Differential risk of 23 site-specific incident cancers and cancer-related mortality among patients withmetabolic dysfunction-associated fatty liver disease:a population-based cohort study with 9.7 million Korean subjects

Introduction:Although an association between metabolic dysfunctionassociated fatty liver disease(MAFLD)and cardiovascular disease or overall mortality has been reported,it is unclearwhether there is an association between MAFLD and cancer incidence or mortality.We aimed to investigate the differential risk of all-and site-specific cancer incidence and mortality according to MAFLD subgroups categorized by additional etiologies of liver disease.Methods:Using the Korean National Health Insurance Service database,we stratified the participants into three groups:(1)single-etiology MAFLD(SMAFLD)or MAFLD of pure metabolic origin;(2)mixed-etiology MAFLD(M-MAFLD)or MAFLD with additional etiological factor(s)(i.e.,concomitant liver diseases and/or heavy alcohol consumption);and(3)non-MAFLD.Hepatic steatosis and fibrosiswere defined using the fatty liver index and the BARDscore,respectively.Cox proportional hazards regression was performed to estimate the risk of cancer events.Results:Among the 9,718,182 participants,the prevalence of S-MAFLD and M-MAFLD was 29.2%and 6.7%,respectively.During the median 8.3 years of follow-up,510,330(5.3%)individuals were newly diagnosed with cancer,and 122,774(1.3%)cancer-related deaths occurred among the entire cohort.Compared with the non-MAFLD group,the risk of all-cancer incidence and mortality was slightly higher among patients in the S-MAFLD group(incidence,adjusted hazard ratio[aHR]=1.03;95%confidence interval[CI]:1.02−1.04;mortality,aHR=1.06;95%CI:1.04−1.08)and highest among patients with M-MAFLD group(incidence,aHR=1.31;95%CI:1.29−1.32;mortality,aHR=1.45;95%CI:1.42−1.48,respectively).The M-MAFLD with fibrosis group(BARD score≥2)showed the highest relative risk of all-cancer incidence(aHR=1.38,95%CI=1.36–1.39),followed by the M-MAFLD without fibrosis group(aHR=1.09,95%CI=1.06–1.11).Similar trends were observed for cancer-related mortality.Conclusions:MAFLD classification,by applying additional etiologies other than pure metabolic origin,can be used to identify a subgroup of patients

Nutrition, insulin resistance and dysfunctional adipose tissue determine the different components of metabolic syndrome

Obesity is an excessive accumulation of body fat that may be harmful to health. Today, obesity is a major public health problem, affecting in greater or lesser proportion all demographic groups. Obesity is estimated by body mass index(BMI) in a clinical setting, but BMI reports neither body composition nor the location of excess body fat. Deaths from cardiovascular diseases, cancer and diabetes accounted for approximately 65% of all deaths, and adiposity and mainly abdominal adiposity are associated with all these disorders. Adipose tissue could expand to inflexibility levels. Then, adiposity is associated with a state of low-grade chronic inflammation, with increased tumor necrosis factor-α and interleukin-6 release, which interfere with adipose cell differentiation, and the action pattern of adiponectin and leptin until the adipose tissue begins to be dysfunctional. In this state the subject presents insulin resistance and hyperinsulinemia, probably the first step of a dysfunctional metabolic system. Subsequent to central obesity, insulin resistance, hyperglycemia, hypertriglyceridemia, hypoalphalipoproteinemia, hypertension and fatty liver are grouped in the so-called metabolic syndrome(MetS). In subjects with MetS an energy balance is critical to maintain a healthy body weight, mainly limiting the intake of high energy density foods(fat). However, high-carbohydrate rich(CHO) diets increase postprandial peaks of insulin and glucose. Triglyceride-rich lipoproteins are also increased, which interferes with reverse cholesterol transport lowering highdensity lipoprotein cholesterol. In addition, CHO-rich diets could move fat from peripheral to central deposits and reduce adiponectin activity in peripheral adipose tissue. All these are improved with monounsaturated fatty acid-rich diets. Lastly, increased portions of ω-3 and ω-6 fatty acids also decrease triglyceride levels, and complement the healthy diet that is recommended in patients with MetS.

