Osteomyelitis is a devastating disease caused by microbial infection in deep bone *** high recurrence rate and impaired restoration of bone deficiencies are major challenges in *** have evolved numerous mechanisms to effectively evade host intrinsic and adaptive immune attacks to persistently localize in the host,such as drug-resistant bacteria,biofilms,persister cells,intracellular bacteria,and small colony variants(SCVs).Moreover,microbial-mediated dysregulation of the bone immune microenvironment impedes the bone regeneration process,leading to impaired bone defect *** advances in surgical strategies and drug applications for the treatment of bone infections within the last decade,challenges remain in clinical *** development and application of tissue engineering materials have provided new strategies for the treatment of bone infections,but a comprehensive review of their research progress is *** review discusses the critical pathogenic mechanisms of microbes in the skeletal system and their immunomodulatory effects on bone regeneration,and highlights the prospects and challenges for the application of tissue engineering technologies in the treatment of bone *** will inform the development and translation of antimicrobial and bone repair tissue engineering materials for the management of bone infections.

Introduction: Microbiology of effusion fluids in children in Burkina Faso is characterized by the scarcity of data. This work aimed to study the bacteriological and antibiotics susceptibility profile of bacteria involved in effusion fluid infections in paediatrics in order to improve the choice of probabilistic antibiotics therapy. Methods: A cross-sectional, descriptive study was used in children aged 0 to 15 years from 2017 to 2020 at the Charles De Gaulle Pediatric University Hospital Center (CHUP-CDG) in Ouagadougou. Classical bacteriology methods such as macroscopy, Gram staining, identification galleries and antibiotics susceptibility testing were used. Results: Of 231 samples, 64 bacteria were isolated. The most common bacterial strains of pleural fluid were Staphylococcus aureus (25%) and 40% for Enterobacteriaceae. Of the peritoneal fluid, 77% were Enterobacteriaceae with 57% Escherichia coli;and from joint fluid, 33% were S. aureus and 22% for P. aeruginosa. The overall susceptibility profile showed 29% extended-spectrum beta-lactamase-producing Enterobacteriaceae (ESBL), 10% methicillin-resistant S. aureus (MRSA), and 8% carbapenemases. Conclusion: Bacteriological profile is characterized by ESBL-producing Enterobacteriaceae and MRSA. The most active antibiotics were macrolides, aminoglycosides, and cefoxitin (methicillin) for Gram-positive cocci, carbapenems, and aminoglycosides for Gram-negative bacilli. Then, the monitoring of antibiotics resistance must be permanent.

Although the advent of antibiotics has significantly improved the quality of life of infected patients,bacterial infections continue to pose a serious threat to public health[1,2].According to a recent report,within the next 30 years,bacterial infections are projected to surpass cancer in terms of lethality rates,resulting in an alarming 10 million deaths annually by 2050 due to the development of bacterial resistance[3].Moreover,the formation of bacterial biofilms hampers the penetration of antibacterial agents and inhibits the host immune response,making biofilm infections extremely challenging to treat[4-7].Hence,the development of innovative antimicrobial biofilm therapeutics is imperative.

Severe alcoholic hepatitis (sAH) is defined by a modified discriminant function ≥32 or model for end-stage liver disease (MELD) >20. Patients with sAH are in an immu-nocompromised state attributed to cirrhosis-related immu-noparesis and corticosteroid use. Individuals with sAH often develop severe infections that adversely impact short-term prognosis. Currently, the corticosteroid prednisolone is the only treatment with proven efficacy in sAH;however, the combination of corticosteroid treatment and altered host defense in sAH has been thought to increase the risk of acquiring of bacterial, opportunistic fungal, and viral infec-tions. Newer studies have shown that corticosteroids do not increase occurrence of infections in those with sAH;unfor-tunately, the lack of response to corticosteroids may instead predispose to infection development. Prompt and appropri-ate antibiotic treatment is therefore essential to improving patient outcomes. This review highlights common infections and risk factors in patients with sAH. Additionally, current diagnostic, therapeutic, and prophylactic strategies in these patients are discussed.

Community-acquired serious bacterial infections in the first 90 days of life:a revisit in the era of multi-drug-resistant organisms

Background Infants in the first 90 days of life are more prone to develop serious bacterial infections(SBIs).Multi-drug-resistant organisms(MDROs)are emerging as important pathogens causing *** reviewed the epidemiology of SBIs in infants 0-90 days old and compared the clinical features,laboratory values and final outcome for SBIs due to MDROs *** Episodes of culture-proven SBIs(bacteremia,urinary tract infections,or meningitis)with age at onset of 0-90 days during a 7-year period were retrospectively *** care-associated infections were *** collected demo-graphics,clinical features,and laboratory and microbiology *** compared clinical characteristics,laboratory data,microbiologic results and final outcome for SBIs due to MDROs *** Ninety-four episodes(88 patients)including bacteremia(42.6%),urinary tract infections(54.3%)and meningi-tis(3.1%)were caused by Gram-negative bacteria(67%),and Gram-positive bacteria(33%).Escherichia coli,Klebsiella pneumoniae and GBS were the most common *** caused SBIs in 39 patients(44.3%).SBIs due to MDROs were associated with more delay in providing targeted antimicrobial therapy compared to non-MDROs(74.4%vs.0%,P≤0.001,but no difference in case-fatality rate(12.8%vs.12.2%,P=1.0).Clinical features or basic laboratory values were not statistically different between the two *** The bacteriology of SBIs in the first 90 days of life is changing to include more MDROs,which causes more delay in providing targeted antimicrobial *** of the local epidemiology is crucial to ensure appropriate antibiotics are provided in a timely manner.

Coronaviruses have in the past been known to be the etiologic agents of mild upper respiratory infections in humans,similar to the ubiquitous and relatively benign"common cold"-type upper respiratory illnesses induced by the human rhinoviruses in adults and *** to the severe acute respiratory syndrome(SARS)outbreak in China 2003,and the Middle East respiratory syndrome(MERS)outbreak in the Middle East in 2012,global concerns regarding the pathogenicity and epidemic/pandemic potential of novel human coronaviruses began to emerge,with some experts predicting that novel coronaviruses could likely again cross the species barrier and present humans with future pandemic-potential infections.[1]These concerns have proven prescient with the emergence,late in 2019,of the 2019 coronavirus disease(COVID-19)or novel coronavirus pneumonia.
