Aim:We aimed to analyze temporal trends in mortality from intrahepatic(ICC)and extrahepatic(ECC)cholangiocarcinoma in selected countries ***:Official death certification data for ICC and ECC and populations estimates for 29 countries worldwide(17 from Europe,8 from the Americas,and 4 from Australasia)and for Hong Kong Special Administrative Region of the People’s Republic of China(SAR),from 1995 to 2018,were extracted from the World Health Organization and the Pan American Health Organization ***-standardized mortality rates were computed.A joinpoint regression analysis was ***:In both sexes,ICC mortality rates increased in most countries considered,including the USA,the UK,and Australia;in some countries,including Italy and France,the increasing trends leveled off over the most recent *** men,around 2016,the highest rates(1.7-2.3/100,000)were observed in Hong Kong SAR,Portugal,France,Spain,Australia,Austria,the UK,and Canada;Latin American countries and some eastern European countries had the lowest rates(0.2-0.8/100,000).A similar pattern was observed in women,but with lower rates(from 1.7/100,000 in Hong Kong SAR to 0.14/100,000 in Argentina).ECC mortality declined in most European and Australasian countries,but it tended to increase in *** both sexes,rates were below 1/100,000 around 2016,with the only exceptions being Japan(2.6/100,000 men and 1.2/100,000 women)and Hungary(1.5/100,000 men and 1.1/100,000 women).Conclusion:ICC mortality increased in most areas of the world,likely due to increased prevalence of risk factors and improved cancer recognition and *** mortality fell in most countries,largely due to the widespread use of cholecystectomy.

Reduced expression of P120 catenin in cholangiocarcinoma correlated with tumor clinicopathologic parameters

AIM： To investigate the relationship between the expression of P120 and the clinicopathologic parameters in intrahepatic cholangiocarcinoma （ICC）. METHODS： An immunohistochemical study of E-cadherin and P120 catenin was performed on 42 specimens of ICC with a Dako Envision kit. RESULTS： The expression of E-cadherin and P120 was reduced in 27 cases （64.3%） and 31 cases （73.8%）, respectively. Both E-cadherin and P120 expressions were significantly correlated with the tumor histological grade （χ^2 = 9.333, P = 009 and χ^= 11.71, P = 0.003）, TNM stage （χ^= 8.627, P = 0.035 and χ^= 13.123, P = 0.004）, intrahepatic metastasis （χ^= 7.292, P = 0.007 and χ^= 4.657, P = 0.041, respectively） and patients′ survival （χ^= 6.351, P = 0.002 and χ^= 4.023, P = 0.000, respectively）. In addition, the expression of P120 was in concordance with that of E-cadherin （χ^ = 13.797, P = 0.000）, indicating that the expression of P120 may be dependent on that of E-cadherin. Finally, only P120 expression was found to be an independent prognostic factor in Cox regression model （r = 0.088, P = 0.049）. CONCLUSION： Down-regulated expression of E-cadherin and P120 occurs frequently in ICC and contributes to the progression and development of tumor. Both of them may be valuable biologic markers for predicting tumor invasion, metastasis and patients′ survival, but only P120 is an independent prognostic factor for ICC.

Prognosis of hepatocellular carcinoma expressing cytokeratin 19:Comparison with other liver cancers

AIM:To investigate whether expressing biliary phenotype predicted poor outcome after the surgical treatment in primary liver cancers. METHODS:Out of 204 patients that underwent liver resection due to hepatocellular carcinoma (HCC), liver specimens of 70 patients with HCC were evaluated for biliary components by cytokeratin (CK) 19 immunostain (CK19 - HCC and CK19 + HCC). CK19 positivity was defined as membranous and/or cytoplasmic expression in ≥ 5% of tumor cells with moderate or strong intensity. Patients with other primary liver cancers, such as com- bined HCC and cholangiocarcinoma (cHCC-CC), intrahe- patic cholangiocarcinoma (ICC) who received curative liver resection, were also included in the study. Clinical characteristics of CK19-HCC and CK19 + HCC patients, including survival outcome after curative liver resection, were compared with that of cHCC-CC and ICC patients. RESULTS: The overall survival (OS) rate of CK19 - HCC(n = 49) after the curative surgical treatment was 90.7%, and 80.4% at 1 and 5 years after the resection. OS rate of CK19 + HCC (n = 21) was 74.3%, 28.9% and OS rate of cHCC-CC (n = 22) was 66.7%, 32.2% at 1 and 5 years after the surgery. For ICC (n = 19), 1 and 5-year-OS rate was 50.2% and 14.3% after the cura-tive resection. The OS rates of CK19 + HCC and cHCC-CC were significantly lower than that of CK19-HCC, but higher than the OS rate of ICC (P = 0.000). There was no statistically significant difference in OS rate between CK19 + HCC and cHCC-CC. The disease free survival (DFS) rate of CK19-HCC was 72.0% and 54.5% at 1 and 3 years after the surgical treatment. DFS rate of CK19 + HCC was 53.3%, 34.3% and DFS rate of cHCC- CC was 51.5%, 39.2% at 1 and 3 years after the resection. For ICC, 1 and 3-year-DFS rate was 28.0% and 14.0% after the curative resection. DFS rate of CK19-HCC was significantly higher than that of ICC (P = 0.017), but marginally higher than DFS rate of either CK19 + HCC or cHCC-CC (P = 0.097, P = 0.089, respec-tively).