Use of fibrates in the metabolic syndrome:A review

The use of fibrates in the treatment of dyslipidaemia has changed significantly over recent years.Their role appeared clear at the start of this century.The Helsinki Heart Study and Veterans Affairs High-Density Cholesterol Intervention Trial suggested significant benefit,especially in patients with atherogenic dyslipidaemia.However,this clarity disintegrated following the negative outcomes reported by the Bezafibrate Infarction Prevention,Fenofibrate Intervention and Event Lowering in Diabetes and Action to Control Cardiovascular Risk in Diabetes randomised controlled trials.In this review we discuss these and other relevant trials and consider patient subgroups such as those with the metabolic syndrome and those needing treatment to prevent the microvascular complications associated with diabetes in whom fibrates may be useful.We also discuss observations from our group that may provide some explanation for the varying outcomes reported in large trials.The actions of fibrates in patients who are also on statins are interesting and appear to differ from those in patients not on statins.Understanding this is key as statins are the primary lipid lowering agents and likely to occupy that position for the foreseeable future.We also present other features of fibrate treatment we have observed in our clinical practice;changes in creatinine,liver function tests and the paradoxical high density lipoprotein reduction.Our purpose is to provide enough data for the reader to make objective decisions in their own clinical practice regarding fibrate use.

Glucagon-like peptide-1 receptor agonists favorably address all components of metabolic syndrome

Cardiovascular death is the leading cause of mortality for patients with type 2 diabetes mellitus. The etiologyof cardiovascular disease in diabetes may be divided into hyperglycemia per se and factors operating through components of metabolic syndrome(Met S). Hyperglycemia causes direct injury to vascular endothelium and possibly on cardiac myocytes. Met S is a cluster of risk factors like obesity, hyperglycemia, hypertension and dyslipidemia. The incidence of this syndrome is rising globally. Glucagon-like peptide-1 receptor agonists(GLP-1RA) are a group of drugs, which address all components of this syndrome favorably. Experimental evidence suggests that they have favorable actions on myocardium as well. Several compounds belonging to GLP-1RA class are in market now and a large number awaiting their entry. Although, originally this class of drugs emerged as a treatment for type 2 diabetes mellitus, more recent data generated revealed beneficial effects on multiple metabolic parameters. We have studied literature published between 2000 and 2016 to look into effects of GLP-1RA on components of Met S. Results from recently concluded clinical trials suggest that some of the molecules in this class may have favorable effects on cardiovascular outcome.

Excessive 5-year weight gain predicts metabolic syndrome development in healthy middle-aged adults

AIM:To quantitatively examine the impacts of an easyto-measure parameter-weight gain-on metabolic syndrome development among middle-aged adults. METHODS:We conducted a five-year interval observational study.A total of 1384 middle-aged adults not meeting metabolic syndrome(MetS)criteria at the initial screening were included in our analysis.Baseline data such as MetS-components and lifestyle factors were collected in 2002.Body weight and MetS-components were measured in both 2002 and 2007.Participants were classified according to proximal quartiles of weight gain(WG)in percentages(%WG≤1%,1%10%, defined as:control,mild-WG,moderate-WG and severe-WG groups,respectively)at the end of the follow-up. Multivariate models were used to assess the association between MetS outcome and excessive WG in the total population,as well as in both genders. RESULTS:In total,175(12.6%)participants fulfilled MetS criteria within five years.In comparison to the control group,mild-WG adults had an insignificant risk for MetS development while adults having moderate-WG had a 3.0-fold increased risk for progression to MetS [95%confidence interval(CI),1.8-5.1],and this risk was increased 5.4-fold(95%CI,3.0-9.7)in subjects having severe-WG.For females having moderate-and severe-WG,the risk for developing MetS was 3.6(95% CI,1.03-12.4)and 5.5(95%CI,1.4-21.4),respectively. For males having moderate-and severe-WG,the odds ratio for MetS outcome was respectively 3.0(95%CI, 1.6-5.5)and 5.2(95%CI,2.6-10.2). CONCLUSION:For early-middle-aged healthy adults with a five-year weight gain over 5%,the severity of weight gain is related to the risk for developing metabolic syndrome.

