Kawasaki disease(KD)is an acute febrile ilness characterized by systemic vasculitis affecting the small-and mediumsized arteries in children [1,2].One of the most critical complications of KD is the development of coronary artery lesions(CALs)approximately after 12 days of ilness,which may lead to myocardial infarction[3].High-dose intravenous Immunoglobulin(IVIG)and oral administration of aspirin have been established as the first-line therapy for KD[4,5].

Background:Aberrant activation of the complement system plays an important role in the pathogenesis and development of Immunoglobulin A nephropathy(IgAN).The relationship between serum complement and the clinical-histopathological features and outcomes of IgAN is controversial.This retrospective study aimed to examine the relationship between the complement 3/4(C3/C4)ratio and the clinicopathologic changes and prognosis of patients with IgAN.Methods:A total of 397 patients with primary IgAN from January 2007 to December 2012 at the Chinese People's Liberation Army General Hospital were included in this study.The correlation test and Chi-square test or one-way analysis of variance test were performed to evaluate the relationship between the C3/C4 ratio and other clinical-pathological factors.Propensity score matching and a multivariate Cox regression model were used to calculate the risk factors of renal outcome.Results:The median follow-up period was 75 months.During the follow-up period,62 patients(15.6%)developed into the endstage renal disease(ESRD).The C3/C4 ratio at baseline was associated with the level of serum creatinine(SCr),24 h urinary protein excretion(24 h Upre),global glomerular sclerosis,and tubulointerstitial lesion.The level of SCr and 24 h Upre and the degree of chronic kidney injury were statistically different among groups defined by different C3/C4 ratio levels.The survival rates of patients among groups with different C3/C4 ratio levels were different.After propensity score matching,eighty-eight pairs of patients were successfully matched,and the C3/C4 ratio was an influencing factor for the patients5 outcome(hazard ratio 0.587,95%confidence interval 0.329-0.880).Patients with a C3/C4 ratio<3.6 had a poorer outcome compared with the others(P=0.002).Conclusions:IgAN patients with decreased C3/C4 ratio displayed significantly more severe clinical symptoms and chronic renal injury than patients with higher ratios.A low C3/C4 ratio could be a risk factor for patients developing to ESRD.

Detection of antibodies against severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is a supplemental option for reverse transcription polymerase chain reaction(RT-PCR)detection,regardless of whether it is Immunoglobulin M(IgM),Immunoglobulin G(IgG)or combined detection.However,there are inadequate data on the sensitivity and specificity of IgM/IgG detection,which is of paramount importance for the decision of whether,when,and how to use IgM/IgG antibody detection.The aim of this study was to evaluate the sensitivity,specificity,accuracy,positive predictive value(PPV),and negative predictive value(NPV)of IgM/IgG antibody detection,especially the anti-interference ability and kappa coefficient compared with the SARS-CoV-2 RT-PCR,which was defined as a standard diagnosis in this study.

Background: Immunoglobulin A nephropathy (IgAN) is the most common pathological type of glomerular disease. Kidney biopsy, the gold standard for IgAN diagnosis, has not been routinely applied in hospitals worldwide due to its invasion nature. Thus, we aim to establish a non-invasive diagnostic model and determine markers to evaluate disease severity by analyzing the serological parameters and pathological stages of patients with IgAN. Methods: A total of 272 biopsy-diagnosed IgAN inpatients and 518 non-IgA nephropathy inpatients from the Department of Nephrology of Chinese People's Liberation Army General Hospital were recruited for this study. Routine blood examination, blood coagulation testing, Immunoglobulin-complement testing, and clinical biochemistry testing were conducted and pathological stages were analyzed according to Lee grading system. The serological parameters and pathological stages were analyzed. The receiver operating characteristic (ROC) analysis was performed to estimate the diagnostic value of the clinical factors. Logistic regression was used to establish the diagnostic model. Results: There were 15 significantly different serological parameters between the IgAN and non-IgAN groups (all PImmunoglobulin A (IgA), and Immunoglobulin G (IgG) were more than 0.70. The AUC of the "TC + FIB + D2 + IgA + age" combination was 0.86, with a sensitivity of 85.98% and a specificity of 73.85%. Pathological grades of Ⅰ,Ⅱ,Ⅲ,Ⅳ, and Ⅴ accounted for 2.21 %, 17.65%, 62.50%, 11.76%, and 5.88%, respectively, with grade Ⅲ being the most prevalent. Hepatitis B virus(HBV)immunization is safe and has been accepted worldwide as a routine practice.The target of such vaccination is to induce the immune response in the host,resulting in the prevention of replication of HBV.There are several immunological and clinical factors which determine the clinical efficacy and safety of the HBV vaccine.In this article we have highlighted the response of the host immune system to HBV vaccination(immunogenicity),efficacy,and safety of the vaccine,issues with booster dosing,paths of development(preclinical and clinical)of the HBV vaccine,novel and upcoming strategies for improvement of HBV vaccination,and the concept of therapeutic HBV vaccination.The different aspects and regulatory recommendations pertaining to HBV vaccine development are also discussed.The new strategies for improvement of HBV vaccination include pre-S1 and pre-S2 portions of the HBV surface antigen,increasing the antigen dose,accelerated vaccination schedules,alternative vaccination route,use of adjuvants like immunostimulatory DNA sequences,etc.Therapeutic vaccination is being explored for initiation of a multifunctional and multispecific T cell response against the major HBV antigens and also effective activation of humoral immunity for viral control.

