Plaque rupture with subsequent thrombus formation is the common pathophysiological substrate of acute coronary syndrome （ACS）. Moreno et al reported mat neovascularization as manifested by the localized appearance of microvessels is increased in ruptured plaques in the human aorta. Microvessel density is also increased in inflammatory lesions, with intraplaque hemorrhage and in thin-cap fibroatheromas. Microvessels at the base of the plaque are independently correlated with plaque rupture, suggesting a contributory role for neovascularization in the process of plaque rupture. Soluble CD105, a sensitive serum marker of neovascularization, is thought to be associated with cardiovascular disease. The purpose of this study was to assess the relationship between the level of soluble CD105 and the morphological plaques by intravascular ultrasound （IVUS） in patients with stable angina （SA） and those with unstable angina （UA） and whether soluble CD105 may serve as a non-invasive marker of coronary plaque destabilization.

Objective:Stenting for coronary artery disease is generally accepted as a percutaneous coronary intervention(PCI),but there remain many challenges with this *** coronary atherectomy(DCA)can reduce plaque volume,and its use alone or in combination with drug-coated balloons can reduce stent ***,clinical studies on DCA are limited,and its safety and effectiveness remain *** aimed to evaluate the safety and effectiveness of the new DCA device-ATHEROCUT with intravascular ultrasound(IVUS)***:Between May 2015 and May 2018,45 patients with 48 lesions were treated electively with ATHEROCUT in Kusatsu Heart *** retrospectively analyzed in-hospital major adverse cardiac events(MACE),overall clinical follow-up MACE,complications rate,and dual antiplatelet therapy(DAPT)*** definition of MACE included cardiac death,myocardial infarction,and target lesion revascularization(TLR).Our definition of complications included coronary perforation,thrombosis,and major ***:The overall follow-up rate was 95.6%,and the average follow-up period was 453±320 ***-hospital and overall MACE rates were 2.2%(TLR 0)and 11.1%(TLR 8.9%),*** were no complications,except for one cerebral hemorrhage case(2.1%),which was considered irrelevant to the PCI *** DAPT duration was 8.3±8.3 months,only 4.8±4.9 months in the subgroup that did not require another ***:Using the ATHEROCUT device with IVUS guidance is safe,might be effective in selected patients and can shorten the DAPT period.

Background The intravascular ultrasound(IVUS)-guided percutaneous coronary intervention(PCI)can benefit the treatment of coronary artery disease(CAD).However,the beneficial effect of IVUS-guided PCI in patients with acute myocardial infarction(AMI)remains *** A total of 537 AMI patients were included in this study,divided into two groups:the angiography-guided group(n=289)and the IVUS-guided group(n=248)according to whether the IVUS was *** primary outcome was a composite of major adverse cardiovascular events(MACEs),including cardiovascular death,recurrent myocardial infarction(MI),and target lesion revascularization(TLR).The secondary outcome was procedural radiation exposure *** The IVUS-guided group was associated with a reduced incidence of MACEs(12.8%vs.8.5%,P=0.032).The results were consistent after adjusting for confounders in the multivariable Cox analysis,which showed that the absence of IVUS(HR 1.194,95%CI 1.061-1.323,P=0.011)was an independent predictor of ***,the IVUS-guided group experienced significantly lower float time(893.3±265.2 min vs.623.2±137.3 min,P<0.001).Conclusions The use of IVUS was associated with better long-term cardiovascular *** use of IVUS in PCI should be considered for patients with AMI to optimize procedural outcomes and enhance long-term prognosis.

intravascular ultrasound (IVUS) is a new technology for the diagnosis of coronary artery disease, and for the support of coronary intervention. IVUS image segmentation often encounters difficulties when plaque and acoustic shadow are present A novel approach for hard plaque recognition and media-adventitia border detection of IVUS images is presented in this paper. The IVUS images were first enhanced by a spatial-frequency domain filter that was constructed by the directional filter and histogram equalization. Then, the hard plaque was recognized based on the intensity variation within different regions that were obtained using the k-means algorithm. In the next step, a cost matrix representing the probability of the media-adventitia border was generated by combining image gradient, plaque location and image intensity. A heuristic graph-searching was applied to find the media-adventitia border from the cost *** results showed that the accuracy of hard plaque recognition and media-adventitia border detection was 89.94% and 95.57%, respectively. In conclusion,using hard plaques recognition could improve media-adventitia border detection in IVUS images.

Background To investigate the relationship btween serum uric acid （SUA） and cardiovascular plaque composition by intravascular ultrasound （IVUS）. Methods From March 2005 to August 2009, 81 patients were explored cardiovascular plaque composition by IVUS and taken SUA of measurement. According to American Heart Association IVUS criterion, coronary artery plaque was divided into four categories：（1） Fibrous plaque; （2） Soft plaque; （3） Calcified plaque; （4） Mixed plaque. We investigated the relationship btween SUA and cardiovascular plaque composition by IVUS. Results It was shown that the level of SUA in calcified plaque group was significantly higher than other three groups（P0.01）, which in fiber plaque group was significantly higher than mixed plaque group（P=0.032）, there was no significant difference between soft plaque group and mixed plaque group（P=1.00）among the four groups. The level of SUA among the four groups from high to low was as follows：calcified plaque, fibrous plaque group, soft plaque group and mixed plaque group. Conclusions The present study found that higher SUA in calcified plaque group, which proved that the relationship between SUA and cardiovascular plaque composition.

Objective: We used intravascular ultrasound (IVUS) to analyze the features of coronary artery atheromatous plaque in patients with impaired glucose tolerance and mild-to-moderate angiographic coronary stenosis. The aim was to determine the clinical significance of plaque characteristics as well as the relationship between hemoglobin A1c (HbA1c) levels and coronary artery lesions. Methods: HbA1c levels were evaluated in 85 patients (96 lesions), of whom 46 had impaired glucose tolerance (IGT Group) and 39 had normal blood glucose (NBG Group). IVUS was used to analyze the lesion vessel of both groups qualitatively and quan-titatively. The external elastic membrane area (EEMA), minimal lumen area (MLA), plaque area (PA), and plaque burden (PB) were measured for both the target lesion and the reference segments (reference external elastic membrane area (REEMA), reference minimal lumen area (RMLA), reference plaque area (RPA), and reference plaque burden (RPB), respectively). Results: HbA1c levels were significantly higher in the IGT Group than in the NBG Group (P<0.05). In the IGT Group there was more soft plaque, eccentric plaque, and positive remodeling, and less calcification, while in the NBG Group there was much harder plaque and calcification, no reconstruction, and negative remodeling (P<0.05). MLA was smaller in the IGT Group than in the NBG Group, while EEMA, PA, and PB were clearly greater (P<0.05). In the meantime, RMLA was clearly smaller in the IGT Group than in the NBG Group, while RPA and RPB were greater (P<0.05). HbA1c levels were positively correlated with PA and PB, and negatively correlated with MLA. Conclusion: IVUS is very valuable for the evaluation of mild-to-moderate coronary lesions. The coronary artery lesions in patients with IGT are more serious and widespread than those in patients with NBG. HbA1c levels might be of some value in assessing the severity of coronary artery lesions.
