OBJECTIVE:To formulate the first clinical practice guideline for the treatment of cough using Chinese Medicine based on the grading of recommendations assessment,development,and evaluation(GRADE)systematic approach,including clinical evidence,evaluation of ancient literature,and expert consensus.METHODS:In the process of development,the regulation of"evidence-based,consensus-assisted,and empirical"was followed,and a comprehensive systematic approach of recommendation assessment,GRADE,evidence-based evaluation,expert consensus,and the Delphi method was used.In the process of guideline development,evidence-based evaluation of ancient literature was included for the first time,and clinical evidence was fully integrated with clinical expert consensus.RESULTS:The clinical practice guidelines for the treatment of cough with Chinese herbal Medicine were developed after a comprehensive consideration of evidence-based evaluation and expert opinions.The guideline recommendations focused on recommending herbal compound decoctions and Chinese patent Medicines for cough in different conditions.Based on the GRADE systematic approach,we conducted an evidence-based evaluation of the recommended Chinese patent Medicines one by one;meanwhile,the expert consensus method was used to unify the recommendations of both.CONCLUSION:Based on clinical evidence,ancient literature evaluation,and expert consensus,a clinical practice guideline for Traditional Chinese Medicine(TCM)in the treatment of cough was developed,providing the first current clinical practice guideline for domestic and foreign TCM and Western Medicine practitioners,especially respiratory professionals at home and abroad.

Sepsis-associated encephalopathy(SAE)is a common manifestation of sepsis,ranging from mild confusion and delirium to severe cognitive impairment and deep coma.SAE is associated with higher mortality and long-term outcomes,particularly substantial declines in cognitive function.The mechanisms of SAE probably include neuroinflammation that is mediated by systemic inflammation and ischemic lesions in the brain,a disrupted blood–brain barrier,oxidative stress,neurotransmitter dysfunction,and severe microglial activation.Increasing evidence suggests that complementary and alternative Medicine,especially Traditional Chinese Medicine(TCM),is favorable in alleviating cognitive decline after sepsis.Here,we summarized the studies of traditional herbal remedies,TCM formulas and acupuncture therapy in animal models of neurological dysfunctions after sepsis in recent decades and reviewed their potential mechanisms.

Effects of Traditional Chinese Medicine on the survival of patients with stage Ⅰ gastric cancer and high-risk factors:a real-world retrospective study

OBJECTIVE:To investigate the benefits of Traditional Chinese Medicine(TCM) therapy for improving the survival of patients with stage I gastric cancer(GC) and high-risk factors in a real-world setting.METHODS:The clinical data of patients who were diagnosed with stage I GC from March 1,2012 to October 31,2020 were collected.Prognostic analysis was performed to explore the high-risk factors that affected the survival of the patients.A Cox multivariate regression model was used to compare the hazard ratios for the mortality risk of patients,especially those with high-risk factors.Kaplan-Meier survival curve and log-rank test were utilized to assess the survival time.RESULTS:Prognostic analysis demonstrated that female sex,tumor invasion into blood vessels,and Ib stage were independent risk factors.The 1-,3-,and 5-year survival rates of the TCM group vs those of the non-TCM group were 100.0% vs 91.0%,97.6% vs 64.5%,and 81.4% vs 55.5%,respectively.A significant difference in median overall survival(m OS) was found between the two groups(χ2 = 7.670,P = 0.006).Subgroup analysis showed that the m OS of female patients and stage Ib patients in the TCM group were longer than those in the non-TCM group (P ≤ 0.001 and 0.001,respectively).CONCLUSIONS:TCM treatment can improve the survival of patients with stage I GC and high-risk factors.

Efficacy and safety of activating blood circulation and removing blood stasis of Traditional Chinese Medicine for managing renal fibrosis in patients with chronic kidney disease:a systematic review and Meta-analysis

