The manifest right accessory pathway led to reversible left ventricular dyssynchrony and dysfunction

We report observations in a 7 year old girl with manifest right accessory pathway (AP) and depressed cardiac function. Her Echocardiogram showed LV dilatation with asynchronous ventricular wall motion and LVEF reduction. The electrophysiological study demonstrated a para-his AP. She underwent successfully ablation and without any complications. The Echo validated that asynchronous ventricular wall motion disappeared after ablation. Her cardiac size and function was normal after follow up of three month. Our findings suggested that manifest AP leading to asynchronous ventricular motion was a possible cause of left ventricular dilation and dysfunction. Catheter ablation could reverse cardiac remodeling in such patient. Case report A seven year old girl was referred to our hospital for fainting episode in last six month. To exclude cephalic syncope, she underwent electroencephalogram, the result was negative. The electrocardiogram (ECG) showed right AP. An apparent delta wave looked like LBBB pattern with short PR interval (80ms) and long QRS duration (180ms) (figure 1 left panel). Echocardiogram (Echo) revealed left ventricle dilatation and left ventricular ejection fraction reduction (LVDd=52mm, LVEF=46%). Asynchronous ventriclular wall motion and moderate mitral valve regurgitation also observed on Tissue Doppler Imaging (TDI). She underwent electrophysiology study (EPS) and ablation procedure on the 5th hospital day. The intracardiac electrogram showed a para-His AP with antegrade conduction. The orthodromic SVT (201 bpm) was induce right ventricular apex programmed stimulation. During SVT episode, her symptom was similar to spontaneous one. After successful ablation, delta wave on ECG disappeared and SVT can not be induced with or without isoproterenol. Normal atrial-ventricular conduction was kept without any complications. 引言:在Windows 7环境下,当使用普通用户账号运行某些应用程序时,系统会弹出对话框要求输入管理员账号和密码,否则无法运行。但同一个应用程序在Windows XP环境下却能基本运行。为了安全起见,公司电脑一般都不会赋予用户账号管理员权限,这样很多程序无法在Windows 7下运行。我们可以通过修改manifest文件,来控制应用程序的运行权限。

1291 CASES OF CHOLELITHIASIS TREATED WITH ELECTRIC SHOCK ON OTOACUPOINTS

Since 1985,the authors began to use electric shock on otoacupoints of varying electric resistancefor the treatment of *** instrument used was the Channel Therapeutic Instrumentmade in China,and the otoacupoints of varying electric resistance were Sympathetic,Pancreas-Gall Bladder,Stomach,Liver,Sanjiao(三焦),Endocrine,and Ermigen(耳迷根).Inthe 1291 cases treated,the total effective rate was 99.69%,the rate of calculus excretion was91.32%,and the rate of total excretion was 19.51%.The composition of the calculi wascholesterol crystals(31.25%),biliruhin crystals(28.17%),and mixed crystals(40.58%).Thelargest calculus excreted was an extrahepatic biliary duct calculus of 1.75 cm×1.5 cm;the larg-est number of calculi excreted was 152 cholecystic stones 0.3 cm x 0.5 cm in *** 100 randomcases,the biliary system was shown to manifest vigorous dilations and constrictions underUltrasonic B-scan when the relevant otoacupoints were stimulated with electric *** 78 control cases,no cholecystic stones were excreted,inspite of the Magnesium Sulfate,Folium Cassiae and fatty meals administered to many cases with constipation.

The fundamental deficiency and outward excesssyndrome of coronary heart disease can be classifiedinto Qi Yang deficiency with blood stasis（QYD） andYin deficiency with blood stasis（YD）.The patientsshowed disturbances in immunofunction manifest-ed as marked increase of serum IgG,CIC,IC-IgG and IC-C3 levels and the percentage of Bcells in the peripheral blood,while the percentages ofOKT3+,OKT4+ and OKT8+ cells markedly decreas-ed,especially the percentage of OKT8+ cells,resultingin an increase of the ratio of T4/8 and imbalancebetween TS and TH.The authors deem that thefundamental deficiency of coronary heart disease isrelated to the low cellular immunity,especially theimbalance between TS and TH cells,while outwardexcess of coronary heart disease is related to hyper-activity of humoral immunity.

Role of irrational beliefs in depression and anxiety: a review

Irrational beliefs play a central role in cognitive theory and therapy;they have been shown to be related to a variety of disorders such as depression and anxiety. Irrational beliefs, which can be assessed via clinical interviewing techniques, are frequently assessed by self-report measures, both clinically and for research purposes. Much of the research demonstrating the effect of irrational beliefs has utilized such measures. The present article reviews the empirical work on irrational beliefs assessment and identifies 25 scales and techniques. The measures are organized according to their theoretical affiliation (i.e., either the Ellis or Beck model), with the goal of providing investigators a source to identify the available tests, their shortcomings, and potential applications. The authors conclude with recommendations which would strengthen empirical cohesion and precision in the measurement of irrational beliefs.
