Dear Editor,In this work, we evaluated a special case. Acquired progressive restrictive strabismus is very rare in the infants. We have been accessed to literatures and found only 5 infants had been reported. So, we report a case of progressive restrictive strabismus in a 4-month-old infant and using an anterior orbitotomy approach for the strabismus surgery.

Objective:To compare the differences in the clinical effect of congenital muscular torticollis between the modified tuina(Chinese massage)manipulation and the textbook-version tuina manipulation in infant.Methods:From January 2017 to December 2018,68 infants with congenital muscular torticollis were collected from the Department of Acupuncture-Moxibustion of the Affiliated Hospital of Yanbian University.They were randomized into a modified tuina group and a textbook-version tuina group,34 cases in each one.In the modified tuina group,the infants were treated with the stroking and circular kneading manipulation,200 times,the plucking manipulation,500 times and the modified rolling manipulation,500 times.In the textbook-version tuina group,the infants were treated with the tuina for muscular torticollis described in Tuina,in the sequence of the circular kneading for 5-6 min,the pinching and grasping for3-5 min,neck traction for several times,the pushing and circular kneading for 3-5 min and gently holding Jianjing(肩井GB21)for 3-5 times.In each group,the treatment was given once every day and there was 1-day rest after every 6-day treatment.Totally,the duration of treatment was 60 days.Separately,before treatment,in 30 days and 60 days of treatment,the lump diameter and the infants'adaptability were observed and compared between the two groups.Results:The effective rates were 94.1%and 70.6%in the modified tuina group and the textbook-version tuina group respectively,presenting a significant difference(P<0.05).In 30 days of treatment,12 cases were cured in the modified tuina group and 7 cases in the textbook-version tuina group.In 60 days of treatment,the color Doppler ultrasound examination found that the lump diameter in the modified tuina group was obviously smaller than that in the textbook-version tuina group,indicating a significant difference(P<0.05).During the treatment,there was no case dropped out in the modified tuina group,but in the textbook-version tuina group,because of crying and low adaptability durin

Dear Editor,My name is Jun Chen and I am currently working as an ophthalmologist at the Affiliated Nanping First Hospital of Fujian Medical University.I am writing this letter to present a case of a tooth lesion in the eyelid.Choristomas are believed to be developmental malformations of normal,mature-appearing tissues in an abnormal anatomic location[1].Choristomas consist of dermis-like tissue or ectopic tissues of mesoectodermal origin(lacrimal and other glands,fat,nerve,brain,cartilage,bone,and teeth)[2].An ectopic tooth in the eyelid was first reported by Van Der Straeten[3]in 1934,then reported by Subramaniam et al[4]in 1966 and finally by Jakobiec et al[5]who named it palpebral odontogenic choristoma in 2009.Here,we describe a case of a tooth lesion in the eyelid in a Chinese infant.According to Declaration of Helsinki,a written voluntary informed consent was obtained from the patient for publication of this case report and accompanying images.

Introduction The acquisition of non-contaminated urine samples in pre-continent infants remains a challenge.The Quick Wee method uses bladder stimulation to induce voiding.A previous randomized trial showed a higher rate of voiding within 5 minutes using this method.We evaluated this method in an Irish hospital providing secondary care.Methods A non-blinded,randomized,controlled trial was carried out.Eligible infants were between 1 and 12 months of age,who required urine sampling as part of clinical care.Participants were randomly allocated to receive the intervention(Quick Wee Method—supra-pubic stimulation with cold saline)or the control(usual care—clean catch with no bladder stimulation)for 5 min.Primary outcome was voiding of urine within 5 min.Results A total of 140 infants were included in this study(73 in intervention group;67 in control group).Baseline characteristics were similar.25%in the intervention group passed urine in the 5-min trial period compared with 18%in the control group[P=0.4,absolute diference 7%(95%confdence interval:−7%to+20%)].Conclusion The Quick Wee method is a simple and inexpensive intervention that did not show a statistically signifcant increase in urine samples obtained in pre-continent infants.

Parenting the infant in the neonatal intensive care unit(NICU)comes with a multitude of unique challenges,and NICU parents are often unprepared and ill equipped for the challenges.Moreover,a gap in the literature still exists concerning a full understanding of the breadth of the process parents of NICU infants undergo from the time of admission to discharge.This study utilized Noblit and Hare's meta-ethnography synthesis method for literature extraction and data analysis to illuminate the NICU parenting process by metaphorically comparing the process to Patricia Benner's novice to expert theory.Fourteen studies including 12 published qualitative research articles and 2 dissertations from the nursing discipline were included in the analysis.Findings illuminated the process of becoming a NICU parent more fully and revealed the most salient and effective facilitators of the process from being a novice and advanced beginner to becoming a competent,proficient,even expert NICU parent.In addition,this paper discusses four influencing factors in a NICU parent's progression:contact with and proximity to their infant,relationship with the nurse,having information,and social support.Nurses must engage with NICU parents in such a way as to maximize the likelihood that these parents will reach at minimum the proficient and at maximum the expert parenting stage by discharge.

