Natural killer/T-cell lymphoma with concomitant syndrome of inappropriate antidiuretic hormone secretion: A case report and review of literature

We report a case of natural killer(NK)/T-cell lymphoma with concomitant syndrome of inappropriate antidiuretic hormone secretion(SIADH).The patient was a 64-yearold woman with a history of nasopharyngeal carcinoma of over 30 years.She was admitted with a chief complaint of intermittent fever for 2 mo.Palpation after admission indicated a swollen lymph node below the left jaw.Multiple imaging examinations on admission indicated multiple enlarged lymph nodes throughout the body.We performed a left submandibular lymph node biopsy,and the results revealed NK/T-cell lymphoma.A biochemical examination indicated Epstein-Barr virus positivity.At the same time,the patient developed hyponatremia.Based on her laboratory examination and clinical manifestation,decreased plasma osmolality,urine osmolality greater than plasma osmolality,lack of skin swelling,normal blood pressure,normal renal function,no adrenal function detected on serology,and no abnormalities in imaging examination of the adrenal glands,the likelihood of SIADH in the patient was high.After fluid restriction and administration of sodium chloride,the patient’s blood sodium level gradually increased.Subsequently,the immune function of the patient declined,there were severe symptoms of infection,and she died of respiratory failure.NK/T-cell lymphoma associated with SIADH has not,to our knowledge,been previously reported in PubMed.This case emphasizes the importance of monitoring serum ion levels,especially serum sodium,in patients with NK/T-cell lymphoma.

Case of inappropriate ADH syndrome:Hyponatremia due to polyethylene glycol bowel preparation

Colonoscopic screening has been reported to reduce deaths from colorectal cancer.Adequate bowel preparation is essential for this and safety is an important issue in choosing the methods.Polyethylene glycol(PEG)is regarded as a safe method for cleansing,especially compared with oral sodium phosphate.Here,we present a case of hyponatremia caused by the syndrome of inappropriate antidiuretic hormone(ADH)syndrome after PEG precolonoscopic cleansing resulting in generalized tonic-clonic seizures.A 62-year-old women had ingested PEG for precolonoscopic bowel cleansing.While waiting for the colonoscopy,she developed a stuporous mentality and generalized tonic-clonic seizures,which did not correlate with brain magnetic resonance imaging.Her serum sodium level was 113 mEq per liter and laboratory analyses were consistent with inappropriate ADH syndrome.Her thyroid and adrenal functions were normal.There were no malignancies,infections,respiratory disorders or central nervous disorders and she had no history of taking either diuretics or other medications,which might have caused inappropriate ADH syndrome.She was treated with 3%hypertonic saline and showed a complete neurological recovery as her sodium levels recovered.Follow-up visits showed the patient to have a normal sodium level without neurologic deficits.This case shows that inappropriate ADH syndrome can be caused by PEG preparation,which implies that physicians have to be aware of the possible side effects of this colonic cleansing approach and mindful of the possible ensuing symptoms.

Radiofrequency Catheter Ablation Of inappropriate Sinus Tachycardia

Objective:To investigate the mechanism of inappropriate sinustachycardia and the safety,effectiveness on radiofrequency catheterablation of it.Methods:Radiofrequency catheter ablation wasperformed on a 52-year-old female patient and changes in Holter,heart rate variability,intrinsic heart rate and syndromes were comparedbetween before and after radiofrequeney catheter ablation.Results:The inappropriate sinus tachycardia could not be induced and terminatedby atrial program electric stimulation and burst stimulation,whichsuggests that the mechanism of inappropriate sinus tachycardia isreentry.After radiofrequency catheter ablation,the total heart beatsdecreased from 173490 to 129172 times,the lowest heart rate did from79 to 71 bpm,the highest heart rate from 200 to 122 bpm,the averageheart rate from 117 to 90 bpm,and intrinsic heart rate from 166 to 93Variables of heart rate variability varied too.PNN50 increased from O to11,RMSSD did from 8 to 35.However,LF/HF ratio decreased from12.23 to 0.86.Transesophageal eleetrophysiologic studies demonstratethat sinoatrial node function is normal after radioflequency catheterablation.The patient has been followed up for six months,she has beenbeing free of palpitation,chest distress and dizziness,etc.Conclusion:The inappropriate sinus tachycardia could be due to abnormal sinoatrialnodal automaticity resulting from excessive sympathetic nerve influencesor deficient vagal nerve influences and modification of sinus node withradiofrequency catheter ablation is a safe,effective method for thetreatment of inappropriate sinus tacbycardia.

