BACKGROUND Situs inversus totalis(SIT)is a rare congenital condition that is characterized by a complete mirror image of the typical arrangement of the thoracic and abdominal *** thoracoscopic segmentectomy for a patient with lung cancer and SIT is an extremely skilled and challenging surgical *** SUMMARY A 41-year old woman with a medical history of dextrocardia since childhood was admitted to our hospital with a mixed ground-glass opacity(mGGO)in her left lung field,discovered by computed tomography during her health *** order to facilitate surgical orientation,three-dimensional computed tomography bronchography and angiography(3D-CTBA)was preoperatively carried *** result of 3D-CTBA was consistent with the diagnosis of SIT and an mGGO in the posterior segment of the left upper lobe(LS2).Surgery was conducted in accordance with preoperative 3D-CTBA and designed surgical procedure,combined with intraoperative *** pathological examination revealed in situ *** patient’s postoperative condition was uneventful and no complications were *** We present the first case of lung cancer in a patient with SIT who successfully underwent thoracoscopic segmentectomy assisted by *** is a new technique that covers precise confirmation and dissection of targeted structures and intersegmental demarcation,and can help achieve a meticulous anatomical segmentectomy.

Laparoscopy-assisted distal gastrectomy for advanced gastric cancer with situs inversus totalis:A case report

Situs inversus totalis(SIT) is a rare anomaly in which the abdominal and thoracic cavity structures are located opposite to their usual positions. Occasionally,patients with this condition are diagnosed with malignant tumors. We report a case of a 60-yearold woman with gastric cancer and SIT. Laparoscopyassisted distal gastrectomy(LADG) with D2 lymph node dissection and Billroth Ⅱ anastomosis were performed successfully on the patient by careful consideration of the mirror-image anatomy. The operation required 230 min, and no intraoperative complications occurred. The final pathological report was p T4 a N0M0,according to the American Joint Committee on Cancer 7th edition staging guidelines. The postoperative course was favorable, and the patient was discharged on postoperative day 8. We believe that this is the first case of LADG with D2 lymphadenectomy reported in a SIT patient with advanced gastric cancer.

A rare complication of a rare disease: Amyloidosis in situs inversus

A 31-year-old female admitted with productive cough and progressively increasing edema in her legs. Four years ago she had the diagnosis of situs inversus (SI) totalis. She had6.5 g/day proteinuria in 24-hour urine analysis. High resolution computed tomography revealed bronchiectatic areas in the left lung. In renal biopsy, kongo and amyloid AA antibody positivity was detected in arteriolar walls and mesangial areas. The patient is currently being followed with ramipril and colchicine treatments. Primary ciliary dyskinesia (PCD) and resultant SI totalis are very rare anomalies characterized by the total inversion of all organs and chronic bronchiectasis. There are very few reports on the association of SI totalis with nephrotic syndrome. Patients with SI totalis and chronic bronchiectasis have increased risk of AA amyloidosis and possibly end stage renal disease and thus these patients should be regularly followed with proteinuria and serum creatinine levels.

Alternate Technique for Doing Laparoscopic Cholecystectomy in Situs inversus

A 45-year-old female with known situs inversus totalis presented with left-sided abdominal discomfort. Chest X-ray, abdominal ultrasonography and CT scan confirmed the diagnosis of a gallstone, as well as, situs inversus;Laparoscopic cholecystectomy was safely performed with mirror image of standard 4 ports. Callots triangle dissection was done with epigastric working port by surgeon, but gallbladder fossa dissection was done by surgical assistant from midclavicular port as main working port. Laparoscopic surgeon should be careful for view of reversed relationships and also for existence of other anomalies.

Isolated Blunt Traumatic Diaphragmatic Rupture in a Case of Situs inversus

Situs inversus, a very rare congenital anomaly of reversal site of thoracic and abdominal organs, can be very problematic to surgeon while dealing with a case of trauma in emergency. Surgical procedures are considered difficult, complex and more challenging in patients with this condition due to the anatomical difference and position of organs. We came across an interesting and very rare case of isolated blunt traumatic diaphragmatic injury in a case of situs inversus. Traumatic injuries of the diaphragm are uncommon and isolated blunt traumatic injuries of diaphragm are very rare. Our case is very unique of its kind of situs inversus with isolated right sided diaphragmatic rupture in a 60-year-old male patient presenting 4 hours after blunt trauma to chest and abdomen.

Hypertrophic osteoarthropathy as the cause of situs inversus totalis: A case report

Situs inversus totalis is a very rare anomaly. Even more, cases of situs inversus totalis plus hypertrophic osteoarthropathy. A woman presented with cyanosis, clubbing of fingers and distal edema. Chest examination revealed heart sounds at the right side and a heart murmur. Abdominal examination showed liver dullness in the left side. The X-rays showed dextrocardia and liver in the left side. The Upper digestive barium test showed the stomach in the right side, the barium enema showed caecum on the left. Treatment for heart failure improved the patient’s condition and she remained well. When she became pregnant, developed heart failure again and required admission. She was compensated and continued her pregnancy. 3 years later she developed heart and respiratory refractory failure, was admitted again and after few days she died. We report this case of situs inversus totalis in association with hypertrophic osteoarthropathy because of the rarity of this anomaly.

Situs inversus during Bariatric Surgery: A Retrospective Analyze

Background: We were reporting three cases of Situs inversus in morbidly obese patients that were submitted a laparoscopic gastric bypass (LGBP). The condition itself was not a serious hazard to normal, healthy patients but, may cause difficulties in the diagnosis and management of abdominal pathology. The incidence of surgical disease did not seem to be different as in the general population, but its presentation, diagnosis and the operative procedure were. This was particularly true for laparoscopic surgery. Methods: From January 1999 to June 2015, 3047 morbidly obese patients underwent. Three patients with Situs Inverse (SI), two of the three with Situs inversus Totalis (SIT), and one with Situs inversus Parcialis (SIP) were submitted to a laparoscopic Roux-en-Y gastric bypass with no major difficulties. The mean age was 45 years (39 - 49), BMI 37.7 kg/m2, two of the above patients were female. Results: The LGBP was completed successfully in two of the three patients with SI. The mean duration of surgery was a 167 minutes, ranging from 110 - 210 minutes. The hospital stay was 5 days (3 - 8). Conclusions: The benefits of surgery were evident, even in the patients with SI;as there was no need for ICU admission and patients had an early hospital discharge. Morbidly obese patients with SI might be submitted to LGBP. This procedure was feasible and safe when the diagnosis was pre-operatively confirmed, and the surgical team made proper adjustments.