OBJECTIVE:To evaluate the efficacy and safety of activating blood circulation and removing blood stasis in terms of Traditional Chinese Medicine(TCM) for managing renal fibrosis(RF) in patients with chronic kidney disease(CKD).METHODS:We searched randomized controlled trials(RCTs) from eight databases.RESULTS:Sixteen eligible studies with 1,356 participants were included in this study.Compared to treatment with Western Medicine(WM) alone,the combined treatment with activating blood circulation and removing blood stasis in terms of TCM(ARTCM) and WM to manage RF in patients with CKD significantly ameliorated type Ⅳ collagen(C-Ⅳ)(SMD:-2.17,95% CI:3.01 to-1.34),type Ⅲ procollagen(PC-Ⅲ)(SMD:-1.08,95% CI:-1.64 to-0.53),laminin(LN)(SMD:-1.28,95% CI:-1.65 to-0.90),transforming growth factor β 1(TGF-β1)(SMD:-0.65,95% CI:-1.18 to-0.12),serum creatinine(Scr)(SMD:-1.36,95% CI:-1.85 to-0.87),blood urea nitrogen(BUN)(MD:-1.51,95% CI:-2.59 to-0.43),and 24 h urine protein(24h Upro)(SMD:-1.23;95% CI:-1.96 to-0.50).The level of hyaluronic acid(HA) was similar in both types of treatment(SMD:-0.74,95% CI:-1.91 to 0.44).The subgroup analysis showed that the duration of 8 weeks might affect the concentration of C-Ⅳ,PC-Ⅲ,and LN(P < 0.05).The effectiveness of the longer duration to C-Ⅳ,PC-Ⅲ,and LN was not certain.However,the result should be interpreted in care.The safety of the treatment using ARTCM and WM could not be evaluated because a few studies had reported adverse effects.The results of the Meta-analysis were not stable enough.There was publication bias for the reports on Scr(P = 0.001),C-Ⅳ(P = 0.001),PC-Ⅲ(P = 0.026),and LN(P = 0.030) and no publication bias for the reports on BUN(P = 0.293).The quality of evidence varied from low to very low.CONCLUSIONS:The combined treatment using ARTCM and WM to manage RF in patients with CKD has some advantages over treatment with WM alone.High-quality RCTs need to be conducted for the strong support.

OBJECTIVE:To investigate the antipyretic effect of early treatment with Traditional Chinese Medicine(TCM) on coronavirus disease 2019(COVID-19) patients.METHODS:We retrospectively analyzed 369 patients from January 26th,2020 to April 15th,2020,who had been diagnosed with COVID-19.Among 92 eligible cases,45 cases were identified as treatment group Ⅰ(n = 45) and 47 cases were identified as treatment group Ⅱ.Patients in the treatment group Ⅰ were treated with TCM herbal decoction within 5 d after admission.Patients in the treatment group Ⅱ were treated with TCM herbal decoction after the 6th admission day.The onset time of antipyretic effect,the antipyretic time,the time of negative oropharyngeal swab nucleic acid conversion,and the changes of cell count in blood routine test were compared.RESULTS:The treatment group Ⅰ showed shorter average antipyretic duration(4 vs 7 d;P 38 ℃,patients in the treatment group I had shorter median onset time of antipyretic effect than those in the treatment group Ⅱ(3 vs 4 d;P < 0.05).The absolute lymphocyte(LYMPH) count and absolute eosinophil(EOS) count on the 3rd day after admission and the neutrophil/lymphocyte ratio on the 6th day after admission of patients in the treatment group I were notably different from those in the treatment group Ⅱ at the same time point(P < 0.05).Based on Spearman’s rank correlation analysis,the change of body temperature on the 3rd day after admission was positively correlated with the increase of EOS count and the increase of EOS count and LYMPH counts on the 6th day after admission(P < 0.01).CONCLUSIONS:Early TCM intervention within 5 d after hospital admission shortened the onset time of antipyretic effect and fever duration of COVID-19 patients,reduced the time required for PCR test results to turn negative.Moreover,early TCM intervention also impr

Effective therapy options for pneumoconiosis are lacking.Traditional Chinese Medicine(TCM)presents a favorable prospect in the treatment of pneumoconiosis.A pilot study on TCM syndrome differentiation can evaluate the clinical efficacy and safety of TCM and lay a foundation for further clinical research.A double-blind,randomized,and placebo-controlled trial was conducted for 24 weeks,in which 96 patients with pneumoconiosis were randomly divided into the control and treatment groups.Symptomatic treatment was conducted for the two groups.The treatment group was treated with TCM syndrome differentiation,and the control group was treated with placebo.The primary outcomes were the six-minute walking distance(6MWD)and the St.George Respiratory Questionnaire(SGRQ)score.The secondary outcomes were the modified British Medical Research Council Dyspnea Scale(mMRC),Chronic Obstructive Pulmonary Disease Assessment Test(CAT),Hospital Anxiety and Depression Scale(HADS),and pulmonary function.Only 83 patients from the 96 patients with pneumoconiosis finished the study.For the primary outcome,compared with the control groups,the treatment group showed a significantly increased 6MWD(407.90 m vs.499.51 m;95%confidence interval(CI)47.25 to 135.97;P<0.001)and improved SGRQ total score(44.48 vs.25.67;95%CI 27.87 to 9.74;P<0.001).The treatment group also significantly improved compared with the control group on mMRC score(1.4 vs.0.74;95%CI 1.08 to 0.23;P=0.003),CAT score(18.40 vs.14.65;95%CI 7.07 to 0.43;P=0.027),and the total symptom score(7.90 vs.5.14;95%CI 4.40 to 1.12;P<0.001).No serious adverse events occurred.This study showed that TCM syndrome differentiation and treatment had a favorable impact on the exercise endurance and quality of life of patients with pneumoconiosis.

