Effects of acupuncture in treating insomnia due to spleen-stomach disharmony syndrome and its influence on Intestinal microbiome: Study protocol for a randomized controlled trial

Background: Insomnia is a common complaint that is closely related to gastroIntestinal symptoms, which is consistent with the traditional Chinese medicine classical theory of ‘‘stomach disharmony leading to restless sleep." Acupuncture is an effective complementary and alternative medicine therapy to improve gastroIntestinal function and restore the normal sleep-wake cycle. However, studies on the effectiveness of acupuncture for insomnia due to spleen-stomach disharmony syndrome are limited to case reports and few randomized controlled trials; deeper research on its mechanism is still lacking. This randomized controlled trial aims to assess the treatment efficacy of ‘‘harmonizing stomach to tranquilize mind"acupuncture for insomnia and its influence on the Intestinal microbiome.Methods/design: This is a randomized, single-blind, parallel-group study. Sixty eligible patients with insomnia due to spleen-stomach disharmony syndrome will be randomly divided into two groups(1:1 allocation ratio). The intervention group will use ‘‘harmonizing stomach to tranquilize mind"acupuncture, and the control group will receive sham acupuncture. Participants will receive 5 acupuncture treatment sessions per week for 4 consecutive weeks. The Pittsburgh Sleep Quality Index will be used to evaluate the clinical efficacy of acupuncture treatment by making assessments at baseline, the end of treatment and the end of the follow-up. High-throughput 16 S ribosomal ribonucleic acid gene sequencing will be performed to detect changes in the Intestinal microbial composition before and after treatment.Discussion: The results of this trial are expected to confirm that ‘‘harmonizing stomach to tranquilize mind" acupuncture can effectively relieve insomnia and alter the Intestinal microbiome.

Effect of graphene oxide exposure on Intestinal Wnt signaling in nematode Caenorhabditis elegans

Exposure to engineered nanomaterials(ENMs), such as graphene oxide(GO), can potentially induce the response of various molecular signaling pathways, which can mediate the protective function or the toxicity induction.Wnt signaling pathway is conserved evolutionarily in organisms.Using Caenorhabditis elegans as an in vivo assay model, we investigated the effect of GO exposure on Intestinal Wnt signaling.In the intestine, GO exposure dysregulated Frizzled receptor MOM-5, Disheveled protein DSH-2, GSK-3(a component of APC complex), and two β-catenin proteins(BAR-1 and HMP-2), which mediated the induction of GO toxicity.In GO exposed nematodes, a Hox protein EGL-5 acted as a downstream target of BAR-1, and fatty acid transport ACS-22 acted as a downstream target of HMP-2.Functional analysis on HMP-2 and ACS-22 suggested that the dysregulation of these two proteins provides an important basis for the observed deficit in functional state of Intestinal barrier.Our results imply the association of dysregulation in physiological and functional states of Intestinal barrier with toxicity induction of GO in organisms.

Biomimetic Intestinal barrier based on microfluidic encapsulated sucralfate microcapsules

Intestinal barriers play an important role in preventing Intestinally derived diseases,and maintaining their function is a promising approach to prevent and treat those diseases.Here,inspired by the protection effect of Intestinal barriers in live organisms and the mucosa adhesive property of sucralfate,we present a biomimetic Intestinal barrier based on microfluidic encapsulated sucralfate microcapsules.Benefiting from the flexible selectivity and precise control of microfluidic electrospray flows,the generated microcapsules were imparted with stomach-tolerant dietary-fibre shells and controllable released sucralfate cores,both of which could contribute to forming a continuous biomimetic Intestinal barrier on the intestine.Through in vitro adhesive study,in vivo computed tomography(CT)imaging and in vivo imaging system(IVIS)methods,we have demonstrated that the microcapsule-derived biomimetic Intestinal barrier can effectively block food fermentation in the gut,reduce generation of fat,decrease disease risk indexes,and prevent obesity.These features make the microfluidic encapsulated sucralfate microcapsules and their resultant biomimetic Intestinal barrier an approach for treating obesity and other Intestinal diseases.

Alterations in Intestinal microbiota of colorectal cancer patients receiving radical surgery combined with adjuvant CapeOx therapy

An intricate relationship exists and interactions occur between gut microbiota and colorectal cancer(CRC).Radical surgery combined with adjuvant chemotherapy(AC) serves as the mainstream therapeutic scheme for most CRC patients.The current research was conducted to assess the effect of surgery or chemotherapy on gut microbiota.Forty-three CRC patients who received radical surgery and AC were enrolled.Fecal samples were collected preoperatively,postoperatively,and after the first to fifth cycles of postoperative chemotherapy.The microbial community of each sample was analyzed using high throughput 16S rRNA amplicon sequencing.Compared with preoperative samples,fecal samples collected postoperatively exhibited a significant decrease of obligate anaerobes,tumor-related bacteria,and butyric acid-producing bacteria.However,a significant increase of some conditional pathogens was observed.In addition,the AC regimen(CapeOx) was found to alter Intestinal microbiota dramatically.In particular,several changes were observed after chemotherapy including an increase of pathogenic bacteria,the "rebound effect" of chemotherapy-adapted bacteria,the shift of lactate-utilizing microbiota from Veillonella to Butyricimonas and Butyricicoccus,as well as the decrease of probiotics.Both radical surgery and CapeOx chemotherapy exert a non-negligible effect on the gut microbiota of CRC patients.Microbiota-based intervention may be beneficial for patients during postoperative clinical management.

