Glucocorticoid pulse therapy in an elderly patient with post-COVID-19 organizing pneumonia:A case report

BACKGROUND Pulmonary fibrosis often occurs as a sequel of coronavirus disease 2019(COVID-19);however,in some cases,it can rapidly progress,similar to the acute exacerbation of interstitial lung *** are the standard treatment for severe COVID-19 pneumonia requiring oxygen supply;however,the post-COVID-19 efficacy of high-dose steroid therapy remains ***,we presented a case of an 81-year-old man who developed acute respiratory failure after COVID-19 and was treated with glucocorticoid pulse *** SUMMARY An 81-year-old man with no respiratory symptoms was admitted due to a diabetic *** had been previously treated for COVID-19 pneumonia six weeks ***,upon admission,he suddenly complained of dyspnea and required a high-flow oxygen *** simple chest radiography and computed tomography(CT)revealed diffuse ground-glass opacities and consolidation in both ***,repeated sputum tests did not identify any infectious pathogens,and initial broad-spectrum antibiotic therapy did not result in any clinical improvement with the patient having an increasing oxygen *** patient was diagnosed with post-COVID-19 organizing ***,we initiated glucocorticoid pulse therapy of 500 mg for three days followed by a tapered dose on hospital day(HD)*** three days of pulse treatment,the patient's oxygen demand *** patient was subsequently discharged on HD 41,and chest radiography and CT scans have almost normalized nine months after *** Glucocorticoid pulse therapy may be considered when the usual glucocorticoid dose is ineffective for patients with COVID-19 sequelae.

Correlation of pulmonary functions of COPD patients to those of their first-degree children

Objective To assess the risk factors correlating to the likelihood for airflow obstruction among first degree children of chronic obstructive pulmonary disease (COPD) patients and whether familial aggregation of pulmonary function abnormality exists Methods Fifty nine smokers with COPD and 28 smokers without COPD as control and all their children available were recruited into the study Their history was recorded and a binary logistic regression analysis was carried out to ascertain the effects of their relationship to a proband with COPD, when other potential risk factors were controlled Results Children with COPD probands showed increased risk of FEV1 below the 70% predicted (OR=1 987) after accounting for the effects of smoking, sex and clinical symptoms The lower the pulmonary function of the COPD proband, the higher the risk to their children for FEV1 below the 70% predicted Conclusions Our finding demonstrates the presence of a household aggregation inclination of COPD and pulmonary function impairment Genetic factors might act as the basis of the familial aggregation

We present a case of necrotizing pneumonia in an 87-year-old man without severe respiratory instability. Clinical suspicion arose due to the need for supplementary oxygen and persistent fever during treatment for community-acquired pneumonia. The diagnosis was confirmed by chest computed tomography. Although necrotizing pneumonia typically requires major surgical intervention upon diagnosis, we chose conservative management with antimicrobials and chest drainage alone. The patient experienced significant improvement and resolution of pneumonia with conservative management.

BACKGROUND While extensive information exists relating cigarette smoking to the risk of lung cancer,chronic obstructive pulmonary disease(COPD),ischaemic heart disease(IHD)or acute myocardial infarction(AMI),and stroke,far less information is available on risks from moist snuff(“snus”)or smokeless tobacco(ST)in United States/Canada,Europe or *** To summarize data from the selected countries on risks of the four diseases associated with current ST or snus *** Publications in English in 1990-2020 were considered that,based on epidemiological studies in North America,Europe or Japan,estimated risks of lung cancer,COPD,IHD/AMI,or stroke according to use of ST or *** studies should involve at least 100 cases of the disease considered,and not be restricted to those with specific other *** literature searches were conducted,selecting papers initially from examination of titles and abstracts,and then from full *** papers were sought from reference lists in selected papers,reviews and *** each disease,relative risk estimates adjusted at least for age were extracted relating ST or snus use to risk,and combined using random-effects *** estimates were mainly for current *** or non-current use,but results for ever vs never use were also *** Seven publications reported results for ST use from six United States *** most useful results came from four studies which provided results for current *** ***-effects meta-analyses of these results showed an increased risk for each disease,clearest for lung cancer(relative risk 1.59,95%confidence interval 1.06-2.39,based on 4 estimates)and COPD(1.57,1.09-2.26,n=3),but also significant(at Plung cancer estimate to 1.80(1.23-2.64,n=6),but had little effect on the other *** snus,16 pu