Pulmonary hypertension and metabolic syndrome: Possible connection, PPARγ and Caveolin-1

A number of disparate diseases can lead to pulmonary hypertension(PH), a serious disorder with a high morbidity and mortality rate. Recent studies suggest that the associated metabolic dysregulation may be an important factor adversely impacting the prognosis of PH. Furthermore, metabolic syndrome is associated with vascular diseases including PH. Inflammation plays a significant role both in PH and metabolic syndrome. Adipose tissue modulates lipid and glucose metabolism, and also produces pro-and anti-inflammatory adipokines that modulate vascular function and angiogenesis, suggesting a close functional relationship between the adipose tissue and the vasculature. Both caveolin-1, a cell membrane scaffolding protein and peroxisome proliferator-activated receptor(PPAR) γ, a ligandactivated transcription factor are abundantly expressed in the endothelial cells and adipocytes. Both caveolin-1 and PPARγ modulate proliferative and anti-apoptotic pathways, cell migration, inflammation, vascular homeostasis, and participate in lipid transport, triacylglyceride synthesis and glucose metabolism. Caveolin-1 and PPARγ regulate the production of adipokines and in turn are modulated by them. This review article summarizes the roles and inter-relationships of caveolin-1,PPARγ and adipokines in PH and metabolic syndrome.

Investigating the prescription compatibility of Rhodiolarosea,Cordycepsmilitaris,and Rhubarbroot for the treatment of metabolic syndrome

OBJECTIVE To investigate the compatibility of the prescription Rhodiolarosea,Cordycepsmilitaris,and Rhubarb root.METHODS The DIO mice accompanying with insulin resistance,hypercholesterolemia and obesity were induced by high fat diet(HFD)in C57BL/6mice.For evaluating the insulin sensitivity in vivo,the values of serum insulin,HOMA-IR,and the glucose infuse rate(GIR)in hyperinsulinemiceuglycemic clamp test were determined;the time-dependent blood glucose changes were measured after insulin loading in insulin tolerance test(ITT),or after glucose loading in glucose tolerance test(GTT),respectively.Recombinant human protein tyrosine phosphatase 1B(PTP1B)was overexpressed by hGST-PTP1BBL21 E.coli and purified by GST affinity chromatography.The activities of PTP1 Band glutamine:fructose 6-phosphate amidotransferase(GFAT)were measured by the pNPP and GDH method,respectively.The activity ofα-glucosidase was determined with the substrates of p-nitrophenyl-alpha-D-glucopyranoside(pNPG).The IC50 value was calculated using the software GraphPad Prism 5.RESULTS 1st,the standard for the raw materials and for the extraction process of Rhodiolarosea,Cordycepsmilitaris,and Rhubarbroot are formulated according to the inhibition on PTP1 B,α-glucosidase,and GFAT in vitro,and the improvement on impaired glucose tolerance(IGT),hypercholesterolemia,and obesity in vivo,respectively.2nd,the composition was determined by the synergistic effects of Rhodiolarosea,Cordycepsmilitaris,and Rhubarb root on insulin resistance in DIO mice.3rd,in accordance with the IC50 values from the L9(34)and L8(27)orthogonal experiments targeting on PTP1 B and α-glucosidase,respectively,the best rate of Rhodiolarosea∶Cordycepsmilitaris∶Rhubarbroot was selected.4th,the effective dose for the treatment of metabolic syndrome was assessed based on the effects on IGT,hypercholesterolemia,obesity,insulin resistance,respectively,in DIO mice.CONCLUSION The prescription compatibility of Rhodiolarosea,Cordycepsmilitaris,and Rhubarbroot with the rate of 20∶1∶1was formed

Systemic omics analysis of the hub genes, proteins, metabolites and metabolic pathways related to the hypoxia preconditioning in mice