Background Recurrent respiratory tract infection (RRTI) is a disease occurred frequently in preschool children.Methods A total of 120 RRTI children were randomly divided into active group,remission group,intervention group and control group,meanwhile 30 healthy children were selected as the healthy group.Children in the intervention group were given oral Bifidobaeterium tetravaccine tablets (Live) for 2 months,while the control group received routine treatment.Stool sample were detected to analyze the bacterial strains.The occurrence of respiratory tract infection (RTI) was compared between different groups during 1 year follow-up.Results Compared with the healthy group,the number of Bifidobacteria and Lactobacilli in the active group,remission group,intervention group and control group was significantly decreased (P Intestinal flora imbalance.Oral probiotics can effectively improve the RRTI Intestinal microecological balance in children and reduce the frequency of RTI.

Houttuynia cordata polysaccharide alleviated Intestinal injury and modulated Intestinal microbiota in H1N1 virus infected mice

Houttuynia cordata polysaccharide(HCP) is extracted from Houttuynia cordata, a key traditional Chinese medicine. The study was to investigate the effects of HCP on Intestinal barrier and microbiota in H1N1 virus infected mice. Mice were infected with H1N1 virus and orally administrated HCP at a dosage of 40 mg×kg^(–1)×d^(–1). H1N1 infection caused pulmonary and Intestinal injury and gut microbiota imbalance. HCP significantly suppressed the expression of hypoxia inducible factor-1α and decreased mucosubstances in goblet cells, but restored the level of zonula occludens-1 in intestine. HCP also reversed the composition change of Intestinal microbiota caused by H1N1 infection, with significantly reduced relative abundances of Vibrio and Bacillus, the pathogenic bacterial genera. Furthermore, HCP rebalanced the gut microbiota and restored the Intestinal homeostasis to some degree. The inhibition of inflammation was associated with the reduced level of Toll-like receptors and interleukin-1β in intestine, as well as the increased production of interleukin-10. Oral administration of HCP alleviated lung injury and Intestinal dysfunction caused by H1N1 infection. HCP may gain systemic treatment by local acting on intestine and microbiota. This study proved the high-value application of HCP.

Effects of probiotics and prebiotics on Intestinal microbiota in mice with acute colitis based on 16S rRNA gene sequencing

Background:Imbalance of Intestinal microbiota was closely related to colitis.Under these circumstances,regulation of enteric flora may be beneficial to the repair of inflammation.We aimed to investigate the effects of probiotics (Bifidobacterium and Lactobacillus),prebiotics and their combination on inflammation,and microflora in mice of acute colitis.Methods:C57BL/6J mice were divided into six groups randomly (blank control group,model control group,probiotics group,synbiotics group,lactitol group and probiotics + lactitol group).Each group was given 2.5% dextran sulfate sodium drinking water for 5 days other than the blank control group.Except for the model control group,the other four groups were intervened with probiotics,synbiotics (probiotics and inulin),lactitol,and probiotics + lactitol.Mice were sacrificed after 1 week of gavage,and pathologic scores were calculated.The feces of different periods and Intestinal mucosa samples were collected to analyze the differences of Intestinal microbiota by 16S rRNA sequencing.Differences of two groups or multiple groups were statistically examined through unpaired Student t test and analysis of variance (ANOVA),respectively.ANOVA,Tukey,Anosim,and metastats analysis were used to compare differences of microbiota among different groups.Results:After gavage for 1 week,the pathologic scores of groups with the intervention were significantly lower than those in the model control group,and the difference was statistically significant (P < 0.05).The model control group was higher in the genus of Bacteroides (relative abundance:0.3679 vs.0.0099,P =0.0016) and lower in Lactobacillus (relative abundance:0.0020 vs.0.0122,P =0.0188),Roseburia (relative abundance:0.0004 vs.0.0109,P =0.0157),compared with the blank control group.However,the same phenomenon was not found in groups gavaged with probiotics and lactitol.Compared with model control group,mice with intervention were increased with Bifidobacterium (relative abundance:0.0172 vs.0.0039,P =0.0139),Lachnospiraceae_NK4A136_g