Background:The fetal growth charts in widest use in China were published by Hadlock>35 years ago and were established on data from several hundred of American pregnant women.After that,>100 fetal growth charts were published around the world.We attempted to assess the impact of applying the long-standing Hadlock charts and other charts in a Chinese population and to compare their ability to predict newborn small for gestational age(SGA).Methods:For this retrospective observational study,we reviewed all pregnant women(n=106,455)who booked prenatal care with ultrasound measurements for fetal biometry at the Shenzhen Maternity and Child Healthcare Hospital between 2012 and 2019.A fractional polynomial regression model was applied to generate Shenzhen fetal growth chart ranges for head circumference(HC),biparietal diameter(BPD),abdominal circumference(AC),and femur length(FL).The differences between Shenzhen charts and published charts were quantified by calculating the Z-score.The impact of applying these published charts was quantifed by calculating the proportions of fetuses with biometric measurements below the 3rd centile of these charts.The sensitivity and area under the receiver operating characteristic curves of published charts to predict neonatal SGA(birthweight<10th centile)were assessed..Results:Following selection,169,980 scans of fetal biometry contributed by 41,032 pregnancies with reliable gestational age were analyzed.When using Hadlock references(<3rd centile),the proportions of small heads and short femurs were as high as 8.9%and 6.6%in late gestation,respectively.The INTERGROWTH-21st standards matched those of our observed curves better than other charts,in particular for fat-free biometry(HC and FL).When using AC<10th centile,all of these references were poor at predicting neonatal SGA.Conclusions:Applying long-standing Hadlock references could misclassify a large proportion of fetuses as SGA.INTERGROWTH-21st standard appears to be a safe option in China.For fat-based biometry,AC,a reference base

The surgical emergency treatments and cura-tive effects of postrenal acute renal failure following obstruction due to upper urinary calculi in infants were evaluated.Of the 13 infants with postrenal acute renal failure following obstruction due to upper urinary calculi,11 received retrograded catheterizations of the ureter with semi-rigid ureteroscopy(F 6.8),and two received open ureterolithotomy.The results showed that only one infant had anuresis and continuous reduction of hemoglobin 5 h after the open ureterolithotomy and received exploration via excision and peritoneal dialysis,and the remaining 12 patients well recovered in this group.The renal function of all the patients was restored without postoperative complications.It is concluded that the retrograded catheterization of the ureter with ureteroscopy is a minimally invasive,safe and effective therapy for postrenal acute renal failure following obstruction due to upper urinary calculi in infants.For those infants whose urethras are thin and small,the open ureterolithotomy is a suitable method.But patients with bleeding tendency need to be corrected prior to the open ureterotomy to remove obstructions.

BACKGROUND Fibrous hamartoma of infancy(FHI)is a rare disease of infancy with unknown etiology.The disease mainly involves soft tissue,has no specific clinical manifestations,and is difficult to diagnose.At present,the diagnosis is mainly confirmed by histopathological examination,and the main treatment is surgical resection of the pathological tissue,which is prone to recurrence.CASE SUMMARY A five-month-old female patient was admitted to our hospital with swelling in the right calf.Two biopsies were performed in our hospital and another hospital,respectively,confirming the diagnosis as fibrous hamartoma.After exclusion of surgical contraindications,resection was performed with clear margins of 1 cm.Radiographic examination showed tumor recurrence more than four months after the operation,and surgery was performed again to extend the resection margins to 1.5 cm.The patient is recovering well,and after a follow-up of 36 mo,shows no signs of recurrence.CONCLUSION Our case report demonstrates that FHI should be considered in the differential diagnosis for a lower extremity mass with bone destruction.For FHI with bone destruction and unclear boundaries,excision margins of 1.5 cm could be superior to margins of 1 cm.

BACKGROUND Congenital tuberculosis(TB),tuberculous meningitis,and situs inversus totalis are rare diseases.We here report a patient who simultaneously suffered from these three rare diseases.There is currently no such report in the literature.Congenital TB is easily misdiagnosed and has a high case fatality rate.Timely anti-TB treatment is required.CASE SUMMARY A 19-day-old male newborn was admitted to hospital due to a fever for 6 h.His blood tests and chest X-rays suggested infection,and he was initially considered to have neonatal pneumonia and sepsis.He did not respond to conventional antiinfective treatment.Finally,Mycobacterium tuberculosis was found in sputum lavage fluid on the 10th day after admission.In addition,the mother's tuberculin skin test was positive,with an induration of 22 mm,and her pelvic computed tomography scan suggested the possibility of tuberculous pelvic inflammatory disease.The child was diagnosed with congenital TB and immediately managed with anti-TB therapy and symptomatic supportive treatment.However,the infant's condition gradually worsened and he developed severe tuberculous pneumonia and tuberculous meningitis,and eventually died of respiratory failure.CONCLUSION If conventional anti-infective treatment is ineffective in neonatal pneumonia,anti-TB treatment should be considered.
