Biological markers of Undernutrition and Risk of Complications in Children Hospitalized at the Teaching Hospital of Brazzaville

Objective:To evaluate neutrophil-(NLR),monocyte-(MLR),eosinophil-(ELR) and basophil-lymphocyte ratios(BLR)in systemic autoimmune rheumatic diseases(SARDs).Methods:A total of 1139 SARD patients and 170 healthy individuals were enrolled,x-test was used to compare difference between *** were plotted and AUCs were calculated to assess differentiating performance of nonlymphocyte-lymphocyte ratios between one SARD and ***:(1) NLR was ssignificantly higher in SARD patients than in healthy *** was higher in most SARD patients except OA than in healthy *** was significantly decreased in SLE patients,but increased in other SARD patients,compared with healthy *** was lower in most SARD patients except PM/DM and PMR than healthy controls.(2) NLR,MLR,ELR and BLR had some differentiating performance for some *** showed the largest AUC for differentiating SLE from other SARDs and healthy individuals.(3) Markedly higher levels of MLR and ELR in AS patients and of NLR and MLR in those with pSS,SSc and PMR were observed in male than in *** age significantly correlated with NLR,MLR and ELR in RA,with MLR and ELR in OA,with ELR and BLR in MCTD and with ELR in PMR.(4) In SLE,CRP showed positive correlation with NLR,MLR and *** significantly negatively correlated with NLR and positively with *** RA,AS and OA,NLR and MLR significantly positively correlated with ESR and *** pSS,ESR showed markedly positive correlation with NLR and MLR,while CRP,IgG and IgA did only with ***:The study demonstrated increased NLR and MLR and deceased BLR in most SARDs,decreased ELR in SLE and increased ELR in other *** and MLR may be useful tools to reflect inflammatory status of SARDs.

Introduction: Undernutrition is a condition frequently encountered in pediatrics. It leads to an increased morbidity and mortality regardless of the underlying condition or other risk factors such as age. In countries with limited resources such as ours, the diagnosis of undernutrition is often limited to the clinical presentation, and the contribution of biology is not often taken into account. Objectives: To establish the relationship between anthropometric parameters and biological markers in the diagnosis and classification of undernutrition and to assess the risk of infectious complications during undernutrition in children. Materials and Methods: A cross-sectional study conducted in Brazzaville among undernourished children aged 1 - 59 months between October 2018 and April 2019. Clinical diagnosis was based on WHO growth charts. The CRP, orosomucoid, albumin and transthyretin were obtained using the Cobas c311 analyzer, which enabled the calculation of Prognostic inflammatory and Nutritional Index (PINI). The comparison of the means of the biological markers used the Student’s t-test, the risk of infectious complications the chi-square. The correlation of the diagnostic value of Z-score weight/height and PINI was also investigated. The significance level was set at 0.05. Results: Of the 95 children enrolled 63 (66.3%) were clinically severely malnourished, including 26 acute (41.3%) and 37 chronic (58.7%). The PINI revealed severe undernutrition in 85 children (89.4%) including 50 acute (58.8%) and 35 chronic (41.2%). CRP and orosomucoid were statistically higher in severe acute undernutrition (p Conclusion: Anthropometric parameters have a front-line advantage for assessing and classifying undernutrition. However, biological markers of undernutrition with PINI should be systematized in the diagnosis and management of undernutrition.

Background: Coronavirus disease 2019 which is officially known as COVID-19 belongs to family viruses. COVID-19 manifestations vary among affected people. These symptoms may become more serious among patients suffering from chronic disease and those who are on treatment which may af-fect their defense mechanism or immune-compromised patients who become more vulnerable to complications of COVID-19, and at high risk for morbidity and mortality with any bacterial or viral illness. Method: A retrospective, non-experimental research design was applied with a quantitative approach among patients with COVID-19 who were admitted to COVID-19 department at AVH with a total of 72 patients. Data were extracted from a patients’ elec-tronic medical record. Results: During COVID-19 outbreak 72 patients were admitted to COVID department at AVH, 54.2% were female and 33% of study participants were from Gaza governorate followed by Jerusalem 27%. Most participants 34.7% had first clinic visit after two days from the onset of COVID-19 symptoms. 45.8% were discharged to home while 13.9% died. All inflammatory markers that include ferritin, C-RP, IL-6 and D-dimer are in-creasing in all patients that were admitted to hospital;IL-6 and D-dimer were significant inflammatory markers in relation to the mortality rate. The study found the risk of mortality with IL-6 mean (218.5), and D-dimer mean (12). Furthermore there was a relation between increased risk of mortality and im-mune comprised. Conclusion: Mortality rate increased among COVID-19 pa-tients when IL-6 was higher than 218.5 and D-dimer higher than 12, and there was a relationship between increased risk of mortality and immune comprised.