Pulmonary cryptococcosis in an immunocompetent patient—A case report

Cryptococcosis is an infection caused by fungi that belong to the genus Cryptococcus. There are several species of Cryptococcus, but two species—Cryptococcus neoformans and Cryptococcus gattii—cause nearly all cryptococcal infections in humans. Cryptococcosis is one of the most common fungal infections in Brazil and becomes even more important after the onset of the AIDS epidemic. The lung is the main gateway, and also is the most common site, with diverse clinical manifestations ranging from an asymptomatic to severe pneumonia. When symptommatic, most patients present with fever and cough with expectoration or hemoptysis. The most common radiological findings in immunocompetent patients are located images of masses and nodules, while immunosuppression shows interstitial infiltrates and diffuses interstitial opacity. This is a case report of a patient in the third decade of life with breathing-dependent pain at the base of the left hemithorax, who has radiological examination of a mass in the left lung base in contact with the pleura mimicking malignant lung. Lobectomy was performed, as well as the pathological diagnosis of cryptococcosis.

目的初步探讨肺癌病性证素的分布特点,为肺癌中医临床和基础研究提供参考依据。方法采用临床回顾性研究,搜集整理98例肺癌住院患者的临床资料,运用"中医智能(辅助)诊疗系统"进行系统辨证,提取病性证素,结合患者相关血液检测指标,建立Excel数据库,采用SPSS 19.0统计软件运用频数分析及逐步回归分析等进行统计。结果肺癌常见病性证素分布依次为:阴虚、气虚、饮、痰、阳虚、血虚、热、湿、寒、血瘀、脓、动血、血热、气滞、毒、气逆、燥,其中以阴虚、气虚、饮、痰多见,其中虚性证素之间无明显差异(P>0.05),实性证素饮、痰、热较其他出现频数高(P1),cAl25降低是血瘀的危险因素(OR值=10.908>1),CEA降低是血瘀形成的抑制因素(OR值=0.267<1),红细胞减少是痰、热病理变化形成的抑制因素(OR值=0.412、0.295<1),CA125降低是饮形成的抑制因素(OR值=0.376<1)。结论肺癌的病性证素以虚性证素为主,虚实夹杂,其中虚性病性证素以阴虚、气虚为主,实性病性证素主要为痰、热、饮及寒;随着红细胞的减少程度的增加,越容易出现血虚的病理变化,却越不容易出现痰、热的病理变化;CA125降低可能能加速血瘀、减慢饮病理变化的形成;CEA的降低可能减慢血瘀的形成。

Objectives To assess the effects of an exercise training program combining power-oriented resistance training(RT)and high-intensity interval training(HIIT)on metabolic syndrome(MetS)markers in older people with *** Twenty-nine older people(66–90 years old)with COPD were randomly assigned to 12 weeks of exercise training(ET;power-oriented RT+HIIT)or a control group(CON).Waist circumference,diastolic(DBP)and systolic blood pressure(SBP),and serum fasting glucose,triglycerides and HDL cholesterol levels were assessed at baseline and after 12 *** mixed-effects models were used to assess the effects of the intervention,and data were reported as mean and 95%confidence interval *** Waist circumference increased in the CT group,but not in the ET group(2.0[0.2,3.7]vs.1.0[−1.3,3.2]cm,respectively).No changes in fasting glucose(−4.1[−10.3,2.1]vs.−1.0[−8.7,6.7]mg dL−1),triglycerides(3.9[−13.4,21.3]vs.−13.9[−35.6,7.7]mg dL−1)or HDL cholesterol(1.0[−3.4,5.4]vs.2.9[−2.6,8.4]mg dL−1)were found in the CT or ET group,*** ET group exhibited decreased DBP(−5.2[−9.5,−0.8]mmHg)and SBP(−2.7[−22.7,−2.7]mmHg),while no changes were found in the CT group(0.3[−3.2,3.7]and−3.5[−11.4,4.5]mmHg).MetS z-score declined in ET but remained unchanged in CT(−0.88[−1.74,−0.03]vs.0.07[−0.62,0.76],respectively).Conclusions A 12-week exercise training program led to a reduction in blood pressure and MetS z-score in older people with COPD.