Hypoxia preconditioning （HPC） is associated with many complicated pathophysiological and biochemical processes that integrated and regulated via molecular levels. HPC could protect cells, tissues, organs and systems from hypoxia injury, but up to date, the molecular mechanism still remained unclear. The acute and repetitive hy- poxia preconditioning model was constructed and the related parameters were observed. The high-throughput mi- croarray analysis and multiple bioinformatics were used to explore the differentially expressed genes in HPC mice brain and the related gene network, pathways and biological processes related to HPC. The 2D-DIGE coupled with MALDI-TOF/TOF-MS was performed to identify these proteins that were differentially expressed during HPC. The UPLC-HRMS based metabolomics method was utilized to explore the key endogenous metabolites and metabolic pathways related to HPC. The results showed that （1） 1175 differentially expressed genes in HPC mice brain were identified. Fourteen of these genes were the related hub genes for HPC, including Cacna2dl, Grin2a, Npylr, Mef2c, Epha4, Rxfpl, Chrm3, Pdela, Atp2b4, Glral, Idil , Fgfl, Grin2b and Cda. The change trends of all the detected genes by RT-PCR were consistent with the data of gene chips. There were 113 significant functions up- regulated and 138 significant functions down-regulated in HPC mice. （2） About 2100 proteins were revealed via the gel imaging and spot detection. 66, 45 and 70 of proteins were found to have significantly difference between the control group and three times of HPC group, the control and six times of HPC, and the three times of HPC and six times of HPC group. （3）Some endogenous metabolites such as phenylalanine, valine, proline, leucine and glu- tamine were increased, while ereatine was decreased, both in HPC brain and heart; in addition, y-aminobutyric acid was markedly decreased in brain. Global left ventricular performance in non-diabetic non-hypertensive metabolic syndrome adults

AIM:To evaluate the left ventricular structure and function in isolated metabolic syndrome.METHODS:One hundred and fifty six consecutive adults with metabolic syndrome were enrolled in the study.Fifty nine had isolated metabolic syndrome(group A) and 97 had metabolic syndrome with hypertension and/or diabetes(group B).There was a control group of 34 healthy adults.In addition to classic echocardiographic assessment of myocardial structural and functional changes,the Tei index was used to evaluate global left ventricular performance.RESULTS:There were no statistically significant differences between group A and controls in all parameters of left ventricular structural,systolic,and diastolic function except global myocardial performance(Tei index).On the other hand,significant differences were observed between group B and the control group in most of the parameters of left ventricular structural and global performance.CONCLUSION:The early identification of isolated metabolic syndrome in non-diabetic,non-hypertensive adults may be an indication that aggressive preventive measures should not be postponed until overt obesity,hypertension or diabetes mellitus has developed.

Mechanisms of developmental programming of the metabolic syndrome and related disorders

There is consistent epidemiological evidence linking low birth weight, preterm birth and adverse fetal growth to an elevated risk of the metabolic syndrome (obesity, raised blood pressure, raised serum triglycerides, lowered serum high-density lipoprotein cholesterol and impaired glucose tolerance or insulin resistance) and related disorders. This "fetal or developmental origins/programming of disease" concept is now well accepted but the "programming" mechanisms remain poorly understood. We reviewed the major evidence, implications and limitations of current hypotheses in interpreting developmental programming and discuss future research directions. Major current hypotheses to interpret developmental programming include: (1)thrifty phenotype; (2) postnatal accelerated or catchup growth; (3) glucocorticoid effects; (4) epigenetic changes; (5) oxidative stress; (6) prenatal hypoxia; (7) placental dysfunction; and (8) reduced stem cell number. Some hypothetical mechanisms (2, 4 and 8) could be driven by other upstream "driver" mechanisms. There is a lack of animal studies addressing multiple mechanisms simultaneously and a lack of strong evidence linking clinical outcomes to biomarkers of the proposed programming mechanisms in humans. There are needs for (1) experimental studies addressing multiple hypothetical mechanisms simultaneously; and (2) prospective pregnancy cohort studies linking biomarkers of the proposed mechanisms to clinical outcomes or surrogate biomarker endpoints. A better understanding of the programming mechanisms is a prerequisite for developing early life interventions to arrest the increasing epidemic of the metabolic syndrome, type 2 diabetes and other related disorders.
