Titanium allergy or not? “Impurity” of titanium implant materials

For patients suffering from allergies to nickel, chrome and cobalt, titanium implants are the implants of choice. Nevertheless, titanium implant sensitivity has been reported in the form of “allergies” and an increasing number of patients are confused. This paper aims to use spectral analysis as a diagnostic tool for analyzing different titanium implant alloys in order to determine the percentage of the alloy components and additions that are known to cause allergies. Different materials, such as sponge titanium, TiAl6Nb7, Ti21SRx, TiAl6V4 [forged alloy], TiAl6V4 [cast alloy], TMZF, pure titanium [c. p. 1] and iodide titanium were analyzed for the presence of the elements that have been associated with allergic reactions using spectral analysis. All the implant material samples contained traceable amounts of Be, Cd, Co, up to a maximum of 0.001 percent by weight [wt.%], Cr up to 0.033 wt.%, Cu up to 0.007 wt.%, Hf up to 0.035 wt.%, Mn up to 0.007 wt.%, Ni up to 0.031 wt.%, and Pd up to 0.001 wt.%. This paper demonstrates that all the investigated implant material samples contained a low but consistent percentage of components that have been associated with allergies. For example, low nickel contents are related to the manufacturing pro- cess and are completely dissolved in the titanium grid. Therefore, they can virtually be classified as “impurities”. Under certain circumstances, these small amounts may be sufficient to trigger allergic reactions in patients suffering from the corresponding allergies, such as a nickel, palladium or chrome allergy.

Background: Despite scientific and technological developments on dentistry, man is still losing their teeth due to several reasons. Tooth loss may result in esthetical and functional discrepancies. Although beginning of dental transplantation is quite old, it started to take attraction on scientific basis recently. In recent years, oral implants are being used nearly in all fields of dentistry. Purpose: The purpose of this study was to evaluate 5 year follow-up of a patient with an impacted maxillary canine transplantation and a single-tooth implant. Materials and Methods: In our study an impacted canine was removed and transplanted immediately. Thereafter the missing premolar was replaced by an implant retained cemented crown. The patient was evaluated clinically and radiographically. Clinical periodontal parameters and implant mobility was evaluated, marginal bone level was measured on radiographs. Results: No significant changes have been observed in these parameters and implant mobility between year one and year five of the study. Conclusion: This study has demonstrated that stable long-term results can be achieved with replacement of single teeth with MIS implant, and cemented crown on abutment with adjacent transplanted canine, with cumulative success rate for five years.

Nowadays there are lots of patients who are wearing removable partial dentures (RPD)among elder population in our ***,outcome of this therapy is not that ideal especially Kennedy I partially edentulous *** are many complications such as mucosa tenderness,abutment trauma and so *** idea of combination of RPD and implant therapy came up to solve those *** treatment is called implant-supported Removable Partial Dentures(ISRPDs).ISRPDs contains both advantages of RPD and implant,which means improvement of stability and supporting. Objective:Analyze the stress distributions of ISRPDs and supporting tissues in Kennedy I partially edentulous mandible,and compare influence of different retention attachments on stress among supporting structures. Material and Method:By 3-dimensional laser scanner and CAD software,a basic partially edentulous model was *** attachment and RPD were added in to build three models:RPD only,RPD + implant + locator attachment,RPD + implant + magfit *** 80N vertical and inclined loads on artificial teeth *** stress distribution by 3-dimensional finite element analysis software. Results:Addition of implant transferred the stress concentration area to the cortical bone next to cervical of *** of cortical bone at the same location of edentulous area decreased *** of denture was smaller in Locator and Magfit *** two attachments,cortical bone stress was higher in Locator model. Conclusion:Existence of implant could reduce maximum von Mises stress value of supporting *** magfit and locator attachment,stress value of implant’s apex was lower in magfit *** attachment could improve stability of denture.

Extraordinary sneeze:Spontaneous transmaxillarytransnasal discharge of a migrated dental implant

This case report describes an extraordinary case of the spontaneous transmaxillary-transnasal discharge of a dental implant, which occurred during a sneeze. The patient was complained of symptoms of acute sinusitis. She underwent a computed tomography scan that revealed a dental implant dislocated in the maxillary sinus. Medical treatment based on antibiotics and mucolytics was administered to the patient in order to prepare her for endoscopic endonasal surgery. The implant was spontaneously discharged two days after during a sneeze. Mucociliary clearance in combination with a local osteolytic inflammatory process and mucolytics therapy are the likely causes of this unusual discharge.

Cranial defects often occur due to trauma. The treatment of such defects is a challenge to the skill and knowledge of the practitioner. This article presents one such case, where a 15-year-old boy had suffered extensive loss of the right cranium following a road traffic accident. The patient required rehabilitation of the right fronto-temporal cranial anatomy and was managed using a custom made heat polymerized acrylic alloplastic implant.

implant retention after acute and hematogenous periprosthetic hip and knee infections: Whom, when and how?

Periprosthetic joint infections(PJI) of the hip and the knee are grossly classified as early post-operative, acute hematogenous and late chronic infections. Whereas two-stage exchange arthroplasty is the standard of care in North America for treating chronic infections, irrigation and debridement(I and D) with retention of implants has been used in an attempt to treat the other two types of PJIs. The rationale of this approach is that a PJI may be eradicated without the need of explanting the prostheses, as long as it has not transitioned into a chronic state. With the present paper, we review current evidence regarding the role of I and D with implant retention for treating PJIs of the hip and the knee. While a very wide range of success rates is reported in different studies, a short period of time between initiation of symptoms and intervention seems to play a prominent role with regards to a successful outcome. Moreover, pathogens of higher virulence and resistance to antibiotics are associated with a poorer result. Specific comorbidities have been also correlated with a less favorable outcome. Finally, one should proceed with serial I and Ds only under the condition that a predefined,aggressive protocol is applied. In conclusion,when treating a PJI of the hip or the knee, all the above factors should be considered in order to decide whether the patient is likely to benefit from this approach.

Objective:To analyze the clinical efficacy of Mixing natural teeth and implant supported denture sleeve,and provide reference for clinical ***:46 patients with Molarless in hospital from December 2010-December 2014 were selected,patients were randomly divided into observer group and control group,two groups of patients were designed and planted natural teeth fixed bridge and natural teeth and implants mixed support sleeve denture,regular follow patients and the surrounding alveolar bone height changes,5-year cumulative retention rate,clinical efficiency and other ***:During clinical observation,patients with no obvious symptoms,the use of feel good,no loose superstructure *** groups of patients were 2 cases of patients with implant loosening occurs,the amount of bone resorption annual observation group(0.22±0.10)mm less than the control group(0.24±0.08)mm,but no significant difference(P>0.05),clinical observation group efficiency(100%)was significantly higher(72.3%)(Pimplants 5-year cumulative retention rate was 94.4%.Conclusion:Mixing natural teeth and implant-supported telescopic denture success rate of 100%was observed during the clinical results were satisfactory,with use value.
