Endotension Distribution in Fluid-Structure Interaction Analysis of Abdominal Aortic Aneurysm Following Endovascular Repair

Endovascular aneurysm repair is a new and minimally invasive repair for patients with abdominal aortic aneurysm (AAA). However, endotension is one of the post-operative compliances of endo-vascular aneurysm repair in abdominal aortic aneurysm. Typically, endotension is mainly a result of pressure transmitted to the aneurysm sac through endovascular implanted graft (EVG) by intermediary of the stagnant blood filled aneurysm cavity. Focusing on a representative AAA with an EVG, a fluid-structure interaction (FSI) solver has been employed to provide physical insight for evaluating the blood flow dynamics, maximum AAA-stresses and deformations. Although implanting an EVG can reduce the sac pressure, mechanical stress and wall deformation in AAAs significantly, they remain non-zero. These magnitudes depend on multi-factors including blood flow conditions such as velocity and pressure, as well as EVG and aneurysm geometries. In this study, it was found that blood flow velocity deceleration occurs on the graft due to the curvature of its neck, so greater curvature of the graft neck can contribute to vortex formation in this area and exert load on the graft wall. In the iliac bifurcation region, divaricating of the flow leads to a large net flow momentum change. It results in additional stress on the implant graft and may lead to graft migration. One of the peak wall stress points is in the neck region where the stent-graft is in contact with the aneurysm wall. This necessitates considering adequate graft fixation to withstand the stresses of blood flow through the implanted graft. Also, maximum deformation of sac wall occurs in around the large diameter of the sac, and deformation during the systole phase is higher than that during the diastole phase.

Objective Investigate the degradation process of polyactic acid (PLA) filaments within peripheral neres and its adverse effects on peripheral nerve. Methods Bilateral sciatic nerves of 20 SD rats were exposed, and PLA filaments were implanted among the fascicles at one side, while Vicryl filaments were implanted into the oomtrakteral nerve as control. The filaments and nerves were harvested at 1,4,8 and 12 weeks postoperatively, and inspected by macroscopy, light microscope and transmission electron microscope. Results The degradation occurred not only on the surface but also inside the PLA filaments after they were implanted into the sciatic nerve. PLA fragments still could be found at the end of this study. During the degradation of PLA filaments, macrophages invaded into the filaments and exhibited active phagocytosis. There were mild degree infiltration of lymphoscytes and fibroblast around the filaments during their degradation. However, the structure of peripheral nerve didn’ t change

In comparison with the low-beam current, the pulsed ionbeam implantation (PIBI) causes high rate of heat and coding as well as larger atomic mixing depth, which is much deeper than the stopping range corresponding to the ion energies, therefore, the study of metal noncrystallization could be greatly extended.

AIM:To investigate the nature and severity of AE related to sacral neurostimulation(SNS).METHODS:Based on Pubmed and Embase searches,we identified published trials and case series of SNS for fecal incontinence(FI)and extracted data on adverse events,requiring an active *** problems were operationally defined as infection,device removal explant or need for lead and/or generator *** addition,we analyzed the Manufacturer and User Device Experience registry of the Federal Drug Administration for the months of August-October *** were included if the report specifically mentioned gastrointestinal(GI),bowel and FI as indication and if the narrative did not focus on bladder *** classification,reporter,the date of the recorded complaint,time between initial implant and report,the type of AE,steps taken and outcome were extracted from the *** cases of device removal or replacement,we looked for confirmatory comments by healthcare providers or the ***:Published studies reported adverse events and reoperation rates for 1954 patients,followed for 27(1-117)*** rates were 18.6%(14.2-23.9)with device explants accounting for 10.0%(7.8-12.7)of secondary surgeries;rates of device replacement or explant or pocket site and electrode revisions increased with longer follow *** the period examined,the FDA received 1684 reports of AE related to SNS with FI or GI listed as indication.A total of 652 reports met the inclusion criteria,with 52.7%specifically listing *** or loss of benefit(48.9%),pain or dysesthesia(27.8%)and complication at the generator implantation site(8.7%)were most commonly *** led to secondary surgeries in 29.7%of the *** were performed to explant(38.2%)or replace(46.5%)the device or a lead,or revise the generator pocket(14.6%).Conservative management changes mostly involved changes in stimulation parameters(44.5%),which successfully addressed concerns in 35.
