Freehand-robot-assisted laparoscopic colorectal surgery:Initial experience in the Trinidad and Tobago

BACKGROUND laparoscopic colorectal surgery is still developing in the Anglophone Caribbean,having been first performed in the region in the year 2011.We report the initial outcomes using a robot camera holder to assist in laparoscopic colorectal operations.AIM To report our initial experience using the FreeHand®robotic camera holder(Freehand 2010 Ltd.,Guildford,Surrey,United Kingdom)for laparoscopic colorectal surgery in Trinidad&Tobago.METHODS We retrospectively collected data from all patients who underwent laparoscopic colorectal resections using the Freehand®(Freehand 2010 Ltd.,Guildford,Surrey,United Kingdom)robotic camera holder between September 30,2021 and April 30,2022.The following data were recorded:patient demographics,robotic arm setup time,operating time,conversions to open surgery,conversions to a human camera operator,number and duration of intra-operative lens cleaning.At the termination of the operation,before operating notes were completed,the surgeons were administered a questionnaire recording information on ergonomics,user difficulty,requirement to convert to a human camera operator and their ability to carry out effective movements to control the robot while operating.RESULTS Nine patients at a mean age of 58.9±7.1 years underwent colorectal operations using the FreeHand robot:Right hemicolectomies(5),left hemicolectomy(1),sigmoid colectomies(2)and anterior resection(1).The mean robot docking time was 6.33 minutes(Median 6;Range 4-10;SD±1.8).The mean duration of operation was 122.33±78.5 min and estimated blood loss was 113.33±151.08 mL.There were no conversions to a human camera holder.The laparoscope was detached from the robot for lens cleaning/defogging an average of 2.6±0.88 times per case,with cumulative mean interruption time of 4.2±2.15 minutes per case.The mean duration of hospitalization was 3.2±1.30 days and there were no complications recorded.When the surgeons were interviewed after operation,the surgeons reported that there were good ergonomics(100%),with no limitation on inst

Liver metastasis of colorectal cancer is common. Resection of solitary tumors of primary and metastatic colorectal cancer can have a favorable outcome. Open resection of primary colorectal tumor and liver metastasis in one operation or in separate operations is currently common practice. Reports have shown that synchronous resections do not jeopardize short or long-term surgical outcomes and that this is a safe and effective approach in open surgery. The development of laparoscopic colorectal surgery and laparoscopic hepatectomy has made a minimally invasive surgical approach to treating colorectal cancer with liver metastasis feasible. Synchronous resections of primary colorectal tumor and liver metastasis by laparoscopy have recently been reported. The efficacy and safety of laparoscopic colorectal resection and laparoscopic hepatectomy have been proven separately but synchronous resections by laparoscopy are in hot debate. As it has been shown that open resection of primary colorectal tumor and liver metastasis in one operation results in an equally good short-term outcome when compared with that done in separate operations, laparoscopic resection of the same in one single operation seems to be a good option. Recent evidencehas shown that this new approach is a safe alternative with a shorter hospital stay. Large scale randomized controlled trials are needed to demonstrate the effectiveness of this minimally invasive approach.

As an increasing proportion of surgeons mount their learning curves in laparoscopic liver resections,a larger percentage of liver surgery is being performed laparoscopically(1).Syn et al.(2)published an impressive statistical analysis of existing randomized controlled trials(RCT)and propensity-score matched(PSM)studies that model a long-term survival benefit in favor of laparoscopic over open resection for colorectal liver metastases(CRLM).Whilst this is an encouraging outcome,it is critical to analyze the studies included that led to these improved survival outcomes.

BACKGROUND: The short-term results of repair of lapa- roscopic bile duct injuries have been well discussed, but the long-term results have been rarely reported. This study was undertaken to evaluate the factors influencing the outcome of repair of bile duct injuries caused by laparoscopic chole- cystectomy. METHODS: The outcomes of repair of bile duct injuries caused by laparoscopic cholecystectomy in 31 patients were reviewed retrospectively, and the effects of injury recogni- tion, cholangiography, repair modality and techniques on the long-term results were analyzed. RESULTS: Bile duct injuries were repaired successfully in 19 (95%) of 20 patients with injuries who had been recog- nized intraoperatively, and in 10 (90%) of 11 patients with injuries who had been recognized postoperatively. Repair was successful in 29 (93% ) of the 31 patients after complete cholangiography. Closure of partial division, laceration, or small perforation of the bile duct with or without T tube drainage was satisfactory in the 23 patients. End to end re- pair over T tube was successful in 2 transection patients, who were detected intraoperatively. Roux-en-Y hepatico- jejunostomy was used successfully to repair transection, ex- cision or stricture of the bile duct in 4 of 5 patients (80% ). CONCLUSION: Early detection of bile duct injuries caused by laparoscopy, complete evaluation of the biliary duct, and appropriate surgical modality and techniques are help- ful to improve the results of repair for laparoscopic bile duct injuries.

AIM:To describe the clinical characteristics,technical procedures,and outcomes of patients undergoing laparoscopic spleen-preserving distal pancreatectomy(LSPDP)for benign and malignant pancreatic neoplasms.METHODS:The clinical data of 38 patients who underwent LSPDP in the Sir Run Run Shaw Hospital between January 2003 and August 2013 were analyzed retrospectively.Surgical techniques for LSPDP included preservation of the splenic artery and vein(Kimura’s technique)and ligation of the splenic pedicle with preservation of the short gastric vessels(Warshaw’s technique).RESULTS:There were no conversions to open surgery in the 38 patients.Splenic vessels were conserved during spleen-preserving pancreatectomy,except in two patients who underwent resection of the splenic vessels and preservation only of the short gastric vessels.The mean operation time was 123.2±52.4 min,the mean intraoperative blood loss was 78.2±39.5 mL,and the mean postoperative hospital stay was 7.6±2.9 d.The overall rate of postoperative complications was 18.4%(7/38),and the rate of clinical pancreatic fistula was13.2%(5/38).All postoperative complications were treated conservatively.The postoperative pathological diagnoses were 22 cases of benign pancreatic disease and 16 cases of borderline or low-grade malignant lesions.During a median follow-up of 38 mo(range:5-133mo),no recurrence was observed.CONCLUSION:LSPDP is a safe,feasible and effective procedure for the treatment of benign and low-grade malignant tumors of the distal pancreas.

Lymphomas form a heterogenous group of clonal(neoplastic) diseases.Primary renal lymphoma(PRL) is rare. Diagnosis of primary renal lymphoma is important for the patients to receive appropriate therapy.laparoscopic nephrectomy should be the standard procedure in most cases of both malignant and benign,renal tumors with the possible exception of tumor>10cm.Probably this case report of laparoscopic surgery of primary renal lymphoma is the first report of this kind and may be useful for the other laparoscopic surgeons.A 53 years-old man with unilateral primary renal lymphoma who had a history of renal colic and ESWL(Extracorporeal Shock Wave Lithotripsy) of the left kidney stone 3 years ago,he underwent laparoscopic radical left nephrectomy and chemotherapy. The patient was monitored for follow up for 4 months and had a significant improvement.Although treatment of lymphoma is now guided by phenotype of tumor,we found that appropriate treatment is possible after radical nephrectomy and assessment of pathology.Renal lesions may compeletly regress by appropriate treatment.There was not any report of laparoscopic surgery of PRL in our literature.We are reporting the first case of successful laparoscopic surgery of PRL successfully.