Patterns of Medication Use among Romanian Nursing Home Residents

Background: the elderly are frequently exposed to drug-related problems and their consequences. Information relating to the appropriateness of the medication used by Romanian elderly is scarce. Objective: to identify the main potential inappropriate prescribing (PIP) instances in a sample of Romanian elderly nursing-home residents, concerning prevalence and subtypes. Methodology: the residents’ medical data were reviewed by a clinical pharmacist considering published geriatric pharmacotherapy recommendations, including four published PIP evaluation tools (the Beers criteria, STOPP-START tools, the PRISCUS list). Results: 91 residents were evaluated;mean age (mean ± SD) was 80.77 ± 6.82 (years), 28 (31%) were ≥85 years old and 58 (64%) had dementia. The median number of diagnoses was 6 (range 2 - 11) and the mean number (±SD) of daily medications was 8.26 (±3.52). An estimated mean (±SD) of 2 (±1.41) PIP per resident was identified, with 117 misuse-PIP (46.24%), 60 underuse-PIP (23.72%), 41 lack of monitoring-PIP (16.21%) and 35 overuse-PIP (13.83%). The most frequent PIP subtypes were the use of nonsteroidal anti-inflammatory drugs as chronic analgesic treatment in osteoarthritis (29.67%), underuse of fall prevention (100%) and analgesic therapies (23.08%), overuse of nootropic medications (26.98%) and lack of annual creatinine assessment (30.77%). Conclusion: a more geriatric-oriented care seemed to be necessary, but larger studies are needed to confirm these findings.

Differentiating between SIADH and CSW Using Fractional Excretion of Uric Acid and Phosphate: A Narrative Review

Background: Sodium imbalances are among the most common electrolyte abnormalities encountered in the acute care setting. The syndrome of inappropriate anti-diuretic hormone (SIADH) and cerebral salt wasting (CSW) are characterized by hyponatremia and can be difficult to differentiate. Failure to accurately diagnose these conditions and implement the correct treatment results in an increased mortality risk, a longer length of stay in the hospital, and an increase in the cost of hospitalization. Objective: The purpose of this review is to summarize the key diagnostic findings in each disorder and to review the use of the fractional excretion of uric acid (FeUA) and the fractional excretion of phosphate as additional diagnostic measures to differentiate between SIADH and CSW. Observation: Publications from MEDLINE, CINAHL, and Google Scholar from 2009 through 2017 were reviewed. Articles were included if original data was presented and diagnosed either SIADH or CSW. Articles were excluded if they did not discuss diagnostic measures or were review articles. Results: Thirteen out of 51 publications met the inclusion criteria;four (31%) were clinica trials, seven (54%) were case reports, one (7.5%) was a prospective study and one (7.5%) was a retrospective-observational study. The populations studied, the etiologies causing hyponatremia, and diagnostic criteria used to distinguish between SIADH and CSW varied. Conclusion and Relevance: There is a need for consistent diagnostic criteria for SIADH and CSW. Based on current evidence, the use of FeUA and the fractional excretion of phosphate have consistently and accurately differentiated between SIADH and CSW.

Objective:To investigate the constitution and health status of the Beijing residents, and the primary influencing factors on nutrition and health, as to finding out the epidemiological characters and the changing trend of chronic diseases and to establish scientific basis for the establishment of public health and disease prevention policies of Beijing. Methods:A stratified multi-stage cluster randomly sampling method was used. In total, 18 districts were surveyed, and among them, 9 were conducted dietary survey. The investigation was based on household, and all family members were surveyed at their home. After having got the compliance from the family members, questionnaire, medical examination, laboratory test and dietary investigation were carried out in these people. Results:The incidence rate of hypertension was 25.0% for the resident≥15 year-old, and over 50% for middle-aged and elderly people ( ≥45 years old).

Background: Kind of delivery affect many aspects of individuals, families and health systems situations. The aim of this study was to identify the Role of non-medical factors on choice of delivery (CS/NVD) in hospitals affiliated to Tehran University of Medical Sciences. Materials and Method: This cross-sectional study was performed to identify the factors involved in the choice of delivery (CS/NVD) among women attending hospitals affiliated to Tehran University of Medical Sciences. Data was derived from the medical records of women who gave birth in the hospitals. Results: A significant statistical relationship was found between factors such as age of mother, level of education, occupational status, type of previous delivery, person supervising the pregnancy and dissatisfaction about delivery were more frequent in women who underwent CS than those who gave birth by NVD. Conclusion: The highly increasing rise in the rate of unnecessary CS during throughout the world, and its adverse effect on maternal and child health, the financial burden imposed on families and health systems, has highlighted the importance of studies to identify the non-medical factors that affect decision-making concerning type of delivery as well as to determine the appropriate medical indications of CS.
