Association between baseline levels of muscular strength and risk of stroke in later life:The Cooper Center Longitudinal Study

Can long-term thiamine treatment improve the clinical outcomes of myotonic dystrophy type 1？

Myotonic dystrophy type 1, also known as Steinert＇s disease, is an autosomal dominant disorder with multisystemic clinical features affecting the skeletal and cardiac muscles, the eyes, and the endocrine system. Thiamine （vitamin B1） is a cofactor of fundamental enzymes involved in the energetic cell me- tabolism; recent studies described its role in oxidative stress, protein processing, peroxisomal function, and gene expression. Thiamine deficiency is critical mainly in the central and peripheral nervous system, as well as in the muscular cells. Our aim was to investigate the potential therapeutical effects of long-term treatment with thiamine in myotonic dystrophy type 1 in an observational open-label pilot study. We de- scribed two patients with myotonic dystrophy type 1 treated with intramuscular thiamine 100 mg twice a week for 12 or 11 months. We evaluated the patients using the grading of muscle strength according to Medical Research Council （MRC）, the muscular Impairment Rating Scale （MIRS）, and the Modified Barthel index. High-dose thiamine treatment was well tolerated and effective in improving the motor symptomatology, particularly the muscle strength evaluated with the MRC scale, and the patients＇ activi- ties of daily living using the Modified Barthel Index. At the end of treatment, the MRC score was 5 in the proximal muscles and 2-4 in the distal muscles （the MRG score before the treatment was 3-4 and 1-3, re- spectively）. The MIRS grade improved by 25% compared to baseline for both patients. In patient #1, the Modified Barthel Index improved by 44%, and in patient #2 by 29%. These findings suggest that clinical outcomes are improved by long-term thiamine treatment.

Prevalence of physical fitness in Chinese school-aged children： Findings from the 2016 Physical Activity and Fitness in China -The Youth Study

Purpose: This study was to present the 2016 prevalence estimates of Chinese school-aged children meeting physical fitness standards and to examine differences by sex and residence locales in children who did not meet fitness ***: We conducted cross-sectional analyses of 171,991 children and adolescents(boy: 50.0%, Grades 1–12) who participated in the 2016 Physical Activity and Fitness in China—The Youth Study. The main outcomes were fitness measures, assessed by the 2014 revised Chinese National Student Physical Fitness Standard(CNSPFS), covering areas of aerobic capacity, upper body strength, flexibility, body mass index,abdominal strength, and trunk strength. Children's overall physical fitness performance was categorized, per CNSPFS standards, as excellent,good, pass, or no pass. Data on the prevalence of physical fitness categories and not meeting fitness standards(i.e., among children who received a "no pass" mark) were analyzed, through logistic regression, by sex(boy, girl) and residence locales(urban, rural) across 3 school grades(primary,junior middle, and junior high).Results: In 2016, 5.95% of Chinese children and adolescents achieved an "excellent" mark, 25.80% received a "good" rating, 59.90% received a "pass", and 8.35% received a "no pass". Overall, boys were more likely to not pass the fitness standards compared with girls(adjusted odds ratio(a OR) = 1.710; 95% confidence interval(CI): 1.708–1.712) and children living in urban areas were more likely to not pass the standards than those living in rural areas(a OR = 1.298; 95%CI: 1.296–1.299). Consistent patterns of not meeting fitness standards were also found by sex and residence locales across all 3 school ***: In the Chinese school-aged population, about 3 in 10 children achieved an "excellent" or "good" fitness standard in 2016, and about8% of this population did not meet CNSPFS standards. Inter-Individual Rapid Force Improvements after Mixed Session and Traditional Periodization in Aging Adults:A Randomized Trial

