Rifaximin improves survival in cirrhotic patients with refractory ascites: A real-world study

BACKGROUND Rifaximin has been shown to reduce the incidence of hepatic encephalopathy and other complications in patients with ***,few studies have investigated the effect of rifaximin in cirrhotic patients with refractory *** To evaluate the effects of rifaximin in the treatment of refractory ascites and to preliminarily explore its possible *** A total of 75 cirrhotic patients with refractory ascites were enrolled in the study(50 in a rifaximin and 25 in a control group).Patients in the rifaximin group were divided into two subgroups according to the presence of spontaneous bacterial peritonitis and treatment with or without other antibiotics(19 patients treated with rifaximin and 31 patients treated with rifaximin plus intravenous antibiotics).All patients received conventional treatment for refractory ascites,while patients in the rifaximin group received oral rifaximin-α200 mg four times daily for at least 2 *** ascites grade,fasting weight,liver and kidney function,and inflammatory factors in the plasma were evaluated before and after *** addition,the gut microbiota was determined by metagenomics sequencing to analyse the changes in the characteristics of the gut microbiota before and after rifaximin *** patients were followed for 6 *** Compared with the control group,the fasting weight of patients significantly decreased and the ascites significantly subsided after treatment with rifaximin(P=0.011 and 0.009,respectively).The 6-mo survival rate of patients in the rifaximin group was significantly higher than that in the control group(P=0.048).The concentration of interferon-inducible protein 10 decreased significantly in the rifaximin group compared with that in the control group(P=0.024).The abundance of Roseburia,Haemophilus,and Prevotella was significantly reduced after rifaximin treatment,while the abundance of Lachnospiraceae_noname,Subdoligranulum,and Dorea decreased and the abundance of Co

A preliminary study on the establishment of an animal model of conjunctivochalasis

AIM： To explore a feasible method on the establishment of an animal model of conjunctivochalasis （CCH）. METHODS： Twelve clean-grade New Zealand white rabbits were divided into four groups （n=3/group）： the control group （one received no interventions, and the others underwent subconjunctival injection of sterile water）, the matrix metalloproteinases （MMPs） group （administered subconjunctival injection of MMP-3）, the aging group （administered subcutaneous injection of D-galactose）, the tumor necrosis factor-α （TNF-α） solution group （administered eye drops of TNF-α）. Anterior segment photography, conjunctival tissue light microscopy and transmission electron microscopy （TEM） were performed after 12wk. RESULTS： Among all groups, the MMPs group had the following changes： the looser connection between the inferior bulbar conjunctiva and sclera; the more disordered collagen fibers （Trichrome staining） and the broken elastic fibers （Aldehyde-fuchsin staining）; the focal necrosis of fibroblasts （TEM）. CONCLUSION： Administration of MMPs may be a feasible method for the establishment of an animal model of CCH.

Clinical efficacy of Xuebijing injection for the treatment of sepsis:A retrospective cohort study

Objective: The objective of this study was to investigate the clinical efficacy and safety of treating sepsis patients with Xuebijing injection(XBJI).Methods: We conducted a retrospective analysis of 418 patients who experienced severe infections and were treated with XBJI from June 2018 to June 2021. Propensity score matching was used to match the patient cases. The study population included 209 pairs of cases(418 individuals), and the analysis included data from before and after a 14-day course of treatment with carbapenem alone, or carbapenem with ***: There were no significant differences in the 14-day mortality or length of hospital stay(P > 0.05)between the two groups. The combined treatment group had more patients with C-reactive protein that returned to normal levels(compared to baseline) than the non-combined treatment group(14.4% vs 8.1%;odds ratio [OR]: 0.528; 95% confidence interval [CI]: 0.282–0.991; P = 0.026). Similarly, the combined treatment group had higher procalcitonin attainment rate(55.0% vs 39.7%; OR: 0.513; 95% CI: 0.346–0.759; P = 0.001) than the non-combined treatment group. Further, more patients in the combined treatment group achieved normal creatinine levels than in the non-combined treatment group(64.1% vs54.1%; OR: 0.659; 95% CI: 0.445–0.975; P = 0.037).Conclusion: The combination of XBJI with carbapenem did not reduce the 14-day mortality rate of patients with severe infection, but it was able to reduce the level of inflammatory factors in patients with sepsis, and had a protective effect on liver and kidney *** cite this article as: Gong ZT, Yang HX, Zhu BB, Liu HH, Siri GL. AIM:To investigate the mechanism underlying the anti-inflammatory effects of epigallocatechin-3-gallate(EGCG)in lipopolysaccharide(LPS)-stimulated human retinal endothelial cells(HRECs).METHODS:HRECspre-treatedwithEGCG(0-100μmol/L)were stimulated with LPS(250 ng/mL).Levels of tumor necrosis factor alpha(TNF-α),vascular endothelial growth factor(VEGF),monocyte chemotactic protein-1(MCP-1)and nitric oxide(NO)in the supernatants were determined by enzyme-linked immunosorbent assay(ELISA)and Griess *** protein expression of phosphorylated extracellular signal-regulated kinase(ERK)1/2 and p38 mitogen-activated protein kinases(p38)were determined by Western blot ***:EGCG pre-treatment significantly inhibited the secretion of TNF-α,VEGF,MCP-1 and NO in LPSstimulated ***,EGCG effectively attenuated LPS-induced activation and phosphorylation of ERK1/2 and p38 in HRECs in a dose-dependent ***:EGCG exhibited inhibitory effects on LPS-induced pro-inflammatory cytokines production by modulating ERK1/2 and p38 pathways in HRECs,suggesting EGCG as a potential candidate for antiinflammatory intervention.

Influences of dexmedetomidine on stress responses and postoperative cognitive and coagulation functions in patients undergoing radical gastrectomy under general anesthesia

BACKGROUND Radical gastrectomy(RG)is commonly used in the treatment of patients with gastric cancer(GC),but this procedure may lead to stress responses,postoperative cognitive dysfunction,and blood coagulation abnormalities in *** To investigate the influences of dexmedetomidine(DEX)on stress responses and postoperative cognitive and coagulation functions in patients undergoing RG under general anesthesia(GA).METHODS One hundred and two patients undergoing RG for GC under GA from February 2020 to February 2022 were retrospectively *** these,50 patients had received conventional anesthesia intervention[control group(CG)]and 52 patients had received DEX in addition to routine anesthesia intervention[observation group(OG)].inflammatory factor(IFs;tumor necrosis factor-α,TNF-α;interleukin-6,IL-6),stress responses(cortisol,Cor;adrenocorticotropic hormone,ACTH),cognitive function(CF;Mini-Mental State Examination,MMSE),neurological function(neuron-specific enolase,NSE;S100 calciumbinding protein B,S100B),and coagulation function(prothrombin time,PT;thromboxane B2,TXB2;fibrinogen,FIB)were compared between the two groups before surgery(T0),as well as at 6 h(T1)and 24 h(T2)after *** Compared with T0,TNF-α,IL-6,Cor,ACTH,NSE,S100B,PT,TXB2,and FIB showed a significant increase in both groups at T1 and T2,but with even lower levels in OG vs *** groups showed a significant reduction in the MMSE score at T1 and T2 compared with T0,but the MMSE score was notably higher in OG compared with *** In addition to a potent inhibitory effect on postoperative IFs and stress responses in GC patients undergoing RG under GA,DEX may also alleviate the coagulation dysfunction and improve the postoperative CF of these patients.
