A Comparative Study of Bone Resorption between implant Placement Sites and Non-implant Placement Sites after Autogenous Block Bone Grafts in the Anterior Maxilla

The aim of this study was to compare the bone resorption differences between implant placement sites (IPS) and non-implant placement sites (NIPS) after autogenous block bone grafts in the anterior maxilla. Fourteen patients (58 edentulous sites) with alveolar atrophy in the anterior maxilla were treated with autogenous block bone grafts. CBCT examinations were performed at 1 month before surgery (T0), immediately after surgery (T1), 3 to 4 months after surgery (T2), 6 to 7 months after surgery before implant placement (T3), 12 to 13 months after surgery (T4), and the longest follow-up point (T5). Alveolar crestal and basal bone width (ACBW, ABBW), and alveolar bone height (ABH) were measured and divided into IPS (30 sites) and NIPS (28 sites). All results were compared by the Wilcoxon Signed Rank test. The bone resorption changes for both groups were the same. For these three parameters, ACBW didn’t change significantly from T2 to T3 and T4 to T5, ABBW didn’t change at every period from T2 to T5, and ABH didn’t change from T4 to T5. The bone resorption volume of ACBW and ABH in NIPS were more than in IPS after implant placement surgery, while the volume of ABBW was similar in both groups. At T5, the bone resorption percentages of ACBW, ABBW, and ABH were 25.57%, 16.85% and 43.84% in IPS, and 33.55%, 15.92% and 46.44% in NIPS. A more rapid loss of alveolar crest in NIPS resulted from implant placement surgery, and this reminded us of the importance of immediate implant placement.

Correlation between Endometrial Endothelin and Irregular Uterine Bleeding after implant Insertion

Objective To investigate endometrial endothelin type I (ET-I) level during menstrual cycle and its change after implant insertionMaterials &. Methods It was a prospective control study based on endometrial samples from 27 healthy women using radio-immunity technology. ET-I level in proliferation period and secretion period were measured and compared. ET-I level before and 3 to 7 months after implant insertion were also compared. The side effect of long term use of implant was also study by measuring the ET-I level among women 24 to 36 months after implant insertion.Results ET-I level in proliferation period zvas 2. 76±1. 25 pg/mg, which was sig-nificantly(Pimplant insertion were obtained, were3. 38± 1. 17 pg/mg and 10. 86± 3. 93 pg/mg respectively, with significant statistical difference (Pimplant insertion, the average ET-I level was 10. 86+ 3. 93 pg/mg within 2 to 3 years after implant insertion, significantly higher than that of women have implant insertion for more than 24 to 36 months year.Conclusion ET-I level changes periodically. ET-I level 3 to 7 months after implant insertion was significantly higher than that before insertion, and drops as time goes. The increase of ET-I level is possibly related to irregular uterine bleeding after the use of implant.

A Totally Absorbable Multilayer PLGA implant Device Containing Doxorubicin Inhibited Tumor Growth and Metastasis without Systemic Toxicity in Murine Breast Cancer and an Ideal Pharmacological Paradigm for Regional Chemotherapy

We hypothesize that a cylinder implant made of multilayer Poly-lactic-co-glycolic-acid (PLGA) membrane can be a method for controlled and extended drug release. We fashioned a multilayer cylindrical implant termed STID100 that released doxorubicin for 3 weeks in an orthotopic 4T1 breast cancer model in Balb/C mice. This implant starts as a thin doxorubicin-embedded PLGA membrane, and is then rolled into a cylinder containing an air gap between the membrane layers. Its controlled sustained release delivered 2× the amount of the intravenous (IV) equivalent of doxorubicin, inhibited the primary tumor, and prevented lung metastasis. Importantly it did not cause weight loss, splenomegaly, or cardiac toxicity vs systemically administrated doxorubicin. This favorable safety profile is further substantiated by the finding of no detectable plasma doxorubicin in multiple time points during the 3-week period, and low tumor doxorubicin concentration. The implant system delivered to the specification of an ideal pharmacological paradigm might offer a better coverage of the local tumor, significantly preventing metastatic spread with less drug toxicity to many vital organs, compared to the traditional pharmacology of IV route. The profile of STID made it an attractive therapeutic alternative in metastatic tumor prevention, pain management and many other diverse clinical scenarios.

