Validation of a novel imaging approach using multi-slice CT and cone-beam CT to follow-up on condylar remodeling after bimaxillary surgery

The main goal of this study was to introduce a novel three-dimensional procedure to objectively quantify both inner and outer condylar remodelling on preoperative multi-slice computed tomography （MSCT） and postoperative cone-beam computed tomography （CBCT） images. Second, the reliability and accuracy of this condylar volume quantification method was assessed. The mandibles of 20 patients （11 female and 9 male） who underwent bimaxillary surgery were semi-automatically extracted from MSCT/CBCT scans and rendered in 3D. The resulting condyles were spatially matched by using an anatomical landmark-based registration procedure. A standardized sphere was created around each condyle, and the condylar bone volume within this selected region of interest was automatically calculated. To investigate the reproducibility of the method, inter- and intra-observer reliability was calculated for assessments made by two experienced radiologists twice five months apart in a set of ten randomly selected patients. To test the accuracy of the bone segmentation, the inner and outer bone structures of one dry mandible, scanned according to the clinical set-up, were compared with the gold standard, micro-CT. Thirty-eight condyles showed a significant （P〈O.05） mean bone volume decrease of 26.4%_ 11.4% （502.9 mm3＋ 268.1 mm3）. No significant effects of side, sex or age were found. Good to excellent （ICC〉 0.6） intra- and inter-observer reliability was observed for both MSCT and CBCT. Moreover, the bone segmentation accuracy was less than one voxel （0.4 mm） for MSCT （0.3 mm __. 0.2 mm） and CBCT （0.4 mm _ 0.3 mm）, thus indicating the clinical potential of this method for objective follow-up in pathological condylar resorption.

Improvement of Efficiency and Flexibility in multi-slice Helical CT

One of the main aspects in computed tomography (CT) development is to make CT rapidly scan a large longitudinal volume with high z-axis resolution. The combination of helical scanning with multi-slice CT is a promising approach. Image reconstruction in multi-slice CT becomes, therefore, the major challenge. Known algorithms need to derive the complementary data or work only for certain range of pitches. A reconstruction algorithm was presented that works with the direct data as well as arbitrary pitches. Filter interpolation based on the proposed method was implemented easy. The results of computer simulations under kinds of conditions for four-slice CT were presented. The proposed method can obtain higher efficiency than the conventional method.

Utility of 64-MSCT in assessing acute non-reperfused myocardial infarct size

Objective To evaluate the utility of multi-slice computed tomography (MSCT) in assessing acute non-reperfused myocardial infarct size. Methods Seven domestic pigs (mean weight 17.3 ± 1.9 kg) underwent ligation of the distal left anterior descending artery to establish a model of acute myocardial infarction (MI). MSCT and triphenyltetrazolium chloride (TTC) staining were performed two hours later. The following data were acquired and analyzed:MI volume (%), CT values of the infarcted region, left ventricular cavity and normal cardiac tissue at various scanning time-points (1, 5, 10, 15, 20 min after contrast injection). Results Using MSCT, the overall MI volume showed a time-dependent decrease, with a reduction of 28.87%after 20 min. The greatest reduction occurred at the 5 min time-point. In TTC staining, MI volume was 9.87%± 2.44%. When MI size, as determined by MSCT, was compared with that by TTC staining in Bland-Altman plots, there was a better agreement at 5, 10, and 15 min time-points at 1 and 20 min. Conclusions The study indicates that double-phase scanning examination using MSCT is a useful tool to assess MI size, and the optimal late-phase scanning time-point set within 5-15 min of contrast injection.

Prognostic value of coronary artery calcium score in patients with stable an-gina pectoris after percutaneous coronary intervention

Objectives To evaluate the prognostic value of the coronary artery calcium （CAC） score in patients with stable angina pectoris （SAP） who underwent percutaneous coronary intervention （PCI）. Methods A total of 334 consecutive patients with SAP who underwent first PCI following multi-slice computer tomography （MSCT） were enrolled from our institution between January 2007 and June 2012. The CAC score was calculated according to the standard Agatston calcium scoring algorithm. Complex PCI was defined as use of high pressure bal-loon, kissing balloon and/or rotablator. Procedure-related complications included dissection, occlusion, perforation, no/slow flow and emer-gency coronary artery bypass grafting. Main adverse cardiac events （MACE） were defined as a combined end point of death, non-fatal myo-cardial infarction, target lesion revascularization and rehospitalization for cardiac ischemic events. Results Patients with a CAC score〉300 （n=145） had significantly higher PCI complexity （13.1%vs. 5.8%, P=0.017） and rate of procedure-related complications （17.2%vs. 7.4%, P=0.005） than patients with a CAC score≤300 （n=189）. After a median follow-up of 22.5 months （4-72 months）, patients with a CAC score≤300 differ greatly than those patients with CAC score＆gt;300 in cumulative non-events survival rates （88.9 vs. 79.0%, Log rank 4.577, P=0.032）. After adjusted for other factors, the risk of MACE was significantly higher [hazard ratio （HR）：4.3, 95%confidence inter-val （95%CI）：2.4-8.2, P=0.038] in patients with a CAC score〉300 compared to patients with a lower CAC score. Conclusions The CAC score is an independent predictor for MACE in SAP patients who underwent PCI and indicates complexity of PCI and proce-dure-related complications.

Gastrointestinal Stromal Tumors: Correlation of multislice CT Findings to Histopathologic Features and Preliminary Validation of New Scoring System

Purpose: The purpose of this study is to demonstrate the correlation between radiologic and pathologic features of the gastrointestinal stromal tumors. Patients and methods: A retrospective review from 2004 to 2014 identified 50 resected cases of confirmed gastrointestinal stromal tumors (GIST) is done. All these lesions were visualized in the first multi-slice computed tomography (MSCT) investigation. Radiologic and pathologic features were reviewed and compared. A radiologic score with MSCT findings was established. Four levels of risk were defined and compared to the Miettinen-Lasota prognostic classification. Results: Mean patients’ age was 57.6 with a sex-ratio (M/F) of 1.17. Of the 50 GISTs lesions, 29 were located in the stomach (58%), 3 in the duodenum (6%), 16 in the small intestine (32%), one in the rectum and one in the great omentum. MSCT images were evaluated for origin and size of the tumor, as well as growth pattern, density before and after contrast, relationship with adjacent structures, presence of lymph nodes, ascitis and metastasis. The presence of mucosal ulceration, calcification, necrosis, cystic area or hemorrhage into the lesion was emphasized for each case. The histologic equivalent criteria were gathered from histopathology examination review of all specimens. Significant correlation was found for all these findings except the hemorrhage (p = 0.071). A radiologic score of fifteen items variable between 0 and 18 was established. Miettinen risk classification was noted for each lesion. GISTs with very low risk had MSCT score < 4. GIST with low risk had a MSCT score between 5 and 9. GIST with moderate risk had a score between 10 and 14 and those with high risk had an MSCT score between 15 and 18. Significant correlation was found between the radiologic and histopathologic risk classification (p = 0. 001). Conclusion: MSCT is helpful in risk prediction for GIST.
