Significances of Peripheral inflammatory cells and Neutrophil/Platelet-Lymphocyte Ratio in Breast Cancer after Resection

Introduction: Breast cancer had become top leading cause of death in Taiwan and endangered women’s health worldwide. Therefore, we try to invest the peripheral inflammatory cell counts and neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) from our routine practice for the predictor of prognosis of breast cancer after resection. Patients and Methods: There were 574 breast cancer patients accepted surgical resection and registered in Cancer Registry Center of our hospital. Patient’s basic profiles, peripheral neutophil, lymphocyte and platelet count were measured for study. The scales of NLR and PLR were derived from the lower and higher normal range in cell count from neutrophil, lymphocyte and platelet respectively. Therefore, the scales for NLR and PLR were ≤1.62, 1.63 - 2.57, ≥2.58 and ≤224, 225 - 253, ≥254 respectively for analysis. Results: Poor 5-yr survival rate was found if higher cell counts of neutrophil and platelet (p ≤ 0.05). Three scales of NLR were ≤1.62, 1.63 - 2.57, ≥2.58, and their 5-year survival rates were 94%, 91% and 84% respectively (p = 0.019). In the subgroup of HER-2 (negative), and 3-Negative breast patients had a higher NLR of poor prognosis. But higher PLR was found less in 3-Negative and non in 3-Positive patients (p = 0.039). The PLR was ≤224, 225 - 253, ≥254 and their 5-year survival rates were 92%, 87%, and 64% respectively (p = 0.001);Multivariate Cox regression model for predictor of breast cancer patients who have 3.39 (PLR ≥ 254) and 2.45 (NLR ≥ 2.58 ) times risk (p = 0.02 and p = 0.002) of poor prognosis respectively. Conclusion: Peripheral inflammatory cell counts are easily to take in our clinical practice and have a potential role as predictors of prognosis. We have to pay attention to the trends of peripheral inflammatory cell count and their ratio in our clinical practice where possible.

Autoimmune pancreatitis(AIP)is a rare form of pan-creatitis characterized by prominent lymphocyte inf iltration and pancreatic f ibrosis resulting in organ *** pathogenesis and pathology of AIP remain unknown.A 64-year-old Chinese man presented with symptoms and signs of bile duct obstruction diffuse enlargement of the head of pancreas,elevated IgG levels,and negative autoimmune antibody responses.A pylorus-preserving pancreatoduodenectomy was per-formed and a pancreatic tumor was ***-er,periductal lymphoplasmacytic inf iltration and f ibrosis were found in the head of pancreas and nearby organs instead of tumor *** months after surgery,the patient was readmitted because of reoccurrence ofsevere jaundice and sustained abdominal *** 30 mg/d was administered orally as an AIP was *** and a half months later,the symp-toms of the patient disappeared,and globulin,amino-transferase and bilirubin levels decreased signif *** a 9-mo follow-up period,the dose of prednisone was gradually decreased to 10 mg/d and the patient remained in good *** further demonstrated dominant CD3+/CD8+ populations,CD20+ cells and a few CD4+ cells in the pancreatic parenchyma,duo-denum and gallbladder wall by immunohistochemical *** AIP case presented with signif icant CD8+ T lymphocyte inf iltration in the pancreas and extra-pan-creatic lesions,indicating that this cell population may be more important in mediating AIP pathogenesis than previously known and that AIP might be a poorly defined autoimmune disease with heterogeneous pathogenesis.

Interferon gamma （IFN-7） and leukemia inhibitory factor （LIF） are key gestational factors that may differentially affect leukocyte function during gestation. Because IFN-,/induces a pro-inflammatory phenotype in macrophages and because trophoblast cells are principal targets of LIF in the placenta, we investigated whether and how soluble factors from trophoblast cells regulate the effects of IFN-7 on macrophage activation. IFN-7 reduces macrophage motility, but enhances Statl activation, pro-inflammatory gene expression and cytotoxic functions. Soluble factors from villous cytotrophoblasts （vCT＋LIF cells） and BeWo cells （BW/ST＋LIF cells） that were differentiated in the presence of LIF inhibit macrophage Statl activation but inversely sustain Stat3 activation in response to IFN-7. vCT＋LIF cells produce soluble factors that induce Stat3 activation; this effect is partially abrogated in the presence of neutralizing anti-interleukin 10 （IL-IO） antibodies. Moreover, soluble factors from BW/ST＋LIF cells reduce cell proliferation but enhance the migratory responses of monocytes. In addition, these factors reverse the inhibitory effect of IFN-y on monocyte/macrophage motility. BW/ST＋LIF cells also generate IFN-y-activated macrophages with enhanced IL-IO expression, but reduced tumor-necrosis factor alpha （TNF-cd, CD14 and CD40 expression as well as impaired cytotoxic function. Additional assays performed in the presence of neutralizing anti-lL-lO antibodies and exogenous IL-IO demonstrate that reduced macrophage cytotoxicity and proliferation, but increased cell motility result from the ability of trophoblast IL-IO to sustain Star3 activation and suppress IFN-y-induced Star1 activation. These in vitro studies are the first to describe the regulatory role of the LIF-trophoblast-lL- 10 axis in the process of macrophage activation in response to pro-inflammatory cytokines.

Aim:Negative pressure wound therapy(NPWT)has achieved widespread success in the treatment of chronic ***,its effects have been only partially explored,and investigations have generally concentrated on the wound-dressing interface;a detailed histopathological description of the evolution of wounds under NPWT is still *** present study was performed to investigate the effect of a limited access dressing(LAD)which exerts intermittent NPWT in a moist environment on chronic ***:A total of 140 patients were randomized into 2 groups:LAD group(n=64)and conventional dressing group(n=76).By histopathological analysis of the granulation tissue,the amount of inflammatory infiltrate,necrotic tissue,angiogenesis,and extracellular matrix(ECM)deposition was studied and compared to determine healing between the 2 ***:After 10 days of treatment,histopathological analysis showed a significant decrease in necrotic tissue with LAD compared to the conventional dressing group(mean±standard error,11.5±0.48 vs.10.1±0.30,P=0.007),the number of inflammatory cells(12.6±0.60 vs.8.63±0.35,P=0.018),a significant increase in new blood vessels(12.8±0.58 vs.9.3±0.29,P=0.005)and ECM deposit(13.3±0.50 vs.9.6±0.24,P=0.001).Conclusion:LAD exerts its beneficial effects on chronic wound healing by decreasing the amount of necrotic tissue and inflammatory cells while increasing the amount of ECM deposition and angiogenesis.
