplished by a pre-existing electroacupuncture instrument in the control group. The duration of treatment will be 2 weeks. The primary outcome is the change of Visual Analog Scale(VAS) score after one course of treatment. The secondary outcomes include the VAS scores after each treatment, the responder rate, drug-usage rate of non-steroidal antipyretic analgesics, the rate of adverse events occurrence, and the performance of instrument. Discussion: This study will evaluate the effect and safety of the CX-DZ-Ⅱ intelligent electroacupuncture therapeutic instrument in comparison with a pre-existing non-intelligent instrument in the treatment of Neck pain caused by CS. The results will hopefully demonstrate a more optimal electroacupuncture instrument for the treatment of Neck pain.(Trial registration No. gov NCT03005301).

Background:Head and Neck squamous cancer(HNSC)frequently occurs in the clinic.Revealing the role of the genes that correlate with cancer cell outgrowth will contribute to potential treatment target identification and tumor inhibition.Methods:The gene expression profiles and gene ontology of the proton-sensing G-protein-coupled receptor OGR1 were analyzed using the TCGA(The Cancer Genome Atlas)database.The effects of sex,age,race,and degree of malignancy on HNSC were investigated,and the survival times of HNSC patients with high or low/medium expression levels of OGR1 were compared.Methylation of the OGR1 promoter CpG sites was also investigated and OGR1-related genes were analyzed using gene set enrichment analysis.Results:OGR1 is overexpressed in HNSC patients.However,compared with the low/median expression group,the high OGR1 expression group did not have different survival rates.The OGR1 expression level differed across sex,age,race,and degree of malignancy,while the methylation of the OGR1 promoter CpG sites was maintained at a similar level.Gene set enrichment analysis revealed that OGR1 was positively correlated with head and Neck cancer,cisplatin resistance,hypoxia,angiogenesis,cell migration,and TGF-β.Conclusion:The expression of OGR1 correlated with HNSC progression and survival and thus can serve as a potential treatment target and prognostic marker.

Background:Refractory femoral Neck fractures cannot be anatomically reduced by closed traction reduction which may affect fracture healing.We evaluated the biomechanical effects of positive,negative,and anatomic reduction of various degrees of displacement in Pauwels I femoral Neck fractures by a finite element analysis.Methods:Five reduction models of Pauwels type I femoral Neck fracture were established using the Mimics 17.0(Materialize,Leuven,Belgia)and Hypermesh 12.0(Altair Engineering,Troy,MI,USA).According to the degree of fracture displacement,there were three models of positive support,an anatomic reduction model,and a negative 2mm reduction model.Finite element analysis was conducted using the ABAQUS 6.9 software(Simulia,Suresnes,France).The von Mises stress distribution and the stress peak of internal fixation in different models,the displacement between fracture blocks,and the principal strain of the femoral Neck cancellous bone model were recorded under the axial stress of 2100 N.Results:The peak von Mises stress on screw of each model was located at the thread of the screw tip.The peak von Mises stress was the lowest at the tip of the anatomic reduction model screw(261.2 MPa).In the positive 4mm model,the von Mises stress peak was the highest(916.1 MPa).The anatomic reduction model showed the minimum displacement(0.388 mm)between fracture blocks.The maximum displacement was noted in the positive 4mm model(0.838 mm).The displacement in the positive 3mm model(0.721 mm)was smaller than that in the negative 2mm model(0.786 mm).Among the five models,the strain area of the femoral Neck cancellous bone was mainly concentrated around the screw hole,and the area around the screw hole could be easily cut.Conclusions:Compared with negative buttress for femoral Neck fracture,positive buttress can provide better biomechanical stability.In Pauwel type I fracture of femoral Neck,the range of positive buttress should be controlled below 3mm as far as possible.

This contribution reports on a case involving the death of a female with a history of depression in treatment found dead in her house,with her Neck compressed by a hydraulic lift elevator used for domestic purposes.The manner of death was determined as suicide based on an on-scene investigation and witness testimony along with a comprehensive postmortem examination.This case is reported on the grounds of its rarity and it highlights the potential danger of using advanced domestic equipment in the elder people’s residencies,especially those having a mental state of self-destruction.

BACKGROUND Squamous cell carcinoma of head and Neck(SCCHN) is the fifth most common cancer worldwide. Inhibition of epidermal growth factor receptor signaling has been shown to be a critical component of therapeutic option. Herein, we report a case of durable complete response to erlotinib.CASE SUMMARY An 81-year-old Caucasian male who presented with metastatic poorly differentiated squamous cell carcinoma of right cervical lymph nodes(levels 2 and 3). Imaging studies including(18)F-fluorodeoxyglucose positron emission tomography/computed tomography(CT) and contrast-enhanced CT scan of Neck and chest did not reveal any other disease elsewhere. Panendoscopic examination with random biopsy did not reveal malignant lesion in nasopharynx,oropharynx, and larynx. He underwent modified Neck dissection and postoperative radiation. Within 2 mo after completion of radiation, he developed local recurrence at right Neck, which was surgically removed. Two mo after the salvage surgery, he developed a second recurrence at right Neck. Due to suboptimal performance status and his preference, he started erlotinib treatment.He achieved partial response after first 2 mo of erlotinib treatment, then complete response after total 6 mo of erlotinib treatment. He developed sever skin rash and diarrhea including Clostridium difficile infection during the course of erlotinib treatment requiring dose reduction and eventual discontinuation. He remained in complete remission for more than two years after discontinuation of erlotinib.CONCLUSION We report a case of metastatic SCCHN achieving durable complete response from erlotinib. Patient experienced skin rash and diarrhea toxicities which were likely predictors of his treatment response.