Background: Advancing surgical reconstructive methods and demanding prosthetics need accurate and precise implant placements. Positioning dental implant is vital in both prosthetic and aesthetic perspectives. The optimal three-dimensional placement not only reduces biomechanical complications but also imprecates the odds of implant failure. Materials & Methods: By using robust text mining, searching and retrieval tools, 350 relevant articles were found and then out of them 161 articles were short listed for our review. They included systematic reviews, meta-analyses, case series and experimental studies. Conclusions & Results: Conventional freehand implant placement techniques remain experts’ favorite for uncompromised cases. However, for compromised cases that demand accuracy and predictability, various computer-based methods are in use. While computerized tomography techniques and use of interactive software prevalent in treatment planning, computer-aided design or computer-aided manufacturing (CAD/ CAM) fabricated surgical guides enable implantologists for more successful implantations.

High-Resolution Ultrasound in Research of Mouse Orthotopic Glioma and Ultrasound-Guided Cell implant

The purpose is to evaluate the feasibility of imaging mouse brain with high resolution ultrasound (HiRes US), and generation of mouse brain tumor (glioma) model under HiRes US guided implantation of glioma cells. Normal mouse brain was imaged with 30 MHz HiRes US in six pups and ten adult nude mice. Glioma model was developed by injecting human glioma cells (5 × 105), U-87MG-luc, under HiRes US guidance, in three pups and five adult mice. Bioluminescence imaging, magnetic resonance imaging, and HiRes US were used for in vivo tumor imaging. In addition, brain imaging ex vivo with HiRes US were also performed in three tumor bearing and five normal brains. The brain parenchyma was seen as a homogeneous low echo on HiRes US without locoregional echo differences. An inverted U shaped linear echo structure (fissure) differentiated the telencephalon from the diencephalon. Bilaterally budding structure at the base of the skull represented the trigeminal nerve. The inserted needle, used to implant the glioma cells, was seen as a high linear echogenic reflection. Brain tumor on ex vivo HiRes US was well demarcated, homogeneous and hyperechoic compared to the surrounding healthy brain. In conclusion, imaging the brain with HiRes US in small animal model like mouse is possible and convenient. Real-time guiding is possible to perform any intervention from tumor implantation to percutaneous therapeutic procedures. Ex vivo, HiRes US is extremely useful to study the detailed anatomical features.

Is the immediate use of guided bone regeneration on exposed dental implant threads beneficial?

Thin buccal bone has a risk of thread exposure, and placement of a graft is the mainline of treatment for thread exposure. However, this process takes time as the patient is expected to wait for healing to occur before the implant can be placed. To overcome the waiting period, this study views a different method. This study examines the use of guided bone regeneration on the day of implant placement on buccal bone where thread exposure occurs. This process has the advantage of avoiding the patient to go through multiple operations till receiving the final prosthetic treatment, also saving time and money for both the patient and the practitioner.

Effects of Laser Pulse Numbers on Surface Biocompatibility of Titanium for implant Fabrication

Generally, materials with high biocompatibility are more appropriate for bone and tissue transplant applications, due to their higher effectiveness in the healing process and infection problems. This study presents the effects of laser surface texturing on the surface topography properties, roughness, and wettability of thin titanium sheets, which consequently enhance the biocompatibility of this material. Creating line patterns across the surfaces, the titanium samples are prepared using variety of laser parameters. The apatite inducing ability of each sample is tested through the use of simulated body fluid (SBF). The final biocompatibility level of titanium samples is analyzed through wettability, surface angle measurements, and average surface temperature profile. Overall, the effects of laser parameter, pulse numbers, upon the biocompatibility of titanium are thoroughly examined, with results indicating that a scanning speed of 100 μm/ms results in desirable bone type apatite inducing abilities across the surface of treated titanium sheets.