Background Different periodization models have been widely used to improve maximum and rapid force in aging ***,it is unclear if some specific model can influence the remarkable heterogeneity of responsiveness to rapid torque improvements following resistance *** Compare the effects of mixed session periodization(MSP)and traditional(TP)models on neuromuscular performance and individual responsiveness in aging *** Twenty-two aging men and women(64.6±5.2 years old;1.68±0.1 m;77.8±16.0 kg)completed the intervention *** were assigned into MSP(n=11)or TP(n=11)resistance training for lower *** outcome measures normalized dynamic leg press(5-RM:BM),absolute and normalized knee isometric peak torque(PT and PT:BM)and rate of torque development,and impulse were assessed at baseline and after three blocks of a 9-wk resistance training(3 sessions per week).Thigh leg lean mass was assessed by dual-energy x-ray absorptiometry at baseline and after the *** Similar increases were observed for 5-RM:BM(MSP:28.8%±13.5%and TP:26.4%±7.6%,P0.05).Absolute and normalized RTD remained unchanged throughout intervention(P>0.05),but both groups improved impulse(MSP:22.1%±25.7%and TP:12.6%±45.2%,P<0.001).Only MSP responders(n=5)presented significant time effect for absolute RTD(P<0.05)and impulse(P<0.05).Conclusion Although both periodization protocols increased 5-RM:BM and PT,it was not interchangeable through improve-ments in RTD even adopting power training ***,these results suggest a great inter-individual variability following different periodization models for aging people.

Measurement of respiratory muscle strength is performed through static measurements of maximal respiratory pressures and is an important tool for determining the existence and prognosis of neuromuscular and pulmonary disorders, such as weakness or fatigue of this musculature. Objectives: To evaluate and compare respiratory muscle strength among healthy adult and elderly women. Methods: A total of 163 healthy women were recruited through convenience sampling at the family centers of the city of Manaus (AM, Brazil), divided into two groups: adult women (age 20 - 59 years) and elderly women (over 60 years of age). The evaluation was performed by Manovacuometry, in order to measure the maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP). For statistical analysis we used the Student’s T test, considering significant p value ≤ 0.05. Results: The final sample consisted of 116 adult women and 47 elderly women. The mean age in the group of women between 20 - 59 years was 44.5 ± 12 and the elderly group was 65 ± 6.6. MIP values in adult women were significantly higher when compared to the elderly (−108 ± 5.3 cmH2O × −79.5 ± 4 cmH2O, p = 0.0014, respectively). The same behavior was observed in the evaluation of MEP (87.1 ± 2.9 cmH2O in adults and 69.8 ± 3.5 cmH2O in the elderly;p = 0.0008). Conclusion: The results indicate that inspiratory and expiratory muscle strength is lower in older women, suggesting that the aging process tends to interfere with those muscles, resulting in altered respiratory system function.

Puerarin alleviates skeletal muscle atrophy in type 1 diabetic rats

OBJECTIVE Puerarin is an important isoflavone component of traditional Chinese medicine Pueraria lobata and has a wide range of pharmacological *** have shown that puerarin has a significant alleviation effect on diabetes and its complications. This article investigates the therapeutic effect of puerarin on rat skeletal muscle atrophy caused by type 1 diabetes. METHODS SD rats were fed adaptively for 1 week, then 10 rats were randomly divided into normal control *** rest were injected intraperitoneally with 65 mg·kg-1streptozotocin（STZ）. Blood was taken from the tip of the tail after 1 week and rats with fasting blood glucose levels above 16.5 mmol · L-1were chosen as type 1 diabetic rats, which randomly divided into two groups: diabetic model control group and puerarin administration group（n=10）. The administration group was treated with orally administration of puerarin at 100 mg·kg-1·d-1, while the normal control group and the diabetic model group were treated with corresponding dose of normal saline. At the end of the 8-week treatment, diabetes related indicators were detected including body weight, food-intake, fasting blood glucose and glucose tolerance test. Meanwhile,skeletal muscle atrophy correlative indicators like foreleg tensile force, time of wire hanging, persistent time on sloping plate, area of skeletal muscle and atrophy markers（F-box protein 32, Fbxo32 and tripartite motif containing63, Trim63） were also detected. RESULTS Puerarin(100 mg · kg-1) can lighten body weight and decrease food-intake of type 1 diabetic rats induced by STZ（Pstrength, puerarin can significantly enhance the foreleg tensile force（P<0.01）, time of wire hanging（P<0.05） and persistent time on sloping plate（P<0.01）.Besides, puerarin also showed a significant effect on enhancing the area of skeletal muscle（P<0.01）while reducing expressio