Traditional Chinese Medicine treatment for COVID-19:An overview of systematic reviews and meta-analyses

Background:Coronavirus disease 2019(COVID-19)is a rapidly spreading disease that has caused an extensive burden to the world.Consequently,a large number of clinical trials have examined the efficacy of traditional Chinese Medicine(TCM)for treating and preventing COVID-19,with coinciding proliferation of reviews summarizing these studies.Objective:This study aimed to evaluate the methodological quality and evidence quality of systematic reviews and meta-analyses on the efficacy of TCM.Search strategy:Seven electronic databases,including PubMed,Cochrane Library,Web of Science,China National Knowledge Infrastructure,Chongqing VIP,Wanfang Data and SinoMed,were searched for systematic reviews and meta-analyses in October 2021.Search terms such as“Chinese Medicine,”“Lianhua Qingwen”and“COVID-19”were used.Inclusion criteria:Systematic reviews and meta-analyses of randomized controlled trials that evaluated the efficacy of TCM treatment of COVID-19 were included.Data extraction and analysis:A Measurement Tool to Assess Systematic Reviews Version 2.0(AMSTAR 2)was used to evaluate the methodological quality.The quality of evidence was graded using the Grading of Recommendations Assessment,Development,and Evaluation(GRADE)system.Data extraction and analysis were performed by two reviewers independently.Results:There were 17 meta-analyses included in our overview.The intervention group was defined as TCM combined with Western Medicine,while the control group was Western Medicine alone.The methodological quality of all the included studies was moderate to poor.A total of 89 outcome indicators were evaluated,of which,8 were rated as moderate quality,39 as low quality,and 41 as very low quality.Only one outcome measure was graded as being of high quality.The moderate quality of evidence indicated that,for the treatment of COVID-19,the clinical efficacy of TCM in combination with Western Medicine was better,in terms of lung recovery,rate of conversion to severe/critical cases,symptom scores,duration of symptoms,mortality,and len

Registration of intervention trials of Traditional Chinese Medicine for four neurological diseases on Chinese Clinical Trial Registry and Clinical Trials.gov:a narrative review

OBJECTIVE:To analyze the current status of clinical trial registration of Traditional Chinese Medicine(TCM)for the treatment of neurological diseases.METHODS:Interventional clinical trials of TCM treatment for ischemic stroke,hemorrhagic stroke,vascular cognitive impairment,tension-type headache before September 22,2020 on the platform of Chinese Clinical Trial Registry(Chi CTR),and Clinical Trials.gov were searched.Two researchers independently selected the literature and extracted data.RESULTS:A total of 180 interventional clinical trials were included for analysis.Out of 180 trials,127 were from Chi CTR and 53 from Clinical Trials.gov.The countries primary sponsoring the included trials were China(176,97.8%),and the common categories of primary sponsors were hospital(131,72.8%).Among the study design,the largest proportion of allocation was randomized(172,95.6%),interventional model assignment was parallel(163,90.6%),masking was double blind 49(27.2%),and the sample size was≤400(144,80.0%).The trials were most carried out at a single center(102,56.7%).Among the included studies,112(62.2%)registered on Chi CTR attached the ethical approval documents.In terms of trial stages,50(27.7%)studies were in phaseⅣ.The mostly used intervention was Chinese herbal Medicines(99,55%),acupuncture(68,37.8%)was the second.By searching the registration number on China National Knowledge Infrastructure Database and Pub Med,38(21.1%)registered trials were published,including 25 protocol studies and 14 research results with one(NCT02275949)published both the protocol and the results.CONCLUSIONS:Irregular and inadequate reporting,untimely update and publication,insufficient information on traditional Medicine unique characteristics,and lack of international collaborations are the problems existing in the interventional clinical registration trials of traditional Medicine treatment on neurological diseases.More efforts need to be made from the above aspects to standardize and improve the registration of traditional Medicine trials.

Toxic Medicine used in Traditional Chinese Medicine for cancer treatment: are ion channels involved?

Cancer has been one of the most serious public health issues in the world. Traditional Medicines are widely used in adjunctive therapies in clinical cancer treatment in many countries. One of the unique traditional Medicine usages in tumor treatment is the high-dose application of traditional toxin Medicine, including venom or body from toxin animals. Evidence has shown that they are very likely to have direct effects on cancer cells. One of the potential pharmacological effects of traditional toxin Medicines is their regulation of ion channels in cancers. Many ion channels are found critical in cancers. This study suggested that ion channels were involved in the effect of traditional toxin Medicine on cancers. However, so far, the study of the effect of traditional toxin Medicine on ion channels in cancers is relatively lacking. This perspective article urged the study in this field because, given the fact that these traditional toxin traditional Medicines have been widely used in cancer treatment, the identification of the effective components and pharmacological targets can improve their clinical application.
