Epidemiological, Clinical and Paraclinical Profile of Osteoarthritis of the lower limbs in Obese Patients at the Cocody University Teaching Hospital

Objective: To describe the epidemiological, clinical and paraclinical characteristics of osteoarthritis of the lower limbs in obese patients at the Cocody University Teaching Hospital. Methodology: This was an analytical cross-sectional study carried out in the rheumatology department of the Cocody UTH in Abidjan (Ivory Coast) from March 1 to April 30 2023. Patients who came for rheumatology consultation presenting with mechanical arthralgia of the lower limbs, who were obese, had radiographic images were included. All patients without radiographic images were excluded. Obesity was defined as a body mass index (BMI) greater than or equal to 30 kg/m. The Chi2 test was used to compare proportions and determine the existence of associations between osteoarthristis and obesity, obesity severity and radiographic stage of osteoarthritis as well as the functional impact. A p-value below a predefined threshold (p = 0,05) indicates a significant relationship between the variables. Result: Out of 185 patients received for osteoarthritis of the lower limbs during the study period, 136 were obese (74%). There were 115 women (84.6%) with an average age of 56.03 with a standard deviation of 12.72 years (extremes: 22 and 84 years). The main socio-professional category was the informal sector (30%). The majority of patients had a low socio-economic level (80.2%) and lived in urban areas (92.6%). The most common past medical history was hypertension (33.08%) followed by peptic ulcer disease (16.91%). Patients had a body mass index class 1 (81.6%), class 2 (15.40%) and class 3 (2.90%). The average duration of symptom progression until diagnosis was 11 months. Genu varum was the main static disorder (56.10%) and the knee joint was the dominant topography (90.4%) with a bilateral localization (80%). The average Lequesne index was greater than 8 (59.5%). The Kellgren and Lawrence radiographic stages were stage 1 (9.20%), stage 2 (46.90%), stage 3 (29.20%) and stage 4 (6.90%).

Development of electric cart for improving walking ability — application of control theory to assistive technology

This paper explains the development of an electric cart that helps the elderly maintain or improve their physical strength. Unlike commercially available ones, it has a pedal unit that provides some exercise for a user in training his lower limbs. An impedance model describes the feeling of pushing the pedals. The largest pedal load is determined based on a pedaling experiment. An H∞ controller is designed for each of the largest pedal load and virtually no load. A control law, which is based on the concept of dynamic parallel distributed compensation, is designed using the rating of perceived exertion of a driver as a criterion to choose a pedal load between the largest and almost zero. Five university students and twelve elderly people participated experiments to verify the system design and the validity of the system.

Propofol-controlled infusion technique in assessing the safe dosage for sedation during epidural anesthesia

BACKGROUND： Epidural anesthesia requires a convenient and accurate method to determine the plasma concentration of the given subsidiary drug. The target-controlled infusion （TCI） technique, controlling infusion pump by computer, has been used. Intravenous injection of propofol was given for the purpose of measuring plasma concentrations and to observe the effects of different target concentrations on the memory, consciousness, hemodynamics and blood gases of patients, in order to find a safe dosage for sedation in epidural anesthesia. OBJECTIVE： To observe the influence of different target concentrations on the memory, consciousness hemodynamics and blood gases of patients accepted epidural anesthesia. DESIGN ： A randomized and controlled study SETTING ： Department of Anesthesiology, the Second Affiliated Hospital, Medical College of Xi＇an Jiao Tong University. PARTICIPANTS： The experiment was conducted in the Department of Anesthesiology, the Second Affiliated Hospital, Medical College of Xi＇an Jiao Tong University from June 2005 to June 2006. Fifty patients underwent surgery for lower abdomen and lower limbs were selected, including 30 males and 20 females, 30-60 years old with an average of （46±6） years, weighing （65.8±8.5） kg, and American Society of Anesthesiology （ASA） grading of Ⅰ- Ⅱ. All the patients were informed and agreed with the items for detection.METHODS： All the patients were given diazepam （10 mg） and atropine （0.5 mg） intramuscularly 30 minutes before surgery, They were placed supinely and given oxygen inhalation, their blood pressure, heart rate and blood oxygen saturation were determined, then an epidural puncture was made in the left lateral decubitus position with hands crossed over knees. Study on relationship between paraneoplastic syndrome and nerver system injury

Non-metastatic or invaded neoplasms sometimes produce distal effects in nervous system,nerve-muscle junction and muscle i.e paraneoplastic syndrome(PNS).PNS is diverse in clinical *** system symptoms and neoplasm may occur simultaneously or months-years ***,clinical diagnosis and management are very *** is our report about clinical manifestations in 23 PNS patients admitted in our department. 1 Subjects data 25 PNS patients including 18 males and 7 females,aging from 20～ 76(mean age:54.2) were included in our *** consisted of pulmonary cancer(n=16),digestive system tumor(n=3),leukemia(n=1),cervical carcinoma(n=1). All nervous system symptoms occur earlier than neoplasm *** cervical carcinoma patients(1 month～ 6 year,mean:10 months). 64 cases of tibia platform fracture were received and treated from February1994 to December 2000. The patients were guided for rehabilitation treatment as early function exercise and herb washing esc, the effects were satisfactory and it is reported as following. 1 Objects and methods 1.1 Objects 64 cases,39 male,25 female, aged 16～ 72 years, 35 cases were at the left and 29 were at the right. 52 cases were treated in one week and 12 were from one week to two *** fractures were of three types: There were 37 cases of exstrophy type, Degree I: Split fracture or compress fracture of tibia external condyle with little or no transportation. The indented articular facet of the compress was no more than 0.5 cm. There were 15 cases of Degree I in the group. Degree II: 1/3 articular facet at the outside of the external condyle was of split fracture and there were external transportation, the indentation of the internal 2/3 articular facet caused by the external condyle of the femur was no more than 1.0 cm. There were 13 cases of degree II in the group. Degree III: Fracture on both condyles and there were 7 cases of Degree III in the group. There were 18 cases of entropion type and 9 cases of vertical type in the group. 1.2 Methods ( 1) Non- operation treatment ① The lower limbs were fixed with plaster support for two weeks, after swelling at the knee joints disappeared, legs were fixed for another 2～ 4 weeks with plaster support or tube support, there were 30 cases in the group and the method was suitable for Degree I and II of the exstrophy type and entropion or vertical types with little transportation.② Bone traction for 2 weeks, and 2～ 4 weeks of lower limb plaster support or tube support, or continual bone traction combined with early function exercise, there were 9 cases in the group and the method was suitable for Degree III and IV of the exstrophy type, entropion and vertical types with major transportation.( 2) Operation treatment ① Open reduction and inner