Appendicular Mucosal Tube implant with Dartos Wrap Operation for Hypospadias

Aims and Objective: To develop a better operation for hypospadias patients with poor and bad quality urethral plate and damaged urethral plate as in hypospadias cripples. Materials and Methods: I operated 21 cases of hypospadias with Appendicular mucosal tube implant with Dartos wrap operation from 17/04/2017 to 03/03/2019, in Jawale Institute of pediatric Surgery, and that group was labeled as group A. 27 cases of hypospadias were operated in same time span with conventional techniques such as extended Snodgrass operation and Byar’s two-stage operation (group B), kept as control. 7 patients (33.33%) were cases of hypospadias cripples with multiple surgeries done in the past. 4 patients (9.52%) patients had congenital short urethra. Remaining 10 patients (50%) were fresh cases with no operation done in past but with bad and fibrotic urethral plate. The longest follow up was 3 years and the shortest of 1 year. Technique of Operation: Appendicectomy performed by open technique and the serosa of Appendix cut longitudinally and stripped off the mucosal tube. The proximal hypospadias opening sutured with the appendicular mucosal tube with 6 sutures of 5-0 Vicryl. Dartos fascia is raised from the scrotum wrapped over the tube and 8 - 10 interrupted stitches taken with 5-0 Vicryl. Glanuloplasty performed over it. Suprapubic diversion did and kept for 3 weeks postoperatively. Results: In group A, 3 patients developed fistula and only 1 (4.76%) required repair at the end of 6 weeks. 2 (9.52%) patients developed grade 3 infection and settled with conservative treatment. UFR was normal at the end of 12 weeks (12.85 Ml/sec. In group B, 11 patients developed fistula and 9 (33.33%) required repaired. 9 (33.33%) patients developed strictures and all of them required multiple urethral dilatation under GA. 7 (25.92%) patients developed meatal *** was badly reduced with average of 5.78 ML/Sec. Intensity Modulated Proton Therapy as a Boost Treatment after Prostate Seed implant: A Treatment Planning Study

Purpose: Combination of Prostate Seed implant (PSI) with External Beam Radiation Therapy (EBRT) remains as an attractive option for patients with intermediate or high-risk prostate cancer. One of the most widely used approaches is to use external beam radiation therapy (EBRT) to deliver boost doses after permanent prostate seed implant (PSI). In this study, the feasibility of using Intensity Modulated Proton Therapy (IMPT) as an alternative EBRT boost treatment for PSI patients was investigated in the presence of a large number of high Z metallic seeds. The dosimetry of IMPT boost plans was compared with that of conventional Intensity Modulated Radiation Therapy (IMRT) boost plans. Methods: Ten post prostate seed implants with seeds of I-125 were randomly selected for this study. Proton treatment plans were created with two lateral opposed proton beams in Eclipse treatment planning system. IMRT boost plans were generated with seven co-planner beams for comparison. Several plan evaluation parameters such as the planning target volume (PTV) dose homogeneity, dose conformity and dose to surrounding normal tissues were evaluated. Results: Compared to conventional IMRT boost plans, IMPT demonstrated better sparing of normal tissues while providing similar satisfactory PTV coverage. The high Z implanted seeds is not a problem for IMPT as boost treatment. Conclusions: PSI with an IMPT boost can be a valuable option for prostate cancer patient treatment. It delivers comparable or better radiation dose distribution in terms of normal tissue sparing compared to IMRT boost plan.

implant placement in severely atrophic jaws is especially challenging because of the poor quality and quantity of the future implant bed. Although various bone augmentation procedure like ridge augmentation, sinus lift these procedures are possible today but it may lead to surgical morbidity they increase the risks and costs of dental implant treatment as well as the number of necessary operations. Also sometimes the patient is not agreeing for such extensive surgical procedures, according to the well-known implantological rules for dental restorations, crestal implants are indicated in situations when an adequate vertical bone supply is given but single piece dental implant (basal implants) is a viable treatment option derives support from the basal bone area which usually remains free from the infection and less prone to resorption.
