Uncharted Territory: Frequent Relapsing, Steroid Sensitive Secondary minimal change nephrotic syndrome Cause by Solid Tumor of the Gastro-Esophageal Junction —(Case Presentation and Review of the Literature)

We reported a biopsy proved case of minimal change nephrotic syndrome in a 72-year-old patient. The minimal change nephrotic syndrome has been steroid sensitive, but the patient had 7 relapses over a span of 5 years. Each time the dose of steroid is tapered, a relapse of the nephrotic syndrome occurred. Eventually, the patient was complaining of dysphagia and difficulty swallowing. Hospital work-up with barium swallow, endoscopy, and CT of the chest, abdomen and pelvis, revealed a focal stenotic lesion with mild to moderate esophageal dysmotility 7/15/2022. A diagnosis of an ulcerating lesion with biopsy confirmed a neuro-endocrine carcinoma of the gastro-esophageal junction was entertained. The CT of the chest/abdomen/pelvis, 7/19/2022, has shown, an esophageal mass of 5.1 × 5.6 × 7 cm of the gastro-esophageal junction with ulceration. No evidence of spread beyond the esophagus and stomach. The histology revealed a poorly differentiated neuroendocrine tumor of the gastro-esophageal junction. The patient underwent several rounds of chemotherapy, radiation, and surgery culminating in tumor control. His nephrotic syndrome was resolved after the tumor has been controlled by surgery and chemotherapy.

A large amount of evidence has implicated minimal change nephrotic syndrome(MCNS)may result from the dysfunction of T cells and B cells,while the precise mechanism are not well *** has been widely known that MCNS is a Th2-dominant glomerular disease resulting from imbalanced Th1/Th2 immune *** the increasing levels ofTh2 cytokines,IL-4 and IL-13,are closely correlated with disease ***,basophils which can affect Th1/Th2balance by enhancing Th2 response and impairing Th1 response then be involved in the development of many diseases,however,whether basophils are vital in the pathogenesis of MCNS,remains *** has been reported that frequent positivity of the human basophil degranulation test in patients with ***,basophils should be analyzed in the pathogenesis of MCNS.

Background:Myeloid-derived suppressor cells(MDSCs)are bone marrow-derived immature heterogeneous myeloid cells and they are morphologically divided into two distinct subsets:monocytic(M)and granulocytic(G)*** have been shown to suppress T cell responses and contribute to the tumor immunosuppressive *** contrast,mouse studies have shown that MDSCs may contribute to autoimmune ***,the role of MDSCs in the pathogenesis of human autoimmune diseases remains relatively poorly ***:To determine the role of MDSCs in the pathogenesis of adult minimal change nephrotic syndrome(MCNS),a disorder of T cell function that accounts for 10-15% of all primary adult nephrotic ***:This study involved 31 adult patients with new onset MCNS identified by renal biopsy pathology and 26healthy controls(HC).The study protocols were approved by the ethics committee of the First Hospital of Jilin *** blood mononuclear cells(PBMCs)were isolated by Histopaque1077 and were stained with ***11b/HLADR/CD33/CD14/*** were acquired by BD FACS LSR II and analyzed by using Flowjo *** analysis was performed with GraphPad Prism ***:The percentages of total MDSCs(CD11bHLADRCD33)in PBMCs were significantly increased in patients with MCNS compared to HC(10.82±8.58%vs 2.33±1.22%,p<0.0001),and the percentages of MDSCs were correlated positively with proteinuria(r=0.559,p=0.003).Furthermore,the percentages of both CD14M-MDSCs(4.15±2.75%vs 1.11±0.66%,p<0.0001)and CD66b+G-MDSCs(5.46±1.53%vs 0.92±0.14%,p<0.0001)were significantly higher in MCNS patients than in ***:Compared to HC,PBMCs from patients with MCNS showed significant increase in the percentages of MDSCs,including both M-MDSCs and ***,the proportion of MDSCs in PBMCs were correlated positively with proteinuria in MCNS patients.

Pathogenesis of childhood idiopathic nephrotic syndrome:a paradigm shift from T-cells to podocytes

Background:nephrotic syndrome is the most common cause of kidney disease in children,but its pathogenesis remains *** article reviews the novel aspects of the mechanisms underlying massive proteinuria in minimal-change disease,which is the most common form of childhood nephrotic *** sources:This article integrates the findings of a PubMed database search for English language articles published in the past 40 years(from September 1974 to February 2014)using the key words"pathogenesis","minimal change nephrotic syndrome"or"idiopathic ne phrotic syndrome".Results:Unknown humoral factors associated with T-cell dysfunction have been thought to play an important role in the pathogenesis of minimal-change ***,recent findings are changing this paradigm,i.e,visceral glomerular epithelial cells(podocytes)may be involved via expression of molecules such as CD80 and angiopoietin-like ***:Recent evidence suggests that minimal-change disease results from interactions between humoral factors and dysfunctional *** addition to immunosuppressant drugs that target lymphocytes,a biological agent such as an antibody against the abnormal molecule(S)expressed by podocytes may provide novel drug treatment for minimal-change disease.