The error-prone V(D)J recombination process generates considerable amounts of nonproductive Immunoglobulin(Ig)pre-mRNAs.We recently demonstrated that aberrant Ig chains lacking variable(V)domains can be produced after nonsense-associated altered splicing(NAS)events.Remarkably,the expression of these truncated Ig polypeptides heightens endoplasmic reticulum stress and shortens plasma cell(PC)lifespan.Many questions remain regarding the molecular mechanisms underlying this new truncated Ig exclusion(TIE-)checkpoint and its restriction to the ultimate stage of B-cell differentiation.To address these issues,we evaluated the extent of NAS of Ig pre-mRNAs using an Ig heavy chain(IgH)knock-in model that allows for uncoupling of V exon skipping from TIE-induced apoptosis.We found high levels of V exon skipping in PCs compared with B cells,and this skipping was correlated with a biallelic boost in IgH transcription during PC differentiation.Chromatin analysis further revealed that the skipped V exon turned into a pseudo-intron.Finally,we showed that hypertranscription of Ig genes facilitated V exon skipping upon passive administration of splice-switching antisense oligonucleotides(ASOs).Thus,V exon skipping is coupled to transcription and increases as PC differentiation proceeds,likely explaining the late occurrence of the TIE-checkpoint and opening new avenues for ASO-mediated strategies in PC disorders.

The importance of Immunoglobulin Y(IgY) as a specific antibody equivalent to mammalian Immunoglobulin G(IgG) is well recognized. However, production of highly purified IgY is still difficult due to the lack of specific purification methods. In this study, adsorption behaviors of Ig Y on four mixed-mode resins with functional ligands of 4-mercatoethyl-pyridine(MEP), 2-mercapto-1-methyli-midazole(MMI), 5-aminobenzi-midazole(ABI) and tryptophan-5-aminobenzi-midazole(W-ABI) were evaluated. The results showed that high adsorption ratio were found at p H 6.0–7.0 with little adsorption under acidic conditions. The resin with ABI ligand was then used to separate IgY from immunized chicken serum. An efficient process with Ig Y purity of 95% and recovery of 90% was developed after optimization of loading and elution p H and injection volume. The biological activity of the purified Ig Y was fully maintained. These results indicated that mixed-mode chromatography with specially-designed ligands has great potential for the separation of Ig Y from crude feedstock.

BACKGROUND Immunoglobulin G4-related disease (IgG4-RD) is an immune-mediated condition that consisted of disorders that share particular clinical,serologic and pathologic properties.The common presentation of disease includes tumor-like swelling of involved organs and the histopathological findings are a lymphoplasmacytic infiltrate enriched with IgG4-positive plasma cells,and a variable degree of fibrosis that has a characteristic "storiform" pattern in biopsy specimens of tumorlike masses.Major presentations of this disease,which often affects more than one organ,include autoimmune pancreatitis,salivary gland disease (sialadenitis),orbital disease and retroperitoneal fibrosis.The steroid treatment is essential for the treatment of the disease however,other immunosuppressive drugs including cyclophosphamide or rituximab could be an option in resistant cases.CASE SUMMARY Herein,we reported a 34-year-old woman whom previously had diagnosed with asthma,rheumatoid arthritis and Sj?gren’s syndrome (SS) referred our nephrology department due to acute kidney failure development at the last rheumatology visit.After kidney biopsy she has been diagnosed with IgG4-RD and tubuluointerstitial nephritis.She had been accepted resistant to steroid,mycophenolate mofetil,methotrexate and azathioprine therapies due to receiving in last two years.She refused to receive cyclophosphamide due to potential gonadotoxicity of the drug.Thus,rituximab therapy was considered.She received 1000 mg infusion,15 d apart and 6 mo later it has been administered same protocol.After one year from the last rituximab dose serum creatinine decreased from 4.4 mg/dL to 1.6 mg/dL,erythrocyte sedimentation rate decreased from 109 mm/h to 13 mm/h [reference range (RR) 0-20],and Creactive protein decreased from 55.6 mg/L to 5 mg/L (RR 0–6).All pathologic lymph nodes and masses were also disappeared.CONCLUSION Patients with IgG4-RD usually misdiagnosed with rheumatologic diseases including systemic lupus erythematous or SS and also they were screened for the

Pyoderma gangrenosum(PG)is an uncommon ulcerative cutaneous condition of an unknown etiology and is often associated with immune diseases.However,PG rarely shows visceral involvement,especially in the kidney.A 20-year-old female presented with pedal edema and skin ulceration of both lower limbs.The skin lesion began as an erythematous plaque and then became a blister.She also complained of abdominal distension and a decreasing urine volume.Laboratory data showed high proteinuria,hypoalbuminemia and hyperlipidemia.Her skin and kidney were biopsied.The pathological results indicated PG and Immunoglobulin A(IgA)nephropathy.The patient was finally cured with prednisolone in combination with cyclosporine A(CsA).

To the Editor:The epidemic caused by 2019 novel coronavirus(2019-nCoV)has drawn public attention.[1]Huge infected population and enormous economic loss make it the urgent public health event to deal with.Confirmatory test targeting virus RNA was established at the early stage of outbreak and then used for 2019-nCoV infection diagnosis.[2]However,high risk of laboratory infection,high-qualified personnel,and strict operation condition hampered its application into primary hospitals and community clinics.[3]In this study,the serum Immunoglobulin(Ig)M and IgG antibodies were detected in 2019-nCoV confirmed cases of different stages.Furthermore,three different immunological assays,chemiluminescent immunoassay(CLIA),gold immunochromatographic assay(GICA),and enzyme-linked immunosorbent assay(ELISA)were used for IgM and IgG detection.