A Neck scar contracture can severely and negatively affect the function of mastication, phonic, or breathing and result in Neck pain and issues with esthetics. The best way is of course to avoid such contracture by means of non-surgical treatment such as use of a growth factor. The basic fibroblastic growth factor is clinically well proven in decreasing scar formation and improving healing. There are numerous reconstructive methods for Neck contracture, especially when the lesions are relatively limited in part of the Neck. However, a very severe and full circumferential scar contracture requires extensive reconstruction. The thin groin flap is one of the answers and well matches with the tissue texture and maintains the flexibility. Even with extensive burns and delayed reconstructions due to resuscitation first, the groin area is well preserved and can be safely harvested by dual vasculature systems of the superficial circumflex iliac artery and superficial epigastric artery, which warrant more reliability compared to the perforator flaps in this area. More demanding and stringent forms of the Neck burn scar contracture are the sequelae of radiation. A radiation burn or radiation injury can be progressing and hard to heal. Adipose-derived stem cells can reverse the scar contracture as the surrounding tissue is softened and can accelerate wound healing. In this review, different types of Neck burn scar contracture and reconstructive methods are summarized, including innovative use of bFGF and ADSCs in the management of difficult wound healing and scar contracture.

BACKGROUND Odontogenic infection is one of the common infectious diseases in oral and maxillofacial head and Neck regions.Clinically,if early odontogenic infections such as acute periapical periodontitis,alveolar abscess,and pericoronitis of wisdom teeth are not treated timely,effectively and correctly,the infected tissue may spread up to the skull and brain,down to the thoracic cavity,abdominal cavity and other areas through the natural potential fascial space in the oral and maxillofacial head and Neck.Severe multi-space infections are formed and can eventually lead to life-threatening complications(LTCs),such as intracranial infection,pleural effusion,empyema,sepsis and even death.CASE SUMMARY We report a rare case of death in a 41-year-old man with severe odontogenic multi-space infections in the oral and maxillofacial head and Neck regions.One week before admission,due to pain in the right lower posterior teeth,the patient placed a cigarette butt dipped in the pesticide"Miehailin"into the"dental cavity"to relieve the pain.Within a week,the infection gradually spread bilaterally to the floor of the mouth,submandibular space,Neck,chest,waist,back,temporal and other areas.The patient had difficulty breathing,swallowing and eating,and was transferred to our hospital as an emergency admission.Following admission,oral and maxillofacial surgeons immediately organized consultations with doctors in otolaryngology,thoracic surgery,general surgery,hematology,anesthesia and the intensive care unit to assist with treatment.The patient was treated with the highest level of antibiotics(vancomycin)and extensive abscess incision and drainage in the oral,maxillofacial,head and Neck,chest and back regions.Unfortunately,the patient died of septic shock and multiple organ failure on the third day after admission.CONCLUSION Odontogenic infection can cause serious multi-space infections in the oral and maxillofacial head and Neck regions,which can result in multiple LTCs.The management and treatment of LTCs such as multi-space inf

Purpose: Reduction and fixation in femoral Neck fracture in young patients have a problem of nonunion, requiring additional procedures like valgus osteotomy;but fixation devices are technically difficult for in experienced surgeons. We aims to assess the results of valgus osteotomy in femoral Neck fracture in our setup. Methods: We reported a series of 20 patients of higher Pauwel's angled fracture of femoral Neck fracture presenting late wherein valgus osteotomy was added to reduction fixation which was secured with a commonly available 135 dynamic hip screw and plate. Results: Femoral Neck fractures united in 16 patients (80%). Excellent to good results (Harris hip score >80) were seen in 70% patients. Angle of correction of preoperative Pauwels has been changed from 68.3 to 34.3. Conclusion: The 135 dynamic hip screw and plate provides rigid internal fixation after valgus osteotomy and being a more familiar fixation device simplifies the procedure with good results.

BACKGROUND Simultaneous bilateral femoral Neck fractures are relatively rare injuries.They are usually associated with underlying metabolic bone disorders or systemic diseases.Long-term use of narcotics and bisphosphonates can also result in similar fracture patterns;however,association of this fracture type with longterm use of antiepileptic drugs is not very common.Only one such case has been reported in the literature.This article describes the second.CASE REPORT We report a case of simultaneous displaced bilateral femoral Neck fractures in a 50-year-old epileptic patient,who had taken phenytoin for the past 3 years.The fractures were a result of low-velocity injury following a fall from the bed.The fractures were managed with a bilateral hemi-replacement arthroplasty.Oral bisphosphonates were given to improve the bone quality in the post-operative period.The patient had a good post-operative outcome,that was sustained throughout the entire follow-up period of 1 year.CONCLUSION Antiepileptic drugs should be supplemented with bisphosphonates and vitamin D to improve bone quality and prevent fractures in epileptic patients.
