Evaluation of modifi ed Alvarado scoring system and RIPASA scoring system as diagnostic tools of acute appendicitis

BACKGROUND: Acute appendicitis is the most common surgical condition presented in emergency departments worldwide. Clinical scoring systems, such as the Alvarado and modified Alvarado scoring systems, were developed with the goal of reducing the negative appendectomy rate to 5%–10%. The Raja Isteri Pengiran Anak Saleha Appendicitis(RIPASA) scoring system was established in 2008 specif ically for Asian populations. The aim of this study was to compare the modif ied Alvarado with the RIPASA scoring system in Kuwait ***: This study included 180 patients who underwent appendectomies and were documented as having "acute appendicitis" or "abdominal pain" in the operating theatre logbook(unit B) from November 2014 to March 2016. The sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), diagnostic accuracy, predicted negative appendectomy and receiver operating characteristic(ROC) curve of the modified Alvarado and RIPASA scoring systems were derived using SPSS statistical ***: A total of 136 patients were included in this study according to our criteria. The cut-off threshold point of the modif ied Alvarado score was set at 7.0, which yielded a sensitivity of 82.8% and a specif icity of 56%. The PPV was 89.3% and the NPV was 42.4%. The cut-off threshold point of the RIPASA score was set at 7.5, which yielded a 94.5% sensitivity and an 88% specif icity. The PPV was 97.2% and the NPV was 78.5%. The predicted negative appendectomy rates were 10.7% and 2.2% for the modif ied Alvarado and RIPASA scoring systems, respectively. The negative appendectomy rate decreased significantly, from 18.4% to 10.7% for the modified Alvarado, and to 2.2% for the RIPASA scoring system, which was a signif icant difference(P<0.001) for both scoring ***: Based on the results of this study, the RIPASA score is a simple scoring system with better sensitivity and specif icity than the modif ied Alvarado scoring system in A

To overcome the fast or burst release of hydrophilic drugs from hydrophilic alginate-based carriers,hydrophobic molecule(vinyl acetate,VAc)was grafted on alginate(Alg),which was further used to prepare drug *** Alg-g-PVAc hydrogel beads were firstly prepared by emulsification/internal gelation technique for the loading of bovine serum albumin(BSA).Then,chitosan was coated on the surface of beads to form novel amphiphilic Alg-g-PVAc/chitosan(Alg-g-PVAc/CS)*** BSA-loading amphiphilic Alg-g-PVAc/chitosan(Alg-g-PVAc/CS)microcapsules display similar morphology and size to the hydrophilic alginate/chitosan(AC)***,the drug loading and loading efficiency of BSA in Alg-g-PVAc/CS microcapsules are higher,and the release rate of BSA from Alg-g-PVAc/CS microcapsules is *** results demonstrate that the introduction of hydrophobic PVAc on alginate can effectively help retard the release of BSA,and the higher degree of substitution is,the slower the release rate *** addition,the complex membrane can also be adjusted to delay the release of *** a whole,amphiphilic sodium alginate-vinyl acetate/CS microparticles could be developed for macromolecular drug delivery.

Ni_2P/ZrO_2-SBA-15 Dibenzothiophene Hydrodesulfurization Catalysts: Preparation,Characterization and Evaluation

Ni_2P/ZrO_2-SBA-15 catalysts with different zirconium n-propoxide/SBA-15 mass ratios were synthesized to evaluate their dibenzothiophene hydrodesulfurization catalytic activity. Effect of ZrO_2 introduction was investigated. Supports and catalysts were characterized by BET,XRD,^(29)SiNMR,XPS and FTIR. The results indicated that zirconium was incorporated into SBA-15 in the forms of [(–O–)_2 Si(–O–Zr)_2 ] and/or [(–O–)_3 Si–O–Zr],and that the SBA-15 framework structure was maintained after incorporation of ZrO_2. With zirconium content increasing,ZrO_2 was transformed from amorphous phase to tetragonal phase. Zirconium incorporation into SBA-15 supports could facilitate to form more dispersed Ni_2P active phase. There might be some interaction occurring between the P and Zr species. In addition to Ni_2P,another kind of active phase,ZrP,was formed,which might exhibit a better HDS activity than Ni_2P. It was observed that at a temperature of 280 °C,pressure of 3.0 MPa,WHSV of 6.5 h^(-1) and H_2 to oil ratio of 450,the Ni_2P/Zr-SBA(1.5) catalyst,where 1.5 represents zirconium n-propoxide/SBA-15 mass ratio,showed the highest DBT conversion,which was 86.6%,almost 35% higher than that of the Ni_2P/Zr-SBA(0) catalyst.