BACKGROUND Some substances of plant origin have been reported to exert an effect in reducing Intestinal neoplasm development,especially in animal models.Adenomatous polyposis coli multiple Intestinal neoplasia-ApcMin/+is the most studied murine model of genetic Intestinal carcinogenesis.AIM To assess whether an enriched nutritional formulation(silymarin,boswellic acid and curcumin)with proven“in vitro”and“in vivo”anti-carcinogenetic properties may prevent inherited Intestinal cancer in animal model.METHODS Forty adenomatous polyposis coli multiple Intestinal neoplasia-ApcMin/+mice were used for the study of cancer prevention.They were divided into two groups:20 assumed standard and 20 enriched diet.At the 110th d animals were sacrificed.In each group,four subgroups received intraperitoneal bromodeoxyuridine injection at different times(24,48,72 and 96 h before the sacrifice)in order to assess epithelial turnover.Moreover,we evaluated the following parameters:Intestinal polypoid lesion number and size on autoptic tissue,dysplasia and neoplasia areas by histological examination of the whole small intestine,inflammation by histology and cytokine mRNA expression by real-time polymerase chain reaction,bromodeoxyuridine and TUNEL immunofluorescence for epithelial turnover and apoptosis,respectively.Additionally,we performed western blotting analysis for the expression of estrogen alpha and beta receptors,cyclin D1 and cleaved caspase 3 in normal and polypoid tissues.RESULTS Compared to standard,enriched diet reduced the total number(203 vs 416)and the mean±SD/animal(12.6±5.0 vs 26.0±8.8;PIntestinal carcinoma(IC;P<0.001)were significantly decreased in enriched diet group.IC was observed in 100%in standard and 85%in enriched formulation assuming animals.Enriched diet showed a faster epitheli

Chidamide based combination regimen for treatment of monomorphic epitheliotropic Intestinal T cell lymphoma following radical operation:Two case reports

BACKGROUND Monomorphic epitheliotropic Intestinal T cell lymphoma(MEITL)is a rare extranodal T-cell lymphoma that has uniformly aggressive features with a poor prognosis.No standardized treatment protocols have been established.Previous experience has demonstrated favorable outcomes with combination chemotherapy followed by autologous hematopoietic stem cell transplant.However,many patients are unable to tolerate the toxicities.Chidamide is a new histone deacetylase inhibitor that has shown preferential efficacy in mature T-cell lymphoma.CASE SUMMARY We herein present two cases of MEITL who were both intermediate risk according to enteropathy-associated T cell lymphoma prognostic index.Case one was a 61-year-old man.He complained of upper abdominal pain and intermittent black stool for 2 mo.Imaging examination revealed that the Intestinal wall was thickened.He received a partial excision of the small intestine.A chidamidebased combination regimen was given postoperatively.Eleven months later,he presented with recurrence in the bilateral lungs.He passed away 15 mo after his diagnosis.Case two was a 35-year-old woman who complained of abdominal distention for 1 mo.Positron emission tomography/computed tomography demonstrated wall thickening of the small intestine and upper sigmoid colon.Colon perforation and septic shock occurred on the fourth day of her admission.She was treated by sigmoid colostomy.Chidamide-based combination therapy was then provided.She was recurrence-free for 6 mo until lesions were found in the bilateral brain and lived for 17 mo since her diagnosis.Compared to historical data,chidamide seems to improve the prognosis of MEITL slightly.CONCLUSION MEITL is a type of aggressive lymphoma.Chidamide is a new promising approach for the treatment of MEITL.

Identification of active compound combination contributing to anti-inflammatory activity of Xiao-Cheng-Qi Decoction via human Intestinal bacterial metabolism

Human Intestinal bacteria play an important role in the metabolism of herbal medicines, leading to the variations in their pharmacological profile. The present study aimed to investigate the metabolism of Xiao-Cheng-Qi decoction(XCQD) by human Intestinal bacteria and to discover active component combination(ACC) contributing to the anti-inflammatory activity of XCQD. The water extract of XCQD was anaerobically incubated with human Intestinal bacteria suspensions for 48 h at 37 °C. A liquid chromatography-hybrid quadrupole time-of-flight mass spectrometry(LC-Q-TOF/MS) method was performed for identification of the metabolites. In addition, the anti-inflammatory effects of XCQD and biotransformed XCQD(XCQD-BT) were evaluated in vitro with cytokines in RAW264.7 cells induced by lipopolysaccharide(LPS). A total of 51 compounds were identified in XCQD and XCQD-BT. Among them, 20 metabolites were proven to be transformed by human Intestinal bacteria. Significantly, a combination of 14 compounds was identified as ACC from XCQD-BT, which was as effective as XCQD in cell models of inflammation. In conclusion, this study provided an applicable method, based on Intestinal bacterial metabolism, for identifying combinatory compounds responsible for a certain pharmacological activity of herbal medicines.
